    MONOCYTES AND LYMPHOCYTES PROGNOSIS TOOLS AMONG STROKE PATIENTS

    Objectives: The values of white blood cell types were evaluated to determine which one is the most potential index in the diagnosis of patients with ischemic stroke (IS). Besides, neutrophil-lymphocyte ratio, neutrophil-to-monocyte ratio, and lymphocyte-to-monocyte ratio (LMR) were determined to confirm whether there is a relationship between these parameters and stroke. Methods: One thousand files of patients admitted between January 2017 and March 2020 to intensive care units at Alia Governmental Hospital in Hebron-Palestine were reviewed and evaluated. Only a total of 87 patients were found to meet our inclusion criteria and included in the study and a total of 913 patients were excluded. Besides, complete blood counts of a total of 95 patients’ data were also collected randomly from Private Lab in Hebron district, and were considered as healthy individuals (HI); that is, a control group. The data were introduced to the excel program 2010 version to facilitate reading and analysis. GraphPad Prism version (8.3.4.) was used to analyze the data. The data of patients and HI were compared to determine which value/s of these blood components is associated with the risk of stroke. Results: Different types of white blood cells; monocytes, lymphocytes, and neutrophils for stroke and healthy patients were collected and compared. Besides, the platelet count was also compared. It was found a marked significant decrease in the monocytes counts in all stroke patients (100–1500 cell/ul) compare to monocytes counts in the healthy donors (310–1740 cell/ul). Lymphocytes were also significantly decreased among all stroke patients (300–8600 cells/ul) compare to HI (910–9000 cells/ul). There was no significant increase in neutrophils in stroke patients. The ratios of neutrophils to monocytes and neutrophils to lymphocytes in stroke patients were compared too. The results showed a significantly elevated neutrophil ratio compare to monocytes and lymphocytes in all stroke patients. The platelet counts in stroke patients were also found less compared to HI. The percentages of these blood components were also evaluated. Conclusion: It is demonstrated that patients with stroke have lower lymphocyte and higher monocyte counts, and therefore, lower LMR values compared to the control group. It was found that MLR was significantly correlated with IS. LMR is associated with functional outcomes in patients with stroke

    Trichosporonasahii is a basidiomycete yeast responsible for white piedra and onychomycosis in the immunocompetent host. In the immunocompromised patients, invasive infections are reported; their diagnosis is difficult and they are associated with high mortality rate. Urinary infection due to Trichosporon Asahi is rare but its incidenceis increasing. We report the case of a 58 year old diabetic patient. The yeast was isolated from urine samples of three  consecutive crops in pure form. The patient improved after antifungal therapy

    Unified representation of fixtures: Clamping, locating and supporting elements in CNC manufacture

    International audienceA CNC machining operation is the outcome of the application of the integrated capabilities of various resources within the CNC machining centre. Part fixtures, clamping and other location mechanisms are essential subsets of CNC machining resources. Today, various fixturing techniques and attachments available on the market allow manufacturers to enhance their production capability without buying expensive machine tools. This technology rich fixturing domain is detached while representing and exchanging machine tool resource information for making manufacturing decisions. The research work in this paper utilizes the STEP-NC compliant Unified Manufacturing Resource Model (UMRM) for representing fixtures in conjunction with the parent CNC machining centre. Thus UMRM is enhanced in this context to represent various fixtures such as universal vises, chucks, pallets, auxiliary rotary tables among others. The major contribution of this paper is the application of the extension of the UMRM approach for representing fixturing domain, which allows generic modelling of fixtures and loading devices in addition to machine workpiece and process modelling. This would enable the stage of automated process planning and manufacturing. The universal approach in representing resource information allows the data to be utilised for making a wide variety of manufacturing decisions

    Bringing business risk management and technical security risk management together is one of the major challenges banks currently struggle with in order to increase their resilience against cyber security threats. This short paper presents a systematic approach for such an integrated security risk management which is currently developed in cooperation with a system-relevant bank. The approach uses well known methods and existing standards, it takes advantage of knowledge databases and available generic domain specific models. A first case study has just started. With tool support and especially with a high level of automation the presented approach might become applicable even for large banks

    Exogenous lipoid pneumonia caused by Nigella sativa oil – A case report

    Introduction: Exogenous lipoid pneumonia (ELP) is an uncommon and chronic infiltrative pulmonary process secondary to continued aspiration of exogenous lipids. Nigella sativa, or the black cumin seed, is a herb used in traditional medicine in many Middle Eastern and Asian countries to treat a broad array of diseases. Objective: The purpose of the present case report is to reveal the role of the abuse of N. sativa seed oil (black cumin seed oil), as an exceptional cause of ELP, which has not been, to our knowledge, previously documented. Case report: A 50-year-old man was evaluated for a 6-month history of cough, sputum and progressively worsening dyspnea on exertion. His medical history included long-term smoking. We questioned the patient specifically about use of any traditional herbal medicinal products. He acknowledged that he had been taking one tablespoon (15 ml) of N. sativa oil at bedtime since 8 months (8 bottles of 500 ml) for the purposes of tonification. Exogenous lipid pneumonia was suspected. Physical examination found bilateral crackles of the lung bases. Lung function tests were normal. The chest radiograph showed thoracic distension and bilateral basal interstitial disease. Computed tomography (CT) of the chest was performed, and revealed infiltrative lung disease affecting the posterobasal segments of the right and left lower lobes, honeycomb fibrosis was seen at the bases of both lungs. Routine blood tests were normal. Sputum and blood culture were sterile. We performed bronchoscopy with transbronchial biopsies and bronchoalveolar lavage. The color of the lavage liquid was blakish with fat globules on the fluid surface. It contained 220,000 cells/ml. Differential cell count showed 87% macrophages, 13% lymphocytes; stains and cultures for infectious agents were negative. The patient was advised to stop taking N. sativa oil. The only treatment that was instituted was N-acetylcysteine. A clinically significant change in symptoms and chest radiograph was observed. The patient has remained stable 18 months after the diagnosis

    L'association d'un cancer digestif avec la grossesse est rare et concerne avant tout les cancers colorectaux, de l'estomac ainsi que ceux se révélant par une tumeur de Krukenberg. Le diagnostic est souvent tardif expliquant la fréquence élevée des stades avancés. Nous rapportons un cas d'une femme enceinte qui a présenté une occlusion intestinale aigue sur tumeur colique. Il s'agissait d'une patiente enceinte de 32 semaines d'aménorrhées, admise pour syndrome occlusif évoluant depuis trois jours. L'examen abdominal retrouvait une distension abdominale. L'échographie abdominopelvienne retrouvait une grossesse monofoetale évolutive de 32 SA ± 2SA d'aménorrhée, ainsi qu'un épanchement intrapéritonéal de moyenne abondance. Un scanner abdominal a montré une occlusion grêlique sur processus de la dernière anse iléale. La patiente a été opérée six heures après son admission. Le traitement consistait à réaliser une colostomie temporaire afin de dériver les selles et de décomprimer rapidement le côlon et évacuer les gaz et les selles. Les suites postopératoires ont été marquées par des contractions utérines jugulées par la tocolyse intraveineuse

    Risk assessment and security testing of large scale networked systems with RACOMAT

    Risk management is an important part of the software quality management because security issues can result in big economical losses and even worse legal consequences. While risk assessment as the base for any risk treatment is widely regarded to be important, doing a risk assessment itself remains a challenge especially for complex large scaled networked systems. This paper presents an ongoing case study in which such a system is assessed. In order to deal with the challenges from that case study, the RACOMAT method and the RACOMAT tool for compositional risk assessment closely combined with security testing and incident simulation for have been developed with the goal to reach a new level of automation results in risk assessment
