    The scattering of a cylindrical invisibility cloak: reduced parameters and optimization

    We investigate the scattering of 2D cylindrical invisibility cloaks with simplified constitutive parameters with the assistance of scattering coefficients. We show that the scattering of the cloaks originates not only from the boundary conditions but also from the spatial variation of the component of permittivity/permeability. According to our formulation, we propose some restrictions to the invisibility cloak in order to minimize its scattering after the simplification has taken place. With our theoretical analysis, it is possible to design a simplified cloak by using some peculiar composites like photonic crystals (PCs) which mimic an effective refractive index landscape rather than offering effective constitutives, meanwhile canceling the scattering from the inner and outer boundaries.Comment: Accepted for J.     Grating-coupled excitation of multiple surface plasmon-polariton waves

    The excitation of multiple surface-plasmon-polariton (SPP) waves of different linear polarization states and phase speeds by a surface-relief grating formed by a metal and a rugate filter, both of finite thickness, was studied theoretically, using rigorous coupled-wave-analysis. The incident plane wave can be either p or s polarized. The excitation of SPP waves is indicated by the presence of those peaks in the plots of absorbance vs. the incidence angle that are independent of the thickness of the rugate filter. The absorbance peaks representing the excitation of s-polarized SPP waves are narrower than those representing p-polarized SPP waves. Two incident plane waves propagating in different directions may excite the same SPP wave. A line source could excite several SPP waves simultaneously

    Casimir-Polder interaction between an atom and a dielectric slab

    We present an explicit analytic calculation of the energy-level shift of an atom in front of a non-dispersive and non-dissipative dielectric slab. We work with the fully quantized electromagnetic field, taking retardation into account. We give the shift as a two-dimensional integral and use asymptotic analysis to find expressions for it in various retarded and non-retarded limiting cases. The results can be used to estimate the energy shift of an atom close to layered microstructures.Comment: 10 pages, incl 7 figure

    Evaluation of the efficacy and safety of high dose short duration enrofloxacin treatment regimen for uncomplicated urinary tract infections in dogs.

    BackgroundUncomplicated urinary tract infections (UTI) in dogs usually are treated with antimicrobial drugs for 10-14 days. Shorter duration antimicrobial regimens have been evaluated in human patients.HypothesisA high dose short duration (HDSD) enrofloxacin protocol administered to dogs with uncomplicated UTI will not be inferior to a 14-day treatment regimen with amoxicillin-clavulanic acid.AnimalsClient-owned adult, otherwise healthy dogs with aerobic bacterial urine culture yielding ≥ 10(3) CFU/mL of bacteria after cystocentesis.MethodsProspective, multicenter, controlled, randomized blinded clinical trial. Enrolled dogs were randomized to group 1 (enrofloxacin 18-20 mg/kg PO q24h for 3 days) or group 2 (amoxicillin-clavulanic acid 13.75-25 mg/kg PO q12h for 14 days). Urine cultures were obtained at days 0, 10, and 21. Microbiologic and clinical cure rates were evaluated 7 days after antimicrobial treatment was discontinued. Lower urinary tract signs and adverse events also were recorded.ResultsThere were 35 dogs in group 1 and 33 in group 2. The microbiologic cure rate was 77.1 and 81.2% for groups 1 and 2, respectively. The clinical cure rate was 88.6 and 87.9% for groups 1 and 2, respectively. Cure rates between groups did not differ according to the selected margin of noninferiority.Conclusions and clinical importanceHDSD enrofloxacin treatment was not inferior to a conventional amoxicillin-clavulanic acid protocol for the treatment of uncomplicated bacterial UTI in dogs. Further research is warranted to determine if this protocol will positively impact owner compliance and decrease the emergence of antimicrobial resistance

    Parallel Search with no Coordination

    We consider a parallel version of a classical Bayesian search problem. kk agents are looking for a treasure that is placed in one of the boxes indexed by N+\mathbb{N}^+ according to a known distribution pp. The aim is to minimize the expected time until the first agent finds it. Searchers run in parallel where at each time step each searcher can "peek" into a box. A basic family of algorithms which are inherently robust is \emph{non-coordinating} algorithms. Such algorithms act independently at each searcher, differing only by their probabilistic choices. We are interested in the price incurred by employing such algorithms when compared with the case of full coordination. We first show that there exists a non-coordination algorithm, that knowing only the relative likelihood of boxes according to pp, has expected running time of at most 10+4(1+1k)2T10+4(1+\frac{1}{k})^2 T, where TT is the expected running time of the best fully coordinated algorithm. This result is obtained by applying a refined version of the main algorithm suggested by Fraigniaud, Korman and Rodeh in STOC'16, which was designed for the context of linear parallel search.We then describe an optimal non-coordinating algorithm for the case where the distribution pp is known. The running time of this algorithm is difficult to analyse in general, but we calculate it for several examples. In the case where pp is uniform over a finite set of boxes, then the algorithm just checks boxes uniformly at random among all non-checked boxes and is essentially 22 times worse than the coordinating algorithm.We also show simple algorithms for Pareto distributions over MM boxes. That is, in the case where p(x)1/xbp(x) \sim 1/x^b for 0<b<10< b < 1, we suggest the following algorithm: at step tt choose uniformly from the boxes unchecked in 1,...,min(M,t/σ){1, . . . ,min(M, \lfloor t/\sigma\rfloor)}, where σ=b/(b+k1)\sigma = b/(b + k - 1). It turns out this algorithm is asymptotically optimal, and runs about 2+b2+b times worse than the case of full coordination

    Exterior optical cloaking and illusions by using active sources: a boundary element perspective

    Recently, it was demonstrated that active sources can be used to cloak any objects that lie outside the cloaking devices [Phys. Rev. Lett. \textbf{103}, 073901 (2009)]. Here, we propose that active sources can create illusion effects, so that an object outside the cloaking device can be made to look like another object. invisibility is a special case in which the concealed object is transformed to a volume of air. From a boundary element perspective, we show that active sources can create a nearly "silent" domain which can conceal any objects inside and at the same time make the whole system look like an illusion of our choice outside a virtual boundary. The boundary element method gives the fields and field gradients (which can be related to monopoles and dipoles) on continuous curves which define the boundary of the active devices. Both the cloaking and illusion effects are confirmed by numerical simulations

    Improving recognition of anxiety and depression in rheumatoid arthritis: a qualitative study in a community clinic.

    BACKGROUND: Comorbid anxiety and depression are common in patients with rheumatoid arthritis (RA) but are often under-recognised and treated, contributing to worse outcomes. National Institute for Health and Care Excellence (NICE) recommends that patients with RA should be offered a holistic annual review, including an assessment of mood. AIM: To explore patients' perspectives of anxiety and depression in RA and preferences for disclosure and management of mood problems. DESIGN AND SETTING: Qualitative interview study with patients recruited from a nurse-led RA annual review clinic in the Midlands, England. METHOD: Patients attending the clinic who scored ≥3 on the case-finding questions (PHQ-2 and GAD-2) were invited for interview. Data were analysed thematically using principles of constant comparison. RESULTS: Participants recognised a connection between their RA and mood, though this was perceived variably. Some lacked candidacy for care, normalising their mood problems. Fear of stigmatisation, a lack of time, and the perception that clinicians prioritise physical over mental health problems recursively affected help-seeking. Good communication and continuity of care were perceived to be integral to disclosure of mood problems. Participants expressed a preference for psychological therapies, though they reported problems accessing care. Some perceived medication to be offered as a 'quick fix' and feared potential drug interactions. CONCLUSION: Prior experiences can lead patients with RA and comorbid anxiety and depression to feel they lack candidacy for care. Provision of equal priority to mental and physical health problems by GPs and improved continuity of care could help disclosure of mood concerns. Facilitation of access to psychological therapies could improve outcomes for both mental and physical health problems

    Clinical Use of Cinacalcet in MEN1 Hyperparathyroidism

    Background. Management of multiple-endocrine neoplasia type 1- (MEN1-) associated hyperparathyroidism is associated with high recurrence rates and high surgical morbidity due to multiple neck explorations. Cinacalcet, a calcimimetic agent licensed for the treatment of secondary hyperparathyroidism and parathyroid carcinoma, may provide a medical alternative for the management of these complex patients. Methods. A prospective audit was performed of eight patients; three males and five females, aged 20–38 at diagnosis. Two patients commenced cinacalcet as primary treatment and six had previous surgery. Six patients had complications of hyperparathyroidism: renal calculi, renal dysfunction, and reduced bone mineral density. All were commenced on cinacalcet 30 mg bd for MEN1 associated hyperparathyroidism; doses were subsequently reduced to 30 mg od in four patients. Results. Significant reductions were observed in serum calcium and PTH measurements. Serum calcium reduced by a median of 0.35 mmol/L (P = .012 Wilcoxon Signed Rank). Serum PTH levels decreased by a median of 5.05 pmol/L (P = .012). There was no change in urine calcium. Duration ranged from 10–35 months with maintenance of control. Cinacalcet was well tolerated by six patients; one experienced nausea and one experienced diarrhoea. Conclusion. Cinacalcet is an effective and well-tolerated medical treatment for the management of complex primary hyperparathyroidism
