    Critical Access Hospitals and Retail Activity: an Empirical Analysis

    This paper takes an empirical approach to determining the effect that a critical access hospital (CAH) has on local retail activity. Previous research on the relationship between hospitals and economic development has primarily focused on single-case, multiplier oriented analysis. The positive empirical results provide additional evidence on the far-reaching economic development impacts of CAHs.     Expanding the multicolor capabilities of basic confocal microscopes by employing red and near-infrared quantum dot conjugates

    <p>Abstract</p> <p>Background</p> <p>Confocal microscopy is a widely employed methodology in cellular biology, commonly used for investigating biological organization at the cellular and sub-cellular level. Most basic confocal microscopes are equipped to cleanly discriminate no more than four fluorophores in a given sample, limiting the utility of this method for co-localization, co-expression, and other multi-parameter analyses. In this study, we evaluated the use of red and near-infrared emitting quantum dot staining reagents to expand the multi-parameter capabilities of basic confocal microscopes.</p> <p>Results</p> <p>We modified a three-laser Zeiss Pascal confocal microscope by the addition of two band-pass filters and one long-pass filter for the detection of three different red to near-infrared quantum dot conjugates. We then performed direct comparisons between organic dye- and quantum dot-labeled detection reagents for the detection of subcellular structures. We found that the quality of staining was generally indistinguishable, although quantum dot reagents do have certain limitations, relative to organic dye conjugates. Using the modified Pascal system, three quantum dot conjugates, two organic dye conjugates, and one fluorescent protein, we demonstrated clean discrimination of six distinct fluorescent labels in a single sample.</p> <p>Conclusion</p> <p>Our data demonstrate that nearly any basic confocal microscope can be modified by the simple addition of appropriate emission filters, allowing the detection of red and near-infrared quantum dot conjugates. Additionally, quantum dot- and organic dye-based secondary reagents can be successfully combined in complex intracellular staining experiments. Substantial expansion of the multi-parameter capabilities of basic confocal instruments can be achieved with a financial investment that is minimal in comparison to instrument replacement or upgrade with additional lasers.</p

    Neurological and neurocognitive function of HIV-infected children commenced on antiretroviral therapy

    Aim: To describe neurological and neurocognitive deficits in HIV-infected children and the short-term effect of highly active antiretroviral therapy (HAART) on the observed deficits. Methods: In this prospective study, 39 children (15 females) were evaluated before the start of HAART and 30 reassessed 6 months later. The subjects were evaluated with a range of cognitive tests used in everyday clinical practice. Results: At enrolment, the mean (±SD) age was 60±46 months, 17 (44%) and 22 (56%) had Centers for Disease Control (CDC) clinical category B and C disease respectively, and 36 (92%) had severe immunosuppression. At the start of HAART no child had cranial nerve or cerebellar dysfunction, but 13/29 (33.3%) had evidence of motor dysfunction. By 6 months 1 child had developed cerebellar dysfunction, but there was no statistically significant change in the frequency of motor dysfunction. Mean baseline performances on cognitive testing were generally subnormal. Between 33% and 81% of the children recorded subnormal intelligence quotients on various cognitive tests. Mean performances did not change significantly after 6 months of HAART. Conclusion: Neurological and neurocognitive deficits are frequent in HIV-infected children. The prevalence and extent of deficits did not change significantly in response to short-term HAART, indicating neither spontaneous improvement nor deterioration during early treatment

    Magneto-electrostatic trapping of ground state OH molecules

    We report the magnetic confinement of neutral, ground state hydroxyl radicals (OH) at a density of ∼3×103\sim3\times10^{3} cm−3^{-3} and temperature of ∼\sim30 mK. An adjustable electric field of sufficient magnitude to polarize the OH is superimposed on the trap in either a quadrupole or homogenous field geometry. The OH is confined by an overall potential established via molecular state mixing induced by the combined electric and magnetic fields acting on the molecule's electric dipole and magnetic dipole moments, respectively. An effective molecular Hamiltonian including Stark and Zeeman terms has been constructed to describe single molecule dynamics inside the trap. Monte Carlo simulation using this Hamiltonian accurately models the observed trap dynamics in various trap configurations. Confinement of cold polar molecules in a magnetic trap, leaving large, adjustable electric fields for control, is an important step towards the study of low energy dipole-dipole collisions.Comment: 4 pages, 4 figure
