Mutant pattern of p53 predicts local recurrence and poor survival rate in gastric cancer
Background. TP53 mutation is a poor
prognostic factor for various organ malignancies such as
colorectal cancer, breast cancer, ovarian cancer,
hepatocellular carcinoma, lung adenocarcinoma and
clinical pathologists previously evaluated it using
immunohistochemistry for p53. The clinicopathologic
significance of p53 expression in gastric cancer remains
unclear due to inconsistent classification methods.
Methods. Immunohistochemistry for p53 protein
was performed using tissue microarray blocks generated
from 725 cases of gastric cancer, and p53 expression
was divided into three staining patterns using a semiquantitative ternary classifier: heterogeneous (wild type),
overexpression, and absence (mutant pattern).
Results. Mutant pattern of p53 expression had a
male predominance, greater frequency in cardia/fundus,
higher pT stage, frequent lymph node metastasis, local
recurrence clinically, and more differentiated histology
microscopically compared with wild type. In survival
analysis, p53 mutant pattern was associated with worse
recurrent-free survival and overall survival rates, and
significance was maintained in subgroup analysis of
early versus advanced gastric cancers. In Cox regression
analysis, p53 mutant pattern was a significant predicting
factor for local recurrence (relative risk (RR=4.882,
p<0.001)) and overall survival (RR=2.040, p=0.007).
The p53 mutant pattern remained significant for local
recurrence (RR=2.934, p=0.018) in multivariate
analyses.
Conclusions. Mutant p53 pattern on immunohistochemistry was a significant prognostic factor for
local recurrence and poor overall survival in gastric
cancer
Current Concepts in the Mandibular Condyle Fracture Management Part I: Overview of Condylar Fracture
The incidence of condylar fractures is high, but the management of fractures of the mandibular condyle continues to be controversial. Historically, maxillomandibular fixation, external fixation, and surgical splints with internal fixation systems were the techniques commonly used in the treatment of the fractured mandible. Condylar fractures can be extracapsular or intracapsular, undisplaced, deviated, displaced, or dislocated. Treatment depends on the age of the patient, the co-existence of other mandibular or maxillary fractures, whether the condylar fracture is unilateral or bilateral, the level and displacement of the fracture, the state of dentition and dental occlusion, and the surgeonnds on the age of the patient, the co-existence of othefrom which it is difficult to recover aesthetically and functionally;an appropriate treatment is required to reconstruct the shape and achieve the function ofthe uninjured status. To do this, accurate diagnosis, appropriate reduction and rigid fixation, and complication prevention are required. In particular, as mandibular condyle fracture may cause long-term complications such as malocclusion, particularly open bite, reduced posterior facial height, and facial asymmetry in addition to chronic pain and mobility limitation, great caution should be taken. Electric field control of nonvolatile four-state magnetization at room temperature
We find the realization of large converse magnetoelectric (ME) effects at
room temperature in a multiferroic hexaferrite
BaSrCoFeO single crystal, in which rapid
change of electric polarization in low magnetic fields (about 5 mT) is coined
to a large ME susceptibility of 3200 ps/m. The modulation of magnetization then
reaches up to 0.62 /f.u. in an electric field of 1.14 MV/m. We find
further that four ME states induced by different ME poling exhibit unique,
nonvolatile magnetization versus electric field curves, which can be
approximately described by an effective free energy with a distinct set of ME
coefficients
Analysis of different tumor volume thresholds of insignificant prostate cancer and their implications for active surveillance patient selection and monitoring
PurposeWe compared oncological outcomes according to tumor volume (TV) thresholds defining both classical and updated insignificant prostate cancer (IPC), since the TV threshold can be used as clinical parameter for active surveillance.MethodsBetween 2001 and 2012, we retrospectively analyzed 331 organ-confined prostate cancer patients who had preoperative Gleason score 6, preoperative PSA under 10 ng/mL and pathologic TV less than 1.3 mL. Among them, 81 of 331 (24.5%) had Gleason grade 4/5 disease postoperatively. Patients were stratified into two groups: (1) TV less than 0.5 mL, using the classical definition; and (2) TV between 0.5 mL and 1.3 mL, using the range of updated definition. We compared biochemical recurrence (BCR)-free survival and identified independent predictors of BCR in each group.ResultsGroup 2 had more Gleason grade 4/5 disease than group 1 (P<0.001). On multivariate analysis, Gleason grade 4/5 disease was not associated with BCR in group 1 (P=0.132). However, it was an independent predictor for BCR in group 2 (P=0.042). BCR-free survival were not significantly different according to the presence of Gleason grade 4/5 disease in group 1 (P=0.115). However, in group 2, it was significantly different according to the presence of Gleason grade 4/5 disease (P=0.041).ConclusionsAlthough the TV thresholds of the two definitions of IPC vary only slightly, this difference was enough to result in different clinical course if Gleason grade 4/5 disease was present. Therefore, the updated IPC TV threshold should be carefully applied as clinical parameter for active surveillance
Efficacy and patient satisfaction regarding lymphovenous bypass with sleeve-in anastomosis for extremity lymphedema
Background When performing lymphovenous anastomosis, it is sometimes difficult to find venules in the proximity of an ideal lymphatic vessel that have a similar diameter to that of the lymphatic vessel. In this situation, larger venules can be used. Methods The authors evaluated the efficacy of and patient satisfaction with lymphovenous bypass with sleeve-in anastomosis. Between January 2014 and December 2016, we performed this procedure in 18 patients (eight upper extremities and 10 lower extremities) with secondary lymphedema. Lymphovenous bypass with sleeve-in anastomosis was performed under microscopy after injecting indocyanine green dye. The circumferential diameter was measured before lymphovenous bypass and at 1, 2, and 6 months after the procedure. An outcomes survey that included patientsā qualitative satisfaction with lymphovenous bypass was conducted at 6 months postoperatively. Results Almost all patients showed quantitative improvements after surgery. The circumferential reduction rate in patients with stage II lymphedema of both the upper and lower extremities was significantly greater than in their counterparts with stage III/IV lymphedema. The circumferential reduction rate was lower in lower-extremity patients than in upper-extremity patients. Conclusions Lymphovenous bypass surgery with sleeve-in anastomosis in lymphedema patients is beneficial, and appears to be effective, when adequately-sized venules cannot be found in the proximity of an ideal lymphatic vessel
Improved blue light-emitting polymeric device by the tuning of drift mobility and charge balance
We have prepared blue polymer-small molecule hybrid electroluminescence devices with improved
efficiency and lower driving voltage by the statistical design method. Analysis of time-of-flight
measurement shows that amorphous small molecule hole-transporter blended with a blue
light-emitting polymer increases the field-dependent hole mobility, with transition from
nondispersive to dispersive transport induced by the charge-trapping effect. Moreover, at the
electroluminescent devices with different electron injection/transport layer ~LiF/Al, LiF/Ca/Al, and
Alq3 /LiF/Al), efficiency was further increased. We have analyzed that carrier mobility of a
multilayered device can also be controlled by the change of electron injection and transport layers.
We find that structural design and matching overall charge balance is an essential factor to improve
both the operating voltage and efficiency of existing blue polymer devices

As increased agricultural insecticide uses and trends in insecticide resistance, increased labor cost to apply insecticide and limited its application to fertility season in green house. There is a need of a safe, labor-saving and confined space application concept to manage control tiny-scale insect pest such as thrips and whitefly. Fumigation with ethyl formate (EF), which is considered as effective to various insect pest and safely use in quarantine treatment, was evaluated in the confined space (glass house) and semi-confined space (vinyl house). The new application technology for application of liquid EF could be the one of the key options for control of tiny flying insects in greenhouses that would save labor and operation costs. Influencing Factors for Cure of Clonorchiasis by Praziquantel Therapy: Infection Burden and CYP3A5 Gene Polymorphism
Chemotherapy of clonorchiasis with praziquantel (PZQ) is effective but about 15% of treated cases have been reported uncured. The present study investigated correlation of single nucleotide polymorphisms (SNPs) of the cytochrome P450 gene, CYP3A5 and cure of clonorchiasis. A total of 346 egg passing residents were subjected and treated by 3 doses of 25 mg/kg PZQ. Reexamination recognized 33 (9.5%) uncured and 313 cured. Numbers of eggs per gram of feces (EPGs) before treatment were significantly lower in the cured group than in the uncured group (2,011.2Ā±3,600.0 vs 4,998.5Ā±7,012.0, P<0.001). DNAs of the subjects were screened for SNPs at 7 locations of CYP3A5 using PCR. In the uncured group, the SNP frequencies at g.-20555G>A and g.27526C>T of CYP3A5 were 15.2% and 9.1% while those were 3.8% and 1.0%, respectively, in the cured group. The cure rate was significantly lower in the cases with SNP at g.27526C>T and EPGsā„1,000. In conclusion, EPGs and SNPs of CYP3A5 are factors which influence cure of clonorchiasis by PZQ therapy. It is strongly suggested to recommend 2-day medication for individuals with high EPGsā„1,000
