    FEM and Von Mises analysis on prosthetic crowns structural elements: evaluation of different applied materials

    The aim of this paper is to underline the mechanical properties of dental single crown prosthodontics materials in order to differentiate the possibility of using each material for typical clinical condition and masticatory load. Objective of the investigation is to highlight the stress distribution over different common dental crowns by using computer-aided design software and a three-dimensional virtual model. By using engineering systems of analyses like FEM and Von Mises investigations it has been highlighted the strength over simulated lower first premolar crowns made by chrome cobalt alloy, golden alloy, dental resin, and zirconia. The prosthodontics crown models have been created and put on simulated chewing stresses. The three-dimensional models were subjected to axial and oblique forces and both guaranteed expected results over simulated masticatory cycle. Dental resin presented the low value of fracture while high values have been recorded for the metal alloy and zirconia. Clinicians should choose the better prosthetic solution for the teeth they want to restore and replace. Both prosthetic dental crowns offer long-term success if applied following the manufacture guide limitations and suggestions

    Porphyromonas gingivalis, Periodontal and Systemic Implications: A Systematic Review

    In recent scientific literature, oral infections and systemic manifestations, or correlations between oral health and systemic diseases are a topic of discussion. Porphyromonas gingivalis is one of the bacteria implicated in the biofilm formation of bacterial plaque, and plays an important role in the progression of periodontal disease. In this systematic review authors have evaluated the literature of the last 10 years on P. gingivalis and all the systemic implications proven. This study therefore evaluates all the districts of the organism in which this bacterium may have implications. From the results it emerges that P. gingivalis has implications in the onset of different systemic pathologies, including rheumatoid arthritis, cardiovascular pathologies, and neurodegenerative pathologies. Surely, understanding the mechanisms of diffusion of this bacterium, it would be possible to prevent a series of pathologies. Thus, putting the dentist clinician at the center of prevention for these diseases

    Comparison of Two Low-Profile Prosthetic Retention System Interfaces: Preliminary Data of an In Vitro Study

    In recent years, a major research goal of companies has been to create mechanical components suitable for rehabilitation that are safer and more reliable. Evaluating their biomechanical features could be a way to improve them. The purpose of this study was to evaluate the different biomechanical features of low-profile retentive systems (Rhein®). Two different attachment systems were tested: OT Equator® Smart Box and Locator® R-TX. Once a machine was created for the simulation of the connection and disconnection of the attacks in a combined manner, it was possible to evaluate these parameters over time. Attachments were mounted in two different configurations of the divergence angle: 10° and 50°. The drop retention force proved to be stable over time. The Locator® R-TX attachment experienced a more rapid decrement of the retention force than the OT Equator® Smart Box. Both tested systems experienced a high drop in retention; this drop tended to stabilize after 1.5 years of use, and it was correlated with the divergence angle. The OT Equator® Smart Box system underwent this loss of retention more gradually than the Locator® R-TX. This study demonstrates preliminary results from a bioengineering and biomechanical point of view, providing useful information for the continuous improvement of these devices and, therefore, for the quality of patients’ oral health

    Antibiotic Prophylaxis on Third Molar Extraction: Systematic Review of Recent Data

    The aim of this paper was to highlight the most widely antibiotic protocols applied to the dental field, especially in the surgical treatment of impacted wisdom teeth. Once these protocols were screened, all the possible advantages or disadvantages for each drug and each posology were recorded in this review. In recent years, the need to use these protocols has been debated in the literature. The data obtained by this review underlined how antibiotic protocols applied to oral surgery treatments only included surgeries performed on patients who did not present other systemic pathologies. The first literature review obtained 140 results, and then after the application of the inclusion criteria, 12 papers were selected. The results showed that the most commonly used protocol involved the use of penicillin and clavulanate, obtaining safe clinical and prophylactic results in the management of infections. This widely used protocol seems to guarantee high predictability and safety. The presented review highlights the current possibility of antibiotic resistance affecting patients due to drug misuse. Further clinical studies are required to state specific guidelines; however, oral surgeons involved in third molar surgery should evaluate the local and general health conditions of the patients before suggesting any drug measures for patients

    Growth Factors in Oral Tissue Engineering: New Perspectives and Current Therapeutic Options

    The present investigation is aimed at systematically analyzing the recent literature about the innovative scaffold involved in the reconstructive surgeries by applying growth factors and tissue engineering. An extensive review of the contemporary literature was conducted according to the PRISMA guidelines by accessing the PubMed, Embase, and Scopus Elsevier databases. Authors performed the English language manuscript research published from 2003 to 2020. A total of 13 relevant studies were included in the present review. The present systematic review included only papers with significant results about correlation between scaffold, molecular features of growth factor, and reconstructive surgeries in oral maxillofacial district. The initial research with filters recorded about 1023 published papers. Beyond reading and considering of suitability, only 42 and then 36 full-text papers were recorded for the revision. All the researches recorded the possibility of using growth factors on rebuilding atrophic jaws. Different growth factors like morphogenetic factors, cytokines, and inflammatory ones and their application over different scaffold materials were recorded. Further investigations should be required in order to state scientific evidence about a clear advantage of applying tissue engineering for therapeutic purpose

    The Panoramic radiography is the result of a radiological technique that allows the reproduction of the dental arches and the support structures of a unique image. The clinical utility of panoramic radiography allows the overall assessment as well as the detection of structural abnormalities and/or possibly relates to pathological lesions. The disadvantage of this technique is related to the lower quality of the images compared to endoral radiographs and TC scans but especially to the attempt to represent a three-dimensional, curved anatomic region on a two-dimensional plane. This inevitably leads to geometric distortions of shape, enlargements and overlaps that adversely affect the detection of any pathological entities. However, the most important advantage is the possibility of reducing patient exposure to ionizing rays. The aim of digital radiology is to improve the characteristics of radiological machinery (through increasingly innovative and sophisticated data processing software and hardware) in order to obtain high-quality images while reducing dosimetry and to reduce “disturbing elements” in radiological images. To overcome the limitations of the panoramic technique and to improve the quality of radiological images, and thus the effectiveness of diagnosis, image optimization software can be used. In this regard, a tool called Pipeline for Advanced Contrast Enhancement (PACE) is being developed: This is an algorithm that allows image quality to be improved by optimizing contrast and reducing brightness inhomogeneity, resulting in increased definition, making details more evident. The purpose of the proposed investigation is to evaluate if the application of the PACE algorithm in dental radiology could be an advantage because of the possibility of enhancing the quality of radiological images to make details of anatomical structures and related pathological lesions more obvious

    Finite Element Method and Von Mises Investigation on Bone Response to Dynamic Stress with a Novel Conical Dental Implant Connection

    The bioengineering and medical and biomedical fields are ever closer, and they manage to obtain surprising results for the development of new devices. The field of simulations and studies in silica has undergone considerable development in recent years, favoring the advancement of medicine. In this manuscript, a study was carried out to evaluate the force distribution on the implant components (In-Kone\uae Universal) and on the peri-implant tissues subjected to loading. With the finite element analysis and the Von Mises method, it was possible to evaluate this distribution of forces both at 0 degrees (occlusal force) and at 30 degrees; the applied force was 800N. The obtained results on this new type of connection and on all the implant components are satisfactory; the distribution of forces appears optimal even on the peri-implant tissues. Surely, studies like this help to obtain ever more performing devices, improving both the clinic and the predictability of rehabilitations
