Monitoring osteoarthritis: a cross-sectional survey in general practice
<b>Background</b> Despite being a highly prevalent chronic condition managed predominantly in primary care and unlike other chronic conditions, osteoarthritis (OA) care is delivered on an ad hoc basis rather than through routine structured review. Evidence suggests current levels of OA care are suboptimal, but little is known about what general practitioners' (GPs) consider important in OA care, and, thus, the scope to improve inconsistency or poor practice is, at present, limited.<p></p>
<b>Objectives</b> We investigated GPs' views on and practice of monitoring OA. <p></p>
<b>Methods</b> This was a cross-sectional postal survey of 2500 practicing UK GPs randomly selected from the Binley's database. Respondents were asked if monitoring OA patients was important and how monitoring should be undertaken.<p></p>
<b>Results</b> Responses were received from 768 GPs of whom 70.8% were male and 89.5% were principals within their practices. Despite 55.4% (n = 405) indicating monitoring patients with OA was important and 78.3% (n = 596) considering GPs the appropriate professionals to monitor OA, only 15.2% (n = 114) did so routinely, and 45% (n = 337) did not monitor any OA patients at all. In total, 61.4% (n = 463) reported that patients should self-monitor. Respondents favored monitoring physical function, pain, and analgesia use over monitoring measures of BMI, self management plans, and exercise advice.<p></p>
<b>Conclusions</b> The majority of respondents felt that monitoring OA was important, but this was not reflected in their reported current practice. Much of what they favored for monitoring was in line with published guidance, suggesting provision of suboptimal care does not result from lack of knowledge and interventions to improve OA care must address barriers to GPs engaging in optimal care provision
Discussing prognosis with patients with osteoarthritis: a cross-sectional survey in general practice
Osteoarthritis is a leading cause of chronic pain and disability and one of the most common conditions diagnosed and managed in primary care. Despite the evidence that patients would value discussions about the course of osteoarthritis to help them make informed treatment decisions and plan for the future, little is known of GPs’ practice of, or views regarding, discussing prognosis with these patients. A cross-sectional postal survey asked 2500 randomly selected UK GPs their views on discussing prognosis with patients with osteoarthritis and potential barriers or facilitators to such discussions. They were also asked if prognostic discussions were part of their current practice and what indicators they considered important in assessing the prognosis associated with osteoarthritis. Of 768 respondents (response rate 30.7 %), the majority felt it necessary to discuss prognosis with osteoarthritis patients (n = 738, 96.1 %), but only two thirds reported that it was part of their routine practice (n = 498, 64.8 %). Most respondents found predicting the course of osteoarthritis (n = 703, 91.8 %) and determining the prognosis of patients difficult (n = 589, 76.7 %). Obesity, level of physical disability and pain severity were considered the most important prognostic indicators in osteoarthritis. Although GPs consider prognostic discussions necessary for patients with osteoarthritis, few prioritise these discussions. Lack of time and perceived difficulties in predicting the disease course and determining prognosis for patients with osteoarthritis may be barriers to engaging in prognostic discussions. Further research is required to identify ways to assist GPs making prognostic predictions for patients with osteoarthritis and facilitate engagement in these discussions

Photoemission Spectroscopy from Inhomogeneous Models of Cuprates
We investigate the electronic dynamics in the underdoped cuprates focusing on
the effects of one-dimensional charge stripes. We address recent experimental
Angular-Resolved Photoemission Spectra results on
(LaNdSr)CuO. We find that various inhomogeneous
models can account for the distribution of quasiparticle weights close to
momentum and symmetry related points. The observed flat
dispersion region around the same point can only be addressed by
certain classes of those inhomogeneous models which locally break spin
symmetry. Homogeneous models including hopping elements up to second neighbors
cannot reproduce the experimental quasiparticle weight, since most of it is

From text summarisation to style-specific summarisation for broadcast news
In this paper we report on a series of experiments investigating the path from text summarisation to style-specific summarisation of spoken news stories. We show that the portability of traditional text summarisation features to broadcast news is dependent on the diffusiveness of the information in the broadcast news story. An analysis of two categories of news stories (containing only read speech or including some spontaneous speech) demonstrates the importance of the style and the
quality of the transcript, when extracting the summary-worthy information content. Further experiments indicate the advantages of doing

Early phonological and sociocognitive skills as predictors of later language and social communication outcomes
Background: Previous studies of outcome for children with early language delay have focused on measures of early language as predictors of language outcome. This study investigates whether very early processing skills (VEPS) known to underpin language development will be better predictors of specific language and social communication outcomes than measures of language itself.
Method: Participants were 163 children referred to clinical services with concerns about language at 2;6–3;6 years and followed up at 4–5 years. Novel assessments of phonological and sociocognitive processing were administered at Time 1 (T1), together with a standardised test of receptive and expressive language, and parental report of expressive vocabulary. The language test was re-administered at Time 2 (T2), together with assessments of morphosyntax and parental reports of social communication.
Results: Intercorrelations at and between T1 and T2 were high, and dissociations were rare. Ordinal regressions were run, entering predictors singly and simultaneously. With the exception of the phonological task, every early measure on its own was significantly predictive of most outcomes, and receptive language was the strongest all-round predictor. Results of simultaneous entry, controlling for the effect of other predictors, showed that early language was the strongest predictor of general language outcome, but early phonology was the strongest predictor of a measure of morphosyntax, and early sociocognition the strongest predictor of social communication.
Conclusions: Language measures which draw on a wide range of skills were the strongest overall predictors of general language outcomes. However, our VEPS measures were stronger predictors of specific outcomes. The clinical and theoretical implications of these findings are discussed
Affective Man-Machine Interface: Unveiling human emotions through biosignals
As is known for centuries, humans exhibit an electrical profile. This profile is altered through various psychological and physiological processes, which can be measured through biosignals; e.g., electromyography (EMG) and electrodermal activity (EDA). These biosignals can reveal our emotions and, as such, can serve as an advanced man-machine interface (MMI) for empathic consumer products. However, such a MMI requires the correct classification of biosignals to emotion classes. This chapter starts with an introduction on biosignals for emotion detection. Next, a state-of-the-art review is presented on automatic emotion classification. Moreover, guidelines are presented for affective MMI. Subsequently, a research is presented that explores the use of EDA and three facial EMG signals to determine neutral, positive, negative, and mixed emotions, using recordings of 21 people. A range of techniques is tested, which resulted in a generic framework for automated emotion classification with up to 61.31% correct classification of the four emotion classes, without the need of personal profiles. Among various other directives for future research, the results emphasize the need for parallel processing of multiple biosignals
Rabies in southern Africa
The first confirmed outbreak of rabies in Africa, believed to have followed the importation of an infected
dog from England in 1892, occurred in the eastern Cape Province of South Africa, and was brought under
control in 1894. An unconfirmed epidemic of rabies in dogs occurred in western Zambia in 1901. By the
following year the disease had apparently spread along a major trade route, to cause an outbreak in
Zimbabwe which engulfed most of the country before being eradicated in 1913. The existence of endemic
rabies of viverrids (mongooses and genets) was confirmed in South Africa in 1928, and since then the
viverrid disease has continued to occur widely on the interior plateau of the country with spill-over of
infection to cattle and a variety of other animals. From about 1947 onwards, an invasive form of dog
rabies spread from southern Zambia and/or Angola into Namibia, across northern and eastern Botswana
into Zimbabwe and the northern Transvaal by 1950, entered Mozambique in 1952, and spread from there
to Swaziland in 1954. Dog rabies extended from southern Mozambique into Natal in 1961 to cause a
major epidemic which was brought under control in 1968. The disease re-entered northern Natal from
Mozambique in 1976 and since then dog rabies has proved difficult to control in the peri-urban settlements
of Natal-KwaZulu. The disease spread from Natal to Lesotho in 1982, and into the Transkei region of the
eastern Cape Province in 1987, to reach the Ciskei by 1990. The spread of the disease in dogs was
followed by the emergence of rabies of jackals and cattle in central Namibia, northern Botswana, Zimbabwe
and the northern Transvaal. A unique outbreak of rabies in kudu antelope occurred in central Namibia
from 1977 to 1985, apparently involving oral spread of infection between individuals. A few cases of rabies
in the bat-eared fox were recognized each year in Namibia from 1967 onwards, and from the 1970s the
occurrence of the disease in the fox has emerged as a distinct problem in the northern Cape Province
and spread to the west coast. The rabies-related viruses, Lagos bat, Mokola and Duvenhage, associated
with bats, shrews and rodents in Africa, are known to have caused isolated cases of disease in South Africa,
and on one occasion a small outbreak involving six cats and a dog in Bulawayo, Zimbabwe. However,
the results of monoclonal antibody tests on numerous specimens indicate that the rabies-related

Rho GTPase function in flies: insights from a developmental and organismal perspective.
Morphogenesis is a key event in the development of a multicellular organism and is reliant on coordinated transcriptional and signal transduction events. To establish the segmented body plan that underlies much of metazoan development, individual cells and groups of cells must respond to exogenous signals with complex movements and shape changes. One class of proteins that plays a pivotal role in the interpretation of extracellular cues into cellular behavior is the Rho family of small GTPases. These molecular switches are essential components of a growing number of signaling pathways, many of which regulate actin cytoskeletal remodeling. Much of our understanding of Rho biology has come from work done in cell culture. More recently, the fruit fly Drosophila melanogaster has emerged as an excellent genetic system for the study of these proteins in a developmental and organismal context. Studies in flies have greatly enhanced our understanding of pathways involving Rho GTPases and their roles in development
