MANIFESTĂRILE TUBULUI DIGESTIV SUPERIOR LA PACIENTUL CU IRC DIALIZAT
Introducere: bolnavii cu IRC sunt susceptibili la o gamă largă de complicaţii: refl ux gastro-esofagian, esofagită,
gastropatie, gastrită şi duodenită, ulcer gastric şi duodenal, angiodisplazie.
Obiective: studiul a constat în evidenţierea simptomelor gastro-duodenale, a frecvenţei modifi cărilor patologice
ale mucoasei eso-gastro-duodenale şi a prezenţei H.pylori la copiii cu IRC afl aţi în program de dializă.
Material şi metodă: lotul de studiu a fost format din 55 de copii internaţi în Clinica a IV-a Nefrologie, afl aţi în
program de substituţie renală cronică, la care s-a aplicat un protocol ce a sistematizat: simptomele obiective şi
subiective şi rezultatele endoscopiei digestive superioare. Lotul cuprinde bolnavii cu modifi cări ale mucoasei
depistate endoscopic.
Concluzii: modifi cările patologice ale mucoasei eso-gastro-duodenale la copilul dializat cronic se refl ectă clinic inconstant, ceea ce sugerează necesitatea endoscopiei pentru fi ecare caz. Prezenţa H.pylori la 24/55 (43,63%) cazuri biopsiate evidenţiază asocierea relativ frecventă între leziunile gastro-duodenale şi H.pylori la pacienţii
dializaţi cronic
ERADICAREA INFECȚIEI CU HELICOBACTER PYLORI LA COPIL: ACTUALITĂȚI ŞI PERSPECTIVE
Strategiile utilizate în tratamentul infecţiei cu Helicobacter pylori la copii derivă în mare parte din schemele
de tratament stabilite de consensuri clare la pacienţii adulţi. Tripla terapie ce asociază inhibitori de pompă
protonică (IPP), claritromicina şi amoxicilina este utilizată pe scară largă în întreaga lume; alături de aceasta
s-au impus şi alte asocieri bazate fi e pe IPP, fi e pe săruri de bismut. La ora actuală au fost introduse noi
preparate în schemele antibacteriene, alături de esomeprazol, pantoprazol sau lansoprazol afl ându-se
azitromicina şi fl uorochinolonele (la copiii mari). În acelasi timp au fost propuse terapii de linia a doua,
rezervate cazurilor de recădere sau reinfecţie bacteriană, acestea fi ind reprezentate de asocieri cvadruple
bazate pe săruri de bismut sau de regimuri ce includ fl uorochinolone; nu în ultimul rând, introducerea terapiei
secvenţiale la copii reprezintă o alternativă promiţătoare. Pe plan profi lactic, vaccinarea orală sau
intramusculară, aceasta din urmă afl ată în faze avansate de testare clinică, va putea contribui în viitor la
scăderea incidenţei infecţiei cu H.pylori, a gastritei, ulcerului şi cancerului gastric
SINDROMUL ROVIRALTA – PREZENTARE DE CAZ
Sindromul Roviralta reprezintă asocierea dintre stenoza hipertrofi că de pilor şi hernia hiatală. Autorii prezintă
cazul unui copil de sex masculin în vârstă de 2 luni internat în Clinica a V-a de Pediatrie a Spitalului de Urgenţă pentru Copii „Sfânta Maria“, Iaşi pentru vărsături postprandiale şi pierderea progresivă în greutate. Tranzitul esogastroduodenal baritat relevă hernie hiatală, canal piloric îngustat şi refl ux gastroesofagian prezent
inconstant. Ecografi a a confi rmat hipertofi a olivei pilorice şi pH-metria, refl uxul gastroesofagian. Tratamentul medical cu antispastice, rehidratarea, inhibitori de pompă de protoni şi prokinetice a avut succes, iar evoluţia a
fost bună
Effects of prophylactic ibuprofen and paracetamol administration on the immunogenicity and reactogenicity of the 10-valent pneumococcal non-typeable Haemophilus influenzae protein D conjugated vaccine (PHiD-CV) co-administered with DTPa-combined vaccines in children: An open-label, randomized, controlled, non-inferiority trial
Prophylactic paracetamol administration impacts vaccine immune response; this study (www.clinicaltrials.gov: NCT01235949) is the first to assess PHiD-CV immunogenicity following prophylactic ibuprofen administration. In this phase IV, multicenter, open-label, randomized, controlled, non-inferiority study in Romania (November 2010–December 2012), healthy infants were randomized 3:3:3:1:1:1 to prophylactically receive immediate, delayed or no ibuprofen (IIBU, DIBU, NIBU) or paracetamol (IPARA, DPARA, NPARA) after each of 3 primary doses (PHiD-CV at age 3/4/5 months co-administered with DTPa-HBV-IPV/Hib at 3/5 and DTPa-IPV/Hib at 4 months) or booster dose (PHiD-CV and DTPa-HBV-IPV/Hib; 12–15 months). Non-inferiority of immune response one month post-primary vaccination in terms of percentage of infants with anti-pneumococcal antibody concentrations ≥0.2 µg/mL (primary objective) was demonstrated if the upper limit (UL) of the 98.25% confidence interval of difference between groups (NIBU vs IIBU, NIBU vs DIBU) was <10% for ≥7/10 serotypes. Immunogenicity and reactogenicity/safety were evaluated, including confirmatory analysis of difference in fever incidences post-primary vaccination in IBU or DIBU group compared to NIBU. Of 850 infants randomized, 812 were included in the total vaccinated cohort. Non-inferiority was demonstrated for both comparisons (UL was <10% for 9/10 vaccine serotypes; exceptions: 6B [NIBU], 23F [IIBU]). However, fever incidence post-primary vaccination in the IIBU and DIBU groups did not indicate a statistically significant reduction. Prophylactic administration (immediate or delayed) of paracetamol decreased fever incidence but seemed to reduce immune response to PHiD-CV, except when given only at booster. Twenty-seven serious adverse events were reported for 15 children; all resolved and were not vaccination-related
Corporate Governance mechanisms as drivers that enhance the credibility and usefulness of CSR disclosure
The aim of this paper is to examine what kinds of CG mechanisms (institutional,
firm or group level) are driving getting an Assurance or a GRI application
level, like CSR disclosure decisions linked with credibility and usefulness of the
information disclosed, in the particular context of energy companies. Previous evidence
is scarce and does not jointly consider all levels of CG mechanisms. Our sample
is composed by 176 energy companies worldwide which currently report about
CSR through a sustainability report. On the basis of our findings, we could support
the idea that the credibility of the CSR report of the utilities companies will be
greater if the company listed in a Relation-Based country has an Assurance report.
In addition, those companies that have a concentrated ownership and the fewer insiders
sitting in the BoD present more probabilities of having an Assurance. Moreover,
the usefulness of the CSR information provided by this kind of firms will be higher,
the greater the efficiency of the BoD will be. The enhancement of the credibility and
the usefulness of the information reported is essential for companies involved in this
sector due to the frequent claim of window-dressing behaviours
