    Use of Nasal Cytology in Diagnosis of Sinonasal Disorders

    Nasal cytology is an important diagnostic tool in nasal disorders, especially those regarding nasal mucosa. This technique allows clinicians to observe the morphology and the structure of nasal epithelium cells—such as ciliated, mucous-secreting, striated, and basal cells—and to detect the presence of degenerative or phlogistic phenomena in the respiratory epithelium. Moreover, it makes it easy to identify the presence of inflammatory cells, such as neutrophils, eosinophils, and mast cell. Over the past few years, nasal cytology allowed researchers to discover new clinical issues that were still unexplored: the nonallergic rhinitis with eosinophils, the nonallergic rhinitis with mast cells, the nonallergic rhinitis with neutrophils, and the nonallergic rhinitis with eosinophils and mast cells. Nasal cytology is easy to perform and barely invasive; therefore, it can easily be repeated. Since it makes it possible to evaluate the patient’s response to a therapy, the technique is a very useful tool in follow-up checks of nasal disorders. We have reported in the following chapter our working experience, and we observed the results of cytological exams performed in our Center from 2013 to 2018.     Development of a Methodology for Condition-Based Maintenance in a Large-Scale Application Field

    This paper describes a methodology, developed by the authors, for condition monitoring and diagnostics of several critical components in the large-scale applications with machines. For industry, the main target of condition monitoring is to prevent the machine stopping suddenly and thus avoid economic losses due to lack of production. Once the target is reached at a local level, usually through an R&D project, the extension to a large-scale market gives rise to new goals, such as low computational costs for analysis, easily interpretable results by local technicians, collection of data from worldwide machine installations, and the development of historical datasets to improve methodology, etc. This paper details an approach to condition monitoring, developed together with a multinational corporation, that covers all the critical points mentioned above

    A bearing fault model for Independent Cart Conveyor System and its validation

    Independent Cart Conveyor System is one of the most promising technology in automation industries. It combines the benefits of servo motors with the advantages of linear motors. It consists of a close path made up of modular linear motors having a curved or a straight shape that control a fleet of carts independently. Each cart is placed along the motors and it is connected, through rolling bearings, to a rail set on the motors themselves. The bearings are subject to wear and the condition monitoring of these elements is challenging for the non-stationary working conditions of variable load and variable speed profiles. This paper provides a bearing fault vibration model that takes into account the mechanical design of the cart, its motion profile, the shape of the conveyor path, the load variation and the type of fault on the rolling bearing

    The duration of drug efficacy in vivo is a key aspect primarily addressed during the lead optimization phase of drug discovery. Hence, the availability of robust computational approaches that can predict the residence time of a compound at its target would accelerate candidate selection. Nowadays the theoretical prediction of this parameter is still very challenging. Starting from methods reported in the literature, we set up and validated a new metadynamics (META-D)-based protocol that was used to rank the experimental residence times of 10 arylpyrazole cyclin-dependent kinase 8 (CDK8) inhibitors for which target-bound X-ray structures are available. The application of reported methods based on the detection of the escape from the first free energy well gave a poor correlation with the experimental values. Our protocol evaluates the energetics of the whole unbinding process, accounting for multiple intermediates and transition states. Using seven collective variables (CVs) encoding both roto-translational and conformational motions of the ligand, a history-dependent biasing potential is deposited as a sum of constant-height Gaussian functions until the ligand reaches an unbound state. The time required to achieve this state is proportional to the integral of the deposited potential over the CV hyperspace. Average values of this time, for replicated META-D simulations, provided an accurate classification of CDK8 inhibitors spanning short, medium, and long residence times

    Time evolution of restless legs syndrome in haemodialysis patients

    Abstract Background Restless legs syndrome (RLS) is characterized by an urge to move the extremities, accompanied by paraesthesiae, in the evening and at night. Uraemic RLS, a type of secondary RLS, occurs commonly in chronic kidney disease and end-stage renal disease. Progression of uraemic RLS over time is unclear. Therefore we investigated the prevalence, progression over time, risk factors and impact on survival of uraemic RLS in a cohort of dialysis patients. Methods We reviewed at the 7-year follow-up a cohort of haemodialysis (HD) patients we had previously investigated for RLS, through interviews, validated questionnaires and analysis of demographic and clinical data. Results At the 7-year follow-up, RLS was present in 16% of patients, with a persistence rate of 33%. A correlation was obtained between RLS and older age, diabetes, low albumin and low body mass index. RLS was associated with reduced overall survival (median survival of 3.3 versus 3.7 years), particularly with the continuous form of RLS (1.61 years). There was a higher incidence of myocardial infarction and peripheral vascular disease, although not reaching statistical significance. RLS patients had absolute higher scores in all quality of life domains. A large majority of study patients (96%) reported being symptom-free within a few days or weeks following kidney transplantation. Conclusions The development of RLS, especially the continuous form, in patients undergoing HD has important consequences associated with decreased survival. Our results indicated an association between uraemic RLS and ageing, diabetes and malnutrition. Considerable efforts should be focused on the treatment of RLS, since it significantly and persistently impacts the quality of life of HD patients. Kidney transplantation could represent an effective treatment option for that RLS impacts on dialysis patients' quality of life, thus confirming the secondary nature of RLS in most HD patients

    Assessment of the indoor odour impact in a naturally ventilated room

    Indoor air quality influences peopleâ\u80\u99s lives, potentially affecting their health and comfort. Nowadays, ventilation is the only technique commonly used for regulating indoor air quality. CO2 is the reference species considered in order to calculate the air exchange rates of indoor environments. Indeed, regarding air quality, the presence of pleasant or unpleasant odours can strongly influence the environmental comfort. In this paper, a case study of indoor air quality monitoring is reported. The indoor field tests were conducted measuring both CO2 concentration, using a photoacoustic multi-gas analyzer, and odour trends, using an electronic nose, in order to analyze and compare the information acquired. The indoor air monitoring campaign was run for a period of 20 working days into a university room. The work was focused on the determination of both CO2 and odour emission factors (OEF) emitted by the human activity and on the evaluation of the odour impact in a naturally ventilated room. The results highlighted that an air monitoring and recycling system based only on CO2 concentration and temperature measurements might be insufficient to ensure a good indoor air quality, whereas its performances could be improved by integrating the existing systems with an electronic nose for odour detection
