The Quiet-Sun Photosphere and Chromosphere
The overall structure and the fine structure of the solar photosphere outside
active regions are largely understood, except possibly important roles of a
turbulent near-surface dynamo at its bottom, internal gravity waves at its top,
and small-scale vorticity. Classical 1D static radiation-escape modelling has
been replaced by 3D time-dependent MHD simulations that come closer to reality.
The solar chromosphere, in contrast, remains ill-understood although its
pivotal role in coronal mass and energy loading makes it a principal research
area. Its fine structure defines its overall structure, so that hard-to-observe
and hard-to-model small-scale dynamical processes are the key to understanding.
However, both chromospheric observation and chromospheric simulation presently
mature towards the required sophistication. The open-field features seem of

Convective Dynamos and the Minimum X-ray Flux in Main Sequence Stars
The objective of this paper is to investigate whether a convective dynamo can
account quantitatively for the observed lower limit of X-ray surface flux in
solar-type main sequence stars. Our approach is to use 3D numerical simulations
of a turbulent dynamo driven by convection to characterize the dynamic
behavior, magnetic field strengths, and filling factors in a non-rotating
stratified medium, and to predict these magnetic properties at the surface of
cool stars. We use simple applications of stellar structure theory for the
convective envelopes of main-sequence stars to scale our simulations to the
outer layers of stars in the F0--M0 spectral range, which allows us to estimate
the unsigned magnetic flux on the surface of non-rotating reference stars. With
these estimates we use the recent results of \citet{Pevtsov03} to predict the
level of X-ray emission from such a turbulent dynamo, and find that our results
compare well with observed lower limits of surface X-ray flux. If we scale our
predicted X-ray fluxes to \ion{Mg}{2} fluxes we also find good agreement with
the observed lower limit of chromospheric emission in K dwarfs. This suggests
that dynamo action from a convecting, non-rotating plasma is a viable
alternative to acoustic heating models as an explanation for the basal emission
level seen in chromospheric, transition region, and coronal diagnostics from

Locally recurrent rectal cancer and distant metastases:is there still a chance ofcure?
Introduction: Patients with locally recurrent rectal cancer (LRRC) generally have poor prognosis, especially those who have (a history of) distant metastases. The aim of this study was to investigate the impact of distant metastases on oncological outcomes in LRRC patients undergoing curative treatment. Methods: Consecutive patients with surgically treated LRRC between 2005 and 2019 in two tertiary referral hospitals were retrospectively analysed. Oncological survival of patients without distant metastases were compared with outcomes of patients with synchronous distant metastases with the primary tumour, patients with distant metastases in the primary-recurrence interval, and patients with synchronous LRRC distant metastases. Results: A total of 535 LRRC patients were analysed, of whom 398 (74%) had no (history of) metastases, 22 (4%) had synchronous metastases with the primary tumour, 44 (8%) had metachronous metastases, and 71 (13%) had synchronous LRRC metastases. Patients with synchronous LRRC metastases had worse survival compared to patients without metastases (adjusted hazard ratio: 1.56 [1.15â2.12]), whilst survival of patients with synchronous primary metastases and metachronous metastases of the primary tumour was similar as those patients who had no metastases. In LRRC patients who had metastases in primary-recurrence interval, patients with early metachronous metastases had better disease-free survival as patients with late metachronous metastases (3-year disease-free survival: 48% vs 22%, p = 0.039). Conclusion: LRRC patients with synchronous distant metastases undergoing curative surgery have relatively poor prognosis. However, LRRC patients with a history of distant metastases diagnosed nearby the primary tumour have comparable (oncological) survival as LRRC patients without distant metastases.</p
HST Observations of Chromospheres in Metal Deficient Field Giants
HST high resolution spectra of metal-deficient field giants more than double
the stars in previous studies, span about 3 magnitudes on the red giant branch,
and sample an abundance range [Fe/H]= -1 to -3. These stars, in spite of their
age and low metallicity, possess chromospheric fluxes of Mg II (2800 Angstrom)
that are within a factor of 4 of Population I stars, and give signs of a
dependence on the metal abundance at the lowest metallicities. The Mg II k-line
widths depend on luminosity and correlate with metallicity. Line profile
asymmetries reveal outflows that occur at lower luminosities (M_V = -0.8) than
detected in Ca K and H-alpha lines in metal-poor giants, suggesting mass
outflow occurs over a larger span of the red giant branch than previously
thought, and confirming that the Mg II lines are good wind diagnostics. These
results do not support a magnetically dominated chromosphere, but appear more
consistent with some sort of hydrodynamic, or acoustic heating of the outer

The system parameters of DW Ursae Majoris
We present new constraints on the system parameters of the SW Sextantis star
DW Ursae Majoris, based on ultraviolet (UV) eclipse observations with the
Hubble Space Telescope. Our data were obtained during a low state of the
system, in which the UV light was dominated by the hot white dwarf (WD)
primary. Eclipse analysis, using the full Roche lobe geometry, allows us to set
firm limits on the masses and radii of the system components and the distance
between them: 0.67 \leq M_1/M_sun \leq 1.06, 0.008 \leq R_1/R_sun \leq 0.014,
M_2/M_sun > 0.16, R_2/R_sun > 0.28 and a/R_sun > 1.05. For q = M_2/M_1 < 1.5
the inclination must satisfy i > 71 degrees. Using Smith & Dhillon's
mass-period relation for CV secondaries, our estimates for the system
parameters become M_1/M_sun = 0.77 \pm 0.07, R_1/R_sun = 0.012 \pm 0.001,
M_2/M_sun = 0.30 \pm 0.10, R_2/R_sun = 0.34 \pm 0.04, q =0.39 \pm 0.12, i = 82
\pm 4 degrees and a/R_sun = 1.14 \pm 0.06. We have also estimated the spectral
type of the secondary, M3.5 \pm 1.0, and distance to the system, d =930 \pm 160
pc, from time-resolved I- and K-band photometry. Finally, we have repeated
Knigge et al.'s WD model atmosphere fit to the low-state UV spectrum of DW UMa
in order to account for the higher surface gravity indicated by our eclipse
analysis. In this way we obtained a second estimate for the distance, d = 590
\pm 100 pc, which allows us to obtain a second estimate for the spectral type
of the secondary, M7 \pm 2.0. We conclude that the true value for the distance
and spectral type will probably be in between the values obtained by the two

Diagnostic value of a heart-type fatty acid-binding protein (H-FABP) bedside test in suspected acute coronary syndrome in primary care
AbstractBackgroundTo determine the diagnostic accuracy of a rapid heart-type fatty acid-binding protein (H-FABP) test in patients suspected of acute coronary syndrome (ACS) in primary care.MethodsGeneral practitioners included 298 patients suspected of ACS. In all patients, whether referred to hospital or not, ECG and cardiac biomarker testing was performed. ACS was determined in accordance with international guidelines. Multivariate analysis was used to determine the value of H-FABP in addition to clinical findings.ResultsMean patient age was 66years (SD 14), 52% was female and 66 patients (22%) were diagnosed with ACS. The H-FABP bedside test was performed within 24h (median 3.1, IQR 1.5 to 7.1) after symptom onset. The positive predictive value (PPV) of H-FABP was 65% (95% confidence interval (CI) 50â78). The negative predictive value (NPV) was 85% (95% CI 80â88). Sensitivity was 39% (29â51%) and specificity 94% (90â96%). Within 6h after symptom onset, the PPV was 72% (55â84) and the NPV was 83% (77â88), sensitivity 43% (31â57%) and specificity 94% (89â97%). Adding the H-FABP test to a diagnostic model for ACS led to an increase in the area under the receiver operating curve from 0.66 (95% CI 0.58â0.73) to 0.75 (95% CI 0.68â0.82).ConclusionThe H-FABP rapid test provides modest additional diagnostic certainty in primary care. It cannot be used to safely exclude rule out ACS. The test can only be used safely in patients otherwise NOT referred to hospital by the GP, as an extra precaution not to miss ACS (ârule inâ)
GJ 900: A new hierarchical system with low-mass components
Speckle interferometric observations made with the 6 m telescope of the
Special Astrophysical Observatory of the Russian Academy of Sciences in 2000
revealed the triple nature of the nearby ( mas)
low-mass young ( Myr) star GJ 900. The configuration of the triple
system allowed it to be dynamically unstable. Differential photometry performed
from 2000 through 2004 yielded - and -band absolute magnitudes and
spectral types for the components to be =6.660.08,
=9.150.11, =10.080.26, =4.840.08,
=6.760.20, =7.390.31, K5--K7,
M3--M4, M5--M6. The ``mass--luminosity''
relation is used to estimate the individual masses of the components:

. From the observations of the
components relative motion in the period 2000--2006, we conclude that GJ 900 is
a hierarchical triple star with the possible orbital periods
P80 yrs and P20 yrs. An analysis of the 2MASS
images of the region around GJ 900 leads us to suggest that the system can

Modeling Phenotypic Heterogeneity of Glycogen Storage Disease Type 1a Liver Disease in Mice by Somatic CRISPR/CRISPR-associated protein 9-Mediated Gene Editing
BACKGROUND AND AIMS: Patients with glycogen storage disease type 1a (GSDâ1a) primarily present with lifeâthreatening hypoglycemia and display severe liver disease characterized by hepatomegaly. Despite strict dietary management, longâterm complications still occur, such as liver tumor development. Variations in residual glucoseâ6âphosphatase (G6PC1) activity likely contribute to phenotypic heterogeneity in biochemical symptoms and complications between patients. However, lack of insight into the relationship between G6PC1 activity and symptoms/complications and poor understanding of the underlying disease mechanisms pose major challenges to provide optimal health care and quality of life for GSDâ1a patients. Currently available GSDâ1a animal models are not suitable to systematically investigate the relationship between hepatic G6PC activity and phenotypic heterogeneity or the contribution of geneâgene interactions (GGIs) in the liver. APPROACH AND RESULTS: To meet these needs, we generated and characterized a hepatocyteâspecific GSDâ1a mouse model using somatic CRISPR/CRISPRâassociated protein 9 (Cas9)âmediated gene editing. Hepatic G6pc editing reduced hepatic G6PC activity up to 98% and resulted in failure to thrive, fasting hypoglycemia, hypertriglyceridemia, hepatomegaly, hepatic steatosis (HS), and increased liver tumor incidence. This approach was furthermore successful in simultaneously modulating hepatic G6PC and carbohydrate response elementâbinding protein, a transcription factor that is activated in GSDâ1a and protects against HS under these conditions. Importantly, it also allowed for the modeling of a spectrum of GSDâ1a phenotypes in terms of hepatic G6PC activity, fasting hypoglycemia, hypertriglyceridemia, hepatomegaly and HS. CONCLUSIONS: In conclusion, we show that somatic CRISPR/Cas9âmediated gene editing allows for the modeling of a spectrum of hepatocyteâborne GSDâ1a disease symptoms in mice and to efficiently study GGIs in the liver. This approach opens perspectives for translational research and will likely contribute to personalized treatments for GSDâ1a and other genetic liver diseases
Irradiation of the secondary star in X-ray Nova Scorpii 1994 (=GRO J1655--40)
We have obtained intermediate resolution optical spectra of the black-hole
candidate Nova Sco 1994 in June 1996, when the source was in an X-ray/optical
active state (R~15.05). We measure the radial velocity curve of the secondary
star and obtain a semi-amplitude of 279+/-10 km/s; a value which is 30 per cent
larger than the value obtained when the source is in quiescence. Our large
value for K_2 is consistent with 60 +9,-7 per cent of the secondary star's
surface being heated; compared to 35 per cent, which is what one would expect
if only the inner face of the secondary star were irradiated. Effects such as
irradiation-induced flows on the secondary star may be important in explaining
