Corporeal Shock Wave Lithotripsy (ESWL). Like a therapeutic procedure, extracorporeal lithotripsy
may be accompanied by complications. Most of this are minor complications, but in a lower percentage,
major complications can be appear.Materials and methods: The study was made in the Urology and Nephrology Department of
the Republican Clinical Hospital, during January 2015 and November 2015, on a group of 120 (65 male
and 55 female, mean age 41,3 years) patients diagnosed with reno-ureteral lithiasis and treated with
ESWL. The dimension of the calculi has varied between 0,6 and 15 mm. Were analyzed the
complications after ESWL.
Results: Hematoma is the most serious complication of extracorporeal lithotripsy, with a low
incidence (0,83%). Acute pyelonephritis (3,33%) occurs either due to a pre-existing urinary infection,
or by the release of germs located into the calculi during fragmentation. Flanc pain (98,3%) was the most
common symptom was on the side were was made de ESWL. The pain disappear after 2-3 days with
non-steroidal anti-inflammatory drugs. „Steinstrasse” (5.83%) was another complication and was
resolved by administration of conservative treatment.
Conclusions: ESWL is a safe method to treat stones when proper indications are followed. But
when we have the complication after ESWL we must as soon as possible to find this complications and
to treat as well

The role of tamsulosin administration in evolution of stone clearence after shock wave lithotripsy for ureteral stones

condition in urological practice. ESWL, a noninvasive technology, has become one of the main
active interventions for ureteral stones; its success depends on stone size and location, and the
type of lithotripter. Expulsion therapy of the stone requires ureteric peristalsis, tamsulosin must
be the first as an adjunctive medical therapy after ESWL, is more effective for the treatment of
patients with ureteral stones.
Aim of the study. To determine whether the administration of tamsulosin, as a medical therapy,
increases the stone clearance after extracorporeal shock wave lithotripsy (ESWL).
Materials and methods. A total of 250 patients underwent a single ESWL session to treat
ureteral stone up to 15 mm in diameter. After ESWL patients were randomized in two groups.
Group A (control) – 125 patients were administered non-steroidal anti-inflammatory drugs. In
group B, 125 patients additionally were prescribed tamsulosin 400 mg daily. Follow-up visits were performed once per week for 4 weeks after ESWL. Evaluation included a KUB plain film
and an ultrasound examination. Efficacy was evaluated in terms of success rate, stone-free rate,
expulsion time of the fragments and use of tamsulosin.
Results. The success rate was for the control group was 65 % and the tamsulosin group was 80
%, respectively. The mean expulsion time of the fragments was 10.2 days for group A and 8
days for group B. The stone-free rate in group A was 67 % and in group B – 87%.
Conclusions. The results of our study have demonstrated that tamsulosin therapy, as an adjuvant
medical therapy after ESWL, is more effective for the treatment of patients with ureteral stone
up to 15 mm

The role of computed tomography densitometry in assessing the outcomes of extracorporeal shock wave lithotripsy of reno-ureteral calculi: prospective, descriptive study

Introducere. De la introducerea metodei de litotriție extracorporală
cu unde de șoc, în anii 1980, această tehnologie
a devenit una din principalele metode de tratament a litiazei
renoureterale. Pentru evaluarea ratei de succes, aprecierea
densității calculului prin tomografie computerizată se bucură
de cea mai mare popularitate. Scopul acestei lucrări a fost de
a evalua eficacitatea aprecierii densității calculului în evaluarea rezultatelor tratamentului prin litotriție extracoprorală
(ESWL) cu unde de șoc al calculilor renoureterali.
Material și metode. În studiu au fost incluși 60 de pacienți
cu litiază renoureterală cu diametru de 0,5-2,0 cm, diagnosticată
prin tomografie computerizată cu estimarea densității
calculului, care au fost tratați prin litotriție extracorporală. În
timpul procedurii de litotriție extracorporală, au fost aplicate
4000 de impulsuri pe ședință, cu un maxim de energie de 3,0
kV. Numărul de ședințe a variat de la una până la trei. Ulterior,
pacienții au fost invitați la vizite de control, în dinamică, timp
de 1 lună.
Rezultate. Din numărul total de pacienți, incluși în studiu,
39/60 (65%) au fost cu rată de ,,stone free” după prima
ședință de tratament. S-a observat o relație liniară față de
rata mare de ,,stone free” și densitatea calculului. Toți acești
pacienți au avut densitatea calculului ≤600 unități Hounsfield
(UH). La 11/60 (18%) pacienți s-au aplicat două ședințe
de ESWL; aceștia au avut densitatea calculului cuprinsă între
600-1000 UH. La restul pacienților, 10/60 (17%), au fost aplicate
trei sedințe de ESWL; acesția au avut densitatea calculului
cuprinsă între 1000-1200 UH. Rata de ,,stone free”, după trei
sedințe de ESWL, a fost de 92%. Toți pacienții, care au avut
reușită în tratament, au prezentat calculi cu densitate de până
la 1000 UH. La 6 pacienți procedura de tratament a eșuat; toți
au prezentat calculi cu dimensiunea de maxim 17 mm și cu
densitatea de 1000-1200 UH, aceștia fiind referiți către alte
metode de tratament.
Concluzii. Aprecierea densității calculului până la litotriție
permite o estimare mai exactă a numărului de ședințe necesare
pentru atingerea ratei maximale de succes. De asemenea,
cunoașterea exactă a densității calculului poate ajuta la alegerea
metodei optimale de tratament din prima intenție.Abstract. Introduction. Since the introduction of the extracorporeal
shock wave lithotripsy in the 1980s, this method has become
one of the main methods of treatment of reno-ureteral lithiasis.
To evaluate the success rate, the computed tomography,
used to assess the calculus density, is among the most popular
procedures. The purpose of this study is to evaluate the efficacy
of calculus density in assessing treatment outcomes of reno-ureteral calculi by extracorporeal shock wave lithotripsy
(ESWL).
Material and methods. In this study, 60 patients with reno-
ureteral lithiasis of 0.5-2.0 cm were diagnosed by means
of computed tomography, the calculi density being performed.
The patients were treated by extracorporeal lithotripsy. During
the extracorporeal lithotripsy, 4000 pulses per session
were applied with a maximum energy of 3.0 kV. The number
of sessions ranged from one to three sessions. Subsequently,
the patients were monitored and followed up for 1 month.
Results. Of the total number of patients enrolled in the
research study 39/60 (65%) were “stone free” after the first
treatment session. We observed a linear proportion between
the high “stone free” rate and the calculus density, all of these
patients had the calculus density ≤600 Hounsfield units (HU).
In 11/60 (18%) patients, two ESWL sessions were applied and
they had the calculus density of 600-1000 HU. In 10/60 (17%)
patients, three sessions of ESWL were applied, and they had
the calculus density between 1000 and 1200 HU. The rate of
“stone free” after three ESWL sessions was 92%. All patients
who had the therapy success rate, had the calculus density of
up to 1000 HU. In 6 patients, the treatment procedure failed,
all of them had calculi with the maximum size of 17 mm and
the density of 1000-1200 HU, they being referred to other methods
of treatment.
Conclusions. The use of calculus densitometry before lithotripsy
offers the possibility to estimate more accurately the
number of required sessions in order to obtain a maximal success
rate. Also, exact information about the calculus density
may help finding the most optimal treatment method from the
beginning

geographical variation, with documented occurrence ranging from 1 to 3% of all urinary stones, and it
is increasing in developed countries. Objective of the study. The aim of this study is to review 10 years
of extracorporeal shock wave lithotripsy (ESWL) experience in paediatric urolithiasis patients. Material
and Methods. Retrospective data of paediatric urolithiasis patients who underwent ESWL between
2011 to 2021 were used in this retrospective observational study. Ultrasound was used to visualise and
locate the stones during the ESWL session. This required the paediatric patient to be exposed to the least
amount of radiation possible. For X-ray positive stones, X-ray monitoring was used. Results. There
were 28 stones found in total.Recurrent urolithiasis was diagnosed in 2 (7,15%) cases and primary
urolithiasis in 26 (92,85%) cases. The following are the total number of stones: One stone was found in
18 (64,29%) o two stones in 10 (35,71). The majority of the patients 21 (75 %) had stones that were less
than 10 mm long; 7 patients (25%) had stones that were between 11 and 15 mm long. The timing of
stone clearance differed among patients. With the aid of a Kaplan-Meier curve, the possibility of getting
stone clearance after ESWL at various time points was calculated. Conclusion. Extracorporeal shock
wave lithotripsy has been shown to be a safe and successful minimally invasive therapy for kids with
kidney stones.Introducere. Urolitiaza este o afecțiune cu o incidență mică, neobișnuită la populația pediatrică. Studiile
epidemiologice, în funcţie de aria geografică, documentează o incidență de până la 5% din totalul
calculilor sistemului renourinar, fiind în creștere, în special, în țările dezvoltate. Scopul lucrării. Scopul
acestui studiu este de a prezenta experiența de 10 ani, de aplicare a litotriției extracorporale cu unde de
șoc, în tratamentul litiazei renoureterale la copii. Material și Metode. Datele retrospective ale copiilor
cu litiază renoureterală, tratați prin litotriție extracorporală cu unde de șoc, în perioada anilor 2011 –
2021. Ecografia a fost utilizată pentru a vizualiza și a localiza calculii în timpul ședinței de ESWL. Acest
lucru este important, pentru ca pacientul pediatric să fie expus la cea mai mică cantitate de radiații
posibilă. Pentru calculii cu raze X pozitive s-a utilizat monitorizarea cu raze X. Rezultate. Tratamentului
a fost șupuși 28 pacienți. Urolitiaza recurentă a fost diagnosticată în 2 (7,15%) cazuri și urolitiaza
primară în 26 (92,85%) de cazuri. Alți parametri studiați au fost numărul de calculi: un calcul a fost
identificat în 18 (64,29%) cazuri şi doi calculi în 10 (35,71) cazuri. Majoritatea pacienților 21 (75%) au
prezentat calculi cu dimensiuni mai mici de 10 mm; 7 pacienți (25%) au prezentat calculi de dimensiuni
cuprinse între 11 și 15 mm. Perioada eliminării calculului a fost diferită, fiind aplicată curba Kaplan-
Meier, cu scop de a evalua și de a calcula posibilitatea obținerii clearance-ului după ESWL, la diferite
perioade de timp. Concluzii. Litotrizia extracorporală cu unde de șoc s-a dovedit a fi un tratament minim
invaziv, sigur și de succes, pentru pacienții pediatrici cu litiază renoureterală

În 1955, Yutkin, un inginer rus, a descoperit și descris pentru prima dată crearea undei de șoc subacvatice generată de descărcarea electrică a unei bujii. A doua ideie a apărut ca urmare a observațiilor unui grup de ingineri de la Compania Dornier din Germania, privind defecțiunile, fi surile, ce apăreau la diferite componente metalice ale avioanelor supersonice. Extracorporeal shock wave lithotripsy (ESWL) arose as a result of two separate and original ideas. In 1955, Yutkin, a Russian engineer, discovered and described the creation of underwater shock wave, who was generated by the electric discharge of a spark plug. A second idea was developed by the group of engineers from the company Dornier in Germany, they described the faults, fissures, which appeared in various metal components of supersonic aircraft.
