    Review of economic evidence in the prevention and early detection of colorectal cancer.

    This paper aims to systematically review the cost-effectiveness evidence, and to provide a critical appraisal of the methods used in the model-based economic evaluation of CRC screening and subsequent surveillance. A search strategy was developed to capture relevant evidence published 1999-November 2012. Databases searched were MEDLINE, EMBASE, National Health Service Economic Evaluation (NHS EED), EconLit, and HTA. Full economic evaluations that considered costs and health outcomes of relevant intervention were included. Sixty-eight studies which used either cohort simulation or individual-level simulation were included. Follow-up strategies were mostly embedded in the screening model. Approximately 195 comparisons were made across different modalities; however, strategies modelled were often simplified due to insufficient evidence and comparators chosen insufficiently reflected current practice/recommendations. Studies used up-to-date evidence on the diagnostic test performance combined with outdated information on CRC treatments. Quality of life relating to follow-up surveillance is rare. Quality of life relating to CRC disease states was largely taken from a single study. Some studies omitted to say how identified adenomas or CRC were managed. Besides deterministic sensitivity analysis, probabilistic sensitivity analysis (PSA) was undertaken in some studies, but the distributions used for PSA were rarely reported or justified. The cost-effectiveness of follow-up strategies among people with confirmed adenomas are warranted in aiding evidence-informed decision making in response to the rapidly evolving technologies and rising expectations

    Phase and Intensity Distributions of Individual Pulses of PSR B0950+08

    The distribution of the intensities of individual pulses of PSR B0950+08 as a function of the longitudes at which they appear is analyzed. The flux density of the pulsar at 111 MHz varies strongly from day to day (by up to a factor of 13) due to the passage of the radiation through the interstellar plasma (interstellar scintillation). The intensities of individual pulses can exceed the amplitude of the mean pulse profile, obtained by accumulating 770 pulses, by more than an order of magnitude. The intensity distribution along the mean profile is very different for weak and strong pulses. The differential distribution function for the intensities is a power law with index n = -1.1 +- 0.06 up to peak flux densities for individual pulses of the order of 160 Jy

If their sole justification is having the bulk of their recommendations implemented, the institution probably would have died out long ago. Nevertheless, it must be of considerable concern to commissioners that the record has not been good in terms of implementation and one of the questions that perhaps will determine how successful commissions are in this respect has to do with what happens when the report is delivered and thereafter. These questions will be largely the subject of the panel that we are about to present. You will not be treated to profound, scholarly papers on policy analysis or anything like that in this panel. There will be practical comments and suggestions

    A New Look at Mode Conversion in a Stratified Isothermal Atmosphere

    Recent numerical investigations of wave propagation near coronal magnetic null points (McLaughlin and Hood: Astron. Astrophys. 459, 641,2006) have indicated how a fast MHD wave partially converts into a slow MHD wave as the disturbance passes from a low-beta plasma to a high-beta plasma. This is a complex process and a clear understanding of the conversion mechanism requires the detailed investigation of a simpler model. An investigation of mode conversion in a stratified, isothermal atmosphere, with a uniform, vertical magnetic field is carried out, both numerically and analytically. In contrast to previous investigations of upward-propagating waves (Zhugzhda and Dzhalilov: Astron. Astrophys. 112, 16, 1982a; Cally: Astrophys. J. 548, 473, 2001), this paper studies the downward propagation of waves from a low-beta to high-beta environment. A simple expression for the amplitude of the transmitted wave is compared with the numerical solution.Comment: 14 pages, 6 figure

    In 1953 Iowa State College and the Tennessee Valley Authority undertook a research project to determine (1) characteristics of Iowa farmers who use varying amounts of fertilizer and different fertilization practices, (2) informational sources important in the acceptance and use of fertilizer, (3) extent of use and handling of fertilizer on the farm and (4) quantitative relationships between fertilizer use and specified factors considered to influence fertilizer use.     Barriers to avoiding fast-food consumption in an environment supportive of unhealthy eating

    Objective To investigate factors (ability, motivation and the environment) that act as barriers to limiting fast-food consumption in women who live in an environment that is supportive of poor eating habits.Design Cross-sectional study using self-reports of individual-level data and objectively measured environmental data. Multilevel logistic regression was used to assess factors associated with frequency of fast-food consumption.Setting Socio-economically disadvantaged areas in metropolitan Melbourne, Australia.Subjects Women (n 932) from thirty-two socio-economically disadvantaged neighbourhoods living within 3 km of six or more fast-food restaurants. Women were randomly sampled in 2007–2008 as part of baseline data collection for the Resilience for Eating and Activity Despite Inequality (READI) study.Results Consuming low amounts of fast food was less likely in women with lower perceived ability to shop for and cook healthy foods, lower frequency of family dining, lower family support for healthy eating, more women acquaintances who eat fast food regularly and who lived further from the nearest supermarket. When modelled with the other significant factors, a lower perceived shopping ability, mid levels of family support and living further from the nearest supermarket remained significant. Among those who did not perceive fruits and vegetables to be of high quality, less frequent fast-food consumption was further reduced for those with the lowest confidence in their shopping ability.Conclusions Interventions designed to improve women\u27s ability and opportunities to shop for healthy foods may be of value in making those who live in high-risk environments better able to eat healthily

    Reprogramming the Maternal Zebrafish Genome after Fertilization to Match the Paternal Methylation Pattern

    SummaryEarly vertebrate embryos must achieve totipotency and prepare for zygotic genome activation (ZGA). To understand this process, we determined the DNA methylation (DNAme) profiles of zebrafish gametes, embryos at different stages, and somatic muscle and compared them to gene activity and histone modifications. Sperm chromatin patterns are virtually identical to those at ZGA. Unexpectedly, the DNA of many oocyte genes important for germline functions (i.e., piwil1) or early development (i.e., hox genes) is methylated, but the loci are demethylated during zygotic cleavage stages to precisely the state observed in sperm, even in parthenogenetic embryos lacking a replicating paternal genome. Furthermore, this cohort constitutes the genes and loci that acquire DNAme during development (i.e., ZGA to muscle). Finally, DNA methyltransferase inhibition experiments suggest that DNAme silences particular gene and chromatin cohorts at ZGA, preventing their precocious expression. Thus, zebrafish achieve a totipotent chromatin state at ZGA through paternal genome competency and maternal genome DNAme reprogramming
