    We show that by using a feedback loop it is possible to reduce the fluctuations in one quadrature of the vibrational degree of freedom of a trapped ion below the quantum limit. The stationary state is not a proper squeezed state, but rather a ``squashed'' state, since the uncertainty in the orthogonal quadrature, which is larger than the standard quantum limit, is unaffected by the feedback action.Comment: 8 pages, 2 figures, to appear in the special Issue "Quantum Correlations and Fluctuations" of J.     Invariant measures on multimode quantum Gaussian states

    We derive the invariant measure on the manifold of multimode quantum Gaussian states, induced by the Haar measure on the group of Gaussian unitary transformations. To this end, by introducing a bipartition of the system in two disjoint subsystems, we use a parameterization highlighting the role of nonlocal degrees of freedom -- the symplectic eigenvalues -- which characterize quantum entanglement across the given bipartition. A finite measure is then obtained by imposing a physically motivated energy constraint. By averaging over the local degrees of freedom we finally derive the invariant distribution of the symplectic eigenvalues in some cases of particular interest for applications in quantum optics and quantum information.Comment: 17 pages, comments are welcome. v2: presentation improved and typos corrected. Then, also the density operator of those systems is written in terms of the marginal distribution of these quadratures.     Metronomic Chemotherapy with Vinorelbine Produces Clinical Benefit and Low Toxicity in Frail Elderly Patients Affected by Advanced Non-Small Cell Lung Cancer

    Lung cancer is the leading cause of death worldwide. The treatment choice for advanced stage of lung cancer may depend on histotype, performance status (PS), age, and comorbidities. In the present study, we focused on the effect of metronomic vinorelbine treatment in elderly patients with advanced unresectable non-small cell lung cancer (NSCLC). Methods. From January 2016 to December 2016, 44 patients affected by non-small cell lung cancer referred to our oncology day hospital were progressively analyzed. The patients were treated with oral vinorelbine 30 mg x 3/wk or 40 mg x 3/wk meaning one day on and one day off. The patients were older than 60, stage IIIB or IV, ECOG PS ≥ 1, and have at least one important comorbidity (renal, hepatic, or cardiovascular disease). The schedule was based on ECOG-PS and comorbidities. The primary endpoint was progression-free survival (PFS). PFS was used to compare patients based on different scheduled dosage (30 or 40 mg x3/weekly) and age (more or less than 75 years old) as exploratory analysis. We also evaluated as secondary endpoint toxicity according to Common Toxicity Criteria Version 2.0. Results. Vinorelbine showed a good safety profile at different doses taken orally and was effective in controlling cancer progression. The median overall survival (OS) was 12 months. The disease control rate (DCR) achieved 63%. The median PFS was 9 months. A significant difference in PFS was detected comparing patients aged below with those over 75, and the HR value was 0.72 (p<0.05). Not significant was the difference between groups with different schedules. Conclusions. This study confirmed the safety profile of metronomic vinorelbine and its applicability for patients unfit for standard chemotherapies and adds the possibility of considering this type of schedule not only for very elderly patients

    Kepler-447b: a hot-Jupiter with an extremely grazing transit

    We present the radial velocity confirmation of the extrasolar planet Kepler-447b, initially detected as a candidate by the Kepler mission. In this work, we analyze its transit signal and the radial velocity data obtained with the Calar Alto Fiber-fed Echelle spectrograph (CAFE). By simultaneously modeling both datasets, we obtain the orbital and physical properties of the system. According to our results, Kepler-447b is a Jupiter-mass planet (Mp=1.370.46+0.48 MJupM_p=1.37^{+0.48}_{-0.46}~M_{\rm Jup}), with an estimated radius of Rp=1.650.56+0.59 RJupR_p=1.65^{+0.59}_{-0.56}~R_{\rm Jup} (uncertainties provided in this work are 3σ3\sigma unless specified). This translates into a sub-Jupiter density. The planet revolves every 7.8\sim7.8 days in a slightly eccentric orbit (e=0.1230.036+0.037e=0.123^{+0.037}_{-0.036}) around a G8V star with detected activity in the Kepler light curve. Kepler-447b transits its host with a large impact parameter (b=1.0760.086+0.112b=1.076^{+0.112}_{-0.086}), being one of the few planetary grazing transits confirmed so far and the first in the Kepler large crop of exoplanets. We estimate that only around 20% of the projected planet disk occults the stellar disk. The relatively large uncertainties in the planet radius are due to the large impact parameter and short duration of the transit. Planets with such an extremely large impact parameter can be used to detect and analyze interesting configurations such as additional perturbing bodies, stellar pulsations, rotation of a non-spherical planet, or polar spot-crossing events. All these scenarios would periodically modify the transit properties (depth, duration, and time of mid-transit), what could be detectable with sufficient accurate photometry. Short-cadence photometric data (at the 1 minute level) would help in the search for these exotic configurations in grazing planetary transits like that of Kepler-447b.Comment: Accepted for publication in A&A. 13 pages, 8 figures, 4 tables.     On the optimal feedback control of linear quantum systems in the presence of thermal noise

    We study the possibility of taking bosonic systems subject to quadratic Hamiltonians and a noisy thermal environment to non-classical stationary states by feedback loops based on weak measurements and conditioned linear driving. We derive general analytical upper bounds for the single mode squeezing and multimode entanglement at steady state, depending only on the Hamiltonian parameters and on the number of thermal excitations of the bath. Our findings show that, rather surprisingly, larger number of thermal excitations in the bath allow for larger steady-state squeezing and entanglement if the efficiency of the optimal continuous measurements conditioning the feedback loop is high enough.     CoQ10 and vitamin A supplementation support voice rehabilitation. A double-blind, randomized, controlled, three-period cross-over pilot study

    Objectives: To evaluate the effectiveness of an adjuvant therapy (CoQ10 in its watersoluble form and vitamin A) in supporting voice rehabilitation in a large group of patients with muscle tension dysphonia (MTD). Study Design: Twelve-week, double-blind, randomized, controlled, three-period crossover pilot study. The primary endpoint was the change in the Dysphonia Severity Index (DSI) over the 12-week study period. Secondary endpoints were the changes in the subcomponents of DSI, including MPT, F0-high, I-low, and jitter. Exploratory endpoints were the changes in the Shimmer and in Voice Handicap Index (VHI). Methods: Patients were randomly assigned in a 1:1 ratio to two counter-balanced arms. Group A (ADJ-PLA) patients were administered QTer 300 mg and Vit A acetate 500.000&nbsp;Ul/g 1 mg twice daily for a 4-week intervention period, followed by a 4-week period of wash-out, and then were submitted to a last 4-week period of placebo. Patients in Group B (PLB-ADJ) were given the treatment period in reverse order. Both groups received a 45-min voice therapy in a group format once a day for 4 weeks during the first and the second active periods. The therapy was held during the wash-out period. Results: The analysis of main time effect indicated a trend toward recovery of vocal function regardless of group assignment. A significant time by group effect was found on DSI [F = 3.4 (2.5, 80.5), p = 0.03], F0-high [F = 4.5 (2.6, 82.9), p = 0.008] and Shimmer [F = 3.6 (1.5, 46.9), p = 0.048], under CoQ10 and Vit A treatment, with a small effect size. There was no significant time by group effect on the other study measures, namely MPT, I-low, VHI. Conclusions: A trend toward recovery of vocal function was observed in all the patients, likely due to voice rehabilitation. The improvement of DSI was greater under CoQ10 and Vitamin treatment, indicating a more pronounced improvement of vocal quality under adjuvant therapy.
