Mental Health in the Workplace: Situation Analyses, Poland
[From Introduction] The workplace is an appropriate environment in which to educate and raise individuals’ awareness about mental health problems. For example, encouragement to promote good mental health practices, provide tools for recognition and early identification of the symptoms of problems, and establish links with local mental health services for referral and treatment can be offered. The need to demystify the topic and lift the taboos about the presence of mental health problems in the workplace while educating the working population regarding early recognition and treatment will benefit employers in terms of higher productivity and reduction in direct and in-direct costs. However, it must be recognised that some mental health problems need specific clinical care and monitoring, as well as special considerations for the integration or re-integration of the individual into the workforce
Can suicide be prevented?
Sociological, criminological, psychological and psychiatric models of suicide prevention can be construed
on the grounds of the existing extensive both theoretical and practical body of knowledge about
suicide risk factors, as well as social, psychological and biological determinants of suicidal behaviors.
Using this knowledge National Programs and Strategies of Suicide Prevention were developed in many
European countries in the 1990’s, while in England in 2002 and in Ireland in 2005. Regretfully, no such
program has been created so far in Poland. Evaluation of the implemented programs has indicated their
considerable efficacy, as suicide numbers and rates in these countries have decreased by even over
30% during the past 8–10 years. This shows that in many cases suicide can be prevented and that it is
possible to reduce negative socio-economic and health-related effects of suicide.Na temat czynników ryzyka i uwarunkowań społecznych, psychologicznych oraz biologicznych zachowań
samobójczych zgromadzono ogromną wiedzę teoretyczną i praktyczną. Na tej podstawie można
skonstruować socjologiczny, kryminologiczny, psychologiczny i psychiatryczny model zapobiegania samobójstwom.
W latach 90. XX wieku w wielu krajach Europy, a w 2002 roku w Anglii i w 2005 roku w Irlandii,
opracowano, korzystając z tej wiedzy, narodowe programy i strategie zapobiegania samobójstwom. Niestety,
w Polsce dotychczas nie opracowano jeszcze takiego programu. Doświadczenia w realizacji tych
programów wykazały ich znaczną skuteczność w zmniejszeniu współczynników i liczby samobójstw w wymienionych
krajach w okresie 8+10 lat nawet o ponad 30%. Oznacza to, że można zapobiegać znacznej
części samobójstw oraz ograniczać ich negatywne skutki społeczno-ekonomiczne i zdrowotne
Controversy concerning sociological concepts of suicide
Sociological concepts of suicide are outlined from the following four research perspectives, in terms of:
1. social integration, with the paramount classical theory by Emil Durkheim;
2. socio-economic change or modernization;
3. cultural factors - values, norms, and social attitudes;
4. transmission of suicidal behaviors by mass-media and pop culture.
An attempt was made in the paper to present arguments in support of two statements:
1. Controversies, particularly those concerning the theory of suicide proposed by Durkheim in 1897, are
due to misunderstanding of its historical and social determinants, to erroneous interpretations, and to
the enormous development of scientific methodology and computer sciences in the period of over
a century since the theory was put forward.
2. Neither psychological nor biological determinants of suicide are comparable to sociological ones, as
the latter deal with an analysis of impact of political, social and economic factors on the frequency
and variability of individual suicidal acts at the community and society levels, and not at the level of
the individual’s choices and tragedy.Socjologiczne koncepcje samobójstw zostaną przedstawione w czterech perspektywach badawczych:
1. społecznej integracji, w której dominuje klasyczna teoria Emila Durkheima;
2. społeczno-ekonomicznych zmian lub modernizacji;
3. kulturowych czynników — wartości, norm i postaw społecznych;
4. transmisji zachowań suicydalnych przez mass media i całą kulturę masową.
W pracy podjęto próbę uzasadnienia dwóch tez:
1. Kontrowersje, zwłaszcza wokół teorii samobójstw Durkheima z 1897 roku, są wynikiem niezrozumienia
jej historycznych i społecznych uwarunkowań, fałszywych interpretacji oraz ogromnego — w okresie
ponad wieku od jej powstania — rozwoju metodologii nauk społecznych, technologii informatycznych
i komputerowych.
2. Socjologiczne uwarunkowania samobójstw nie są porównywalne z ich uwarunkowaniami psychologicznymi
i biologicznymi, ponieważ odnoszą się do analizy wpływu czynników politycznych, społecznych
i ekonomicznych na częstotliwość oraz zmienność indywidualnych aktów samobójczych na
poziomie całej społeczności i społeczeństwa, a nie na poziomie jednostkowych wyborów i tragedii
Assessment of suicidal behavior risk among persons with mental disorders. A comparative analysis of selected studies conducted in the years 2000-2006
Sixty eight papers were selected for in this review from three English-language databases: Medline, Pro-
Quest and Science-Direct by a single criterion of reporting follow-up studies on suicidal behavior risk
assessment in the context of mental disorders.
A comparative analysis of the reported results indicates that affective disorders, generally regarded as
the most important risk factor for suicidal behaviors, have shown a decreasing tendency since the introduction of antidepressants in the 1960s. Lifetime suicide risk estimated some two or three decades ago
at 15% in persons with affective disorders has decreased at present to 7–8%, and to 4.9% in those
suffering from schizophrenia.
Anxiety disorders increase the relative risk (OR) for suicidal thoughts 2.32 times, and of attempted suicide
3.54 times. Suicide risk is increased most by comorbidity of two or more mental disorders.
As regards eating disorders, women diagnosed with anorexia nervosa attempt suicide twice as often as
do those with bulimia. Suicide risk in women with mental disability is the same as that in women in the
general population, while in mentally disabled men the risk is by 1/3 lower than that for males in the
general population.
The standardized mortality rate (SMR) for suicide among persons with mental disorders associated with
alcohol dependence is 9.79 times higher than that expected in the general population, while the following
SMRs exceed those in the general population: for heavy drinkers - 3.51 times, for opiate users - 13.51
times, for intravenous drug users 13.73 times, and for mixed drug users 16.85 times.
An analysis of research findings concerning the assessment of suicidal behavior risk among persons
diagnosed with various mental disorders may provide useful cues for therapeutic and preventive interventions.Na podstawie kryterium wyboru wyłącznie prac opisujących wyniki badań typu follow-up, dotyczących
oceny ryzyka zachowań samobójczych w kontekście zaburzeń psychicznych, z anglojęzycznych baz
Medline, ProQuest i Science-Direct wybrano 68 artykułów.
Analiza porównawcza wyników badań wykazała, że zaburzenia afektywne - powszechnie uważane za
największy czynnik ryzyka zachowań samobójczych - po wprowadzeniu w latach 60. XX wieku leków
przeciwdepresyjnych są coraz rzadsze. Ryzyko suicydalne w przypadku zaburzeń afektywnych z szacowanego
na 15% jeszcze 20-30 lat wcześniej zmalało do przeciętnie 7-8% obecnie w okresie całego
życia, a dla schizofrenii jest obecnie szacowane na 4,9%.
Zaburzenia lękowe zwiększają współczynnik względnego ryzyka (OR) myśli samobójczych 2,32-krotnie,
a prób samobójczych - 3,64-krotnie. Ryzyko samobójstwa najbardziej zwiększa współwystępowanie
2 lub więcej zaburzeń psychicznych (comorbidity).
Wśród kobiet z zaburzeniami jedzenia próby samobójcze zdarzają się 2-krotnie częściej w przypadku
diagnozy jadłowstrętu psychicznego (anorexia nervosa) niż bulimii. Kobiety z upośledzeniem umysłowym
cechują się takim samym ryzykiem samobójstwa jak kobiety w populacji ogólnej, a mężczyźni - o 1/3
mniejszym niż mężczyźni z populacji ogólnej.
W przypadku zaburzeń psychicznych związanych z uzależnieniem od alkoholu standaryzowany wskaźnik
umieralności (SMR) z powodu samobójstwa jest 9,79-krotnie większy niż oczekiwany w populacji ogólnej,
w przypadku nadmiernego picia (heavy drinking) alkoholu - 3,51-krotnie większy, używania opiatów
- 13,51-krotnie, dożylnego przyjmowania narkotyków - 13,73-krotnie, a w przypadku używania kilku
narkotyków (mixed drugs) - 16,85-krotnie wyższy.
Analiza wyników badań na temat oceny ryzyka zachowań samobójczych w różnych rozpoznaniach zaburzeń
psychicznych może być pożyteczną wskazówką dla działań terapeutycznych i prewencyjnych
Contemporary social-cultural concepts and theories of suicide
The paper being a continuation of the author’s earlier work published in Suicidology vol. 1. outlines major
contemporary social and cultural concepts and theories of suicide. They will be presented from four
research and theoretical perspectives.
Two sociological perspectives - in terms of: the model of social integration; and the modernisation or
socio-economic change model.
Two cultural perspectives - in terms of: the model of patterns and values of a given culture, ethical and
legal norms, attitudes towards suicide prevailing in the society, and economic factors and the model of
suicidal behaviours transmission via mass media and pop culture.
The social integration model includes concepts dealing with age and life cycle, domestic integration in
the family, migration and theory of status integration.
Under the heading of the modernisation model concepts will be described dealing with the influence
exerted on suicide by such processes as industrialisation, urbanisation, education and secularisation, as
well as transition of socio-economic systems. Explanations of suicidal behaviour in terms of the cultural model will be focused on the gender roles
socialisation, ethical and legal norms, religion, ethnic differences (white vs. black race), alcohol consumption,
unemployment, and a demographic conception of the Relative Cohort Size (RCS) in the population
in the productive age.
The model of suicidal behaviour transmission contains the imitation theory (the copycat and the snowball
effect), the differential identification theory, and a concept of suicidal musical subcultures.
Is it possible to construct a single integrative multifactor socio-cultural theory of suicide?Niniejsza praca jest kontynuacją artykułu opublikowanego w I tomie „Suicydologii” (2005 r.). Jej celem
jest opis najważniejszych współczesnych społecznych i kulturowych koncepcji oraz teorii samobójstw. Zostaną
one przedstawione je w czterech perspektywach badawczych i teoretycznych.
Dwie perspektywy socjologiczne: według modelu społecznej integracji oraz według modelu modernizacji
lub zmian społeczno-ekonomicznych.
Dwie perspektywy kulturowe: według modelu wzorów i wartości danej kultury, norm etycznych i prawnych,
postaw społecznych wobec samobójstwa oraz czynników ekonomicznych oraz według modelu
transmisji zachowań samobójczych przez mass media i kulturę masową.
Model społecznej integracji obejmuje koncepcje związane z wiekiem i cyklami życia, integracją rodzinną,
migracji i teorią integracji statusu.
W ramach modelu modernizacyjnego autor opisuje koncepcje wpływu na samobójstwa takich procesów,
jak: industralizacja, urbanizacja, edukacja i sekularyzacja oraz zmiana systemu społeczno-ekonomicznego.
W modelu kulturowych wyjaśnień zachowań samobójczych autor skupi się na roli socjalizacji według płci,
norm etyczno-prawnych, religii, różnic etnicznych (rasa czarna-rasa biała), spożycia alkoholu, bezrobocia
i koncepcji demograficznej - relatywnej wielkości populacji ludności w wieku produkcyjnym (RCS).
Model transmisji zachowań samobójczych zawiera teorię imitacji (naśladownictwa, powielenia, efektu
kuli śniegowej), teorię zróżnicowanej identyfikacji i koncepcję suicydalnych podkultur muzycznych.
Podjęto próbę odpowiedzi na pytanie, czy jest możliwe stworzenie jednej integracyjnej, wieloczynnikowej
społeczno-kulturowej teorii samobójstw
Zachowania samobójcze wśród pacjentów lecznictwa odwykowego - wybrane psychospołeczne i medyczne czynniki ryzyka
Background. One of the two goals set in the study was to answer the following research question: which
of the selected psychosocial and medical variables are statistically significant risk factors for suicidal
behaviors among alcohol dependent people? Nine hypotheses were posed and statistically verified.
Material and methods. Three groups were investigated: 120 alcohol dependent patients after attempted
suicide, 120 patients with alcohol dependence and no suicidal behaviors, and 120 persons with neither
alcohol dependence nor suicidal behaviors. The first two groups were randomly selected from among
patients of 12 alcohol treatment centers (of 4 types) nationwide. All the participants (N = 360) were
examined using the following 6 questionnaires: the UNC Functional Social Support Questionnaire, the
Rand Social Activities Questionnaire, the Orientation to Life SOC-29 by Antonovsky, the CISS by Endler
and Parker, the Beck Depression Inventory (BDI), and a questionnaire covering sociodemographic data,
suicidal behaviors, and the patient’s medical history of alcohol abuse and treatment.
Results. Suicidal behaviors of alcohol dependent persons were not related either to their unemployment/professional position, or severity of their family problems. The following variables were found to be relatively
the strongest risk factors of suicidal behaviors among patients of alcohol treatment centers: a weak
sense of coherence, higher frequency of adverse life events, focusing on negative emotions, distraction
as a strategy of coping with stress, a low tendency to seek social contacts and low social activity, limited
functional social support, and suicidal death of a friend or family member. The most important medical
risk factors of suicide included: severe depressive disorders, longer history of problem drinking, onset of
regular drinking at a younger age, more episodes of inpatient alcohol treatment, and higher frequency of
alcohol psychoses.
Conclusions. Identification of groups at risk for suicide on the grounds of individual diagnoses utilizing
the research findings presented in the paper may contribute to a more effective treatment and prevention
of suicidal behaviors among patients of alcohol treatment centers.
Suicydologia 2009-2010; 5-6: 64-76Wstęp. Jednym z celów w wybranych badaniach przedstawionych w niniejszej pracy było znalezienie
odpowiedzi na pytanie: które z wybranych do badania zmiennych psychospołecznych i medycznych
funkcjonują jako istotne statystycznie czynniki ryzyka zachowań samobójczych wśród osób uzależnionych
od alkoholu? Sformułowano 9 hipotez badawczych i poddano je weryfikacji statystycznej.
Materiał i metody. Z 12 ośrodków lecznictwa odwykowego (4 typów) z terenu całego kraju wybrano
3 grupy metodą doboru losowego: 120 pacjentów uzależnionych od alkoholu po próbach samobójczych
(grupa S), 120 pacjentów uzależnionych od alkoholu bez zachowań samobójczych (grupa A) - I grupa
kontrolna, i 120 osób bez uzależnienia od alkoholu i bez zachowań samobójczych, dobranych parami (grupa
NA) - II grupa kontrolna. Całą próbę badawczą (n = 360) zbadano zestawem 6 narzędzi badawczych:
polskie wersje 2 kwestionariuszy - UNC Functional Social Support Questionnaire i Rand Social Activities
Questionnaire, kwestionariusz orientacji życiowej SOC-29 Antonovsky’ego, kwestionariusz Coping Inventory
for Stressful Situations Endlera i Parkera, Skala Depresji Becka oraz kwestionariusz z danymi socjodemograficznymi,
o zachowaniach samobójczych oraz medycznymi na temat nadużywania alkoholu i leczenia.
Wyniki. Z 9 postawionych hipotez 7 pozytywnie zweryfikowano. Nie zaobserwowano związku między
bezrobociem i pozycją w pracy oraz nasileniem problemów rodzinnych a zachowaniami samobójczymi
wśród osób uzależnionych od alkoholu. Stwierdzono, że relatywnie najsilniejszymi psychospołecznymi
czynnikami ryzyka zachowań samobójczych wśród pacjentów lecznictwa odwykowego są: niski poziom
poczucia koherencji, większa częstotliwość negatywnych zdarzeń życiowych, koncentracja na negatywnych
emocjach, odwracanie uwagi od stresu, mała dyspozycja do poszukiwania kontaktów społecznych
i niska aktywność społeczna, słabe funkcjonalne społeczne wsparcie oraz samobójcza śmierć bliskiej
osoby. Do najważniejszych medycznych czynników zagrożenia suicydalnego należą nasilone zaburzenia
depresyjne, dłuższy staż problemowego picia alkoholu, młodszy wiek rozpoczęcia regularnego picia
alkoholu, większa częstość hospitalizacji odwykowych i częstsze występowanie psychoz alkoholowych.
Wnioski. Identyfikacja grup ryzyka suicydalnego przez indywidualne diagnozy na podstawie wyników
przedstawionych badań powinna być pomocna w poprawie skuteczności leczenia i zapobiegania zachowaniom
samobójczym wśród pacjentów lecznictwa odwykowego.

The Warsaw Educational-Preventive Programme targeted at self-aggressive behaviour among youth
W niniejszej pracy przedstawiono opis warszawskiego programu edukacyjno-profilaktycznego w zakresie
zachowań autoagresywnych wśród młodzieży szkolnej, który realizowano w IV kwartale 2008 roku
w formie szkoleń dla nauczycieli w 20 wybranych szkołach (gimnazja, licea i technika). Program składa się
z trzech części. W pierwszej części artykułu opisano ogólne założenia programów prewencyjnych opracowanych
dla szkół i przykłady kilku takich strategii w wybranych krajach. W drugiej części zawarto metody
i formy realizacji szkoleń w ramach tego programu w poszczególnych szkołach. Trzecia część zawiera
szczegółowy konspekt szkoleń w formie wykładów z elementami warsztatów, a także treści programowe
i materiały edukacyjne dla uczestników, obejmujące: przebieg procesu zachowań samobójczych, sygnały
ostrzegawcze, czynniki ryzyka samobójstwa, prowadzenie rozmowy z uczniem w kierunku określenia
ryzyka zachowań samobójczych, wskazania do hospitalizacji, psychoedukację uczniów i rodziców
w zakresie promocji zdrowia psychicznego, plan pracy z klasą szkolną po próbie samobójczej lub samobójstwie
oraz wskazówki, gdzie szukać pomocy.The paper outlines an educational programme aimed at self-aggressive behaviour prevention among inschool
youth, in the form of training courses for teachers. The programme was implemented in 20 selected
Warsaw schools (junior and senior high, and technical secondary schools) from October to December
2008. It consisted of three parts. In Part 1 a general rationale of school-based prevention programmes
and examples of such strategies introduced in several countries were presented. Part 2 was devoted to
methods and forms of training courses introduced within this programme in particular schools. In Part 3
a detailed curriculum including lectures with workshop elements was given, containing training contents
and educational materials that acquainted the participants with the course of suicidal behaviour process,
warning signals, suicidal risk factors, instructions on how to talk to the student so as to assess suicidal
behaviour risk, indications to hospitalization, psychoeducation for parents and students, how to work with
the class after a suicide attempt, and where to seek help
Czynniki ryzyka majaczenia alkoholowego u osób leczonych stacjonarnie w Polsce z powodu uzależnienia od alkoholu
Introduction: Delirium tremens (DT) is a life-threatening complication that occurs in 5–33% of patients treated for alcohol withdrawal syndrome. The aim of the present study was to analyse the risk factors for delirium tremens among patients treated for alcohol dependence.
Methods: Medical data of 1027 patients hospitalised in Alcohol Detoxification and Rehabilitation Units of Nowowiejski Hospital in Warsaw were analysed. The relationships between variables that are thought to be related to the prevalence of delirium tremens in this population were evaluated. The relevant psycho-social and demographic data as well as information on alcohol use and treatment history were included.
Results: 262 patients (25.5%) confirmed delirium tremens in the past. Among the variables included in the logistic regression model, previous episodes of consciousness loss, suicide attempts and episodes of alcohol withdrawal syndrome, as well as harmful use of other psychoactive substances, proved to be significant risk factors for delirium tremens.
Discussion: The higher prevalence of DT among patients with previous suicide attempts confirms the relationship between the severity of alcohol dependence and suicide attempts and DT. Additionally, higher risk of DT in patients with previous episodes of consciousness loss can be explained by an increased number of head injuries in individuals with severe addiction. Presumably, the higher prevalence of DT among patients with previous episodes of alcohol withdrawal syndrome and other substance abuse can be attributed to the kindling model.
Conclusions: Previous episodes of alcohol withdrawal syndrome are the risk factors of delirium tremens attributable to the severity of alcohol dependence. In our opinion, the other risk factors, such as loss of consciousness, suicide attempts in the past and harmful use of other psychoactive substances, are nonspecific to alcohol dependence
