    Health related quality of life in COVID-19 survivors discharged from acute hospitals: results of a short-form 36-item survey [version 1; peer review: awaiting peer review]

    Background: Health-related quality of life (HRQL) is important for evaluating the impact of a disease in the longer term across the physical and psychological domains of human functioning. The aim of this study is to evaluate HRQL in COVID-19 survivors in Italy using the short form 36-items questionnaire (SF-36). Methods: This is an observational study involving adults discharged home following a coronavirus disease 2019 (COVID-19)-related hospital admission. Baseline demographic and clinical data including the Cumulative Illness Rating Scale (CIRS) and the Hospital Anxiety and Depression Scale (HADS) were collected. The validated Italian version of SF-36 was administered cross-sectionally. The SF-36 contains eight scales measuring limitations in physical and social functioning, the impact on roles and activities, fatigue, emotional wellbeing, pain and general health perception. Results: A total of 35 patients, with a mean age of 60 years, completed the SF-36. The results showed difficulties across the physical and psychological domains, particularly affecting the return to previous roles and activities. A higher burden of co-morbidities as well as a more severe muscle weakness was associated to a lower physical functioning. Younger age, rather than older, correlated to a perceived greater limitation in physical functioning and vitality. Conclusions: COVID-19 survivors particularly the ones of working age may need support for resuming their premorbid level of functioning and returning to work

    A Business Process Reengineering of the Surgical Path through Lean Technique: The Real Case Study of a Midsize Italian Hospital

    This period of pandemic has had important consequences on the flow and the entire organization of any hospital. In particular, the number of accesses to the emergency room has increased, with the consequent urgent need to reorgani ze it quickly. The model proposed in this paper allows to respond to these needs by freeing not only shifts of nursing staff but also surgical staff. This workforce can then be relocated in the emergency room or of the intensive care unit who are in fact at the forefront of emergency management. The aim of this study conducted by the authors is to analyze, inside the context of a midsize Italian hospital, the actual organization model, and then to approach it by Business Process Reengineering (BPR) methodology with the goal to propose a KPI management system that evaluates the efficiency of the whole surgical path. The second objective of the study is to verify if the Operating Rooms (ORs) are properly sized to cover the surgical workload or if it would be necessary to build new ORs (answer to this question is the project mandate by Surgical Wards Chiefs). The last objective is to implement a flexible to cope with emergency situations such as a pandemic. The main result is the approximate maintenance of surgical annual activity (8169 vs 7889). The fewer resources required can be reallocated to deal with emergencies such as the current COVID-19 pandemic. In fact, the surgical shifts decreased during the test case from 464 versus 365 (-15,32%). The rooms’ utilization coefficient rose from 41% to over 52%, whereas the surgeons’ utilization coefficient rose to 61% (with values over 68% for parallel shifts). The results achieved demonstrate that improving efficiency of surgical processes is feasible and a systematic approach allows to respond to new global health challenges

    Proceedings of the 3rd Biennial Conference of the Society for Implementation Research Collaboration (SIRC) 2015: advancing efficient methodologies through community partnerships and team science

    It is well documented that the majority of adults, children and families in need of evidence-based behavioral health interventionsi do not receive them [1, 2] and that few robust empirically supported methods for implementing evidence-based practices (EBPs) exist. The Society for Implementation Research Collaboration (SIRC) represents a burgeoning effort to advance the innovation and rigor of implementation research and is uniquely focused on bringing together researchers and stakeholders committed to evaluating the implementation of complex evidence-based behavioral health interventions. Through its diverse activities and membership, SIRC aims to foster the promise of implementation research to better serve the behavioral health needs of the population by identifying rigorous, relevant, and efficient strategies that successfully transfer scientific evidence to clinical knowledge for use in real world settings [3]. SIRC began as a National Institute of Mental Health (NIMH)-funded conference series in 2010 (previously titled the “Seattle Implementation Research Conference”; $150,000 USD for 3 conferences in 2011, 2013, and 2015) with the recognition that there were multiple researchers and stakeholdersi working in parallel on innovative implementation science projects in behavioral health, but that formal channels for communicating and collaborating with one another were relatively unavailable. There was a significant need for a forum within which implementation researchers and stakeholders could learn from one another, refine approaches to science and practice, and develop an implementation research agenda using common measures, methods, and research principles to improve both the frequency and quality with which behavioral health treatment implementation is evaluated. SIRC’s membership growth is a testament to this identified need with more than 1000 members from 2011 to the present.ii SIRC’s primary objectives are to: (1) foster communication and collaboration across diverse groups, including implementation researchers, intermediariesi, as well as community stakeholders (SIRC uses the term “EBP champions” for these groups) – and to do so across multiple career levels (e.g., students, early career faculty, established investigators); and (2) enhance and disseminate rigorous measures and methodologies for implementing EBPs and evaluating EBP implementation efforts. These objectives are well aligned with Glasgow and colleagues’ [4] five core tenets deemed critical for advancing implementation science: collaboration, efficiency and speed, rigor and relevance, improved capacity, and cumulative knowledge. SIRC advances these objectives and tenets through in-person conferences, which bring together multidisciplinary implementation researchers and those implementing evidence-based behavioral health interventions in the community to share their work and create professional connections and collaborations

    The implementation of a methodology for measuring the performance of nurse managers in a hospital for highly specialized care intensity through the PDCA approach

    Background: the complexity of nurse managers\u2019 role is gradually changed from assistance to management. The aim of this study is to implement an innovative indicator to globally evaluate the features, the professional skills and their performance is described. Methods: the authors found out the features of a valuable nurse managers through an interview to the directors of all the nurses. The method individuated three different aspects (qualitative, quantitative and relational) that were transformed in a single indicator. The approach used by the authors is the PDCA. Results: A total of 18 nurse managers were evaluated in this study. All the results confirmed the judgment of the Healthcare Professions Structure head. Conclusion: This assessment method, validated with these tests, is able to evaluate the nurse manager\u2019s ability to manage personnel, resources and patients

    Le risposte che l\u2019organismo realizza quando e sottoposto ad uno stress metabolico od ambientale rappresentano potenziali biomarcatori utilizzabili per il monitoraggio. Mediante lo studio delle risposte immediate un biomarcatore permette infatti di prevedere con notevole anticipo l\u2019insorgenza di effetti nocivi come cancerogenesi, alterazioni patologiche, diminuzione delle capacita riproduttive, e mortalita. La reazione dell\u2019organismo prevede la sintesi di particolari proteine dello stress quali le metallotioneine (MT) e le heat shock protein (Hsp). In particolare, livelli alterati di MT nei diversi tessuti possono rappresentare un indice specifico, anche se indiretto, di inquinamento ambientale da metalli pesanti, mentre le Hsp, ed in particolare l\u2019HSP70 e 90, che possono essere indotte da un ampio spettro di fattori di stress, sono considerate buoni indicatori del benessere dei pesci in condizioni di allevamento. Questo studio e stato condotto su esemplari di spigola (Dicentrarchus labrax) campionati nel mese di febbraio 2009 a fine ingrasso, presso due diversi tipi di impianto 1) off-shore e 2) in-shore. Campioni di tessuto muscolare sono stati immediatamente congelati per le successiva determinazione del contenuto di metalli pesanti (Hg, Pb, Cd e Cr). I tenori di piombo (Pb), mercurio (Hg), cadmio (Cd) e cromo (Cr) sono stati determinati per via umida previa mineralizzazione del tessuto e successiva lettura in assorbimento atomico. Il fegato e stato prelevato in toto, pesato e immediatamente una piccola porzione (n=10 per ogni allevamento) e stata immersa in RNA later e congelata a -20\ub0C per l\u2019analisi semi-quantitativa dell\u2019espressione di MT e HSP 70 e 90 mediante RT-PCR. I valori di Hg, Pb e Cd espressi in mg/Kg pesce umido sono risultati sempre al di sotto dei limiti previsti dalle normative relative ai contaminanti nei prodotti alimentari (REGOLAMENTO (CE) N. 1881/2006 DELLA COMMISSIONE del 19 dicembre 2006 che definisce i tenori massimi di alcuni contaminanti nei prodotti alimentari. In particolare il Cd e sempre risultato ai limiti della soglia di rilevabilita del metodo. Il dato analitico e supportato dai dati ottenuti con i biomarcatori utilizzati in questo studio. Infatti, l\u2019espressione delle MT e risultata su livelli di base, senza nessuna differenza significativa nei due siti. Inoltre non sono state rilevate differenze significative nell\u2019espressione di HSP70 e HSP90 confermando, con questi parametri, l\u2019adozione di condizioni di allevamento complessivamente atte a ridurre al minimo lo stress.
