Endovascular Detection of Catheter-Thrombus Contact by Vacuum Excitation
Objective: The objective of this work is to introduce and demonstrate the
effectiveness of a novel sensing modality for contact detection between an
off-the-shelf aspiration catheter and a thrombus. Methods: A custom robotic
actuator with a pressure sensor was used to generate an oscillatory vacuum
excitation and sense the pressure inside the extracorporeal portion of the
catheter. Vacuum pressure profiles and robotic motion data were used to train a
support vector machine (SVM) classification model to detect contact between the
aspiration catheter tip and a mock thrombus. Validation consisted of benchtop
accuracy verification, as well as user study comparison to the current standard
of angiographic presentation. Results: Benchtop accuracy of the sensing
modality was shown to be 99.67%. The user study demonstrated statistically
significant improvement in identifying catheter-thrombus contact compared to
the current standard. The odds ratio of successful detection of clot contact
was 2.86 (p=0.03) when using the proposed sensory method compared to without
it. Conclusion: The results of this work indicate that the proposed sensing
modality can offer intraoperative feedback to interventionalists that can
improve their ability to detect contact between the distal tip of a catheter
and a thrombus. Significance: By offering a relatively low-cost technology that
affords off-the-shelf aspiration catheters as clot-detecting sensors,
interventionalists can improve the first-pass effect of the mechanical
thrombectomy procedure while reducing procedural times and mental burden
Deciphering plant richness using satellite remote sensing: a study from three biodiversity hotspots
The ‘spectral variation hypothesis (SVH)’ assumes spectral variability as a result of variation in species richness. In the present study, we explore the potential of satellite datasets in identifying the patterns in species richness in part of three global biodiversity hotspots falling in India viz., Himalaya, Indo-Burma, and Western Ghats. We used generalized linear models to correlate remote sensing based vegetation indices (VIs) and physiographic indices (PIs) with plant richness calculated using 1264, 1114, and 1004 field plots across 21 different forest vegetation classes in Himalaya, Indo-Burma, and Western Ghats respectively. Three different vegetation indices ranked highest in explaining the variance in plant richness in the three hotspots. The variance in species richness explained by models based on only VIs was highest (69%, P < 0.01) for Bamboo vegetation in Indo Burma hotspot with Normalized Difference Vegetation Index, followed by that for dry deciduous forest in Western Ghats (57%, P < 0.001) with Normalized Difference Water Index, and for grasslands (54%, P < 0.05) in Himalaya by Modified Soil Adjusted Vegetation Index. The explained variance increased with combined models that are based on PIs and VIs to up to 85% (P < 0.05). Overall, we observed very high correlation between VIs and plant richness in open canopy vegetation classes with low species richness such as grasslands, scrubs, and dry deciduous forests, followed by vegetation classes with moderately dense canopy. Our study provides crucial insights on utility of satellite datasets as a proxy for estimating plant richness for better conservation of diverse ecosystems
Extraskeletal Chondrosarcoma of Labium Majus
Extraskeletal myxoid chondrosarcoma (ESMC) is a rare tumor seen more often in men. It is seen to arise from soft tissue of lower extremity or buttocks. We report a case of soft tissue swelling of left labium majus in a 66-year-old female. Patient underwent wide excision with uneventful postoperative course. Histopathology of specimen confirmed it to be ESMC. Patient refused adjuvant therapy. Followup of 1 year has shown her to be disease- and symptom- free. Only two cases arising from vulva have been reported in literature . This is the third case and first from Indian subcontinent. A brief review of clinical features, diagnosis, treatment, and outcome of patients with extraskeletal chondrosarcoma is presented
An integrated genomic analysis of lung cancer reveals loss of DUSP4 in EGFR-mutant tumors.
To address the biological heterogeneity of lung cancer, we studied 199 lung adenocarcinomas by integrating genome-wide data on copy number alterations and gene expression with full annotation for major known somatic mutations in this cancer. This showed non-random patterns of copy number alterations significantly linked to EGFR and KRAS mutation status and to distinct clinical outcomes, and led to the discovery of a striking association of EGFR mutations with underexpression of DUSP4, a gene within a broad region of frequent single-copy loss on 8p. DUSP4 is involved in negative feedback control of EGFR signaling, and we provide functional validation for its role as a growth suppressor in EGFR-mutant lung adenocarcinoma. DUSP4 loss also associates with p16/CDKN2A deletion and defines a distinct clinical subset of lung cancer patients. Another novel observation is that of a reciprocal relationship between EGFR and LKB1 mutations. These results highlight the power of integrated genomics to identify candidate driver genes within recurrent broad regions of copy number alteration and to delineate distinct oncogenetic pathways in genetically complex common epithelial cancers

Variation in clinical presentation of childhood group A streptococcal pharyngitis in four countries

The Croatian and Latvian site was funded by the Department of Child and Adolescent Health and Development, World Health Organization, Geneva.We conducted a cross-sectional study from September 2001 to August 2003 during which children between 2 and 12 years of age presenting with complaint of sore throat were recruited from urban pediatric clinics in Brazil, Croatia, Egypt and Latvia. The objective of the study was to compare clinical signs and symptoms of children presenting to urban pediatric clinics with sore throat in and between countries and to identify common clinical criteria predicting group A beta hemolytic streptococcal (GAS) pharyngitis. Using a single standard protocol in all four sites, clinical data were recorded and throat swabs obtained for standard GAS culture in 2040 children. Signs and symptoms were tested for statistical association with GAS positive/negative pharyngitis, and were compared using X2 tests, ANOVA and Odds Ratios. Clinical signs of GAS pharyngitis in children presenting to clinics varied significantly between countries, and there were few signs or symptom that could statistically be associated with GAS pharyngitis in all four countries, though several were useful in two or three countries. Our results indicate that the clinical manifestations of pharyngitis in clinics may vary by region. It is therefore critical that clinical decision rules for management of pharyngitis should have local validation.publishersversionPeer reviewe
A systematic review of participatory scenario planning to envision mountain social-ecological systems futures
Mountain social-ecological systems (MtSES) provide crucial ecosystem services to over half of humanity. However, populations living in these highly varied regions are now confronted by global change. It is critical that they are able to anticipate change to strategically manage resources and avoid potential conflict. Yet, planning for sustainable, equitable transitions for the future is a daunting task, considering the range of uncertainties and the unique character of MtSES. Participatory scenario planning (PSP) can help MtSES communities by critically reflecting on a wider array of innovative pathways for adaptive transformation. Although the design of effective approaches has been widely discussed, how PSP has been employed in MtSES has yet to be examined. Here, we present the first systematic global review of single- and multiscalar, multisectoral PSP undertaken in MtSES, in which we characterize the process, identify strengths and gaps, and suggest effective ways to apply PSP in MtSES. We used a nine-step process to help guide the analysis of 42 studies from 1989 screened articles. Our results indicate a steady increase in relevant studies since 2006, with 43% published between 2015 and 2017. These studies encompass 39 countries, with over 50% in Europe. PSP in MtSES is used predominantly to build cooperation, social learning, collaboration, and decision support, yet meeting these objectives is hindered by insufficient engagement with intended end users. MtSES PSP has focused largely on envisioning themes of governance, economy, land use change, and biodiversity, but has overlooked themes such as gender equality, public health, and sanitation. There are many avenues to expand and improve PSP in MtSES: to other regions, sectors, across a greater diversity of stakeholders, and with a specific focus on MtSES paradoxes. Communicating uncertainty, monitoring and evaluating impacts, and engendering more comparative approaches can further increase the utility of PSP for addressing MtSES challenges, with lessons for other complex social-ecological systems. © 2020 by the author(s)
The interplay of growth differentiation factor 15 (GDF15) expression and M2 macrophages during prostate carcinogenesis
M2 (tumor-supportive) macrophages may upregulate growth differentiation factor 15 (GDF15), which is highly expressed in prostate tumors, but the combined utility of these markers as prognostic biomarkers are unclear. We retrospectively studied 90 prostate cancer cases that underwent radical prostatectomy as their primary treatment and were followed for biochemical recurrence (BCR). These cases also had a benign prostate biopsy at least 1 year or more before their prostate cancer surgery. Using computer algorithms to analyze digitalized immunohistochemically stained slides, GDF15 expression and the presence of M2 macrophages based on the relative density of CD204- and CD68-positive macrophages were measured in prostate: (i) benign biopsy, (ii) cancer and (iii) tumor-adjacent benign (TAB) tissue. Both M2 macrophages (P = 0.0004) and GDF15 (P \u3c 0.0001) showed significant inter-region expression differences. Based on a Cox proportional hazards model, GDF15 expression was not associated with BCR but, in men where GDF15 expression differences between cancer and TAB were highest, the risk of BCR was significantly reduced (hazard ratio = 0.26; 95% confidence interval = 0.09-0.94). In addition, cases with high levels of M2 macrophages in prostate cancer had almost a 5-fold increased risk of BCR (P = 0.01). Expression of GDF15 in prostate TAB was associated with M2 macrophage levels in both prostate cancer and TAB and appeared to moderate M2-macrophage-associated BCR risk. In summary, the relationship of GDF15 expression and CD204-positive M2 macrophage levels is different in a prostate tumor environment compared with an earlier benign biopsy and, collectively, these markers may predict aggressive disease
