    Positive Counseling with College Students

    College students face multiple challenges and can find college life to be overwhelming at times. In this paper, we examine how positive psychology, which embraces a strengths-based focus, has much to offer college students and their counselors. After providing an overview of positive psychology, we examine how positive psychology can be integrated into counseling generally and then more specifically into working with college students. We examine a number of different issues that college students face, and we offer different positive psychology techniques that have been found to be beneficial. Moreover, we provide a case study to demonstrate the benefits of taking a positive psychology approach to enhance counseling effectiveness. We also give attention to the potential benefits of taking a strengths-based approach to improving student retention and to how positive psychology can be applied to enhance the well-being of the counselor

    Breaking voice identity perception: Expressive voices are more confusable for listeners.

    The human voice is a highly flexible instrument for self-expression, yet voice identity perception is largely studied using controlled speech recordings. Using two voice-sorting tasks with naturally varying stimuli, we compared the performance of listeners who were familiar and unfamiliar with the TV show Breaking Bad. Listeners organised audio clips of speech with (1) low-expressiveness and (2) high-expressiveness into perceived identities. We predicted that increased expressiveness (e.g., shouting, strained voice) would significantly impair performance. Overall, while unfamiliar listeners were less able to generalise identity across exemplars, the two groups performed equivalently well when telling voices apart when dealing with low-expressiveness stimuli. However, high vocal expressiveness significantly impaired telling apart in both the groups: this led to increased misidentifications, where sounds from one character were assigned to the other. These misidentifications were highly consistent for familiar listeners but less consistent for unfamiliar listeners. Our data suggest that vocal flexibility has powerful effects on identity perception, where changes in the acoustic properties of vocal signals introduced by expressiveness lead to effects apparent in familiar and unfamiliar listeners alike. At the same time, expressiveness appears to have affected other aspects of voice identity processing selectively in one listener group but not the other, thus revealing complex interactions of stimulus properties and listener characteristics (i.e., familiarity) in identity processing

    A Role for Antimicrobial Stewardship in Clinical Sepsis Pathways: A Prospective Interventional Study

OBJECTIVE To evaluate the impact of early infectious diseases (ID) antimicrobial stewardship (AMS) intervention on inpatient sepsis antibiotic management. DESIGN Interventional, nonrandomized, controlled study. SETTING Tertiary-care referral hospital, Sydney, Australia. PATIENTS Consecutive, adult, non-intensive care unit (non-ICU) inpatients triggering an institutional clinical sepsis pathway from May to August 2015. INTERVENTION All patients reviewed by an ID Fellow within 24 hours of sepsis pathway trigger underwent case review and clinic file documentation of recommendations. Those not reviewed by an ID Fellow were considered controls and received standard sepsis pathway care. The primary outcome was antibiotic appropriateness 48 hours after sepsis trigger. RESULTS In total, 164 patients triggered the sepsis pathway: 6 patients were excluded (previous sepsis trigger); 158 patients were eligible; 106 had ID intervention; and 52 were control cases. Of these 158 patients, 91 (58%) had sepsis, and 15 of these 158 (9.5%) had severe sepsis. Initial antibiotic appropriateness, assessable in 152 of 158 patients, was appropriate in 80 (53%) of these 152 patients and inappropriate in 72 (47%) of these patients. In the intervention arm, 93% of ID Fellow recommendations were followed or partially followed, including 53% of cases in which antibiotics were de-escalated. ID Fellow intervention improved antibiotic appropriateness at 48 hours by 24% (adjusted risk ratio, 1.24; 95% confidence interval, 1.04-1.47; P=.035). The appropriateness agreement among 3 blinded ID staff opinions was 95%. Differences in intervention and control group mortality (13% vs 17%) and median length of stay (13 vs 17.5 days) were not statistically significant. CONCLUSION Sepsis overdiagnosis and delayed antibiotic optimization may reduce sepsis pathway effectiveness. Early ID AMS improved antibiotic management of non-ICU inpatients with suspected sepsis, predominantly by de-escalation. Further studies are needed to evaluate clinical outcomes.     Manganese-enhanced magnetic resonance imaging depicts brain activity in models of acute and chronic pain: a new window to study experimental spontaneous pain?

    Application of functional imaging techniques to animal models is vital to understand pain mechanisms, but is often confounded by the need to limit movement artefacts with anaesthesia, and a focus on evoked responses rather than clinically relevant spontaneous pain and related hyperalgesia. The aim of the present study was to investigate the potential of manganese-enhanced magnetic resonance imaging (MEMRI) to measure neural responses during on-going pain that underpins hyperalgesia in pre-clinical models of nociception. As a proof of concept that MEMRI is sensitive to the neural activity of spontaneous, intermittent behaviour, we studied a separate positive control group undergoing a voluntary running wheel experiment. In the pain models, pain behaviour (weight bearing asymmetry and hindpaw withdrawal thresholds (PWTs)) was measured at baseline and following either intra-articular injection of nerve growth factor (NGF, 10 ”g/50 ”l; acute pain model, n=4 rats per group), or the chondrocyte toxin monosodium iodoacetate (MIA, 1 mg/50 ”l; chronic model, n=8 rats per group), or control injection. Separate groups of rats underwent a voluntary wheel running protocol (n=8 rats per group). Rats were administered with paramagnetic ion Mn2+ as soluble MnCl2 over seven days (subcutaneous osmotic pump) to allow cumulative activity-dependent neural accumulation in the models of pain, or over a period of running. T1-weighted MR imaging at 7 T was performed under isoflurane anaesthesia using a receive-only rat head coil in combination with a 72 mm volume coil for excitation. The pain models resulted in weight bearing asymmetry (NGF: 20.0 ± 5.2%, MIA: 15 ± 3%), and a reduction in PWT in the MIA model (8.3 ± 1.5 g) on the final day of assessment before undergoing MR imaging. Voxel-wise and region-based analysis of MEMRI data did not identify group differences in T1 signal. However, MnCl2 accumulation in the VTA, right Ce amygdala, and left cingulate was negatively correlated with pain responses (greater differences in weight bearing), similarly MnCl2 accumulation was reduced in the VTA in line with hyperalgesia (lower PWTs), which suggests reduced regional activation as a result of the intensity and duration of pain experienced during the 7 days of MnCl2 exposure. Motor cortex T1-weighted signal increase was associated with the distance ran in the wheel running study, while no between group difference was seen. Our data suggest that on-going pain related signal changes identified using MEMRI offers a new window to study the neural underpinnings of spontaneous pain in rats

    Anti-inflammatory and immunoregulatory effects of pinolenic acid in rheumatoid arthritis

    Objectives In pre-clinical studies, pinolenic acid (PNLA), an omega-6-polyunsaturated fatty acid from pine nuts has shown anti-inflammatory effects. We aimed to investigate the effect of PNLA in human cell lines and peripheral blood mononuclear cells (PBMCs) from rheumatoid arthritis (RA) patients and healthy controls (HCs). Methods Modified Boyden chamber was used to assess chemokine-induced migration of THP-1 monocytes. Macropinocytosis was assessed using lucifer yellow and ox-LDL uptake using Dil-oxLDL in THP-1 macrophages and human monocyte-derived macrophages (HMDM). IL6, TNFα and PGE2 release by lipopolysaccharide (LPS) stimulated PBMCs from RA patients and HCs were measured by ELISA. The transcriptomic profile of PNLA treated, LPS activated PBMCs was investigated by RNA-sequencing. Results PNLA reduced THP-1 cell migration by 55% (p< 0.001). Macropinocytosis and Dil-oxLDL uptake were reduced by 50% (p< 0.001) and 40% (p< 0.01) in THP-1 macrophages and 40% (p< 0.01) and 25% (p< 0.05) in HMDM, respectively. PNLA reduced IL6 and TNFα release from LPS stimulated PBMCs from RA by 60% (p< 0.001) and by 50% and 35% respectively (p< 0.01) for HCs. PNLA also reduced PGE2 levels in such PBMCs from RA patients and HCs (p< 0.0001). Differentially expressed genes included upregulated expression of pyruvate dehydrogenase kinase-4, plasminogen activator inhibitor-1, fructose bisphosphatase 1 and N-Myc downstream-regulated gene, which have potential roles in regulating immune and metabolic pathways. Pathway analysis predicted upstream activation of nuclear receptors peroxisome proliferator-activated receptors involved in anti-inflammatory processes, and inhibition of NF-ÎșB and STAT1. Conclusions PNLA has immune-metabolic effects on monocytes and PBMC which are pathogenic in RA and atherosclerosis. Dietary PNLA supplementation may be beneficial in RA

    Designs for Heritage Language Learning: A Photography project in the UK Supplementary Education

    Supplementary Schools in the UK offer educational opportunities for children and young people outside mainstream school provision. The paper reports an enquiry undertaken by practitioners in Greek Supplementary Schools in the UK to explore how features of mobile technologies may be leveraged to foster heritage language learning. It draws on the view that mobile learning can be a way for learners to explore the language informally and direct their own development (Kukulska-Hulme, 2015) and may also shape the learners’ ‘habits of mind’ (Wong, 2012, p.22) in learning—and consequently their language competencies. The project #ItsAllGreekToUS set to investigate how to create learning designs to incorporate effective use of mobile technologies within language learning and teaching. It draws on action research orientation and uses the idea of ‘Bring Your Own Device’ (BYOD) (JISC, 2013) in educational settings. The study involved several sessions around the concept of ‘loanwords’ and representations of this vocabulary in artefacts created with the use of mobile phones and a popular photography application (e.g. Pinterest). The participants were fourteen students (12-13s) attending a pre-GCSE class in a Greek School in London and nine students (12-14s) attending a GCSE class in a Greek School in Leicester. Evidence from user-generated content, the pupils’ views around the project and the practitioners’ observations are considered. The paper will discuss how students’ practices associated with mobile technologies are integrated into teachers’ practice. Particular attention will be drawn to designing language learning by blending traditional language classroom practices along incorporating the practices of sharing and curating content, as well as allowing ‘visibility’ through artefacts created by the learners
