Znaczenie polimorfizmu "IL28B" w odpowiedzi na leczenie interferonem pegylowanym α i rybawiryną przewlekłego zapalenia wątroby wywołanego genotypem 1b HCV
Wstęp: Leczenie przewlekłego zapalenia wątroby typu C
(PZW-C) pegylowanym interferonem α (Peg-IFN) z ryba wiryną
powoduje tzw. trwałą odpowiedź wirusologiczną (sustained
virological response - SVR) u blisko 50% chorych. Odpowiedź
ta ściśle zależy od HCV i czynników genetycznych chorego.
Wykazano, że polimorfizm genu IL28B wiąże się z eradykacją
wirusa i SVR.
Cel: Zbadanie, czy wyniki leczenia PZW-C u chorych z regionu
Polski Południowej zależą od wariantów genetycznych IL28B,
a polimorfizm może być czynnikiem predykcyjnym SVR.
Materiał i metody: Sto czterdzieści dwie osoby z PZW-C spowodowanym genotypem 1b HCV leczono standardowo PegIFN z rybawiryną przez 48 tygodni. Polimorfizmy rs12979860
IL28B (C/T) badano metodą PCR-RFLP. HCV-RNA i aktywność
aminotransferazy alaninowej (ALT) w surowicy mierzono
przed leczeniem i po 12, 48 i 72 tygodniach terapii.
Wyniki: Wiremia HCV-RNA przed terapią i po 12-tygodniowym
leczeniu była wyższa u chorych z genotypem T/C i T/T. Trwałą
odpowiedź wirusologiczną osiągnęło istotnie więcej chorych
z genotypem C/C (71,1%) w porównaniu z 41,4% z genotypem
T/C i 23,5% T/T. Chorzy z genotypem C/C lepiej reagowali na
leczenie w porównaniu z osobami z genotypami T/C i T/T,
osiągali wyższe wskaźniki odpowiedzi wczesnej (12 tygodni),
na koniec leczenia (48 tygodni) i SVR. Aktywność ALT nie
różniła się istotnie podczas terapii pomiędzy grupami. Tolerancja leczenia była podobna i nie zależała od genotypów C/C,
T/C i T/T. Wnioski: Wyniki badania potwierdziły zależność odpowiedzi
na standardowe leczenie PZW-C spowodowanego genotypem 1b HCV od predyspozycji genetycznej chorego, jednak
w przeciwieństwie do niektórych doniesień złe rokowanie
obserwowano u homozygot T/T IL28B.Introduction: Treatment of chronic hepatitis C (CH-C) with
peginterferon α (Peg-IFN) and ribavirin leads to a sustained
virological response (SVR) in 50% of patients and depends on
HCV and host factors. Polymorphism of the IL28B gene is
associated with eradication of HCV and SVR.
Aim: The objective of this study was to examine patients from
the region of southern Poland and determine whether CH-C
therapy depends on the genetic variants of IL28B and
whether this polymorphism may predict SVR.
Material and methods: One hundred forty-two patients
with CH-C and the genotype 1b HCV were treated with PegIFN and ribavirin for 48 weeks. IL28B rs12979860 polymorphisms (C/T) were tested by PCR-RFLP. The HCV-RNA and
alanine aminotransferase (ALT) levels were measured
before treatment and after 12, 48 and 72 weeks.
Results: Viral load before and after 12 weeks of therapy was
higher in the genotypes T/C and T/T than in C/C. 71.1% of
patients with the genotype C/C achieved SVR vs. 41.4% with
the genotype T/C and 23.5% with T/T. Patients with the genotype C/C responded better to treatment as compared to subjects with the genotypes T/C and T/T, and achieved better
early response (12 weeks), at the end of treatment (48 weeks)
and SVR. Alanine aminotransferase activity was similar
among the groups. Tolerance of therapy was similar and independent of the genotypes.
Conclusions: The study confirmed the dependence of
response to standard treatment of CH-C caused by the genotype 1b HCV on the patient's genetic predisposition. In contrast to some other reports, poor prognosis was observed in
T/T homozygotes of IL28B
Cognitive functions in patients with liver cirrhosis : a tendency to commit more memory errors
Background: Minimal hepatic encephalopathy (MHE) is the mildest form of hepatic encephalopathy (HE). For diagnostic purposes, 2 alternative batteries of psychometric screening tests are recommended. They differ from each other in terms of the cognitive domains assessed. The research was designed to provide a profile of cognitive functioning in patients with liver cirrhosis, using an assessment that covers a wider range of cognitive functions than the usual screening battery.
Material and Methods: We examined 138 persons, including 88 with liver cirrhosis and 50 healthy volunteers. The Mini Mental State Examination (MMSE) was used for screening and excluding advanced cognitive impairment. Then, to assess cognitive functions in more detail, the following tests were used: Auditory Verbal Learning Test (AVLT), Letter and Semantic Fluency Tests (LF and SF), Trail Making Test (TMT A&B), Digit Symbol Test (DST), Block Design Test (BDT), and Mental Rotation Test (MRT). The MRT task has not been used in MHE diagnosis so far. Finally, 57 patients and 48 controls took part in the entire study.
Results: Patients with liver cirrhosis commit significantly more errors of intrusions in the AVLT during the delayed free recall trial. Results significantly deviating from the norm in at least 2 tests were found only in 7 cirrhosis patients.
Conclusions: The results do not provide any specific profile of cognitive disturbances in MHE, but suggest that cirrhosis patients have a tendency to commit more memory errors, probably due to subtle impairments of executive function
Czy spektroskopia rezonansu magnetycznego identyfikuje pacjentów z minimalną encefalopatią wątrobową?
Background and purpose
The results of a few studies suggest that magnetic resonance spectroscopy of the brain could allow detection of minimal hepatic encephalopathy. The goal of this study was to assess the ability of magnetic resonance spectroscopy to differentiate between cirrhotic patients with and without minimal hepatic encephalopathy.
Material and methods
Localized magnetic resonance spectroscopy was performed in the basal ganglia, occipital gray matter and frontal white matter in 46 patients with liver cirrhosis without overt encephalopathy and in 45 controls. Neurological and neuropsychological examination was performed in each participant.
Results
The patients with liver cirrhosis had a decreased ratio of myoinositol to creatine in occipital gray matter and frontal white matter (mean: 0.17 ± 0.05 vs. 0.20 ± 0.04, p = 0.01 and 0.15 ± 0.05 vs. 0.19 ± 0.04, p < 0.01, respectively) and a decreased ratio of choline to creatine in occipital gray matter (mean: 0.32 ± 0.07 vs. 0.36 ± 0.08, p = 0.03). Minimal hepatic encephalopathy was diagnosed in 7 patients. Metabolite ratios did not differ significantly between patients with and without minimal hepatic encephalopathy. Metabolite ratios did not differ significantly between patients with Child-Pugh A and those with Child-Pugh B.
Conclusions
Magnetic resonance spectroscopy does not allow accurate diagnosis of minimal hepatic encephalopathy. A similar profile of metabolites in the brain is observed in cirrhotic patients without cognitive impairment.Wstęp i cel pracy
Wyniki pojedynczych badań sugerują, że spektroskopia rezonansu magnetycznego mózgu może być pomocna w wykrywaniu minimalnej encefalopatii wątrobowej. Celem tego badania była ocena przydatności spektroskopii rezonansu magnetycznego w odróżnianiu pacjentów z marskością wątroby z minimalną encefalopatią wątrobową od pacjentów bez tej encefalopatii.
Materiał i metody
Badanie spektroskopii rezonansu magnetycznego mózgu przeprowadzono u 46 pacjentów z marskością wątroby bez jawnej encefalopatii i u 45 osób z grupy kontrolnej. Rejestracji widm dokonano z trzech obszarów mózgu: zwojów podstawy, istoty szarej płata potylicznego i istoty białej płata czołowego. U wszystkich badanych osób przeprowadzono badanie neurologiczne i neuropsychologiczne.
Wyniki
U pacjentów z marskością wątroby stwierdzono obniżenie stosunku mioinozytolu do kreatyny w zakresie płata potylicznego i czołowego (średnie odpowiednio 0,17 ± 0,05 vs 0,20 ± 0,04, p = 0,01 oraz 0,15 ± 0,05 vs 0,19 ± 0,04, p < 0,01) oraz zmniejszenie stosunku choliny do kreatyny w płacie potylicznym (średnia: 0,32 ± 0,07 vs 0,36 ± 0,08, p = 0,03) w porównaniu z grupą kontrolną. Minimalną encefalopatię wątrobową rozpoznano u 7 pacjentów. Stosunek metabolitów nie różnił się istotnie u pacjentów z minimalną encefalopatią wątrobową i bez niej. Nie stwierdzono także różnicy w stężeniu metabolitów u pacjentów z niewydolnością wątroby zakwalifikowanych do kategorii A w skali Child-Pugh w porównaniu z pacjentami zakwalifikowanymi do kategorii B.
Wnioski
Spektroskopia rezonansu magnetycznego nie pozwala na dokładne rozpoznanie minimalnej encefalopatii wątrobowej. Podobny profil metabolitów w mózgu obserwuje się u pacjentów z marskością wątroby bez zaburzeń poznawczych

Polimorfizm –455G/A genu β-fibrynogenu a ryzyko udaru niedokrwiennego w populacji polskiej
Background and purpose: Ischaemic stroke is considered to
be multifactorial and interactions between environmental and
genetic factors play an important role. Although vascular risk
factors are well known, the genetic ones are still undiscover -
ed. In the present study, we assessed the significance of the
β-fibrinogen –455G/A gene polymorphism and the risk of
ischaemic stroke in a Polish population.
Material and methods: 426 ischaemic stroke patients classified according to stroke aetiologies (small vessel disease, large
vessel disease or cardioembolic stroke) and 234 controls were
included in the study. The association of the β-fibrinogen
genotypes with ischaemic stroke was tested using logistic
regression analysis under dominant, recessive or additive
models of inheritance.
Results: The allele and genotype distributions of the β-fibri -
nogen –455G/A gene polymorphism did not differ significantly between patients and controls (patients: G – 75%,
GG – 56.6%, GA – 36.8%, AA – 6.6%; controls: G – 73.7%,
GG – 57.3%, GA – 32.9%, AA – 9.8%; p > 0.05, χ2). In
addition, logistic regression analysis adjusted for the known
risk factors, i.e. hypertension, ischaemic heart disease, myocardial infarction, hypercholesterolaemia, diabetes mellitus and
smoking, did not show a role of the studied polymorphism
in ischaemic stroke. Conclusions: The β-fibrinogen –455G/A gene polymorphism
is not a risk factor for ischaemic stroke in a Polish population
