Epigenetic control of epithelial-mesenchymal transition
El proceso de transición epitelio-mesénquima (TEM)
permite que una célula epitelial, de manera temporal,
adquiera un fenotipo mesenquimal como respuesta
a un estÃmulo interno o externo. Este proceso se
caracteriza por la activación y represión de genes
involucrados en diferentes vÃas de señalización asociadas
con migración, invasión, apoptosis, entre otros. En este
proceso, la epigenética cumple un papel fundamental,
pues comprende cuatro mecanismos: metilación de ADN,
modificación covalente de histonas, ARN no codificantes
(ARNnc) y complejos remodeladores de la cromatina, que
regulan la expresión de un gen sin alterar su secuencia. En
esta revisión de tema los autores describen los principales
mecanismos epigenéticos involucrados en la regulación de
la expresión de genes que se activan y reprimen a lo largo

allows a temporary epithelial cell to acquire a mesenchymal
phenotype in response to an internal or external stimulus.
This process is characterized by the activation and
repression of genes involved in different signaling pathways
associated with migration, invasion and apoptosis, among
others. In this process epigenetics plays a fundamental
role. Epigenetics comprises four mechanisms: DNA
methylation, covalent modification of histones, noncoding RNAs (RNACs) and chromatin remodeling
complexes, which regulate the expression of a gene without
altering its sequence. In this topic review, the authors
describe the main epigenetic mechanisms involved in the
regulation of the expression of genes that are activated and
repressed throughout the TEM process
Evaluation of the association between pre-gestational and gestational diabetes and the development of congenital defects in children born in the city of Bogotá D.C. from 2001 to 2019
En el presente estudio de corte descriptivo retrospectivo, caso control, se planteó el objetivo de determinar la asociación entre Diabetes gestacional o pre-gestacional con el desarrollo de malformaciones congénitas en recién nacidos de la ciudad de Bogotá D.C. estudiados en el periodo comprendido entre 2001 a 2019. Para ello, se seleccionó una base de datos anonimizada del Programa de Vigilancia y Seguimiento de niños con Defectos Congénitos de la Ciudad de Bogotá, D.C, con aproximadamente 600.000 nacimientos, de la cual se determinaron los casos y controles, asà como las variables a estudiar, cualitativas, tipo de parto, sexo del recién nacido, ciudad de reporte del nacimiento, signos y sÃntomas asociados al embarazo, comorbilidades presentadas además de diabetes, tipo de medicamento para tratamiento de la diabetes y el consumo o no de alcohol o cigarrillo; y cuantitativas, talla de gestante, peso de gestante al inicio y al final de la gestación y peso del recién nacido.
Respecto al análisis estadÃstico, el análisis cualitativo se realizó mediante la herramienta de ‘Tablas dinámicas’ de Excel 2016, y el cuantitativo mediante la herramienta estadÃstica ‘EstadÃstica descriptiva’ del mismo programa; asimismo, con el fin de determinar si hay diferencia estadÃsticamente significativa entre la media y por ende clÃnicamente significativa de los pesos de recién nacidos, pesos maternos al inicio y al final del embarazo de las poblaciones de ‘casos’ y ‘controles’, se calculó primeramente la Prueba F para varianzas de dos muestras para conocer si la varianza de la población ‘casos’ es igual a la varianza de la población ‘controles’ en cada variable. Dependiendo de su resultado, se realizó la prueba T student para varianzas iguales o desiguales.
Ahora bien, con el fin de determinar si hay diferencias estadÃsticamente significativas entre los valores de frecuencias de la presencia de malformaciones congénitas en hijos de madres sanas y en en hijos de madres con DMPG y gestacional, se realizó una prueba de Chi cuadrado. Con ese fin se realizaron tablas de contingencia 2 x 2 no estratificadas en las que se evalúa la influencia de una variable independiente en el desarrollo u ocurrencia de una variable dependiente.
En este sentido, en el presente estudio se encontró y confirmó la asociación estadÃsticamente significativa entre Diabetes gestacional o pre-gestacional y el desarrollo de malformaciones congénitas, descrito previamente en la literatura cientÃfica. Sin embargo, aunque el peso en gestantes con DMG es congruente con lo presentado en la literatura, no hubo diferencia significativa entre los pesos de gestantes con diabetes en controles en comparación con casos. Asà también, los efectos del cigarrillo redundan en afectación en la producción de insulina y por ende en los niveles de glicemia, ejerciendo un efecto sinérgico con la embriopatÃa diabética y por ende en el desarrollo de malformaciones congénitas. En el caso del consumo de alcohol, no se concluyó asociación entre éste y el desarrollo de malformaciones congénitas. Las malformaciones presentes en hijos de estas madres se atribuyen entonces principalmente a la embriopatÃa diabética.SecretarÃa de Salud de Bogotá D.C.Pontificia Universidad JaverianaThe aim of this retrospective descriptive case-control study was to determine the association between gestational or pre-gestational diabetes and the development of congenital malformations in newborns in the city of Bogotá, D.C., studied between 2001 and 2019. For this purpose, an anonymized database was selected from the Surveillance and Follow-up Program for Children with Congenital Defects of the City of Bogotá, D.C., with approximately 600. 000 births, from which the cases and controls were determined, as well as the variables to be studied, qualitative, type of delivery, sex of the newborn, city where the birth was reported, signs and symptoms associated with the pregnancy, comorbidities presented in addition to diabetes, type of medication for the treatment of diabetes and the consumption or not of alcohol or cigarettes; and quantitative, height of the pregnant woman, weight of the pregnant woman at the beginning and at the end of gestation and weight of the newborn.
Regarding the statistical analysis, the qualitative analysis was performed using the 'Pivot Tables' tool of Excel 2016, and the quantitative analysis was performed using the 'Descriptive Statistics' statistical tool of the same program; likewise, in order to determine whether there is a statistically significant difference between the mean and therefore clinically significant difference in the weights of newborns, maternal weights at the beginning and end of pregnancy of the 'cases' and 'controls' populations, the F-test for variances of two samples was first calculated to find out whether the variance of the 'cases' population is equal to the variance of the 'controls' population in each variable. Depending on its result, the T student test was performed for equal or unequal variances.
However, in order to determine whether there are statistically significant differences between the frequency values of the presence of congenital malformations in children of healthy mothers and in children of mothers with pre-gestational and gestational diabetes mellitus, a Chi-square test was performed. For this purpose, unstratified 2 x 2 contingency tables were used to evaluate the influence of an independent variable on the development or occurrence of a dependent variable.
In this sense, the present study found and confirmed the statistically significant association between gestational or pre-gestational diabetes and the development of congenital malformations, previously described in the scientific literature. However, although the weight in pregnant women with GDM is congruent with that presented in the literature, there was no significant difference between the weights of pregnant women with diabetes in controls compared to cases. Moreover, the effects of cigarette smoking have an effect on insulin production and therefore on glycemia levels, exerting a synergistic effect on diabetic embryopathy and therefore on the development of congenital malformations. In the case of alcohol consumption, no association was found between alcohol consumption and the development of congenital malformations.
