    Comparative analysis of postoperative results of cataract surgery using monofocal IOLs

    One of the most common surgical interventions in ophthalmology today is lens replacement surgery. But there is still no consensus on whether the choice of a monofocal IOL affects the postoperative functional outcome.The aim: to conduct a comparative analysis of values of visual acuity without correction in the early postoperative period after cataract surgery using monofocal IOLs.Material and methods. The study included data of 2643 eyes operated for cataract, into which one of the monofocal IOLs, included in the top ten most frequently implanted IOLs in the Orenburg Branch of the S. Fyodorov Eye Microsurgery Federal State Institution, was implanted for the period 2019–2021. Statistical analysis was performed using the program Statistica 13.0.Results. The performed analysis of variance showed that the established differences between the compared groups are statistically signifi cant (p < 0.05). However, the subgroup analysis showed that there were statistically signifi cant differences between pairs such as Tecnis and Hydro-4; Tecnis and Rayner. In all other cases, statistically signifi cant uncorrected visual activity (UCVA) values after surgery were not established. It has also been reliably established that the presence of subluxation of the lens affects the value of UCVA after surgery.Conclusion. The results of the performed analysis in general indicate that the value of UCVA in patients in the early postoperative period after the performed surgical intervention does not depend on the manufacturer of the used IOL

    DISSEMINATED CUTANEOUS TUBERCULOSIS

    Cutaneous tuberculosis is a clinically and morphologically heterogeneous group of skin diseases directly or indirectly caused by mycobacteria of the tuberculosis complex, occupying the 5th place among all localizations of extra-pulmonary tuberculosis. Manifestations of cutaneous tuberculosis are extremely diverse and depend on the immune status and ways of penetration of mycobacteria into the skin. Skin involvement can occur as a result of exogenous inoculation, contiguous spread from an adjacent focus, or hematogenous spread from other foci. Family cases of disseminated cutaneous tuberculosis are described. A combination of several clinical forms of cutaneous tuberculosis — scrofuloderma, tuberculosis cutis lichenoides (moniliformis scrophulosorum) and lupus — are observed in a 5 year old child with a family history of tuberculosis. The issues of pathogenesis and advanced methods of diagnosis of cutaneous tuberculosis are also discussed

    Onychomycosis: features external therapy

    Onychomycosis - one of the most frequent diseases with defeat of a nail bed, matrics and nail plate of a fungal origin. Frequency of an onychomycosis in Russia reaches 5% (to 15 million people), and practically every tenth visit of the dermatologist is connected with this pathology. Treatment of an onychomycosis still remains very difficult task. Application of varnishes, ointments with antimycotics and local agents on an oil basis has an essential shortcoming - insufficient receipt of a preparation to the center of pathological changes. It is supposed that for optimum therapy it is necessary to influence: first, from frontal and distal part of a nail, secondly, to apply aqueous-alcoholic solutions of a preparation which can freely get into subnail spaces with the phenomena of an onycholysis. For example, it has been shown highly efficient application of naftifina a hydrochloride under control of solution of the diamond green, showing diffusion distribution active ingredient to subnail space and on channels

    The effect of melatonin on the production of pro-inflammatory cytokines in cataract surgery in patients with non-exudative AMD

    Background. In recent years, the studies concerning the role of melatonin and cytokines in the development of various pathological processes have been conducted in various fields of medicine, including ophthalmology.Purpose. To study the effect of melatonin-containing therapy on the concentration of pro-inflammatory cytokines, melatonin in the lacrimal fluid and melatonin in the blood serum in patients with non-exudative AMD in the course of surgery for uncomplicated senile cataract. Material and methods. During the study, 2 groups have been formed: group 1 (main group) – 40 patients who took the drug «Melaxen» (3 mg at night) for 3 months before the surgery; group 2 (control group) – 40 patients who did not take melatonin before the surgery. Laboratory studies of blood samples and tear fluid for the hormone melatonin and cytokines (IL-1β and IL-8) were carried out using enzyme-linked immunosorbent assay.Results. When taking exogenous melatonin in patients of the main group, the concentration of melatonin in blood serum and lacrimal fluid was significantly higher during the entire observation period, compared with the control group patients. Also, melatonin-containing therapy contributed to a decrease in the concentration of pro-inflammatory IL-8 and IL-1β cytokines. A correlation analysis of the level of cytokines and the content of melatonin in lacrimal fluid of the examined groups patients revealed a significantly higher feedback before surgery and significantly weaker and direct after the surgery.Conclusion. An increase in the concentration of melatonin in the lacrimal fluid after surgery for senile cataract in both groups indicates a possible increase in its production in response to surgical treatment and is aimed at reducing the activity of the local inflammatory process. The obtained results allow us to conclude that melatonin has an antiinflammatory effect

    В статье приведен анализ эксплуатационных свойств набивных тигельных масс в зависимости от их состава, активности компонентов и состава применяемых алюмофосфатных связующих (АФС). Найдены оптимальные режимы механоактивации графитов, а также соотношение активированных компонентов к неактивированным, позволяющее достичь показателя прочности при сохранении термостойкости композиции. Детально исследованы такие свойства, как работа адгезии, плотность, седиментационная устойчивость АФС. Установлено, что с уменьшением в составе АФС содержания гидроксида алюминия повышается кислотность и уменьшается седиментационная устойчивость. Показана зависимость прочности лабораторных образцов тиглей от температуры обжига и состава связующего.The paper analyzes the operational properties of stuffed melting pot masses, depending on their composition, activity of the components and used composition of the aluminum phosphate bonds (APB). Found optimal regimes of mechanical activation graphite, and also found the ratio of activated components to non-activated, allowing to achieve performance of strength when saving thermal stability of the composition. Studied in detail properties such as the work of adhesion, density, sedimentation stability of APB. It is established that a decrease in the content APB of aluminum hydroxide, increased acidity and decreased sedimentation stability. Shows the dependence of the strength of laboratory samples of melting pots from burning temperature and the composition of the binder

    Clinical and morphological heterogeneity of malignant lymphoproliferative lesions of the skin is explained by the peculiarities of their pathogenesis and organization of the lymphoid tissue in the skin. These features determine the authenticity of therapeutic approaches to patients with this pathology and exclude the use of standard regimens of polychemotherapy used in oncology due to their low efficacy.     Application of a preparation of imiquimod in а dosage form of ointment 5% in therapy of basal cell carcinoma

    Objective. To estimate efficiency and safety of application of a preparation of Aldara (imiquimod, 5% external application cream) it is independent or in combination with cryotherapy in therapy of the basal cell carcinoma of the skin (BCC). Material and methods. Research included 78 patients (22 women and 56 men) with various forms of a basal cell carcinoma of the skin. All patients were divided into 3 groups: the main group (n = 30) the patients receiving therapy by Aldara (imiquimod, 5% external application cream) made, second (n = 21) - the patients receiving therapy by Aldara’s, and also 1 course of cryotherapy, and the group of comparison (n = 27) receiving therapy of placebo in the form of external application cream. Therapy was carried out 3 times a week, within 16 weeks. Research was conducted in some stages with histologic confirmation of the diagnosis, and also an assessment of efficiency of therapy and undesirable effects. Results. At 27 of 30 patients of the first group (90%) and at 21 of 21 patients of the second group (100%) receiving treatment by Aldara’s absolute clinical recovery is reached. In group of comparison of clinical recovery at all 27 patients it wasn’t observed. It is expedient to note that at all patients of the second group (100%) already to 75 ± to the 2nd day of therapy (the 4th visit) clinical recovery while at 27 of 30 patients of the first group (90%) the total disappearance (clinical recovery) came for 105 and more days (the 5th and 6th visits) was noted. Recurrence of a disease after the end of therapy it wasn’t observed. Conclusion. Aldara (imiquimod, 5% external application cream) possesses high efficiency at therapy of various forms and sizes of BCC. Undesirable effects of therapy are easily resolved at cancellation of a preparation and not observed at its renewal. Unlike other immunomodulatory preparations (corticosteroids, retinoid, cyclosporin) Aldar’s application doesn’t promote suppression of cellular immunity

    Squamous cell carcinoma of the skin (SSCC) is one of the most common malignant skin tumors. Syphilis is a sexually transmitted disease caused by Treponema pallidum, with human beings as the only host. The combination of syphilis and squamous cell carcinoma of the skin is not uncommon, particularly if the lesions are located on different parts of the body. However, simultaneous development of the chancre and squamous cell carcinoma of the glans penis seems exceptional. Considering rarity of the manifestation observed we feel the rare case of combined syphilis and squamous cell skin cancer is of interest

    Integral Intensities of Absorption Bands of Silicon Tetrafluoride in the Gas Phase and Cryogenic Solutions: Experiment and Calculation

    The spectral characteristics of the SiF4 molecule in the range 3100700 cm1, including the absorption range of the band 3, are studied in the gas phase at P = 0.47 bar and in solutions in liquefied Ar and Kr. In the cryogenic solutions, the relative intensities of the vibrational bands, including the bands of the isotopically substituted molecules, are determined. The absorption coefficients of the combination bands 23, 3 + 1, 3 + 4, and 34 are measured in the solution in Kr. In the gas phase of the one-component system at an elevated pressure of SiF4, the integrated absorption coefficient of the absorption band 3 of the 28SiF4 molecule was measured to be A(3) = 700 ± 30 km/mol. Within the limits of experimental error, this absorption coefficient is consistent with estimates obtained from independent measurements and virtually coincides with the coefficient A(3) = 691 km/mol calculated in this study by the quantum-chemical method MP2(full) with the basis set cc-pVQZ. ©2005 Pleiades Publishing, Inc
