    Concomitant pulmonary vein isolation and percutaneous closure of atrial septal defects: A pilot project

    Background: Patients with an atrial septal defect (ASD) are at increased risk of developing atrial fibrillation (AF). Currently percutaneous ASD closure is the preferred therapeutic strategy and although pulmonary vein isolation (PVI) for AF is feasible after ASD closure, the transseptal puncture can be technically challenging and probably increases the perioperative risk. A staged approach, with PVI several months before ASD closure, has been recommended for patients already scheduled for closure, but no data are available on combined procedures. Purpose: This pilot study evaluates the feasibility of a combined procedure of PVI and ASD closure in patients with a hemodynamic important ASD and documented AF. Methods: In one procedure, PVI was performed prior to placement of the ASD closure device. Transseptal access for PVI was obtained via wire passage through the ASD in all patients. Patients were followed with 5-day-holter monitoring at 3, 6, and 12 months. Recurrence of AF was defined as a documented, symptomatic episode of AF. Results: The study population consisted of five patients (four females, mean age: 58 (±3) years). Acute PVI was achieved in all patients. Only one patient had a small residual ASD after closure. Besides a small groin hematoma in two patients, no complications occurred. After 12-month follow-up, three patients were free of AF recurrence (60%). Conclusion: This study shows that a combined PVI with ASD closure is feasible with an acceptable success rate of AF free survival. These preliminary results in a small patient group warrants a larger trial

    Imaging of perfusion of the myocardium with nuclear cardiac imaging is an uncontested technique for diagnostic as well as prognostic purposes in patients with chest pain and a normal activation pattern of the heart. However, when the activation pattern of the heart is abnormal as happens with intraventricular conduction disorders or artificial cardiac pacing, the value of this technique becomes disputable. This shortcoming is predominantly caused by the presence of perfusion defects not caused by coronary artery disease. These false positive perfusion defects diminish the diagnostic and prognostic value of nuclear imaging in patients with chest pain and abnormal activation who are studied in this thesis. We showed that these false positive defects meet specific diagnostic criteria. In addition it was demonstrated that wall motion abnormalities due to abnormal activation are strongly related to perfusion defects. The limited resolution of the nuclear camera for a movi ng organ is presumably the cause of these false positive defects. We strengthened this assumption by showing a normal myocardial blood-flow in the walls where false positive perfusion are found. The abnormal activation related defects (AARD) follow a typical pattern. We defined a triad to recognize these AARD and demonstrated that the prognosis of patients with AARD compares well those patients with a completely normal perfusion with abnormal activation of the heart. By recognizing these AARD in patients with chest pain it is possible to distinguish patients with perfusion defects that are at a low risk for future cardiac events from those with perfusion defects at high risk for future cardiac events. The results of this thesis allow to identify patients with chest pain and abnormal left ventricular activation pattern but are nevertheless at a low cardiac risk. This category has a prognosis that merits a watchful waiting strategy, and invasive, expensive diagnostic procedures can be av oided. This strategy prevents patient burden and complication risk. However patients with chest pain and abnormal activation of the heart with a high risk estimated by nuclear imaging, require further diagnostic work up often followed by surgical coronary revascularization or Dotter procedures. The results of these cardiac nuclear studies are of relevance for patient care in terms of improved management and saving of costs

    Novel X-ray image noise reduction technology reduces patient radiation dose while maintaining image quality in coronary angiography

This study assessed non-inferiority of image quality and quantified patient dose reduction in interventional cardiology for an anatomy-specific optimised cine acquisition chain combined with advanced real-time image noise reduction algorithms referred to as 'study cine', compared with conventional angiography. METHODS: Fifty patients underwent two coronary angiographic acquisitions: one with advanced image processing and optimised exposure system settings to enable dose reduction (study cine) and one with standard image processing and exposure settings (reference cine). The image sets of 39 patients (18 females, 21 males) were rated by six experienced independent reviewers, blinded to the patient and image characteristics. The image pairs were randomly presented. Overall 85 % of the study cine images were rated as better or equal quality compared with the reference cine (95 % CI 0.81-0.90). The median dose area product per frame decreased from 55 to 26 mGy.cm(2)/frame (53 % reduction, p < 0.001). CONCLUSION: This study demonstrates that the novel X-ray imaging technology provides non-inferior image quality compared with conventional angiographic systems for interventional cardiology with a 53 % patient dose reduction

    Proof of principle of a novel co-pulsating intra-ventricular membrane pump

    In this proof of principle study, we investigated the effectiveness and safety of hemodynamic support with the Intra-Ventricular Membrane Pump (IVMP). The IVMP was implanted into the apex of the left ventricle. Hemodynamic assessment was performed in six ex vivo beating porcine hearts (PhysioHeart platform). The cardiac output (CO), mean arterial pressure (MAP), coronary flow (CF) and pulse pressure (PP) were obtained before and during IVMP support and reported as means ±standard deviations. In two additional visualization experiments, the integrity of the mitral valve was assessed during IVMP support. We found a significant increase of the CO (+1.4 ±0.2 L/min, P &lt; .001), MAP (+13 ±6 mm Hg, P = .008), CF (+0.23 ±0.1 L/min, P = .004), and PP (+15 ±4 mm Hg, P = .002) during IVMP support, when compared to baseline. No interference of the IVMP with mitral valve function was observed. An increase of premature ventricular complexes (PVC) was observed during support with the IVMP (mean PVC-burden 4.3% vs. 0.7% at baseline), negatively influencing hemodynamic parameters. The IVMP is able to significantly improve hemodynamic parameters in a co-pulsatile fashion, without hampering the function of the mitral valve. These findings provide a basis for future development of a catheter-based IVMP

    Aims: A depressed left ventricular function (LVF) is sometimes observed during right ventricular apical (RVA) pacing, but any prediction of this adverse effect cannot be done. Right ventricular outflow tract (RVOT) pacing is thought to deteriorate LVF less frequently because of a more normal LV activation pattern. This study aims to assess the acute effects of RVA and RVOT pacing on LVF in order to determine the contribution of echocardiography for the selection of the optimum pacing site during pacemaker (PM) implantation. Methods and results: Fourteen patients with a DDD-pacemaker (7 RVA, 7 RVOT) and normal LVF without other cardiac abnormalities were studied. PM dependency, because of sick sinus syndrome with normal atrioventricular and intraventricular conduction, was absent in all, allowing acute programming changes. Wall motion score (WMS), longitudinal LV strain, and tissue Doppler imaging for electromechanical delay were assessed with echocardiography during AAI pacing constituting baseline and DDD pacing. The WMS was normal at baseline (AAI pacing) in all patients and LV dyssynchrony was absent. Acute RVA and RVOT pacing deteriorated WMS, electromechanical delay, and longitudinal LV strain, but no difference of the deterioration between both pacing sites was present and dyssynchrony did not emerge. Conclusion: Both acute RVA and RVOT pacing negatively affect WMS, longitudinal LV strain, and mechanical activation times, without clear differences between both pacing sites. Thus echocardiographic techniques do not facilitate the selection between RVOT and RVA pacing to exclude adverse effects on LVF during PM implantation in patients with a normal LVF

The primary outcomes were cardiac power (CP) and cardiac power index (CPI). Survival rates and other haemodynamic data were included as secondary outcomes. For the critical appraisal, we used a modified version of the U.S. Department of Health and Human Services quality assessment form. The systematic review included 12 studies with a total of 596 patients. In 258 patients the CP and/or CPI could be extracted. Our meta-analysis showed an increase of 0.39W [95% confidence interval (CI): 0.24, 0.54], (p= 0.01) and 0.22W/m(2) (95% CI: 0.18, 0.26), (p< 0.01) for the CP and CPI, respectively. The overall survival rate was 56% (95% CI: 0.50, 0.62), (p= 0.09). The quality of the studies was moderate, mostly due to the presence of confounders. Our study suggests that in patients with cardiogenic shock, Impella support seems effective in augmenting CP(I). This study merely investigates the haemodynamic effectiveness of the Impella device and does not reflect the complete clinical impact for the patient
