    Effect of personalized support at home on the prevalence of anemia in pregnancy in Burkina Faso: A cluster randomized trial

    Burkina Faso has high prevalence of anemia in pregnancy (hemoglobin < 11 g/dL), despite the implementation of the WHO recommended guidelines. This study aimed to test the effects of personalized support for pregnant women at home on the trend of anemia prevalence in pregnancy. A cluster randomized trial was conducted from January 2015 to August 2016 at Sindou health district in Burkina Faso. Data were collected from 617 women in their first or second trimester of pregnancy, including 440 and 177 women in the intervention and control groups, respectively. The intervention consisted of a monthly home-based visit to the pregnant woman, focusing on nutritional counseling and pregnancy management, alongside an improvement antenatal visit quality. Compared with the prevalence of anemia in pregnancy in the control group [64.0% (95% confidence interval [CI]: 52.1-74.4%)], that of the intervention group was significantly lower from the fifth home visit onward [36.8% (95% CI: 32.1-41.8%)] (P < 0.001). The adjusted difference-in-differences in anemia prevalence between the two groups was -19.8% (95% CI: -30.2% to -9.4%) for women who received more than four home visits (P < 0.001). The corresponding difference in hemoglobin levels was 0.644 g/dL (95% CI: 0.309-0.167; P < 0.001). Personalized support for pregnant women at home, combined with appropriate antenatal care, can significantly reduce anemia prevalence during pregnancy in rural Burkina Faso.SCOPUS: ar.jinfo:eu-repo/semantics/publishe

    Offre de soins de qualité : Facteurs de survenue des complications au cours du cathétérisme veineux périphérique au Centre hospitalier universitaire Souro Sanou (CHUSS) de Bobo-Dioulasso, Burkina Faso

    L’objectif de cette recherche était d’étudier l’incidence et les facteurs de risque de survenue de complications du cathétérisme veineux périphérique (CVP) chez les patients dans les services des urgences médicale et chirurgicale du Centre hospitalier universitaire Souro Sanou de Bobo Dioulasso. Il s’est agi d’une étude transversale à visée analytique qui s’est déroulée du 4 juin au 3 novembre 2018. Il a été calculé l’incidence et identifié les facteurs de risque de survenue des complications. Des 511 patients inclus dans l’étude, la moyenne d’âge était de 44,7 (95%IC : 43,1 - 46,3) ans. Aussi, 43,1% (95%IC: 39,0 % – 47,0 %) soit 220 des 511 patients ont développé une complication. Les complications étaient : phlébites (29,2%), infiltrations (7,8%), retraits accidentels (4,1%), et extravasations (1,9%). Les facteurs de risque liés à la survenue de complications étaient : sexe des patients, durée du cathétérisme, lavage simple ou friction avec une solution hydroalcoolique des mains, et utilisation d’antibiotiques en thérapie intraveineuse. Pour réduire le risque de complications, il faut respecter les règles de prévention contre les infections, la durée du cathétérisme, les précautions à l’utilisation des antibiotiques en thérapie intraveineuse, surveillance plus minutieuse chez les patientes, et sensibiliser les prestataires hommes.Mots-clés: cathétérisme veineux périphérique, complications, facteurs, contrôle des infections, qualité des soins, management des casEnglish Title: Offering quality care: Factors contributing to the occurrence of complications during the periphéral venous catheterism in the National teaching hospital Souro Sanou of Bobo-Dioulasso, Burkina FasoEnglish AbstractThe objective of this study was to assess the incidence and risk factors for complications of peripheral venous catheterism (PVC) complications in the management of patients from the medical and surgical emergency departments of the National teaching hospital Souro Sanou in Bobo-Dioulasso. This was a cross-sectional study that took place from June 4 to November 3, 2018. The incidence was calculated and risk factors for complications were identified. Of the 511 patients included in the study, the average age was 44.7 (95%IC: 43.1 - 46.3) years-old. Also, 43.1% (CI95%: 39.0% - 47.0%) 220 of the 511 patients developed a complication.. Complications were: phlebitis (29.2%), infiltrations (7.8%), accidental withdrawals (4.1%), and extravasations (1.9%). Risk factors for complications following PVC included: gender of patients, duration of catheterization, simple washing or friction with hydroalcoholic hand solution, and use of antibiotics in intravenous therapy.     Satisfaction des parents accompagnateurs des enfants de moins de 5 ans pris en charge au centre médical de Kokologho au Burkina Faso

    La satisfaction des usagers des centres médicaux (CM) au Burkina Faso n’est pas connue. Notre objectif était d´étudier le niveau de satisfaction et les facteurs associés des parents accompagnateurs des enfants de moins de 5 ans pris en charge au CM de Kokologho en 2018. Il s’est agi d’une étude transversale. Réalisée dans l’aire de santé du CM de Kokologho, les cibles étaient les parents accompagnateurs d’enfants de moins de 5 ans pour l´enquête ménage et pour les entretiens qualitatifs les représentants de la communauté, les agents de santé et les leaders administratifs et coutumiers. Les outils de collecte des données ont été construits à partir des référentiels (SAPHORA-job 2014, conseil Québécois d’Agrément 2005). Des scores de satisfaction, Odds Ratio ajustées et IC95 % ont été calculés. Des analyses thématiques ont été effectuées. Le niveau de satisfaction globale des parents accompagnateurs des enfants pris en charge au CM de Kokologho en 2018 était satisfaisant (70,6 %). Les dimensions étudiées étaient l’accueil (47,2 %), le respect (65,3 %), l’empathie (60,5 %), la confidentialité (73,8 %), la fiabilité (71,9 %), la rapidité (51,4 %), le confort (89,0 %), l’accessibilité géographique (97,5 %) et l’accessibilité financière (28,5 %). Le niveau d’instruction non scolarisé, le sexe féminin et la situation matrimoniale mariée étaient significativement associés à la satisfaction globale des parents. Les usagers sont satisfaits des services du CM de Kokologho. Cependant, des efforts doivent être faits pour améliorer l’accueil, la rapidité de l’offre de service et l’accessibilité financière.Mots-clés: Satisfaction - parents accompagnateurs - enfants - Burkina FasoEnglish AbstractIntroduction: The satisfaction of users of medical centers in Burkina Faso is not known. The data collection tools were built from the repositories (SAPHORA-job 2014, Conseil Québécois d'Agrément 2005). Satisfaction scores, adjusted Odds Ratio and 95% CI were calculated. Thematic analyzes were carried out. The overall level of satisfaction of the parents of the children in the Kokologho medical center in 2018 was satisfactory at 70.6%. The studied dimensions were patient reception (47.2%), respect (65.3%), empathy (60.5%), confidentiality (73.8%), reliability (71.9%), speed (51.4%), comfort (89.0%), geographical accessibility (97.5%) and affordability (28.5%). Level of education gender and marital status were significantly associated with overall parental satisfaction. Users are satisfied with the services in Kokologho medical center.     La satisfaction des usagers des établissements sanitaires fait partie de l’appréciation de la qualité des soins. L’objectif était d’étudier la satisfaction des patients hospitalisés dans les services du département de médecine du Centre hospitalier universitaire Souro Sanou (CHUSS) de Bobo-Dioulasso, Burkina Faso. Il s’est agi d’une étude transversale à visée analytique conduite en 8 mois. Les domaines de satisfaction ont été calculés selon le modèle SAPHORA (version 7). Des 294 patients éligibles, 250 (85,0 %) patients ont été retenus dont 42,0 % de femmes. L’âge moyen des patients était de 47,1 (± 17,9) ans. Les domaines à score faible étaient : niveau global de satisfaction, accueil, communication avec le personnel, restauration et organisation de la sortie. Les patients plus âgés et ceux à durée d'hospitalisation plus longue étaient plus satisfaits. Les 94,0 % des patients sans assurance maladie étaient moins satisfaits.Les scores des domaines de satisfaction par service et le score global de satisfaction étaient inférieurs à 50,0 %. Les différences entre les services étaient en lien avec : accueil, qualité humaine du personnel, soins médicaux, hôtellerie, restauration, coûts, et niveau global de satisfaction. The study type was cross-sectional, analytical, that lasted in 8 months. The satisfaction levels were calculated according to the SAPHORA model (version 7). Among the 294 eligible patients, 250 (85.0%) of them were selected with 42% of women. The mean age of the patients was 47.1 (± 17.9) years old. The low score domains were: overall level of satisfaction, reception, restoration and organization of the outing. Older patients and those  with longer hospital stays were more satisfied. The 94.0% of patients without health insurance were less satisfied. Scores of satisfaction domains by service and the overall satisfaction score were all less than 50.0%. Differences between services were related to: hospitality, human quality of staff, medical care, hospitality, restoration, costs and overall level of satisfaction.
