Utilizarea chirurgiei ghidate în implantologia orală
Catedra de chirurgie OMF şi implantologie orală „Arsenie Guţan”, USMF „Nicolae Testemiţanu”Background. The free hand surgery does not allow the placement of implants in best position from
prosthetic and biomechanical point of view. The usage of digital technologies gives the possibility to
use surgical guides and to minimize the position errors. Objective of the study. To analyze the
advantages and disadvantages of guided implant surgery in comparison to the conventional one.
Material and Methods. The study was axed on 14 patients from which 7 of them were treated using
guided surgery (with 13 implants). In both groups the difference between implants position after the
surgery and those planned before the surgery was analyzed. The analyze was made on CBCT as well as
in oral cavity using position of implant axis in buccal-lingual aspect. Results. Due to the tendency to
minimize the surgical trauma and flap design, the anisotropic structure of the bone as well as the
tendency to put the implant in the middle of the bone crest, in free hand surgery group the deviation of
implants axis from initial plan was greater than in guided surgery group. However, a similar effect was
observed in case of axes guide usage, especially in postextractional or after GBR procedures. In one of
the cases, bad adaptation of the surgical guide was noticed. Conclusion. The usage of surgical guides
offer big advantages in cases when accurate positioning of the implant is necessary. However, the errors
in guide manufacturing and anatomical variables that may influence the implant positioning, even if
guided approach is used.Introducere. Chirurgia convențională nu permite plasarea implanturilor în cea mai bună poziție, din
punct de vedere protetic și biomecanic. Utilizarea tehnologiilor digitale oferă posibilitatea de a utiliza
ghiduri chirurgicale și de a minimaliza erorile de poziție. Scopul lucrării. Analiza avantajelor și
dezavantajelor chirurgiei implantare ghidate, în comparație cu cea convențională. Material și Metode.
Studiul a fost axat pe 14 pacienți , 7 dintre care au fost tratați utilizând chirurgia ghidată (cu 13
implanturi). În ambele grupuri a fost analizată diferența dintre poziția implanturilor după operație și cea
planificată înainte de operație. Analiza a fost făcută pe CBCT, precum și în cavitatea bucală, utilizând
poziția axului implantului în aspect bucal-lingual. Rezultate. Datorită tendinței de a minimaliza trauma
chirurgicală și designul lamboului, structura anizotropă a osului, precum și tendința de a pune implantul
în mijlocul crestei osoase, în grupul de chirurgie convențională, deviația axului implanturilor de la planul
inițial a fost mai mare decât în grupul de chirurgie ghidată. Cu toate acestea, un efect similar a fost
observat în cazul utilizării ghidului pentru stabilirea axului, în special, în procedurile postextracționale
sau după procedurile GBR. Într-un caz, s-a observat o rea adaptare a ghidului chirurgical. Concluzii.
Utilizarea ghidurilor chirurgicale oferă avantaje mari, în cazurile unde este necesară o poziționare exactă
a implantului. Cu toate acestea, erorile în fabricarea ghidului și a variabilelor anatomice pot influența
poziționarea implantului, chiar dacă se utilizează abordarea ghidată
Guided surgery in implant– prosthetic rehabilitation
Rezumat.
Rezultatul protetic precum şi succesul pe
termen lung în reabilitarea implanto-protetică depind de un şir de factori precum: poziţia
şi numărul implanturilor, tehnica chirurgicală, oferta osoasă şi mucoasă ş.a. Chirurgia
convenţională (free hand surgery) nu permite
în toate cazurile plasarea implanturilor în cea
mai bună poziţie din punct de vedere protetic
şi biomecanic. Odată cu dezvoltarea tehnologiilor de scanare şi printare tridimensională,
planificarea rezultatului final permite realizarea ghidurilor chirurgicale pentru asigurarea
unei poziţionări cât mai exacte a implanturilor dentare [1,2]. Analiza comparativă a
metodelor de instalare convenţională şi ghidată de instalare a implanturilor a demonstrat avantaje semnificative în poziţionarea
implanturilor în favoarea chirurgiei ghidate.
Totuşi, odată cu aplicarea diferitor manevre
suplimentare în realizarea tratamentului (amprentare, scanare, planificare, realizare ghid),
au fost observate un şir de neajunsuri şi erori
ce pot periclita realizarea tratamentului.Summary.
The prosthetic result as well as the long–
term success in implant–prosthetic rehabilitation depends on a series of factors such as:
position, number of implants, surgical technique, bone and mucosal offer, and others.
Usually, free hand surgery does not allow in

of view. Due to the development of scanning
technologies and three–dimensional printing,
the planning of final results allows the printing of surgical guides, to ensure the most accurate position of dental implants [1,2]. The
comparative analysis of conventional and
guided implant placement methods has demonstrated significant advantages in favor of
guided surgery. However, with the application
of additional maneuvers in the treatment (impression, scanning, planning, template fabrication), a number of disadvantages and errors

Analizând specifi cul dizabilităţilor motorii în AVC și metodica contemporană de învăţare și reînvăţare a activită ţilor motoare, am încercat să structurăm și să elaborăm un model de recuperare a echilibrului și coordonării în activităţile motorii funcţionale prin prisma transferului calităţilor și deprinderilor formate în baza tehnicilor de biofeedback. În lucrarea dată au fost prezentate particularităţile specifi ce de manifestare a echilibrului şi coordonării în diferite

Determinarea experimentală a impactului tratamentului chirurgical la pacienţii cu patologie stenozantă a arterelor bazinului carotidian cu accident vascular cerebral ischemic asupra procesului de recuperare
Pentru determinarea eficienţei tratamentului chirurgical asupra procesului de recuperare a pacienţilor cu patologie stenozantă a arterelor bazinului carotidian cu accident vascular cerebral ischemic, a fost desfăşurat un experiment longitudinal de constatare. Cercetarea a fost realizată pe un lot de 100 de pacienţi cu patologie stenozantă a arterelor bazinului carotidian cu accident vascular cerebral ischemic. Grupa experimentală a fost constituită din 50 pacienţi la care li s-a aplicat tratament chirurgical, iar grupa martor care a fost la fel în număr de 50 de pacienţi şi li s-a aplicat tratament medicamentos. Pentru constatarea influenţei tratamentului chirurgical asupra procesului de recuperare au fost aplicate teste clinice: ,,Fugl-Meyer”, ,,Barthel”, ,,Rivermed”, ,,Ashworth”. A crescut rezultate performanţelor fi zice în membrul superior şi în membrul inferior, de la 77,2 la 94,3 de puncte; a scăzut tonusul muscular în membrele paretic, unde punctajul la internare a constitult 1,5, iar după 18 luni de la tratamentul chirurgical 0,8 puncte; au sporit capacităţile funcţionale în membrul afectat, de la 5,9 la 10,6 puncte; analiza rezultatelor în recuperarea activităţilor cotidiene a scos în evidenţă o creştere a independenţei funcţionale prin valorile testului ,,Barthel” de la 57,7 la 86,9 puncte
The use of the surgical guides for implant-prosthetic rehabilitation in the aesthetic area
Introduction. Placing a dental implant to replace a central maxillary incisor can be a challenge in many
ways. Many factors need to be considered in order to generate an optimal treatment plan. An important
aspect is the degree of soft and hard tissue atrophy that often requires augmentation and bone grafting
procedures. At the same time, correct implant positioning represents one of the main challenges from
biomechanical and esthetical points of view. In this context, the anterior zone could be crucial for early
rehabilitation of the patient due to the particularities of teeth positioning.
Case presentation. The case study focused on a 32-year-old patient who was clinically and para-clinical
(CBCT) examined, at which was established the D3 (by Misch) bone density, for the following planning of
implant-prosthetic treatment. Previously, the bone addition procedure was performed. In order to achieve
a good result, the examination was performed with the prosthodontist. Following the conventional
impression and the virtual planning of the cast model, the stereolithographic model was obtained (scanned
in the laboratory) and superimposed on the CBCT, after which the surgical template for partial drilling was
made (using 3D printer ASIGA Pro 3D). The surgery was performed using partial drilling through the
surgical guide followed by osseous densification in free hand mode (Versah Drills system) and installation
of the implant followed by immediate screw retained restoration.
Discussion. A single missing central incisor is often the most difficult surgical and prosthetic challenge due
the ambition to obtain good aesthetics, but also the difficulties for the surgeon to make the ideal direction
of osteotomy. In the research it was proposed to place the implant in axial position, which allows the use
of a screw retained restoration (hiding the fixation screw channel in palatal side) as well as to obtain an
axial loading. An important role is also played by the accuracy of the guide and checking of its fitting with
the teeth. The postoperative result evaluated on the CBCT as well as in the oral cavity showed the implant
in the desired position, according to the preoperative plan.
Conclusion. Guided surgery allows us to obtain predictable results related to implant position. The use of
this technology requires additional experience due to the risks that occur both in the planning stage and in
the positioning of the guides in the oral cavity
