    Influence of thoracic epidural analgesia on postoperative pain relief and ileus after laparoscopic colorectal resection: Benefit with epidural analgesia

    Background: Thoracic epidural analgesia (TEA) provides superior analgesia with a lower incidence of postoperative ileus when compared with systemic opiate analgesia in open colorectal surgery. However, in laparoscopic colorectal surgery the role of TEA is not well defined. This prospective observational study investigates the influence of TEA in laparoscopic colorectal resections. Methods: All patients undergoing colorectal resection between November 2004 and February 2007 were assessed for inclusion into a prospective randomized trial investigating the influence of bisacodyl on postoperative ileus. All patients treated by laparoscopic resection from this collective were eligible for the present study. Primary endpoints were use of analgesics and visual analogue scale (VAS) pain scores. Secondary endpoint concerned full gastrointestinal recovery, defined as the mean time to the occurrence of the following three events (GI-3): first flatus passed, first defecation, and first solid food tolerated. Results: 75 patients underwent laparoscopic colorectal resection, 39 in the TEA group and 36 in the non-TEA group. Patients with TEA required significantly less analgesics (metamizol median 3.0g [0-32g] versus 13.8g [0-28g] (p<0.001); opioids mean 12mg [±2.8mg standard error of mean, SEM] versus 103mg [±18.2mg SEM] (p<0.001). VAS scores were significantly lower in the TEA group (overall mean 1.67 [± 0.2 SEM] versus 2.58 [±0.2 SEM]; p=0.004). Mean time to gastrointestinal recovery (GI-3) was significantly shorter (2.96 [±0.2 SEM] days versus 3.81 [±0.3 SEM] days; p=0.025). Analysis of the subgroup of patients with laparoscopically completed resections showed corresponding results. Conclusion: TEA provides a significant benefit in terms of less analgesic consumption, better postoperative pain relief, and faster recovery of gastrointestinal function in patients undergoing laparoscopic colorectal resectio

    SBFT Tool Competition 2024 -- Python Test Case Generation Track

    Test case generation (TCG) for Python poses distinctive challenges due to the language's dynamic nature and the absence of strict type information. Previous research has successfully explored automated unit TCG for Python, with solutions outperforming random test generation methods. Nevertheless, fundamental issues persist, hindering the practical adoption of existing test case generators. To address these challenges, we report on the organization, challenges, and results of the first edition of the Python Testing Competition. Four tools, namely UTBotPython, Klara, Hypothesis Ghostwriter, and Pynguin were executed on a benchmark set consisting of 35 Python source files sampled from 7 open-source Python projects for a time budget of 400 seconds. We considered one configuration of each tool for each test subject and evaluated the tools' effectiveness in terms of code and mutation coverage.     Vitamin D supplementation alleviates insulin resistance in prediabetic rats by modifying IRS-1 and PPARγ/NF-κB expressions

    BackgroundPrediabetes is a condition of intermediate hyperglycemia that may progress to type 2 diabetes. Vitamin D deficiency has been frequently linked to insulin resistance and diabetes. The study aimed to investigate the role of D supplementation and its possible mechanism of action on insulin resistance in prediabetic rats.MethodThe study was conducted on 24 male Wistar rats that were randomly divided into 6 rats as healthy controls and 18 prediabetic rats. Prediabetic rats were induced with a high-fat and high-glucose diet (HFD-G) combined with a low dose of streptozotocin. Rats with the prediabetic condition were then randomized into three groups of 12-week treatment: one group that received no treatment, one that received vitamin D3 at 100 IU/kg BW, and one group that received vitamin D3 at 1000 IU/kg BW. The high-fat and high-glucose diets were continuously given throughout the twelve weeks of treatment. At the end of the supplementation period, glucose control parameters, inflammatory markers, and the expressions of IRS1, PPARγ, NF-κB, and IRS1 were measured.ResultsVitamin D3 dose-dependently improves glucose control parameters, as shown by the reduction of fasting blood glucose (FBG), oral glucose tolerance test (OGTT), glycated albumin, insulin levels, and markers of insulin resistance (HOMA-IR). Upon histological analysis, vitamin D supplementation resulted in a reduction of the islet of Langerhans degeneration. Vitamin D also enhanced the ratio of IL-6/IL-10, reduced IRS1 phosphorylation at Ser307, increased expression of PPAR gamma, and reduced phosphorylation of NF-KB p65 at Ser536.ConclusionVitamin D supplementation reduces insulin resistance in prediabetic rats. The reduction might be due to the effects of vitamin D on IRS, PPARγ, and NF-κB expression

    Hypnozoite depletion in successive Plasmodium vivax relapses

    Genotyping Plasmodium vivax relapses can provide insights into hypnozoite biology. We performed targeted amplicon sequencing of 127 relapses occurring in Indonesian soldiers returning to malaria-free Java after yearlong deployment in malarious Eastern Indonesia. Hepatic carriage of multiple hypnozoite clones was evident in three-quarters of soldiers with two successive relapses, yet the majority of relapse episodes only displayed one clonal population. The number of clones detected in relapse episodes decreased over time and through successive relapses, especially in individuals who received hypnozoiticidal therapy. Interrogating the multiplicity of infection in this P. vivax relapse cohort reveals evidence of independent activation and slow depletion of hypnozoites over many months by multiple possible mechanisms, including parasite senescence and host immunity

    Kontroler on-off tiga posisi memiliki perbedaan mendasar jika dibandingkan dengan kontroler on-off dua posisi, yaitu pada kontroler on-off tiga posisi memiliki tiga kondisi, berbeda dengan dua posisi yang hanya memiliki dua kondisi. Dua kontroler ini diuji pada Temperature Controlled System 734 12 dan dibandingkan dengan kontroler on-off dua posisi. Penelitian ini menggunakan tiga nilai setpoint yang sama tiap kontroler, serta dengan gangguan yang sama. Kontroler on-off tiga posisi menghasilkan kinerja yang lebih baik dalam daya kontroler untuk mencapai setpoint yang tinggi, respons transien yang lebih cepat, serta masa lifetime kontroler yang lebih lama, dibanding dengan kontroler on-off dua posisi. Pada penelitian ini juga membuktikan adanya kondisi mengapung yang terjadi pada kontroler on-off tiga posisi Kata Kunci: Kontroler on-off tiga posisi, kontroler on-off dua posisi, Temperature Controlled System 734 12, kondisi mengapung.   ABSTRACT Three-position on-off controller have a basic difference when compared with two position on-off controller, i.e. three-position on-off controller has three condition, different with two-position which is only has two condition. These two controllers are tested on plant Temperature Controlled System 734 12 and compared with two-position on-off controller. The experiment uses three value of setpoint which is similar for each other controllers, also with the same disturbance.  Three-position on-off controller produces better performance in given power for high value setpoint, faster transient response, and longer controller lifetime, compared to two-position on-off controller. This experiment also proved the existence of the floating condition that occur in the three-position on-off controller.
