    Disc coablation and epidural injection of steroids: a comparison of strategies in the treatment of mechanical spinal discogenic pain

    Summary In this study two strategies in the treatment of Mechanical Spinal Discogenic Pain have been compared: Disc Coablation and Epidural Injection of Steroids. In 2003 50 patients treated with one or two epidural injections have been selected ''ad random'' and 50 patients treated with disc coablation. Comparison of the data indicated an improvement of average VAS when relaxed for both groups ðp < 0:01Þ, while after slightmoderate strain, this value was significant only after coablation ðp < 0:001Þ. Finally, average VAS was clearly lower ðp < 0:01Þ after coablation as compared to epidural injections

    Methylparaben concentration in commercial Brazilian local anesthetics solutions

    FAPESP - FUNDAÇÃO DE AMPARO À PESQUISA DO ESTADO DE SÃO PAULOTo detect the presence and concentration of methylparaben in cartridges of commercial Brazilian local anesthetics. Twelve commercial brands (4 in glass and 8 in plastic cartridges) of local anesthetic solutions for use in dentistry were purchased from the Brazilian market and analyzed. Different lots of the commercial brands were obtained in different Brazilian cities (Piracicaba, Campinas and Sao Paulo). Separation was performed using high performance liquid chromatography (HPLC) with UV-Vis detector. The mobile phase used was acetonitrile: water (75:25 - v/v), pH 4.5, adjusted with acetic acid at a flow rate of 1.0 ml.min(-1). When detected in the solutions, the methylparaben concentration ranged from 0.01% (m/v) to 0.16% (m/v). One glass and all plastic cartridges presented methylparaben. 1. Methylparaben concentration varied among solutions from different manufacturers, and it was not indicated in the drug package inserts; 2.     Monolithic stationary phases represent a new generation of chromatographic separation media. These phases consist of a continuous separation bed prepared by in situ polymerization or consolidation inside the column tubing. In recent years, their simple preparation procedure, unique properties and excellent performance have attracted quite remarkable attention in liquid chromatography and capillary electrochromatography. This review summarizes the preparation, characterization and applications of monolithic stationary phases.     Cyproconazole Translocation in Coconut Palm Tree Using Vegetative Endotherapy: Evaluation by LC-MS/MS and Mathematical Modeling.

    Endotherapy is a potential eco-friendly alternative to conventional pesticide application techniques in coconut (Cocos nucifera Linn.) palms. Field trials applying cyproconazole by vegetative endotherapy using two different injection methods, pressurized and nonpressurized, were tested. Stem samples above the injection point were collected 2 to 45 days after application, and fruits were collected up to 45 to 120 days after application. Residue analyses on the coconut fruit and stem above the point of application of cyproconazole were performed on different days using modified QuEChERS (an acronym for “quick, easy, cheap, effective, rugged, and safe”) and LC-MS/MS MS (liquid chromatography–tandem mass spectrometry). For cyproconazole application in 2 m tall hybrid coconut palms, the residue analysis results revealed that the pressurized method achieves faster translocation and higher concentrations of cyproconazole in the upper part than the nonpressurized method, even when applying half the volume. After 120 days of application, both methods showed residue-free fruits, ensuring fruit quality. Based on these experimental results, mathematical models were developed to aid in interpreting translocation in the stem. The modeling was extrapolated for taller coconut palms, and a prediction was simulated for coconut leaf disease. The modeling was fitted to the stem translocation data, indicating that the pressurized method performs better in translocation efficiency

    Cost analysis of pulmonary lobectomy procedure: comparison of stapler versus precision dissection and sealant

Methods: The cost comparison analysis was based on the clinical pathway and drawn up by collecting the information available from the Thoracic Surgery Division medical team at Mantova Hospital. Direct resource consumption was derived from a previous randomized controlled trial including 40 patients. Use and maintenance of technology, equipment and operating room; administrative plus general costs; and 30-day use of postsurgery hospital resources were considered. The analysis was conducted from the hospital perspective. Results: On the average, a patient submitted to pulmonary lobectomy costs €9,744.29. This sum could vary from €9,027 (using ES) to €10,460 (using ST). The overall lower incidence (50% vs 95%, P=0.0001) and duration of air leakage (1.7 days vs 4.5 days, P=0.0001) in the ES group significantly affects the mean time of hospital stay (11.0 days vs 14.3 days) and costs. Cost saving in the ES group was also driven by the lower incidence of complications. The main key cost driver was staff employment (42%), then consumables (34%) and operating room costs (12%). Conclusion: There is an overall saving of around €1,432.90 when using ES patch for each pulmonary lobectomy. Among patients undergoing this surgical procedure, ES can significantly reduce air leakage incidence and duration, as well as decrease hospitalization rates. However, further multicenter research should be developed considering different clinical and managerial settings.
