    Complementary and Alternative Medicine Treatment for Urinary Incontinence

    Complementary and alternative medicine has been widely used for various diseases and gained acceptance throughout the industrialized world. Basically, complementary and alternative medicine is grouped into five domains: biologically based therapies, mind-body interventions, manipulative and body-based approaches, energy therapies and whole medical systems. Each domain covers a number of therapies. In this chapter, we present the evidence about effectiveness of each complementary and alternative medicine therapy as well as the possible mechanism on the treatment of urinary incontinence. Besides reviewing existed evidence, our research and clinical experience are also presented

    Gas kinematics and star formation in the filamentary molecular cloud G47.06+0.26

    We performed a multi-wavelength study toward the filamentary cloud G47.06+0.26 to investigate the gas kinematics and star formation. We present the 12CO (J=1-0), 13CO (J=1-0) and C18O (J=1-0) observations of G47.06+0.26 obtained with the Purple Mountain Observation (PMO) 13.7 m radio telescope to investigate the detailed kinematics of the filament. The 12CO (J=1-0) and 13CO (J=1-0) emission of G47.06+0.26 appear to show a filamentary structure. The filament extends about 45 arcmin (58.1 pc) along the east-west direction. The mean width is about 6.8 pc, as traced by the 13CO (J=1-0) emission. G47.06+0.26 has a linear mass density of about 361.5 Msun/pc. The external pressure (due to neighboring bubbles and H II regions) may help preventing the filament from dispersing under the effects of turbulence. From the velocity-field map, we discern a velocity gradient perpendicular to G47.06+0.26. From the Bolocam Galactic Plane Survey (BGPS) catalog, we found nine BGPS sources in G47.06+0.26, that appear to these sources have sufficient mass to form massive stars. We obtained that the clump formation efficiency (CFE) is about 18% in the filament. Four infrared bubbles were found to be located in, and adjacent to, G47.06+0.26. Particularly, infrared bubble N98 shows a cometary structure. CO molecular gas adjacent to N98 also shows a very intense emission. H II regions associated with infrared bubbles can inject the energy to surrounding gas. We calculated the kinetic energy, ionization energy, and thermal energy of two H II regions in G47.06+0.26. From the GLIMPSE I catalog, we selected some Class I sources with an age of about 100000 yr, which are clustered along the filament.     High Dose ESAs Are Associated with High iPTH Levels in Hemodialysis Patients with End-Stage Kidney Disease: A Retrospective Analysis

    Objective: Anemia and secondary hyperparathyroidism are the two most common complications associated with chronic kidney disease (CKD). Erythropoiesis-stimulating agents (ESAs) are widely used in the management of anemia in hemodialysis patients. A reverse correlation has been established between hyperparathyroidism and hemoglobin levels. The aim of this retrospective study is to evaluate the relationship of high dose ESAs and hyperparathyroidism in hemodialysis patients with anemia. Methods: A total of 240 uremic patients maintained on regular hemodialysis were enrolled into this study. Among them, 142 patients were treated with Epiao® (epoetin-alfa) and 98 patients were treated with Recormon® (epoetin-beta). The target hemoglobin concentration was 110-130 g/L. Laboratory measurements including hemoglobin, calcium, phosphorus, albumin, intact-parathyroid hormone (iPTH), serum ferritin and transferrin saturation were collected. Results: Hemoglobin concentration increased as iPTH level decreased by stratification. However, no significant association between anemia and calcium or phosphorus level was found. Patients with iPTH levels within 150-300 pg/mL had the highest levels of hemoglobin, serum ferritin and transferrin saturation. Patients treated with Recormon and Epiao had similar hemoglobin concentrations. However, the dose of Recormon for anemia treatment was significantly less than that the dose of Epiao (P<0.05). The level of iPTH in the Recormon group was significantly lower than in the Epiao group. In patients with hemoglobin levels between 110-130 g/L (P<0.05), iPTH level was found to be significantly lower in patients treated with lower doses of ESAs than in patients treated with higher doses of ESAs, no matter which ESA was used (Recormon or Epiao, P<0.05). Conclusions: The dose of ESAs might be positively associated with iPTH level, suggesting that a reasonable hemoglobin target can be achieved by using the lowest possible ESA dose

    Interaction between Granulation and Small-Scale Magnetic Flux Observed by Hinode

    We study the relationship between granular development and magnetic field evolution in the quiet Sun. 6 typical cases are displayed to exhibit interaction between granules and magnetic elements, and we have obtained the following results. (1) A granule develops centrosymmetrically when no magnetic flux emerges within the granular cell. (2) A granule develops and splits noncentrosymmetrically while flux emerges at an outer part of the granular cell. (3) Magnetic flux emergence as a cluster of mixed polarities is detected at the position of a granule as soon as the granule breaks up. (4) A dipole emerges accompanying with the development of a granule, and the two elements of the dipole root in the adjacent intergranular lanes and face each other across the granule. Advected by the horizontal granular motion, the positive element of the dipole then cancels with pre-existing negative flux. (5) Flux cancellation also takes place between a positive element, which is advected by granular flow, and its surrounding negative flux. (6) While magnetic flux cancellation takes place at a granular cell, the granule shrinks and then disappears. (7) Horizontal magnetic fields enhance at the places where dipoles emerge and where opposite polarities cancel with each other, but only the horizontal fields between the dipolar elements point orderly from the positive element to the negative one. Our results reveal that granules and small-scale magnetic flux influence each other. Granular flow advects magnetic flux, and magnetic flux evolution suppresses granular development. There exist extremely large Doppler blue-shifts at the site of one cancelling magnetic element.     Modification of atrioventricular node in a special condition treating paroxysmal supraventricular tachycardia

    Modification of atrioventricular node is a usual and necessary operation to cure atrioventricular nodal reentrant tachycardia (AVNRT). In this operation, atrioventricular block is the most severe complication and its prevention is of our great concern. This complication always occurs under some special circumstances with potential risk. So, it is very important to realize such conditions, as in this paper. A patient with paroxysmal palpitation for 10 years, aggravating to shortness of breath with chest distress for 1 year; cardiac electrophysiological examination found slow conduction in both antegrade and retrograde paths of reentrant loop, and typical AVNRT could be induced. During effective ablation there was no junctional rhythm. In some special cases, modification of atrioventricular node should not only rely on the junctional rhythm to determine the ablation effect, but also on the time of cardiac electrophysiological examination, as such to avoid the severe complication of atrioventricular block caused by excessive ablation

    Characterization of a New M13 Metallopeptidase from Deep-Sea Shewanella sp. E525-6 and Mechanistic Insight into Its Catalysis

    Bacterial extracellular peptidases are important for bacterial nutrition and organic nitrogen degradation in the ocean. While many peptidases of the M13 family from terrestrial animals and bacteria are studied, there has been no report on M13 peptidases from marine bacteria. Here, we characterized an M13 peptidase, PepS, from the deep-sea sedimentary strain Shewanella sp. E525-6, and investigated its substrate specificity and catalytic mechanism. The gene pepS cloned from strain E525-6 contains 2085 bp and encodes an M13 metallopeptidase. PepS was expressed in Escherichia coli and purified. Among the characterized M13 peptidases, PepS shares the highest sequence identity (47%) with Zmp1 from Mycobacterium tuberculosis, indicating that PepS is a new member of the M13 family. PepS had the highest activity at 30°C and pH 8.0. It retained 15% activity at 0°C. Its half life at 40°C was only 4 min. These properties indicate that PepS is a cold-adapted enzyme. The smallest substrate for PepS is pentapeptide, and it is probably unable to cleave peptides of more than 30 residues. PepS prefers to hydrolyze peptide bonds with P1’ hydrophobic residues. Structural and mutational analyses suggested that His531, His535 and Glu592 coordinate the catalytic zinc ion in PepS, Glu532 acts as a nucleophile, and His654 is probably involved in the transition state stabilization. Asp538 and Asp596 can stablize the orientations of His531 and His535, and Arg660 can stablize the orientation of Asp596. These results help in understanding marine bacterial peptidases and organic nitrogen degradation

    Magnetic Feshbach resonances in collisions of 23Na40K with 40K

    We present measurements of more than 80 magnetic Feshbach resonances in collisions of ultracold 23Na40K with 40K. We assign quantum numbers to a group of low-field resonances and show that they are probably due to long-range states of the triatomic complex in which the quantum numbers of the separated atom and molecule are approximately preserved. The resonant states are not members of chaotic bath of short-range states. Similar resonances are expected to be a common feature of alkali-metal diatom + atom systems
