    AbstractObjective Viscosupplementation with hylan G-F 20 has recently become registered for treatment of patients with osteoarthritis (OA) of the knee in most parts of the world. The cost effectiveness and cost utility of this new therapeutic modality were determined as part of a Canadian prospective, randomized, 1-year, open-label, multicentered trial.Design A total of 255 patients were randomized to ‘Appropriate care with hylan G-F 20’ (AC+H) or ‘Appropriate care without hylan G-F 20’ (AC). Costs (1999 Canadian dollars) were collected from the societal viewpoint and included all costs related to OA of the knee and OA in all joints. Patients completed a number of outcomes questionnaires including the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) and the Health Utilities Index Mark 3 (HUI3). Data were collected at clinic visits (baseline, 12 months) and by telephone (1, 2, 4, 6, 8, 10, and 12 months).Results The AC+H group over the year had higher costs (2125–2125–1415=710,P<0.05),morepatientsimproved(69710, P< 0.05), more patients improved (69%–40%=29%,P =0.0001), greater increases in HUI3 (0.13–0.03=0.10, P< 0.0001) and increased quality-adjusted life years (QALYs) (0.071, P< 0.05). The incremental cost-effectiveness ratio was 2505/patient improved. The incremental cost–utility ratio was $10000/QALY gained. Sensitivity analyses and a second cost perspective gave similar results.Conclusion The cost–utility ratio is below the suggested Canadian adoption threshold. The results provide strong evidence for adoption of treatment with hylan G-F 20 in the patients and settings studied in the trial. Copyright 2002 OsteoArthritis Research Society International.     We investigate the entanglement produced by a multi-path interferometer that is composed of two symmetric multiports, with phase shifts applied to the output of the first multiport. Particular attention is paid to the case when we have a single photon entering the interferometer. For this situation we derive a simple condition that characterize the types of entanglement that one can generate.     Selecting and modelling remnant AGNs with limited spectral coverage

    Quantifying the energetics and lifetimes of remnant radio-loud active galactic nuclei (AGNs) is much more challenging than for active sources due to the added complexity of accurately determining the time since the central black hole switched off. Independent spectral modelling of remnant lobes enables the derivation of the remnant ratio, Rrem (i.e. 'off-time/source age'); however, the requirement of high-frequency (≳5 GHz) coverage makes the application of this technique over large-area radio surveys difficult. In this work, we propose a new method, which relies on the observed brightness of backflow of Fanaroff-Riley type II lobes, combined with the Radio AGN in Semi-Analytic Environments (RAiSE) code, to measure the duration of the remnant phase. Sensitive radio observations of the remnant radio galaxy J2253-34 are obtained to provide a robust comparison of this technique with the canonical spectral analysis and modelling methods. We find that the remnant lifetimes modelled by each method are consistent; spectral modelling yields Rrem = 0.23 ± 0.02, compared to Rrem = 0.26 ± 0.02 from our new method. We examine the viability of applying our proposed technique to low-frequency radio surveys using mock radio source populations, and examine whether the technique is sensitive to any intrinsic properties of radio AGNs. Our results show that the technique can be used to robustly classify active and remnant populations, with the most confident predictions for the remnant ratio, and thus off-time, in the longest lived radio sources (>50 Myr) and those at higher redshifts (z > 0.1)

    Extended Gari-Krumpelmann model fits to nucleon electromagnetic form factors

    Nucleon electromagnetic form factor data (including recent data) is fitted with models that respect the confinement and asymptotic freedom properties of QCD. Gari-Krumpelmann (GK) type models, which include the major vector meson pole contributions and at high momentum transfer conform to the predictions of perturbative QCD, are combined with Hohler-Pietarinen (HP) models, which also include the width of the rho meson and the addition of higher mass vector meson exchanges, but do not evolve into the explicit form of PQCD at high momentum transfer. Different parameterizations of the GK model's hadronic form factors, the effect of including the width of the rho meson and the addition of the next (in mass) isospin 1 vector meson are considered. The quality of fit and the consistency of the parameters select three of the combined HP/GK type models. Projections are made to the higher momentum transfers which are relevant to electron-deuteron experiments. The projections vary little for the preferred models, removing much of the ambiguity in electron-nucleus scattering predictions.Comment: 18pp, 7 figures, using RevTeX with BoxedEPS macros; 1 new figure, minor textual changes; email correspondence to [email protected]

    Effect of recent R_p and R_n measurements on extended Gari-Krumpelmann model fits to nucleon electromagnetic form factors

    The Gari-Krumpelmann (GK) models of nucleon electromagnetic form factors, in which the rho, omega, and phi vector meson pole contributions evolve at high momentum transfer to conform to the predictions of perturbative QCD (pQCD), was recently extended to include the width of the rho meson by substituting the result of dispersion relations for the pole and the addition of rho' (1450) isovector vector meson pole. This extended model was shown to produce a good overall fit to all the available nucleon electromagnetic form factor (emff) data. Since then new polarization data shows that the electric to magnetic ratios R_p and R_n obtained are not consistent with the older G_{Ep} and G_{En} data in their range of momentum transfer. The model is further extended to include the omega' (1419) isoscalar vector meson pole. It is found that while this GKex cannot simultaneously fit the new R_p and the old G_{En} data, it can fit the new R_p and R_n well simultaneously. An excellent fit to all the remaining data is obtained when the inconsistent G_{Ep} and G_{En} is omitted.
