    Phase transformation behaviour of porous NiTi alloys fabricated by capsule-free hot isostatic pressing

    Differential scanning calorimetry (DSC) was used to characterize the phase transformation behaviour of porous Ni50Ti50 alloys fabricated by capsule-free hot isostatic pressing (CF-HIP) with different cold compaction pressures. Experimental results reveal that a multi-stage martensitic transformation (MST) exists in the sintered porous NiTi alloys on cooling while the reverse transformation upon heating is either a single or two-stage phase transformation. The DSC thermal analysis indicates that the cold compaction pressure has great effect on the subsequent transformation temperatures. Generally, the phase transformation temperatures of porous NiTi alloys with lower cold compaction pressure are higher than those compacted with higher pressure. With increase in the annealing time, the transformation temperatures increase quickly when the cold compaction pressure was 150MPa. On the other hand, the transformation temperatures change only slightly when the cold compaction pressure was varied from 300MPa to 400MPa. These phenomena can be attributed to the combined effect of larger plastic deformation with higher dislocations density produced by cold compaction and the precipitation of the second phase in the porous NiTi alloys.published_or_final_versio

    Optimization of nutritional requirements and ammonium feeding strategies for improving vitamin B12 production by Pseudomonas denitrificans

    Statistical experiment design and data analysis were used to establish the major factors in a chemically defined medium and to develop an ammonium control strategy to optimize the specific vitamin B12 production rate (Yp) of Pseudomonas denitrificans. Through Plackett-Burman design, the major factors of glucose, ammonium sulfate and KCl were selected as the significant factors affecting vitamin B12 biosynthesis and these were further optimized by central composite design with response surface methodology. The maximum Yp of 34.2 μg/gDCW/h was obtained in batch cultivation under the estimated optimal initial composition of glucose (93.6 g/l), (NH4)2SO4 (7.93 g/l) and KCl (1.24 g/l). Ammonium control strategies in fed-batch fermentation showed that when ammonium concentration was maintained at 40 mmol/l, the maximum Yp reached 36.0 ± 1.31 μg/gDCW/h, which was 57.2% higher than that of the control (22.9 ± 0.83 μg/gDCW/h). This ammonium control strategy successfully enhanced the industrial production, resulting in a stable high vitamin B12 production of 212.02 ± 3.03 mg/l and Yp of 37.1 μg/gDCW/h.Key words: Statistical designs, Pseudomonas denitrificans, chemically defined medium, ammonium controlling strategy, vitamin B12

    Biomimetic deposition of apatite on surface chemically modified porous NiTi shapememory alloy

    Porous NiTi shape memory alloy (SMA) with 48% porosity and an average pore size of 50800 μm was synthesized by capsule-free hot isostatic pressing (CF-HIP). To enhance the surface bioactivity, the porous NiTi SMA was subjected to H"2O"2 and subsequent NaOH treatment. Scanning electron microscopy, X-ray diffraction, and X-ray photoelectron spectroscopy analyses revealed that a porous sodium titanate (Na"2TiO"3) film had formed on the surface of the porous NiTi SMA. An apatite layer was deposited on this film after immersion in simulated body fluid at 37°C, while no apatite could be found on the surface of the untreated porous NiTi SMA. The formation of the apatite layer infers that the bioactivity of the porous NiTi SMA may be enhanced by surface chemical treatment, which is favorable for its application as bone implants. © 2008 World Scientific Publishing Company.postprin

    Herpetic shoulder paresis in a Chinese elderly patient

    A patient with left shoulder girdle weakness secondary to herpetic myotomal paresis is reported. Needle electromyography revealed denervational discharge from the left supraspinatus, deltoid, and brachioradialis muscles, compatible with a radiculopathy that was relevant to his myotomes affected by zoster infection. The patient was managed with range-of-movement and strengthening exercises as well as pain relief for post-herpetic neuralgia. Further studies are required to determine whether antiviral treatment can limit the extent of motor deficit and hasten recovery. Zoster paresis should be one of the differential diagnoses of girdle muscle weakness.published_or_final_versio

    Risk factors, clinical features and prognosis of perioperative stroke in adults

    INTRODUCTION: Perioperative stroke (POS) is an uncommon but severe surgical complication. No widely accepted guidelines for risk prediction or management have been established. Its prevention depends on knowledge about the nature of this disease. METHODS: A total of 36 cases and equal number of controls in Hong Kong West Cluster hospitals were recruited over 43 months. Peri- and intra-operative features were compared and characteristics of POS were described. RESULTS: Atrial fibrillation, diabetes mellitus (DM), and history of stroke were identified as risk factors (P=0.017, 0.002, and 0.003, respectively). Prolonged aortic occlusion (P=0.018) and bypass (P=0.002) contributed in cardiac surgery. Only few BP parameters, but not consistently all, were significant; 78% POS were infarcts. Watershed infarction related to hypotension was uncommon. Beta-blocker use seemed to bare protective effect. Blood loss and haemoglobin levels did not correlate to POS. Three-month mortality rate was 36%. CONCLUSION: Optimal DM control and cardioversion before elective OT, perioperative anticoagulation in AF and old stroke patients, and beta-blockers may be preventive measures for POS.     Suppression of nickel out-diffusion from porous nickel-titanium shape memory alloy by plasma immersion ion implantation

Porous nickel titanium is a promising material for medical application not only because of its super elasticity and shape memory effect but also the porous structure which may enhance bone growth due to the increased surface area. It is thus especially suitable for bone tissue in-growth and fixation of biomedical implants. However, like its dense counterpart, Ni leaching from the materials causes health concern. Thus, in order to suppress Ni diffusion from the materials to body fluids and tissues in humans, a diffusion barrier or similar structure must be introduced. In this work, we produced this diffusion barrier layer by oxygen or nitrogen plasma immersion ion implantation (PIII). In vitro tests were conducted by immersing the plasma-treated NiTi into simulated body fluid (SBF) at 37plusmn0.5degC for 5 weeks and the resulting SBF was analyzed for Ni and Ti using inductively-coupled plasma mass spectrometry (ICMPS).     Clinical and Pathological Findings in Women with Fabry Disease

    Introduction. Fabry disease is a rare metabolic disorder caused by the genetic deficiency of the lysosomal hydrolase alpha-galactosidase A, located on chromosome X. Females with the defective gene are more than carriers and can develop a wide range of symptoms. Nevertheless, disease symptoms generally occur later and are less severe in women than in men. The enzyme deficiency manifests as a glycosphingolipidosis with progressive accumulation of glycosphingolipids and deposit of inclusion bodies in lysosomes giving a myelinlike appearance. Patients and Methods. Records of renal biopsies performed on adults from 1st January 2008 to 31st August 2011, were retrospectively examined at the Renal Pathology Laboratory. We retrieved biopsies diagnosed with Fabry disease and reviewed clinical and laboratory data and pathology findings. Results. Four female patients with a mean age of 49.3±4.5 (44-55) years were identified. The mean proteinuria was 0.75±0.3 g/24h (0.4-1.2) and estimated glomerular filtration rate (CKD EPI equation) was 71±15.7 ml/min/1.73m2 (48-83). Three patients experienced extra-renal organ involvement (cerebrovascular, cardiac, dermatologic, ophthalmologic and thyroid) with distinct severity degrees. Leukocyte α-GAL A activity was below normal range in the four cases but plasma and urinary enzymatic activity was normal. Light microscopy showed predominant vacuolisation of the podocyte cytoplasm and darkly staining granular inclusions on paraffin and plastic-embedded semi-thin sections. Electron microscopy showed in three patients the characteristic myelin-like inclusions in the podocyte cytoplasm and also focal podocyte foot process effacement. In one case the inclusions were also present in parietal glomerular cells, endothelial cells of peritubular capillary and arterioles. Conclusion. Clinical signs and symptoms are varied and can be severe among heterozygous females with Fabry disease. Intracellular accumulation of glycosphingolipids is a characteristic histologic finding of Fabry nephropathy. Since this disease is a potentially treatable condition, its early identification is imperative. We should consider it in the differential diagnosis of any patient presenting with proteinuria and/or chronic kidney disease, especially if there is a family history of kidney disease
