Evaluation of the risk of hemostasis disorders in non-Hodgkin’s lymphomas

Limfoamele non-Hodgkin (LNH) sunt cele mai frecvente hemopatii maligne la nivel mondial, cu o incidență de aproximativ 3% dintre toate maladiile oncologice. Dereglările de hemostază asociate LNH, adesea sunt asimptomatice, dar dezvoltă complicații severe, care alterează vădit calitatea vieții sau poate induce decesul pacientului. Scopul lucrării. Evaluarea riscului trombotic versus hemoragic la pacienții cu LNH. Material și metode. Studiul a fost realizat folosind bazele de date PubMed, Z-library, Medscape cu utilizarea cuvintelor-cheie: „limfom”, „hemostază”, „tromboză”, „hemoragie”, „risc”. Rezultate. Dereglările hemostazei la pacienții cu LNH se manifestă cel mai frecvent prin dezvoltarea evenimentelor trombotice (1,5%-59,5%) în special la nivelul sistemului venos, fiind condiționate de factorii de risc individuali (vârsta≥60ani; comorbidități), specifici LNH (localizarea-mediastin, SNC; dimensiunile tumorii ≥10cm; stadiul avansat; sinteza de anticorpi antifosfolipidici), specifici tratamentului aplicat (chirurgical, chimioterapia, hormonal). Hemoragia poate fi condiționată de dereglarea hemostazei primare (trombocitopenii≤30.000, trombocitopatii), secundare (terapia anticoagulantă, anticorpi anti factori de coagulare), fibrinolizei. Concluzii. Identificarea factorilor de risc procoagulanți și hipocoagulanți permite evaluarea raportului individual de risc-beneficiu al profilaxiei și poate contribui la efectuarea diferitor studii prospective privind profilaxia primară al dereglărilor de hemostază. Managementul riscului de dezvoltare al disfuncției sistemului de hemostază reprezintă un proces complex de elaborare și implementare a multiplelor strategii, pentru a micșora posibilitatea asocierii complicațiilor secundare nefavorabile.Background. IP Universitatea de Stat de Medicină și Farmacie Nicolae TestemițanuRezumat. Maladia oncologică prezintă un risc sporit pentru dezvoltarea evenimentelor trombotice, fiind asociat cu rate înalte
de recurență și mortalitate. Din aceste motive, prevenirea
trombozei asociate cancerului reprezintă o problemă majoră
în conduita pacientului oncologic. Cunoașterea factorilor de
risc tromboembolici în oncologie este absolut necesară pentru
evaluarea raportului risc-beneficiu individual al profilaxiei.
Scopul acestei lucrări este elucidarea factorilor de risc al
complicațiilor tromboembolice în oncologie. În acest sens, au
fost studiate publicațiile științifice medicale din baza de date
GoogleSearch, PubMed, Z-library, NCIB, Medscape, Hinari,
au fost căutate după cuvintele-cheie: „tromboză asociată
cancerului”, „cancer”, „factor de risc”. Informațiile au fost
sistematizate, evidențiind factorii de risc al complicațiilor
tromboembolice la pacienții oncologici.Summary. Oncological disease is a hypercoagulable state with an associated increased risk of venous thromboembolism and is associated with high rates of recurrence and mortality. For those
reasons, the prevention of cancer-associated thrombosis is of
paramount importance. Knowledge of thromboembolic risk
factors in oncology is absolutely necessary for the evaluation
of the individual risk-benefit ratio of prophylaxis. The aim of
this paper is to elucidate the risk factors of thromboembolic
complications in cancer patients. In order to achieve the set
aim, the medical scientific publications from the GoogleSearch
database, PubMed, Z-library, NCIB, Medscape, Hinari, were
searched for the keywords: „cancer-associated thrombosis”,
„cancer”, „risk factor”. The information was systematized,
highlighting the risk factors for thromboembolic complications
in patients with cancer.Резюме. Тромботические осложнения у пациентов с злокачественными заболеваниями являются частыми явлениями и связаны с высокими показателями рецидивов
и смертности. Следовательно, профилактика и оптимальное лечение ВТЭ являются важными компонентами
в ведении онкологических больных. Для оценки индивидуального соотношения риска и пользы профилактики
тромбообразования необходимо определить возможные
факторы риска. Целью данной статьи является определение факторов риска тромбоэмболических осложнений
у онкологических больных. Для достижения поставленной цели был проведен поиск медицинских научных
работ из базы данных GoogleSearch, PubMed, Z-library,
NCIB, Medscape, Hinari, по ключевым словам: «тромбоз»,
«тромбоз ассоциированный с раком», «фактор риска».
Полученная информация была систематизирована,

Clinical, hematological and immunohistochemical aspects of follicular Non-Hodgkin lymphomas: descriptive and prospective study

Medicină şi Farmacie „Nicolae Testemiţanu”, Chişinău, Republica MoldovaIntroducere. Una din cele mai frecvente variante ale limfoamelor non-Hodgkin (LNH) indolente este limfomul folicular. LNH foliculare sunt tumori maligne de origine B-celulară, care, deşi au o evoluţie relativ favorabilă se consideră incurabile la aplicarea programelor de tratament obișnuite. În ultimii ani, direcţiile principale în studierea LNH foliculare sunt orientate în plan morfologic, imunohistochimic, al biologiei moleculare. Însă, aspectele clinice sunt puţin studiate. De ace ea, cercetarea particularităţilor clinice, în conformitate cu modificările morfologice şi imunohistochimice, sunt actuale. Material şi metode. Datele clinice, hematologice şi imunohistochimice ale limfomului non-Hodgkin folicular au fost studiate la 16 pacienţi. Identificarea acestei variante de LNH s-a efectuat în conformitate cu Clasificarea Internaţională Histologică şi Citologică a Patologiilor Tumorale ale Țesutului Hematopoietic şi Limfatic, propusă de OMS în anul 2008. În acest scop, au fost utilizate metode histologice şi imunohistochimice ale bioptatului ganglionilor limfatici şi ale altor ţesuturi afectate. Gradul de răspândire al procesului tumoral a fost determinat în conformitate cu Clasificarea Clinică Internaţională, elaborată în Ann-Arbor (SUA, 1971). În scopul stadializării procesului tumoral, au fost utilizate metode clinice şi paraclinice. Rezultate. LNH folicular s-a dezvoltat preponderent la persoanele cu vârsta de peste 50 de ani. La examenul imunohistochimic, CD19 şi CD20 au fost pozitivi, care au confirmat apartenenţa liniei B-limfocitare, CD10 – pozitiv, iar CD5 şi CD23 – negativi. Focarul tumoral primar s-a dezvoltat mai des în ganglionii limfatici periferici şi splină. Măduva oaselor a fost interesată în 72,7% din cazuri. Supravieţuirea bolnavilor peste 2 ani a constituit 94,0%, iar peste 5 ani – 57,2%. Concluzii. A fost identificat segmentul de vârstă şi sexul persoanelor mai frecvent afectate în LNH foliculare. Au fost determinate zonele de metastazare iniţială şi la distanţă, frecvenţa şi localizarea metastazelor extranodale. A fost elaborat un algoritm de folosire a anticorpilor monoclonali specifici LNH folicular.Introduction. One of the most common variants of indolent non-Hodgkin’s lymphomas (NHL) is follicular lymphoma. Follicular non-Hodgkin’s lymphomas are malignant tumors B cell origin, though they have a relatively favorable evolution is considered incurable to application standard treatment. In recent years, the main directions in the study of NHL are oriented in morphological, immunohistochemical and molecular biology directions. But clinical manifestations are poorly stu died. For this reason the studying of clinical aspects in correlation with morphological and immunohistochemical modifications are actuals. Material and methods. The study included the analysis of clinical, hematological and immunohistochemical data in 16 patients with follicular non-Hodgkin’s lymphoma The identification of this NHL variant was conducted in accordance with the International histological and cytological Classification of the tumor pathologies of the hematopoietic and lymphatic tissue, proposed by the WHO in 2008. For this purpose, histological and immunohistochemical methods of the bioptates of lymph nodes and other tissues affected were used. The stage of the disease was determined in accordance with the International Clinical Classification, developed in Ann Arbor (USA, 1971). Results. Follicular non-Hodgkin’s lymphoma developed only in adults, mainly in people aged over 50 years. At immunohistochemistry, CD19 and CD20 were positive, confirming the relationship of the B-lymphocyte line with positive CD10 and negative CD23 and CD5. The primary tumor focus developed to the same extent in the peripheral lymph nodes (37.5%) and in the spleen (37.5%). During the process of tumor generalization, in 61.5% of cases bone marrow was affected. The survival of patients at 2 years was 94.0% and at over 5 years – 57.2%. Conclusions. It was identified by age and sex of the segment most frequently affected in follicular NHL. They were determined areas of initial and distant metastasis, frequency and extra nodal metastasis location. An algorithm has been developed using specific monoclonal antibodies for follicular NHL
Modern issues in the study of indolent non-Hodgkin’s lymphomas

IMSP Institutul Oncologic din Republica MoldovaThis article describes the modern issues in the study of indolent non-Hodgkin’s
lymphomas (NHL) that during a long period of time evolve asymptomatically, which determines
that they are occasionally identified, usually at the generalized stages. The clinical and
morphological signs of indolent NHL are more or less similar thus creating difficulties in
determining the variants of these lymphomas and requires their identification by applying
immunohistochemical methods. In the literature, the clinical manifestations of the indolent NHL
age generally described without highlighting the clinical and hematological peculiarities of each
variant. In order to more rationally select the monoclonal antibodies for the
immunohistochemical investigation, of great importance are the clinical-hematological and
immunohistochemical comparative studies.
În articol se discută probleme actuale în studierea LNH indolente, care timp îndelungat
evoluează asimptomatic, din care cauză ele deseori sunt depistate ocazional, de obicei, în stadii
generalizate. Semnele clinice şi morfologice ale LNH indolente sunt în mare măsură similare ce
crează dificultăţi în determinarea variantelor acestor limfoame, ce necesită identificarea lor prin
utilizarea metodelor imunohistochimice. Manifestările clinice ale LNH indolente în literatură
sunt descrise la general fără evidenţierea particularităţilor clinico-hematologice a fiecărei
variante. În scopul selectării mai raţională a anticorpilor monoclonali pentru investigarea
imunohistochimică sunt importante studii comparative clinico-hematologice şi
imunohistochimice
Solitary plasmacytoma. Clinical signs and treatment

Institutul OncologicData abouth 8 patients (4 males and 4 females) with solitary plasmacytoma are described. The median age at diagnosis was 51,3 years (range 20-73 years). Three patients had o solitary plasmacytoma of the bone, 5 had solitary extramedullary plasmacytoma. Of 3 cases with solitary plasmacytoma of the bone spinal column (Th 5-6) in one patient was affected, in other tibia with patological fracture, in the third left scapula. Of 5 cases with solitary extramedullary plasmacytoma in three patients tumoral focus appeared in nasal submucosis, in one patient larynx was affected, in one female cervix was affected. The clinical manifestations were determinated by localization of primary tumor. The signs of general intoxication were absent.
The treatment included 3 cycles of chemotherapy COP or CHOP, locoregional radiotherapy (40-44 Gy) folowed by 3 cycles of the same chemotherapy. Of 8 patients 7 finished the treatment with achivement of complete remission. In 6 patients the remission has been lasting one year, one year and 3 months, 5, 6, 7 and 8 years respectively. One patient with solitary plasmacytoma of the bone after achivement of complete remission has disappeared.
The authors consider that combined chemoradiotherapy of solitary plasmacytoma ensured excellent rates results.
Sunt descrise datele despre 8 pacienţi (4 bărbaţi şi 4 femei) cu plasmacitom solitar. Vârsta medie 51,3 de ani (20-73 de ani). Trei bolnavi au fost cu plasmacitom solitar al oaselor, 5 – cu plasmacitom solitar extramedular. Din cele 3 cazuri cu plasmacitom solitar al oaselor la un bolnav au fost afectate vertebrele Th 5-6, la altul tibia cu fractură patologică, la al treilea omoplatul de stânga. Din 5 pacienţi cu plasmacitom solitar extramedular focarul tumoral în 3 cazuri a apărut în submucoasa nasală, la un bolnav a fost afectat laringele. La o pacientă a fost afectat colul uterin. Manifestările clinice au fost determinate de localizarea primară a tumorii. Semne de intoxicare generală au lipsit.
Tratamentul a inclus 3 cicluri de polichimioterapie COP sau CHOP, radioterapie locoregională în doza sumară 40-44 Gy, după care s-au efectuat încă 3 cicluri de polichimioterapie. Din 8 pacienţi au finalizat tratamentul 7 cu obţinerea remisiunii complete, care la 6 pacienţi se prelungeşte respectiv 1 an, 1 an şi 3 luni, 5, 6, 7 şi 8 ani. O bolnavă după obţinerea remisiunii complete ulterior nu s-a prezentat la control.

Results of the treatment of multiple myeloma

Mielomul multiplu (MM) reprezintă 2% în structura maladiilor oncologice și aproximativ 20% din hemopatiile maligne. În Republica Moldova incidența bolii constituie 0,6 la 100.000 de locuitori, cu tendință de creștere în ultimii ani. Tratamentul MM rămâne o provocare majoră în practica medicală. Scopul lucrării. Studierea rezultatelor tratamentului MM în vederea abordării optime a terapiei bolii. Material și metode. Studiul a inclus 52 de pacienți cu mielom multiplu în vârstă de la 36 până la 83 de ani, care s-au aflat la tratament în Departamentul Hematologic a Institutului Oncologic în perioada anilor 2019 – 2022. Diagnosticul a fost confirmat prin efectuarea punctatului medular la 46 bolnavi, trepanobiopsiei osului iliac – în 4 cazuri, biopsiei formațiunii tumorale – la 2 pacienți. Tratamentul de I linie a fost efectuat prin aplicarea MChT cu ciclofosfamidă la 15 pacienți, PChT conform schemelor: VCD – la 25 și VTD – la 12 bolnavi. Rezultate. S-a constatat, că în rezultatul tratamentului de I linie a pacienților cu mielom multiplu, s-a obținut ameliorare clinico-hematologică în 43 (78,2%) cazuri. Eficacitatea terapiei de I linie a fost mai înaltă la bolnavii la care s-au utilizat schemele de PChT cu includerea inhibitorilor de proteozomi (VCD – 88,0% șı VTD – 91,6%, respectiv), spre deosebire de MChT cu ciclofosfamidă-60% cazuri. Perioada stabilizării procesului tumoral la 2 ani a fost mai înaltă la pacienții la care s-a efectuat PChT conform schemei VTD (40,5%) după care a urmat PChT VCD (27,3%). La aplicarea MChT cu ciclofosfamidă perioada de stabilizare a constituit doar 14,3%. Concluzii. Eficacitatea tratamentului a fost mai înaltă la utilizarea PChT cu includerea inhibitorilor de proteozomi, iar metoda optimă de tratament a fost PChT conform schemei VTD.Background. Multiple myeloma (MM) accounts for 2% of oncological diseases and approximately 20% of malignant hematologic disorders. In the Republic of Moldova, the incidence rate of the disease is 0,6 per 100,000 inhabitants, with a tendency to increase in recent years. The treatment of MM remains a major challenge in medical practice. Objective of the study. To investigate the outcomes of MM treatment in order to determine the optimal approach to disease therapy. Materials and Methods. The study included 52 patients with multiple myeloma, aged between 36 and 83 years, who received treatment at the Hematology Department of the Institute of Oncology during the period 2019-2022. The diagnosis was confirmed through bone marrow aspiration in 46 cases, iliac bone trephine biopsy in 4 cases, and tumor formation biopsy in 2 cases. First-line treatments consisted of MChT with cyclophosphamide for 15 patients, and PChT regimens: VCD for 25 cases and VTD for 12 patients. Results. The results of the first-line treatment in patients with multiple myeloma showed that clinical and hematological improvement was achieved in 43 cases (78.2%). The efficacy of first-line therapy was higher in patients treated with PChT regimens including proteasome inhibitors (VCD – 88% and VTD – 91.6%), compared to MChT with cyclophosphamide, which showed an efficacy of 60%. The period of tumor stabilization at 2 years was higher in patients treated with PChT according to the VTD regimen (40.5%), followed by PChT VCD (27.3%). When applying MChT with cyclophosphamide, the stabilization period was only 14.3%. Conclusions. The effectiveness of treatment was higher when using PChT regimens including proteasome inhibitors, and the optimal treatment method was found to be PChT according to the VTD regimen
Management of malignant hematological diseases in the conditions of the COVID-19 pandemic

Introducere. Tratamentul hemopatiilor maligne include:
chimioterapie, radioterapie, imunoterapie, care induc o
imunosupresie şi, respectiv, prezintă un risc major de dezvoltare
al infecţiilor. Factori potenţiali de dezvoltare a infecţiilor
grave sunt: vârsta înaintată, imunodeficienţa secundară, leucopenia, neutropenia. Infecţia COVID-19 are o evoluţie
mult mai gravă la pacienţii imunocompromişi cu hemopatii
maligne. Forma gravă se dezvoltă de 2-3 ori mai frecvent,
decât la persoanele sănătoase. Mortalitatea pacienţilor cu
hemopatii maligne în asocierea infecţiei SARS-CoV-2 este
mai mare de 20%. Pacienţii cu hemopatii maligne au un potenţial
înalt pentru vindecare la efectuarea unui tratament
antitumoral intensiv, cu evitarea ulterioară a complicaţiilor
infecţioase, spre deosebire de pacienţii fără o terapie în deplin
volum. Totodată tratamentul hemopatiilor maligne este
necesar de efectuat la timp şi în volum deplin. Întârzierea
iniţierii chimioterapiei poate afecta negativ prognosticul pacienţilor
cu hemopatii maligne. Măsuri profilactice specifice
pentru infecţia SARS-CoV-2 la pacienţii cu hemopatii maligne
nu sunt. Este necesar de a efectua măsurile generale, care
includ măsurile sanitaro–igienice, izolarea la domiciliu, la
posibilitate activarea de la domiciliu şi testarea pentru infecţia
SARS-CoV-2. Obiectivul acestui articol este prezentarea
managementului hemopatiilor maligne în condiţiile pandemiei
COVID-19.
Material şi metode. Pentru a selecta date din literatură,
a fost folosită baza de date PubMed, utilizând cuvintele cheie:
,,COVID-19”, ,,hemopatii maligne”, ,,pandemie”, ,,management”.
Astfel, a fost sintetizată informaţia despre managementul hemopatiilor
maligne în condiţiile pandemiei COVID-19.
Rezultate. Tratamentul hemopatiilor maligne este necesar
de efectuat la timp şi în deplin volum, deoarece întârzierea
iniţierii terapiei poate afecta negativ prognosticul
pacienţilor cu hemopatii maligne. Tratamentul specific antitumoral
(chimioterapia, imunoterapia, radioterapia) este
însoţit de citopenii şi toxicităţi semnificative, care trebuie
să fie considerate ca factori de risc în infecţia SARS-CoV-2
şi, ca urmare, cu un efect negativ al evoluţiei bolii. Astfel,
administrarea chimioterapiei poate agrava hipersensibilitatea
pneumoniei indusă de COVID-19. În perioada pandemiei
este necesar de a selecta pentru utilizare acele scheme
de tratament, care ar limita vizitele pacienţilor la medic şi
acordarea priorităţii administrării preparatelor perorale sau
subcutane.
Concluzii. Managementul corect al hemopatiilor maligne
în condiţiile pandemiei COVID-19 va reduce numărul cazurilor
de infectare şi evoluţia procesului infecţios. Este obligatorie
gestionarea justă al posibilelor complicaţii infecţioase,
care se pot dezvolta în perioda sau după administrarea tratamentului
specific.Abstract
Introduction. The treatment of hematological malignancies
includes chemotherapy, radiotherapy, immunotherapy
that induce immunosuppression and, respectively, present a
major risk of developing infections. Potential factors for the
development of severe infections are: advanced age, secondary immunodeficiency, leukopenia with neutropenia. SARS-CoV-2
infection has a much more serious course in immunocompromised
patients with malignant hematological diseases. The severe
form develops 2-3 times more frequently than in healthy
people. Mortality in patients with hematological malignancies
associated with SARS-CoV-2 infection is greater than 20%. Patients
with malignant hematological diseases have a high potential
for healing when performing intensive antitumor treatment
with subsequent avoidance of infectious complications
as opposed to patients without full-volume therapy. At the
same time, the treatment of malignant hematological diseases
needs to be performed on time and in full volume. Delayed
initiation of chemotherapy may adversely affect the prognosis
of patients with malignant hematological diseases. There are
no specific prophylactic measures for SARS-CoV-2 infection in
patients with hematological malignancies. It is necessary to
carry out general measures, which include sanitary-hygienic
measures, isolation at home, possible activation at home and
testing for SARS-CoV-2 infection. The objective of this article
was to present the management of malignant hematological
diseases in the conditions of the COVID-19 pandemic.
Material and methods. The PubMed database was used
in order to select the data from the literature, using the key
words: “COVID-19”, “hematological malignancies”, “pandemic”,
“management”. Thus, the information containing the data on
the management of malignant hematological diseases in the
conditions of the COVID-19 pandemic.
Results. The treatment of hematological malignancies
needs to be performed on time and in full amount, because the
delay of initiating the therapy can negatively affect the prognosis
of patients with malignant hematological diseases. Specific
antitumor treatment (chemotherapy, immunotherapy, radiotherapy)
is accompanied by cytopenias and significant toxicities,
which must be considered as risk factors in SARS-CoV-2
infection and as a result of a negative effect on the course of
the disease. Thus, the treatment with chemotherapy may aggravate
the hypersensitivity induced by COVID-19 pneumonitis.
During the pandemic it is necessary to select for use those
treatment schemes, which would limit the visits of patients to
the doctor and to give priority to the oral or subcutaneous drug
administration.
Conclusions. Proper management of hematological malignancies
in pandemic with COVID-19 will reduce the number of
cases of infection and the evolution of the infectious process. At
the same time, it is mandatory to follow a correct management
of possible infectious complications, which may develop during
or after specific treatment
CLINICO-MORPHOLOGICAL ASPECTS OF AGGRESSIVE NON-HODGKIN’S LYMPHOMAS

Limfoamele non-Hodgkin (LNH) agresive reprezintă a 6-a cauză de deces în structura tumorilor maligne în țările europene și SUA. Depistarea patologiei în stadiile ei inițiale și identificarea tipului morfologic sunt elemente esențiale în alegerea tacticii de tratament. Scopul lucrării. Studierea aspectelor clinico-morfologice a LNH agresive pentru a identifica unele particularități de evoluție clinică. Material și Metode. Au fost studiate aspectele clinico-morfologice la 54 de pacienți cu diagnosticul de limfom non-Hodgkin agresiv cu vârsta de la 18 până la 85 de ani(bărbați-24, femei-30). Diagnosticul a fost confirmat morfologic și imunohistochimic. Rezultate. LNH agresive s-au dezvoltat mai frecvent la persoanele cu vârsta de 40-64 de ani(57,4%). Mai des a fost diagnosticat tipul morfologic limfoblastic (64,8%), preponderent la vârsta de peste 65 de ani(80,0%). LNH difuz din celulă mare B a fost constatat în 29,6% cazuri, mai des la vârsta de 18-39 ani(62,5%). Rar au fost diagnosticate LNH zonei de manta(3,8%) și LNH anaplazic (1,8%). Debutul LNH agresive a avut loc în nodulii limfatici periferici în 63,0% cazuri, extranodal-37%. La etapele inițiale, în majoritatea cazurilor (68,8%), LNH s-a răspândit în nodulii regionali. Metastaze extranodale mai des au fost determinate în splină(51,7%), măduva oaselor(34,5%), ficat(31,0%). Concluzii. Particularitățile clinico-morfologice evidențiate ale LNH agresive vor contribui la optimizarea conduitei de diagnostic și tratament a pacienților cu LNH agresive.Background. Aggressive non-Hodgkin’s lymphomas (NHL) are the 6th leading cause of death among malignant tumors in European countries and the USA. Detection of the disease in its early stages and identification of the morphological type are essential elements in the therapeutic approach. Objective of the study. To study the clinico-morphological aspects of aggressive NHL in order to identify some peculiarities of the clinical course. Material and Methods. Clinico-morphological aspects were studied in 54 patients diagnosed with aggressive non-Hodgkin’s lymphoma aged 18 to 85 years (male-24, female-30). The diagnosis was confirmed morphologically and immunohistochemically. Results. Aggressive NHL occurred more frequently in patients aged 40-64 years (57.4%). The most often diagnosed morphological type was lymphoblastic NHL (64.8%), predominantly in people over 65 years old (80.0%). Diffuse large B-cell NHL was found in 29.6% of cases, mostly at the age of 1839 years (62.5%). Seldom were diagnosed variants such as mantle cell NHL (3.8%) and anaplastic NHL (1.8%). The onset of aggressive NHL occurred in peripheral lymph nodes in 63.0% of cases, extranodal in 37%. In the early stages, in the majority of cases (68.8%), NHL spread to regional nodules. Extranodal metastases frequently were determined in spleen (51.7%), bone marrow (34.5%), and liver (31.0%). Conclusion. The highlighted clinical morphological features of aggressive NHL will contribute to optimizing the diagnostic and treatment management of patients with aggressive NHL
THE RESULTS OF TREATMENT APPLIED IN NON-HODGKIN LYMPHOMAS WITH PRIMARY MEDIASTINAL INVOLVEMENT

Limfoamele non - Hodgkin (LNH) sunt tumori maligne limfoproliferative care diferă prin evoluția clinică, structura morfologică, sensibilitatea diferită la programele de tratament. Frecvența LNH cu afectarea primară a mediastinului constituie 2,5 - 5,7%. Scopul lucrării. Studierea rezultatelor tratamentului limfoamelor non- Hodgkin cu afectarea primară a mediastinului. Material și Metode. Au fost studiate rezultatele tratamentului la 49 de pacienți cu vârsta de la 20 până la 74 de ani cu diagnosticul de LNH mediastinal care s-au aflat la tratament și evidență în Departamentul Hematologic al IMSP Institutul Oncologic din Republica Moldova. Evaluarea răspunsului la tratament a fost determinat conform criteriilor OMS. Rezultate. Tratamentul a fost efectuat cu 6-8 cicluri de polichimioterapie (PChT) la 31 pacienți și tratament combinat (PChT + radioterapie) în 18 cazuri. Schemele de PChT utilizate au fost CHOP la 27 pacienți și R-CHOP - la 22. După aplicarea tratamentului combinat remisiunile complete în stadiile locale au constituit 83,3%, în stadiile generalizate- 66,7%, iar după PChT - în 80,0% și 46,2%, respectiv. Procentul remisiunilor complete a fost mai înalt la aplicare a PChT R-CHOP: în stadiile locale - 100%, în stadiile generalizate- 66,6%. La efectuarea PChT CHOP remisiunile complete au fost obținute în stadiile locale la 50,0% pacienți, în stadiile generalizate în 35,3% cazuri. Concluzii. Eficacitatea tratamentului a fost mai înaltă la aplicarea tratamentului combinat și utilizarea PChT conform schemei R-CHOP. Supraviețuirea pacienților peste 5 ani, fără progresie a constituit 82,6%, iar supraviețuirea generală a fost egală cu 69,1 %.Background. The frequency of NHL with primary mediastinal involvement is 2.5- 5.7%. Objective of the study. To study the results of treatment of non- Hodgkin’s lymphomas with primary mediastinal involvement. Material and Methods. They were studied for the treatment of 49 patients aged 20 to 74 years with a diagnosis of mediastinal NHL who were undergoing treatment and evidence in the Hematology Department of the IMSP Oncological Institute of the Republic of Moldova. The evaluation of the response to treatment was determined according to WHO criteria. Results. The treatment was performed with 6-8 cycles of polychemotherapy (PChT) in 31 patients and combined treatment (PChT + radiotherapy) in 18 cases. The PChT regimens used were CHOP in 27 patients and R- CHOP- in 22. When applying the combined treatment, the complete remissions in the local stages constituted 83.3%, in the generalized stages – 66.7%, and after PChT - in 80.0 % and 46.2%, respectively. The percentage of complete remissions was higher when applying PChT R- CHOP, in the local stages- 100%, in the generalized stages- 66.6%. When performing PChT CHOP, complete remissions were obtained in the local stages at 50.0%, in the generalized stages – 35.3% cases. Conclusion. The effectiveness of the treatment was higher when applying the combined treatment and using PChT according to the R- CHOP scheme. The survival of patients over 5 years without progression was 82.6%, and the overall survival was equal to 69.1%
BUNĂSTĂREA PSIHO-EMOȚIONALĂ AL PACIENȚILOR CU LIMFOAME NON-HODGKIN
The given study focuses on the evaluation of the psycho-emotional well-being of patients with non-Hodgkin’s lymphomas (NHL) according to age, gender, living environment, morphological subtype, stage, primary tumor focus, Charlson comorbidities index, and completion of first-line chemotherapy treatment. To carry out the study, a
number of 126 patients aged between 18 and 78 years, included in the treatment and monitored in the Department of Hematology, IMSP Oncological Institute, during the years
2020-2022, were selected. The Global Well Being (GWB) score was used to test the proposed hypotheses. The results illustrate statistically significant correlations between
GWB score and gender, living environment, extranodal onset, and treatment period. The lack of statistically significant correlations was appreciated between the GWB score
and age, disease stage, comorbidities
