    The aim of this study was to investigate a three-year development in BMI and physical fitness of schoolchildren aged 12-16 years with extreme weight status. Taiwan Physical Fitness Test Battery, assessing aerobic fitness (1600-meter walk/run test), power (standing long jump), muscular endurance (sit-up), and flexibility (sit-and-reach) in 16,945 boys, was implemented in September from the year 2006 till 2008. Overweight and underweight were defined by the baseline BMI data values that fall within the highest and lowest 5% of their age population, whereas the BMI data values that fall within one standard deviation of the mean was considered ā€œnormalā€ in this study. The results showed that BMI of schoolchildren in 2006 was ~2ā€“3 kg/m2 above the national average reported in 1993. All physical fitness components in the overweight group were substantially poorer than those in the normal group. Yet, these fitness parameters were improved over the three years in all groups. BMI in the underweight group increased at a faster rate than that in the normal and overweight groups. No difference was found in the jumping distance between the underweight and normal groups. Aerobic fitness in the underweight group was superior but flexibility and muscular endurance were slightly lower than those in the normal group. BMI of Taiwan schoolchildren increased substantially from 1993 to 2006 but leveled off from 2006 to 2008. Underweight schoolchildren during the growing period should not a priori be considered as physically weaker or unfit individuals.Cilj je ovog istraživanja bio ustanoviti trogodiÅ”nji trend razvoja indeksa tjelesne mase (ITM) i tjelesne pripremljenosti u učenika, u dobi između 12 i 16 godina, s ekstremnim vrijednostima tjelesne težine. Tajvanski sklop testova za tjelesnu pripremljenosti (Taiwan Physical Fitness Test Battery), koja uključuje procjenu aerobne pripremljenosti (test hodanja/ trčanja na 1600 metara), eksplozivne snage (skok u dalj s mjesta), miÅ”ićne izdržljivosti (podizanje trupa iz ležanja na leđima) i fleksibilnosti (sjed-i-dohvat), primijenjen je od 2006 do 2008 godine u mjesecu rujnu na uzorku od 16.945 dječaka. PreteÅ”ki (overweight) i prelagani (underweight) ispitanici bili su definirani prema inicijalnoj vrijednosti ITM-a za dobnu skupinu populacije ispitanika kao oni koji se ubrajaju među 5% s najviÅ”im odnosno najnižim vrijednostima ITM. Ispitanici koji su se nalazili unutar jedne standardne devijacije smatrali su se ā€žnormalnimaā€œ. Rezultati su pokazali da je ITM tajvanskih učenika u 2006. godini bio za ~2ā€“3kg/m2 veći od nacionalnog prosjeka objavljenog 1993. godine. Sve varijable tjelesne pripremljenosti u grupi prekomjerno teÅ”kih bile su značajno loÅ”ije u odnosu na rezultate koje su postigli ispitanici u normalnoj grupi. Ipak, rezultati su se u svim mjerenim varijablama u svim grupama poboljÅ”ali tijekom 3 godine mjerenja. ITM je u grupi nedovoljno teÅ”kih porastao većom brzinom nego u grupama normalnih i prekomjerno teÅ”kih učenika. Nije utvrđena značajna razlika u rezultatima skoka u dalj s mjesta između grupa nedovoljno teÅ”kih i normalnih ispitanika. Nedovoljno teÅ”ki ispitanici imali su najbolje rezultate u aerobnoj izdržljivosti u odnosu na ostale grupe, dok su u fleksibilnosti i miÅ”ićnoj izdržljivosti bili neÅ”to slabiji nego ispitanici u normalnoj grupi. ITM je u tajvanske djece značajno porastao u razdoblju od 1993. do 2006. godine, ali je i stagnirao u razdoblju od 2006. do 2008. godine.     Percutaneous Endoscopic Gastrostomy in the Enteral Feeding of the Elderly

    SummaryToday we are faced with an aging society that may develop malnutrition because of dysphagia related to dementia, stroke, and malignancy seen often in the elderly. The preferred form of nutritional supplementation for this group is enteral nutrition, and the most appropriate long-term method is by use of a gastrostomy. Percutaneous endoscopic gastrostomy (PEG) was first introduced in 1980 as an alternative to the traditional operative procedure and rapidly became the preferred procedure. In geriatric patients, the principal indications are neurological dysphagia and malnutrition, related to an underlying disease or anorexia-cachexia in very elderly. PEG is contraindicated in the presence of respiratory distress, previous gastric resection, total esophageal obstruction, coagulation disorders and sepsis in the elderly. Common complications include wound infection, leakage, hemorrhage, and fistula in the general population, but aspiration pneumonia is the major case of death in this group. Risks and complications of PEG must be discussed with patients and their families; and the decision for percutaneous endoscopic gastrostomy insertion should only be made after careful consideration and discussion between managing physicians, allied health professionals, and the patient and/or family. Four ethical principles may help make feeding decisions: beneficence, non-maleficence, autonomy and justice. Attentive long-term care after tube replacement is mandatory. Acceptance of percutaneous endoscopic gastrostomy placement by patients and their families tends to increase once favorable outcomes are offered

    Counteracting UDP flooding attacks in SDN

    Software-defined networking (SDN) is a new networking architecture with a centralized control mechanism. SDN has proven to be successful in improving not only the network performance, but also security. However, centralized control in the SDN architecture is associated with new security vulnerabilities. In particular, user-datagram-protocol (UDP) flooding attacks can be easily launched and cause serious packet-transmission delays, controller-performance loss, and even network shutdown. In response to applications in the Internet of Things (IoT) field, this study considers UDP flooding attacks in SDN and proposes two lightweight countermeasures. The first method sometimes sacrifices address-resolution-protocol (ARP) requests to achieve a high level of security. In the second method, although packets must sometimes be sacrificed when undergoing an attack before starting to defend, the detection of the network state can prevent normal packets from being sacrificed. When blocking a network attack, attacks from the affected port are directly blocked without affecting normal ports. The performance and security of the proposed methods were confirmed by means of extensive experiments. Compared with the situation where no defense is implemented, or similar defense methods are implemented, after simulating a UDP flooding attack, our proposed method performed better in terms of the available bandwidth, centralprocessing-unit (CPU) consumption, and network delay time

Under UV irradiation, the rates of the oxidative polymerization of EDOT monomer along with the reduction of Pt4+ ions were accelerated. In addition, the morphology of PtNPs was also greatly influenced by the UV irradiation; the size of PtNPs was reduced under UV irradiation, which can be attributed to the faster nucleation rate. The immobilized PtNPs showed excellent electrocatalytic activities towards the electroreduction of hydrogen peroxide. The resultant amperometric sensor showed enhanced sensitivity for the detection of H2O2 as compared to that without PtNPs, i.e., only with a layer of PEDOT. Amperometric determination of H2O2 at -0.55 V gave a limit of detection of 1.6 Ī¼M (S / Nā€‰=ā€‰3) and a sensitivity of 19.29 mA cm-2 M-1 up to 6 mM, with a response time (steady state, t95) of 30 to 40 s. Energy dispersive X-ray analysis, transmission electron microscopic image, cyclic voltammetry (CV), and scanning electron microscopic images were utilized to characterize the modified electrode.     SummaryHuman Ī²-glucuronidase, due to low intrinsic immunogenicity in humans, is an attractive enzyme for tumor-specific prodrug activation, but its utility is hindered by low activity at physiological pH. Here we describe the development of a high-throughput screening procedure for enzymatic activity based on the stable retention of fluorescent reaction product in mammalian cells expressing properly folded glycoproteins on their surface. We utilized this procedure on error-prone PCR and saturation mutagenesis libraries to isolate Ī²-glucuronidase tetramers that were up to 60-fold more active (kcat/Km) at pH 7.0 and were up to an order of magnitude more effective at catalyzing the conversion of two structurally disparate glucuronide prodrugs to anticancer agents. The screening procedure described here can facilitate investigation of eukaryotic enzymes requiring posttranslational modifications for biological activity

    A pre-S gene chip to detect pre-S deletions in hepatitis B virus large surface antigen as a predictive marker for hepatoma risk in chronic hepatitis B virus carriers

    <p>Abstract</p> <p>Background</p> <p>Chronic hepatitis B virus (HBV) infection is an important cause of hepatocellular carcinoma (HCC) worldwide. The pre-S<sub>1 </sub>and -S<sub>2 </sub>mutant large HBV surface antigen (LHBS), in which the pre-S<sub>1 </sub>and -S<sub>2 </sub>regions of the LHBS gene are partially deleted, are highly associated with HBV-related HCC.</p> <p>Methods</p> <p>The pre-S region of the LHBS gene in two hundred and one HBV-positive serum samples was PCR-amplified and sequenced. A pre-S oligonucleotide gene chip was developed to efficiently detect pre-S deletions in chronic HBV carriers. Twenty serum samples from chronic HBV carriers were analyzed using the chip.</p> <p>Results</p> <p>The pre-S deletion rates were relatively low (7%) in the sera of patients with acute HBV infection. They gradually increased in periods of persistent HBV infection: pre-S mutation rates were 37% in chronic HBV carriers, and as high as 60% in HCC patients. The Pre-S Gene Chip offers a highly sensitive and specific method for pre-S deletion detection and is less expensive and more efficient (turnaround time 3 days) than DNA sequencing analysis.</p> <p>Conclusion</p> <p>The pre-S<sub>1/2 </sub>mutants may emerge during the long-term persistence of the HBV genome in carriers and facilitate HCC development. Combined detection of pre-S mutations, other markers of HBV replication, and viral titers, offers a reliable predictive method for HCC risks in chronic HBV carriers.</p

    Serum Bone Resorption Markers after Parathyroidectomy for Renal Hyperparathyroidism: Correlation Analyses for the Cross-Linked N-telopeptide of Collagen I and Tartrate-Resistant Acid Phosphatase

    Patients on long-term dialysis may develop secondary hyperparathyroidism (SHPT) with increased serum concentrations of bone resorption markers such as the cross-linked N-telopeptide of type I collagen (NTX) and type-5b tartrate-resistant acid phosphatase (TRAP). When SHPT proves refractory to treatment, parathyroidectomy (PTX) may be needed. Renal patients on maintenance HD who received PTX for refractory SHPT (n=23) or who did not develop refractory SHPT (control subjects; n=25) were followed prospectively for 4 weeks. Serum intact parathyroid hormone (iPTH), NTX, TRAP, and bone alkaline phosphatase (BAP) concentrations were measured serially and correlation analyses were performed. iPTH values decreased rapidly and dramatically. BAP values increased progressively with peak increases observed at 2 weeks after surgery. NTX and TRAP values decreased concurrently and progressively through 4 weeks following PTX. A significant correlation between TRAP and NTX values was observed before PTX but not at 4 weeks after PTX. Additionally, the fractional changes in serum TRAP were larger than those in serum NTX at all times examined after PTX. Serum iPTH, TRAP, and NTX values declined rapidly following PTX for SHPT. Serum TRAP values declined to greater degrees than serum NTX values throughout the 4-week period following PTX
