Effects of Exercise Training on Myocardial Blood Flow Reserve in Patients With Heart Failure and Left Ventricular Systolic Dysfunction

Exercise training has been shown to be effective in improving exercise capacity and quality of life in patients with heart failure and left ventricular (LV) systolic dysfunction. Real-time myocardial contrast echocardiography (RTMCE) is a new technique that allows quantitative analysis of myocardial blood flow (MBF). The aim of this study was to determine the effects of exercise training on MBF in patients with LV dysfunction. We studied 23 patients with LV dysfunction who underwent RTMCE and cardiopulmonary exercise testing at baseline and 4 months after medical treatment (control group, n = 10) or medical treatment plus exercise training (trained group, n = 13). Replenishment velocity (0) and MBF reserves were derived from quantitative RTMCE. The 4-month exercise training consisted of 3 60-minute exercise sessions/week at an intensity corresponding to anaerobic threshold, 10% below the respiratory compensation point. Aerobic exercise training did not change LV diameters, volumes, or ejection fraction. At baseline, no difference was observed in MBF reserve between the control and trained groups (1.89, 1.67 to 1.98, vs 1.81, 1.28 to 2.38, p = 0.38). Four-month exercise training resulted in a significant increase in beta reserve from 1.72 (1.45 to 1.48) to 2.20 (1.69 to 2.77, p <0.001) and an MBF reserve from 1.81 (1.28 to 2.38) to 3.05 (2.07 to 3.93, p <0.001). In the control group, 13 reserve decreased from 1.51 (1.10 to 1.85) to 1.46 (1.14 to 2.33, p = 0.03) and MBF reserve from 1.89 (1.67 to 1.98) to 1.55 (1.11 to 2.27, p <0.001). Peak oxygen consumption increased by 13.8% after 4 months of exercise training and decreased by 1.9% in the control group. In conclusion, exercise training resulted in significant improvement of MBF reserve in patients with heart failure and LV dysfunction. (C) 2010 Elsevier Inc. All rights reserved. Prader-Willi syndrome is a mental retardation genetic disorder also characterized by hypogonadism, hyperphagia and obesity. We report on a four-years-old boy, born to consanguineous parents, with uncommon co-occurrence of Prader-Willi syndrome, 47,XXY karyotype (Klinefelter syndrome) and coronal craniosynostosis. These are different unrelated conditions and it was not described before in the same patient to the best of our knowledge.<br>A síndrome de Prader-Willi é afecção genética de deficiência mental associada a hipogonadismo hipogonadotrófico, hiperfagia e obesidade.

A short time after the creation of the first Geology courses in Brazil (in 1957 with the pioneers in the University of São Paulo and in the Federal Universities of Ouro Preto, Pernambuco and Rio Grande do Sul, and then in the following year in the Federal Universities of Bahia and Rio de Janeiro), there arose other initiatives that spread almost twenty Geology courses throughout Brazil. In addition to expanding the Geology teaching in the South, Southeast and Northeast regions, these initiatives succeeded in allowing access to geological education for the population in the North and Central-west of Brazil. In the 1960s, the courses in the Federal University of Para in Belém (1964), University of Brasilia (1965) and São Paulo State University in Rio Claro (1969) were implanted. In the following decade, the courses in the Federal Rural University of Rio de Janeiro in Seropédica (1970), the Federal University of Ceará in Fortaleza (1970), the University of Rio dos Sinos in São Leopoldo (1973), the Federal University of Paraná in Curitiba (1973), the Federal University of Minas Gerais in Belo Horizonte (1973), the Federal University of Amazonas in Manaus (1976), the Federal University of Mato Grosso in Cuiabá (1976), the Federal University of Rio Grande do Norte in Natal (1976), and the State University of Rio de Janeiro in Rio de Janeiro (1977) were all created. At the close of the twentieth century, the course was implanted in the State University of Campinas (1998). Now, at the beginning of the twenty-first century, new Geology courses are being implanted, accentuating the movement inland of Geology teaching in Brazil. The Federal University of Pará began a new course in its campus in Marabá in the south-east of Pará and the Federal University of Bahia implanted a new course in its campus in Barreiras in the west of Bahia. Finally, the Federal Universities of Sergipe, Espírito Santo and Roraima commenced Geology courses in Aracaju, Alegre and Boa Vista, respectively. This chapter will present the synthesis of the Geology courses which, over the last decades of the twentieth century, contributed to the expansion of Geology teaching in the country, taking it to every region and giving opportunities to a large number of Brazilian citizens to realize their dreams and tread the paths of their professional vocation
