    A systematic review of the impact of psychosocial factors on immunity: Implications for enhancing BCG response against tuberculosis.

    Background: Tuberculosis (TB) remains an urgent global public health priority, causing 1.5 million deaths worldwide in 2018. There is evidence that psychosocial factors modulate immune function; however, how this may influence TB risk or BCG vaccine response, and whether this pathway can be modified through social protection, has not been investigated. This paper aims to: a) systematically review evidence of how psychosocial factors influence the expression of biomarkers of immunity, and b) apply this general evidence to propose plausible TB-specific pathways for future study. Methods: Papers reporting on the impact of psychosocial stressors on immune biomarkers in relation to infectious disease risk were identified through a search of the databases MEDLINE, PsycINFO, Global Health and PsycEXTRA alongside reference list and citation searching of key papers. Data extraction and critical appraisal were carried out using a standardised form. The findings were tabulated and synthesised narratively by infectious disease category, and used to propose plausible mechanisms for how psychosocial exposures might influence immune outcomes relevant to TB and BCG response. Results: 27,026 citations were identified, of which 51 met the inclusion criteria. The literature provides evidence of a relationship between psychosocial factors and immune biomarkers. While the direction and strength of associations is heterogenous, some overarching patterns emerged: adverse psychosocial factors (e.g. stress) were generally associated with compromised vaccine response and higher antibody titres to herpesviruses, and vice versa for positive psychosocial factors (e.g. social support). Conclusions: The evidence identifies pathways linking psychosocial factors and immune response: co-viral infection and immune suppression, both of which are potentially relevant to TB and BCG response. However, the heterogeneity in the strength and nature of the impact of psychosocial factors on immune function, and lack of research on the implications of this relationship for TB, underscore the need for TB-specific research

    Where does brain neural activation in aesthetic responses to visual art occur? Meta-analytic evidence from neuroimaging studies

    Here we aimed at finding the neural correlates of the general aspect of visual aesthetic experience (VAE) and those more strictly correlated with the content of the artworks. We applied a general activation likelihood estimation (ALE) meta-analysis to 47 fMRI experiments described in 14 published studies. We also performed four separate ALE analyses in order to identify the neural substrates of reactions to specific categories of artworks, namely portraits, representation of real-world-visual-scenes, abstract paintings, and body sculptures. The general ALE revealed that VAE relies on a bilateral network of areas, and the individual ALE analyses revealed different maximal activation for the artworks' categories as function of their content. Specifically, different content-dependent areas of the ventral visual stream are involved in VAE, but a few additional brain areas are involved as well. Thus, aesthetic-related neural responses to art recruit widely distributed networks in both hemispheres including content-dependent brain areas of the ventral visual stream. Together, the results suggest that aesthetic responses are not independent of sensory, perceptual, and cognitive processe

    The measurement of household socio-economic position in tuberculosis prevalence surveys: a sensitivity analysis.

    OBJECTIVE: To assess the robustness of socio-economic inequalities in tuberculosis (TB) prevalence surveys. DESIGN: Data were drawn from the TB prevalence survey conducted in Lusaka Province, Zambia, in 2005-2006. We compared TB socio-economic inequalities measured through an asset-based index (Index 0) using principal component analysis (PCA) with those observed using three alternative indices: Index 1 and Index 2 accounted respectively for the biases resulting from the inclusion of urban assets and food-related variables in Index 0. Index 3 was built using regression-based analysis instead of PCA to account for the effect of using a different assets weighting strategy. RESULTS: Household socio-economic position (SEP) was significantly associated with prevalent TB, regardless of the index used; however, the magnitude of inequalities did vary across indices. A strong association was found for Index 2, suggesting that the exclusion of food-related variables did not reduce the extent of association between SEP and prevalent TB. The weakest association was found for Index 1, indicating that the exclusion of urban assets did not lead to higher extent of TB inequalities. CONCLUSION: TB socio-economic inequalities seem to be robust to the choice of SEP indicator. The epidemiological meaning of the different extent of TB inequalities is unclear. Further studies are needed to confirm our conclusions

    Food assistance to tuberculosis patients: lessons from Afghanistan.

    Poverty, food insecurity and poor nutrition in the population are important contributors to the burden of tuberculosis (TB). For poor and food-insecure individuals, accessing and successfully completing anti-tuberculosis treatment over an extended period of time is challenging. Food and nutritional support as an incentive and enabler is employed by national TB control programmes (NTPs) worldwide as a means to encourage treatment initiation and adherence and to improve the nutritional status of patients with TB. It also offers a safety net for food-insecure households affected by TB to mitigate the financial consequences of the disease. This paper reports on the primary lessons from the review of the World Food Programme's (WFP's) Food Assistance Programme for TB patients in Afghanistan. It aims to inform the design, implementation and scale-up of TB programmes in settings where food insecurity and malnutrition are prevalent. It also documents qualitative findings that suggest that patients, their families and providers viewed food support as an important asset and an essential element of the national TB control strategy. While the impact on treatment success or case detection could not be quantified, it is likely that the WFP intervention had a positive impact on the patients and their households, therefore contributing to the success of the DOTS-based NTP

    Optimal Control Problems with Mixed and Pure State Constraints

    This paper provides necessary conditions of optimality for optimal control problems, in which the pathwise constraints comprise both “pure” constraints on the state variable and “mixed” constraints on control and state variables. The proofs are along the lines of earlier analysis for mixed constraint problems, according to which Clarke's theory of “stratified” necessary conditions is applied to a modified optimal control problem resulting from absorbing the mixed constraint into the dynamics; the difference here is that necessary conditions which now take into account the presence of pure state constraints are applied to the modified problem. Necessary conditions are given for a rather general formulation of the problem containing both forms of the constraints, and then these are specialized to problems having special structure. While combined pure state and mixed control/state problems have been previously treated in the literature, the necessary conditions in this paper are proved under less restrictive hypotheses and for novel formulations of the constraints

    Artificial Intelligence in the Water–Energy–Food Model: A Holistic Approach towards Sustainable Development Goals

    This study aims to analyze the role of artificial intelligence (AI) in the Water–Energy–Food (WEF) nexus under the lens of institutional, stakeholder, and innovation theories. Specifically, this study focuses on AI as the technology adopted by companies to promote Sustainable Development Goals (SDGs). A structured literature review has been conducted on 94 articles published from 1990 to 2021 in ISI Web of Science, Scopus, and Google Scholar. This study develops an in-depth review of the literature on the main articles arguing about these issues. The findings highlight the increasing relevance of AI in the water, energy, and food industries individually considered, but the study of AI as a connector between water, energy, and food to achieve SDGs is still under investigation. Research on AI for WEF nexus management has adopted mostly a technical perspective, neglecting the relevance of management tools and the business model concept. Most of the articles did not adopt a specific theoretical lens, but scholars recognize the need to adopt a multi-stakeholder approach and the important role played by AI and other digital technologies to address the WEF nexus challenge. This study proposes an integrated approach for managing the nexus through AI technologies to meet sustainable and responsible business models. The gap between research and policy making could be filled by combining scientific data and policy needs with inclusive tools that are technically viable for sustainable resource utilization

    Acute and cumulative effects of rTMS on behavioural and EMG parameters in Focal Hand Dystonia

    Previous studies suggest that low-frequency repetitive Transcranial Magnetic Stimulation (rTMS) over contralateral premotor cortex (PMC) might ameliorate Focal Hand Dystonia (FHD) symptoms. In the present study behavioral and muscle activity outcomes were explored in a patient with FHD following a single and multiple sessions of rTMS. The patient's behavior was assessed on handwriting tasks, while surface EMG signals were recorded. In Experiment 1 evaluations were performed before and after one session of active and sham 1Hz rTMS over contralateral PMC. In Experiment 2, evaluations were performed before and after six sessions of the same treatment. In Experiment 1 active rTMS improved the patient's performance, although the EMG amplitude did not change. In Experiment 2, the patient showed an improvement of performance along with a decrease of 20% in the EMG amplitude. These results demonstrated that a single session of rTMS ameliorated the patient's performance, while multiple sessions were necessary to reduce muscles activity

    Seasonal and pandemic influenza: the role of communication and preventive strategies

    Appropriate, timely, and data-driven health information is a very important issue in preventive strategies against influenza. Intui- tively, a link between willingness to be vaccinated against seasonal influenza and against pandemic influenza exists, given the similarities in decision-making for this vaccine. International and national literature reviews suggest that progress has been made in order to incorporate and disseminate crisis risk communication principles into public health practice, as such investments in public health could be important for building capacity and practice which aid in the realization of countermeasures in response to a future pandemic and epidemic situation. This study emphasizes the lack of perception by Health Care Workers (HCWs) of the importance of being immunized against seasonal and pandemic influenza and the doubts concerning safety. In the future, particular efforts are needed during vaccination campaigns, to provide more information to HCWs and the general population regarding role and safety of such vaccines
