Diagnostyczne i lecznicze zastosowanie cewnikowania serca z dostępu przez nakłucie tętnicy promieniowej - doświadczenia własne
Wstęp: Cewnikowanie serca z dostępu przez nakłucie tętnicy promieniowej w 1989 r. wprowadził
Campeau. Metoda ta zyskuje coraz więcej zwolenników, ponieważ pozwala zmniejszyć
liczbę powikłań miejscowych i zwiększyć komfort chorych. Celem pracy jest ocena skuteczności
i bezpieczeństwa wykonywania koronarografii i przezskórnych interwencji wieńcowych (PCI)
przez osobę przeprowadzającą zabieg w fazie uczenia.
Materiał i metody: W okresie od września 2004 r. do sierpnia 2005 r. jeden specjalista
podjął się wykonania zabiegu cewnikowania serca przez nakłucie tętnicy promieniowej
u 200 chorych. U wszystkich intencją leczenia był zabieg z zastosowaniem tej metody. Przed
zabiegiem koronarografii i/lub PCI przeprowadzano test z wykorzystaniem pulsoksymetru.
Wyniki: Spośród 200 chorych zabieg z nakłucia tętnicy promieniowej wykonano u 182 (91%).
U 5 (2,5%) badanych zabiegu nie wykonano z powodu nieprawidłowego wyniku testu,
u 13 (6,6%) nie powiodło się nakłucie tętnicy. U 4 (2,2%) chorych konieczne było dokończenie
badania z nakłucia tętnicy udowej. U 182 osób wykonano: 49 (26,9%) zabiegów koronarografii,
24 (13,2%) koronarografie z innymi badaniami naczyniowymi, 95 (52,2%) koronarografii,
które zakończono zabiegiem PCI i 14 (7,7%) planowych zabiegów PCI. U 80 (72,7%)
badanych przeprowadzono PCI jednego naczynia, u 28 (15,4%) chorych - dwóch i u 1 (0,9%)
osoby - trzech naczyń. U 41 (20,5%) chorych wskazaniem do cewnikowania serca był ostry zespół wieńcowy z uniesieniem odcinka ST, u 31 (15,5%) badanych - bez uniesienia odcinka
ST, a u 110 (55%) - stabilna choroba wieńcowa. Nie odnotowano powikłań naczyniowych
wymagających leczenia zabiegowego i/lub przetoczenia krwi.
Wnioski: Cewnikowanie serca z dostępu przez nakłucie tętnicy promieniowej można zastosować
u większości chorych zarówno ze stabilną postacią choroby wieńcowej, jak i z ostrymi zespołami
wieńcowymi. Jest to zabieg charakteryzujący się brakiem poważnych powikłań miejscowych.
Wysoką skuteczność tego zabiegu można osiągnąć, jeśli przeprowadza go specjalista po
dość krótkim okresie szkolenia

Assessment of cardiovascular function following transcatheter aortic valve implantation based on six-minute walk test
Background: Transcatheter aortic valve implantation (TAVI) is presently a recognized treatment modality for patients with severe aortic stenosis ineligible for surgery. It reduces mortality as compared to the conservative treatment. It is further expected from this therapy to improve quality of life by improving of the cardiovascular function performance. The aim of this study is to compare patients’ cardiovascular system efficiency in the 6-minute walk test (6MWT) made before and after TAVI and at the 6–12-month follow-up.
Methods: From January 2009 until February 2012, in the Silesian Center for Heart Diseases in Zabrze, TAVI was performed in 104 patients. Eighty-two patients who underwent 6MWT before surgery were qualified for the analysis. The average age of the patients was 76.0 ± 9.17 years, women made 45.1%. The risk of surgical treatment according to the Logistic Euroscore averaged 22.76 ± 12.63%, and by the Society of Thoracic Surgeons — 5.55 ± 3.34%. The 6MWT was performed within 1 month before the TAVI procedure, up to a month after the procedure and during the 6–12-month follow-up.
Results: The 6-minute walk test after TAVI was performed by 64 patients, and after 6–12 month follow-up by 46 patients. The average distance in 6MWT increased from 268.4 ± 89.0 m before treatment to 290.0 ± 98.2 m after the procedure (p = 0.008) and 276.1 ± 93.5 m to 343.1 ± 96.7 m after 6–12 months (p < 0.0001).
Conclusions: Transcatheter aortic valve implantation procedures significantly improve function of the cardiovascular system evaluated by the 6MWT in 1- and 6–12-month observations. (Cardiol J 2017; 24, 2: 167–175
Primary versus facilitated percutaneous coronary intervention in acute myocardial infarction complicated by cardiogenic shock
Background: Mortality in patients with cardiogenic shock (CS) due to acute myocardial
infarction (MI) may be decreased by fibrynolytic therapy combined with intraaortic balloon
counterpulsation or by invasive treatment, either with percutaneous coronary intervention (PCI)
or coronary artery bypass grafting (CABG). The aim of the study was to compare in-hospital
and long-term outcomes in patients with acute MI complicated by CS who were treated with
primary or facilitated PCI.
Methods: Among 98 consecutive patients with acute MI complicated by CS, 93 patients were
treated with PCI and 5 patients underwent CABG. Patients treated with PCI were divided
into two groups: group I included 59 patients treated with facilitated PCI and group II
included 34 patients treated with primary PCI. Patients in group II were older, had higher
systolic and diastolic blood pressure, and more often presented with 1-vessel disease and
previous MI, while 3-vessel disease was more common in group I (all p < 0.05).
Results: Immediate PCI success rate was similar in both groups (83% in group I vs. 74% in
group II, p = NS), as was in-hospital mortality (41% vs. 36%, respectively, p = NS) and
mortality rate in the cardiac cathetherization laboratory (20% vs. 15%, respectively, p = NS).
The need for repeated PCI was significantly more common in group I (22% vs. 3%, p = 0.02).
The two groups did not differ with respect to the need for CABG or the rate of hemorrhagic
complications. During one year follow-up, three deaths occurred in every group, including two
patients in each group who died suddenly.Conclusions: Comparable immediate PCI success rate, in-hospital mortality, and long-term
mortality were seen in patients with acute MI complicated by CS treated with primary or
facilitated PCI. More coronary reinterventions were needed in patients treated with facilitated
PCI compared to those treated with primary PCI
A comprehensive study on bisphenol A degradation by newly isolated strains Acinetobacter sp. K1MN and Pseudomonas sp. BG12
Bisphenol A (BPA) is an endocrine disrupting
chemical. Its extensive use has led to the wide
occurrence of BPA in various environmental ecosystems,
at levels that may cause negative effects to the
ecosystem and public health. Although there are many
bacteria able to BPA utilization, only a few of them
have a strong capacity for its biodegradation. Therefore,
it is important to search for new bacteria strains,
investigate their BPA biodegradation ability and
potential effect of pH and other organic compounds
on the process. These tasks have become the object of
the present study. The results of our research show that
for the newly isolated strains Acinetobacter sp. K1MN
and Pseudomonas sp. BG12 after 15 days, with an
initial BPA concentration of 100 mg L- 1, the highest
BPA removal was achieved at pH 8, while sodium
glutamate as a biostimulant best accelerated BPA
degradation. Kinetic data for BPA biodegradation by
both strains best fitted the Monod model. The specific
degradation rate and the half saturation constant were
estimated respectively as 8.75 mg L- 1 day- 1 and
111.27 mg L- 1 for Acinetobacter sp. K1MN, and
8.6 mg L- 1 day- 1 and 135.79 mg L- 1 for Pseudomonas
sp. BG12. The half-maximal effective concentration
(EC50) of BPA for Acinetobacter sp. K1MN
was 120 mg L- 1 and for Pseudomonas sp. BG12 it
was 123 mg L- 1. The toxicity bioassay (Microtox
test) showed that elimination of BPA by both strains is
accompanied by reduction of its toxic effect. The
ability of tested strains to degrade BPA combined with
their high resistance to this xenobiotic indicates that
Acinetobacter sp. K1MN and Pseudomonas sp. BG12
are potential tools for BPA removal during wastewater
treatment plant
The incidence and risk factors of stroke in patients with acute myocardial infarction treated invasively and concomitant impaired renal function
Background: Impaired renal function is a marker of poor prognosis in patients with acutemyocardial infarction (AMI). The aim of the study was to assess the incidence and independent predictors of stroke in population of patients with AMI treated invasively and concomitant impaired renal function (IRF).Methods: We analyzed 2,520 consecutive AMI patients admitted to our Center between 2003 and 2007 and treated with percutaneous coronary intervention. The whole population was divided into patients with IRF, defined as glomerular filtration rate < 60 mL/min/1.73 m2 or contrast induced nephropathy (IRF group, n = 933; 37.02%) and patients without IRF (control group, n = 1587; 62.98%). The IRF group was subjected to further analysis. Data onlong-term follow-up were screened to identify the patients who experienced stroke.Results: During median of 25.5 months of follow-up 52 (2.07%) the patients experienced stroke — 33 (3.54%) in the IRF group and 19 (1.2%) patients in the control group. The risk of major adverse cardiovascular events in the IRF group, including repeated AMI (68.8 vs.14.9%, p < 0.001) and death (45.5 vs. 25.1%, p < 0.05) was significantly higher in patients with stroke. Previous stroke (HR 6.85), female gender (HR 3.13), as well as STEMI anterior (HR 2.55) were independent risk factors of stroke in this population.Conclusions: Patients with AMI treated invasively and concomitant IRF were at higher risk of stroke occurrence in the future. Stroke was associated with poor outcome in the studied population. RADIal versus femoral approach for percutaneous coronary interventions in patients with Acute Myocardial Infarction (RADIAMI): A prospective, randomized, single-center clinical trial
Background: The transradial approach for percutaneous coronary intervention (PCI) seems
to be superior to transfemoral. The safety and efficacy of transradial approach for PCI in acute
myocardial infarction is not well-established.
Methods: Hundred patients with acute myocardial infarction qualified to PCI were randomly
assigned to transradial (group I; n = 50) and transfemoral (group II; n = 50) approaches.
Results: PCI was successful for almost all patients, except one from group II. There were no
significant differences between groups in X-ray exposition, volume of contrast and total procedure
duration. Small but significant elongation of door to stent time in group I was caused
mostly by a longer time between beginning of procedure and arterial sheath introduction.
Major bleeding complications occurred in three patients from group I and seven from group II.
There were no significant differences observed between the two groups. Time to ambulation in
group I was significantly shorter then in group II (22.6 ± 10.3 h vs. 34.7 ± 34.6 h; p = 0.003).
Conclusions: The transradial approach for PCI in acute myocardial infarction has the same
efficacy as transfemoral. There are no differences in total procedure duration, X-ray exposition
or volume of contrast between the two approaches. A longer time from the patient’s admission
to the individual stages of the PCI procedure in group I was mostly due to the longer times of
the initial stages of the procedure. The use of transradial approach reduces the time to ambulation
and allows rehabilitation to begin sooner. In both groups, bleeding complications occurred
rarely
Zmienność rytmu zatokowego w rocznej obserwacji u chorych z zawałem serca powikłanym wstrząsem kardiogennym leczonych inwazyjnie
Wstęp: Obniżona zmienność rytmu zatokowego (HRV) jest czynnikiem ryzyka
nagłego zgonu sercowego po przebytym zawale serca (MI). Chorzy z MI powikłanym
wstrząsem kardiogennym (CS) są grupą o szczególnie wysokim ryzyku, które ulega
zmniejszeniu pod wpływem leczenia poprzez rewaskularyzację.
Cel pracy: Ocena HRV u chorych leczonych poprzez rewaskularyzację z powodu
ostrego MI powikłanego CS, na podstawie parametrów oznaczonych pod koniec hospitalizacji,
i HRV u osób zdrowych, a także porównanie parametrów HRV uzyskanych pod koniec
hospitalizacji i po roku w grupie chorych, którzy przeżyli.
Materiał i metody: Badaniem objęto 28 chorych (10 kobiet, 18 mężczyzn,
śr. wiek 53,3 ± 9,8 lat — grupa badana) z ostrym MI powikłanym CS, z których 27
przeżyło rok obserwacji (grupa MICS+), a 1 osoba zmarła w 3. miesiącu obserwacji,
oraz 25 osób zdrowych. U wszystkich chorych natychmiast po przyjęciu do szpitala
przeprowadzono skuteczną rewaskularyzację (u 26 PTCA i u 2 CABG). Oceniono parametry
analizy czasowej HRV: u wszystkich chorych średnio w 16. dobie MI, w grupie MICS+
po roku oraz u osób zdrowych.
Wyniki: Wszystkie parametry HRV w 16. dobie MI były istotnie niższe niż
u osób zdrowych (p < 0,001). W grupie MICS+ istotnie wzrosły po roku (SDNN:
70,9 ± 23,6 vs. 119,3 ± 33,6 ms; p < 0,001), a parametry rMSSD i pNN50 nie
różniły się znamiennie od ich wartości u osób zdrowych. Istotnym zmianom w tym
okresie nie uległa wielkość frakcji wyrzutowej — 42,9 ± 7,2 vs. 41,8 ± 8,6%; p
= 0,6.
Wnioski: U badanych chorych z MI powikłanym CS obniżone parametry HRV pod
koniec hospitalizacji istotnie wzrosły po roku, co można wiązać z przeprowadzoną
natychmiastową skuteczną rewaskularyzacją w okresie wewnątrzszpitalnym
A comparison of the clinical course of preexcitation syndrome in children and adolescents and in adults
Background: Atrioventricular reentrant tachycardia (AVRT) in patients with preexcitation
syndrome (PS) is the main cause of paroxysmal regular arrhythmias in children and adolescents.
While the previously published data most commonly concern clinical consequences of PS
in adults, few researchers have evaluated the problem in children and adolescents. The aim of
the study was to compare the clinical course of PS between the population of children and
adolescents below 19 years of age and the population of adult patients.
Method: The study population consisted of 302 consecutive PS patients managed between January
2001 and June 2005 with radiofrequency catheter ablation (RFCA). The study population was
divided into two groups: Group 1 consisting of 52 patients aged 15.38 years on average (7-18 ± 2.53) and Group 2 consisting of 250 adult patients aged 38.67 years on average (19-72 ± 13.1).
Results: Patients from Groups 1 and 2 experienced their first episode of AVRT at the mean
age of 13.3 years and 29.1 years, respectively (p < 0.05). The mean annual numbers of AVRT
episodes in Groups 1 and 2 were 12.97 (range, 2-96; median, 8) and 8.86 (range, 2-25;
median, 6), respectively (p = non-significant). Two patients from Group 1 (3.85%) and
42 patients from Group 2 (16.8%) experienced episodes of atrial fibrillation (AF) (p < 0.05).
Location of the accessory pathways (AP): In Group 1, the right free wall and anteroseptal AP
locations were significantly more common [11 (21.15%) and 9 (17.31%) patients, respectively,
vs.q 19 (7.6%) and 13 (5.2%) patients in Group 2; p < 0.01]. In Group 2, the left anterolateral
AP location was more common [81 (32.4%) vs. 4 (7.69%) in Group 1; p < 0.01].
Conclusions: In children and adolescents with PS, a significantly lower incidence of AF was
found. In Group 1, RFCA was performed significantly more frequently due to the development
of AVRT caused by right free wall and right anteroseptal AP, while in the group of adults, the
left anterolateral AP location was found more commonly. (Cardiol J 2007; 14: 384-390

Wstęp: Obniżona zmienność rytmu zatokowego (HRV) jest czynnikiem ryzyka
nagłego zgonu sercowego po przebytym zawale serca (MI). Chorzy z MI powikłanym
wstrząsem kardiogennym (CS) są grupą o szczególnie wysokim ryzyku, które ulega
zmniejszeniu pod wpływem leczenia poprzez rewaskularyzację.
Cel pracy: Ocena HRV u chorych leczonych poprzez rewaskularyzację z powodu
ostrego MI powikłanego CS, na podstawie parametrów oznaczonych pod koniec hospitalizacji,
i HRV u osób zdrowych, a także porównanie parametrów HRV uzyskanych pod koniec
hospitalizacji i po roku w grupie chorych, którzy przeżyli.
