    Mechanisms and variability of salt transport in partially-stratified estuaries

    Submitted in partial fulfillment of the requirements for the degree of Doctor of Philosophy at the Massachusetts Institute of Technology and the Woods Hole Oceanographic Institution February 2000The variability of salt transport determines the variation of the length of the salinity intrusion and the large-scale density gradient in an estuary. This thesis contains three studies that address salt transport and the salt balance. The variation of salt transport with the depth, the along-channel salinity gradient, and the amplitude of the tidal velocity is investigated with analytic and numerical models. The results indicate that salt transport increases dramatically during stratified periods when vertical mixing is weak. Analysis of salt transport from observations in the Hudson Estuary show that stratified periods with elevated estuarine salt transport occur in five-day intervals once a month during apogean neap tides. Oscillatory salt transport, which is hypothesized to be primarily caused by lateral exchange and mixing of salt, appears to play a more minor role in the salt balance of the estuary. The salt balance of the estuary adjusts very little to the spring-neap modulation of salt transport but adjusts rapidly to pulses of freshwater flow. A simple model is used to investigate the process and time scales of adjustment of the salt balance by connecting variations of salt transport to the variations of freshwater flow and vertical mixing. The results show the length of the salinity intrusion adjust via advection to rapid and large increases in freshwater flow. The salinity intrusion adjusts more rapidly to the spring-neap cycle of tidal mixing the higher the freshwater flow.The National Science Foundation provided support through a National Science Foundation Graduate Fellowship and NSF Grant OCE94-15617.     The role of tides in bottom water export from the western Ross Sea

    Approximately 25% of Antarctic Bottom Water has its origin as dense water exiting the western Ross Sea, but little is known about what controls the release of dense water plumes from the Drygalski Trough. We deployed two moorings on the slope to investigate the water properties of the bottom water exiting the region at Cape Adare. Salinity of the bottom water has increased in 2018 from the previous measurements in 2008–2010, consistent with the observed salinity increase in the Ross Sea. We find High Salinity Shelf Water from the Drygalski Trough contributes to two pulses of dense water at Cape Adare. The timing and magnitude of the pulses is largely explained by an inverse relationship with the tidal velocity in the Ross Sea. We suggest that the diurnal and low frequency tides in the western Ross Sea may control the magnitude and timing of the dense water outflow

    Stress, salt flux, and dynamics of a partially mixed estuary

    A field study was performed in the lower Hudson River, a partially mixed estuary with a relatively simple geometry (Figure 1), between August and October of 1995. The objectives of the study were (1) to quantify and characterize the turbulent transport of momentum and salt, and (2) to relate the turbulent transport processes to the local and estuary-wide dynamics. The measurement program consisted of fixed and shipboard components. At a central site, a moored array of temperature-conductivity sensors and optical backscatter sensors (OBS), a bottom-mounted acoustic Doppler current profiler (ADCP), and a bottom-mounted array of acoustic travel-time current sensors (BASS), temperature-conductivity sensors, and OBS sensors resolved the vertical structure of velocity, salinity and turbidity and the near-bottom turbulence structure. Moored and bottom-mounted velocity, temperature, conductivity and pressure sensors at five secondary sites quantified the spatial and temporal variabilty of velocity, salinity and bottom pressure. Shipboard measurements with an ADCP and a conductivity-temperature-depth (CTD) profiler, accompanied by an OBS sensor, resolved the spatial structure and tidal variability of velocity, salinity and turbidity along several cross-channel and along-channel transects. This report describes the measurements in detail. Section II describes the instrumentation, Section III describes the deployment and sampling schemes, Section IV describes the data processing, and Section V is a summary of plots of selected data.     Neutropenia in Barth syndrome:characteristics, risks, and management

    PURPOSE OF REVIEW: Barth syndrome (BTHS) is an X-linked disease characterized by defective remodeling of phospholipid side chains in mitochondrial membranes. Major features include neutropenia, dilated cardiomyopathy, motor delay and proximal myopathy, feeding problems, and constitutional growth delay. We conducted this review of neutropenia in BTHS to aid in the diagnosis of this disease, and to improve understanding of both the consequences of neutropenia and the benefits of treatment with granulocyte colony-stimulating factor (G-CSF). RECENT FINDINGS: In 88 patients with BTHS, neutropenia, that is, at least one count below 1.5 × 10/l, was detected in 74 (84%) and 44% had severe chronic neutropenia, with multiple counts below 0.5 × 10/l. The pattern of neutropenia varied between intermittent and unpredictable, chronic and severe, or cyclical with mathematically regular oscillations. Monocytosis, that is, monocytes more than 1.0 × 10/l, was observed at least once in 64 of 85 (75%) patients. G-CSF was administered to 39 of 88 patients (44%). Weekly average G-CSF doses ranged from 0.12 to 10.92 μg/kg/day (mean 1.16 μg/kg/day, median 1.16 μg/kg/day). Antibiotic prophylaxis was additionally employed in 21 of 26 neutropenic patients. Pretreatment bone marrow evaluations predominantly showed reduced myeloid maturation which normalized on G-CSF therapy in seven of 13 examined. Consistent clinical improvement, with reduced signs and symptoms of infections, was observed in response to prophylactic G-CSF ± prophylactic antibiotics. However, despite G-CSF and antibiotics, one adult patient died with multiple infections related to indwelling medical devices and gastrostomy site infection after 15.5 years on G-CSF and a pediatric patient required gastrostomy removal for recurrent abdominal wall cellulitis. SUMMARY: BTHS should be considered in any men with neutropenia accompanied by any of the characteristic features of this syndrome. Prophylaxis with G-CSF ± antibiotics prevents serious bacterial infections in the more severe neutropenic patients although infections remain a threat even in patients who are very compliant with therapy, especially in those with indwelling devices

    Utilization of case presentations in medical microbiology to enhance relevance of basic science for medical students

    Background : Small-group case presentation exercises (CPs) were created to increase course relevance for medical students taking Medical Microbiology (MM) and Infectious Diseases (ID) Methods : Each student received a unique paper case and had 10 minutes to review patient history, physical exam data, and laboratory data. Students then had three minutes to orally present their case and defend why they ruled in or out each of the answer choices provided, followed by an additional three minutes to answer questions. Results : Exam scores differed significantly between students who received the traditional lecture-laboratory curriculum (Group I) and students who participated in the CPs (Group II). In MM, median unit exam and final exam scores for Group I students were 84.4% and 77.8%, compared to 86.0% and 82.2% for Group II students (P < 0.018; P < 0.001; Mann-Whitney Rank Sum Test). Median unit and final ID exam scores for Group I students were 84.0% and 80.0%, compared to 88.0% and 86.7% for Group II students (P < 0.001; P < 0.001). Conclusion : Students felt that the CPs improved their critical thinking and presentation skills and helped to prepare them as future physicians

    Developing a decision aid to guide public sector health policy decisions: A study protocol

    <p>Abstract</p> <p>Background</p> <p>Decision aids have been developed in a number of health disciplines to support evidence-informed decision making, including patient decision aids and clinical practice guidelines. However, policy contexts differ from clinical contexts in terms of complexity and uncertainty, requiring different approaches for identifying, interpreting, and applying many different types of evidence to support decisions. With few studies in the literature offering decision guidance specifically to health policymakers, the present study aims to facilitate the structured and systematic incorporation of research evidence and, where there is currently very little guidance, values and other non-research-based evidence, into the policy making process. The resulting decision aid is intended to help public sector health policy decision makers who are tasked with making evidence-informed decisions on behalf of populations. The intent is not to develop a decision aid that will yield uniform recommendations across jurisdictions, but rather to facilitate more transparent policy decisions that reflect a balanced consideration of all relevant factors.</p> <p>Methods/design</p> <p>The study comprises three phases: a modified meta-narrative review, the use of focus groups, and the application of a Delphi method. The modified meta-narrative review will inform the initial development of the decision aid by identifying as many policy decision factors as possible and other features of methodological guidance deemed to be desirable in the literatures of all relevant disciplines. The first of two focus groups will then seek to marry these findings with focus group members' own experience and expertise in public sector population-based health policy making and screening decisions. The second focus group will examine issues surrounding the application of the decision aid and act as a sounding board for initial feedback and refinement of the draft decision aid. Finally, the Delphi method will be used to further inform and refine the decision aid with a larger audience of potential end-users.</p> <p>Discussion</p> <p>The product of this research will be a working version of a decision aid to support policy makers in population-based health policy decisions. The decision aid will address the need for more structured and systematic ways of incorporating various evidentiary sources where applicable.</p

    Resonances in an evolving hole in the swash zone

When swashes first filled the initially circular hole on the rising tide, the dominant mode observed in the cross-shore velocity was consistent with a zero-order Bessel function solution (sloshing back and forth). As the tide rose and swash transported sediment, the hole diameter decreased, the water depth inside the hole remained approximately constant, and the frequency of the sloshing mode increased according to theory. About an hour after the swashes first reached the hole, it had evolved from a closed circle to a semi-circle, open to the ocean. When the hole was nearly semi-circular, the observed cross-shore velocity had two spectral peaks, one associated with the sloshing of a closed circle, the other associated with a quarter-wavelength mode in an open semi-circle, both consistent with theory.     Tricks and treats: designing technology to support mobility assistance dogs

    Assistance dogs are a key intervention to support the autonomy of people with tetraplegia. Previous research on assistive technologies have investigated ways to, ultimately, replace their labour using technology, for instance through the design of smart home environments. However, both the disability studies literature and our interviews suggest there is an immediate need to support these relationships, both in terms of training and bonding. Through a case study of an accessible dog treats dispenser, we investigate a technological intervention responding to these needs, detailing an appropriate design methodology and contributing insights into user requirements and preferences
