Acceptance of malaria vaccine by a rural community in Nigeria
Background: Introduction of malaria vaccine is imminent. This study evaluated the prevalence of malaria among a non-febrile population and their willingness to accept a malaria vaccine.Methods: This was a cross-sectional, community-based study done in a rural community in south east Nigeria. A total of 156 household heads were interviewed using a structured questionnaire. The questionnaire was pre-tested before commencement of the study to correct ambiguity.Results: Majority (78.2%) acknowledged that malaria is the commonest illness in the community, while 55.1% believed that presumptive treatment is the best malaria preventive measure. Most (98.7%) of the study participants immunized their children against childhood vaccine preventable diseases, while 91.6% would be willing to accept a malaria vaccine. The prevalence of malaria parasitaemia among non-febrile respondents was 35.4% and the use of mosquito nets was 17.9%.Conclusion: The high prevalence of malaria among non-febrile populations, the practice of presumptive treatment of unconfirmed fever as malaria preventive measure and the low use of bed nets, points that it is time to introduce malaria vaccine. The high willingness to receive the vaccine is positive to the introduction of the vaccine.Keywords; Acceptance; Malaria, Vaccine, Nigeria
Factors associated with low rate of exclusive breastfeeding among mothers in Enugu, Nigeria
Background: Exclusive breastfeeding has proven to be beneficial both maternal and child health hence its adopted as a policy for infant feeding in most countries especially developing nations. Its practice has remained low despite the high levels of awareness of exclusive breastfeeding. The study set out to find out the possible reasons that have limited the translation of knowledge of exclusive breastfeeding to action by nursing mothers in an urban city in Nigeria.Methods: A self-administered structured questionnaire was used to collect data from 304 mothers attending immunization clinics of the Institute of maternal and child health in Enugu, Southeast Nigeria.Results: Awareness of exclusive breastfeeding was 98% with an exclusive breastfeeding rate of 26%. The commonest non-human milk given to infant was water- given in the first week of life. Post-natal support from lactation experts and family, beliefs about the sufficiency of human milk and subsequent refusal of complementary foods were major challenges to successful exclusive breastfeeding. Maternal age and education did not determine exclusive breastfeeding.Conclusions: Exclusive breastfeeding rates have remained low in Nigeria. Lack of family support and the belief that human milk is not sufficient food for the less-than-six-months-old infant were major challenges to exclusive breastfeeding
Proteinuria and haematuria in apparently healthy primary school children in Enugu, Nigeria
Background: Mass screening for proteinuria and haematuria is advocated, especially in resource-poor countries with non-existent health insurance policy, to enhance awareness of renal diseases. This screening increases the chance for early detection and treatment, which in turn reduces incidences of resultant kidney failures.
Objective: To determine the prevalence of asymptomatic proteinuria and haematuria in apparently healthy primary school children in Enugu.
Methodology: This was a cross-sectional descriptive survey involving apparently healthy primary school children in Enugu, aged 6 to 12 years, conducted from January 2018 and March 2018 through a pre-tested, caregiver administered questionnaire. Subjects were clinically examined, and samples of on-the-spot mid-stream urine collected from each participant for dipstick urinalysis. The data obtained were analyzed using the Software Package for Social Science (SPSS) version 22 for Windows
Results: One thousandchildren comprising, 437 (44%) males and 563 (56%) females were selected with an age range of 6 to 12 years and a mean of 10.13 ± 1.81 years. A total of 190(19%) urinary abnormalities were noted existing as isolated proteinuria, isolated haematuria or as a combination of both. These urinary abnormalities were more in the females, higher among the early adolescents (9-12 years) with proteinuria predominating.Conclusion: Asymptomatic urinary abnormalities can easily be detected early in the course of renal disease development. A urine dipstick is an important tool in the screening program for everybody, including children of school age. It is important that screening programs be organized for these children at regular intervals and those with positive findings referred immediately for appropriate management.

Profile of chronic kidney disease modifiable risk factors in a rural community of south east Nigeria
Abstract Background Chronic kidney disease (CKD) is on the increase globally. Prevention of this condition is ideal, however early detection of the disease becomes desirable where the disease process has begun as there are known interventions which can slow the progress to end stage renal disease (ESRD). This study aimed at detecting the profile of some modifiable risk factors for CKD in a cohort of household heads in a rural community with limited resources for managing chronic kidney diseases. Methods The study was conducted in a rural community in southeast Nigeria. One hundred and forty five household heads from randomly selected households were interviewed. Their blood pressures were taken and their urine tested. The data was analyzed using SPSS version 21. Simple frequencies and means were calculated. Results A total of 145 house hold heads were enrolled. Their mean age was 45.08 (19.65) years. Forty-seven percent had no prior knowledge of their blood pressure and 31.5% were found to be hypertensive. Only one study participant (1%) had ever had a urinalysis test and proteinuria and glycosuria were found in 50.4 and 27.9% respectively. Most (75%) patronized patent medicine vendors for their primary healthcare while 31.8% had taken herbal mixtures in the past. Conclusion There are presently many modifiable risk factors for the development of chronic kidney disease in rural communities in south-east Nigeria. Urgent targeted intervention is required to forestall an epidemic of CKD in the near future
School health services in Enugu East, Nigeria: perspectives from a resourcepoor setting
School health services (SHS) have widespread impact on the health of a large number of children with implications on access to primary health care especially in developing countries. The aim of this study was to assess health services in primary schools in Enugu East Nigeria. Thirty-three head teachers of primary schools in Enugu east Nigeria and officials of Ministry of Education were interviewed using a questionnaire adapted from school health evaluation scale. Four private schools had health personnel. Only six private schools had a health room. Two public schools had a functional first aid box. There were no health records available in any of the schools. School lunch was given by only one private school. Of a maximum of 45, public and private schools had a mean score of 10.3 and 12.7 respectively on the school health evaluation scale (P=0.01). Three schools only attained the minimum acceptable score of 19. Health services are at a minimal level in primary schools in Enugu East Nigeria. A state school health policy should be developed through inter-sectoral collaboration of the relevant stakeholders to use the platform provided by schools to ensure access to primary health care and also act as bridge for more formal medical care for school children
