Management of labyrinthine fistula in chronic otitis with cholesteatoma: case series.
Labyrinthine fistula is a complication of ear cholesteatoma that increase the risk of sensorineural hearing loss. The management of the fistula must be done contextually with mastoidectomy by: leaving cholesteatoma matrix over the fistula, or remove the matrix reconstructing the defect. Objective: analysis of the two techniques to treat labyrinthine fistula. Methods: retrospective review with case series analysis. Results: a labyrinthine fistula was present in 14% of cholesteatoma patients; CT scan was pre-dictive in all cases; the hearing preservation was obtained with both techniques; a re-currence was detected only in one case; postoperative nystagmus incidence was higher in those cases with matrix left in situ and when the size of the fistula was lar-ger than 2 mm. Conclusions: the labyrinthine fistula have to be treated contextually with cholesteatoma removal, both techniques had good postoperative hearing preser-vation rate. Fat plug myringoplasty: Analysis of a safe procedure for small tympanic perforations
Objective: Assessment of value of fat plug myringoplasty as office-based procedure in small tympanic membrane perforations. Material and Methods: Prospective study of consecutive patients undergoing fat plug myringoplasty under local anesthesia. Minimum follow-up considered was 6 months, which included several microscopic examinations. All data were recorded and analyzed statistically. Results: 33 patients matched the inclusion criteria; the mean perforation size was 2.8 mm. No change in PTA was noted after surgery in the whole series. Recurrence of perforation was detected in 7 cases (21%), which was statistically related with the cause of perforation rather than location or size (p-value <0.01). Conclusions: Fat plug myringoplasty is a safe procedure with very low rate of complications, and it is an office-based procedure well tolerate by all patients. Results in case of primary surgery are excellent, with a high success rate (90%). In case of recurrent perforation after traditional surgery, success rate get decreased
Canal switch and re-entry phenomenon in benign paroxysmal positional vertigo: difference between immediate and delayed occurrence
Studio prospettico ideato per la valutazione delle differenze tra la conversione canalare o il rientro degli otoliti nei canali semicircolari successivo
alle manovre terapeutiche nei pazienti affetti da VPPB. Sono stati valutati 196 pazienti affetti da VPPB, 127 dei quali corrispondevano
ai criteri di inclusione. L’età media dei pazienti era di 54.74 anni. Il canale orizzontale è stato coinvolto in 30 casi e il canale posteriore in
97 pazienti. I pazienti con sordità neurosensoriale presentavano forme ricorrenti di VPPB, rispetto a quelli con udito normale. L’immediato
rientro canalare è stato diagnosticato in 3 pazienti con VPPB del canale laterale, tutti con nistagmo geotropo. 7 pazienti con VPPB del canale
posteriore hanno presentato un rientro canalare immediato e 5 la forma ritardata. I pazienti con rientro canalare ritardato avevano precedentemente
subito più di 2 manovre di riposizionamento. Il rientro canalare non è risultato connesso al tipo di manovra eseguita. Il tempo di
attesa tra l’esecuzione della manovra liberatoria e il test di verifica si è rivelato importante ai fini del rientro canalare immediato. La recidiva
della BPPV dopo un mese dalle manovre liberatorie si è riscontrata in 20 pazienti ed è stata più frequente in quei pazienti che hanno avuto
un fenomeno di rientro canalare. La conversione canalare ed il fenomeno del rientro canalare rappresentano delle entitĂ cliniche che devono
essere considerate dal medico che tratta le VPPB. Appare importante distinguere un rientro da un fallimento della manovra in caso di forme
immediate, o da una recidiva di patologia in caso di forme ritardate. L’esecuzione del test di verifica del successo terapeutico dopo manovre
di riposizionamento deve avere un distacco temporale sufficientemente ampio al fine di evitare il reflusso immediato di otoliti nei canali.This prospective study was designed to evaluate the differences between immediate and delayed canal re-entry of otoliths after therapeutic manoeuvres in patients with benign paroxysmal positional vertigo (BPPV). A total of 196 patients with BPPV were visited and 127 matched our inclusion criteria. The mean age was 54.74 years. The horizontal semicircular canal (HSC) was involved in 30 cases and the posterior semicircular canal (PSC) in 97 patients. Patients with hearing loss in the ear affected by BPPV have a more recurrent form, compared to those with normal hearing. An immediate canal re-entry was recorded in 3 patients with HSC BPPV, all with geotropic nystagmus. In 7 patients with PSC BPPV, the immediate canal re-entry was detected and the delayed form was noted in 5 patients. The patients with the delayed canal re-entry underwent more than 2 previous manoeuvres. The canal re-entry was not related to the manoeuvre performed. The timing of the Dix-Hallpike test to verify the resolution of the BPPV had a significant role in immediate canal re-entry. A recurrence in the follow-up at least one month after treatment was recorded in 20 patients and was more frequent in patients that had canal re-entry. The canal re-entry or canal switch is a clinical entity that should be kept in mind of the neurotologist when approaching BPPV patients. It is important to distinguish it from recurrence when delayed and from manoeuvre failure when immediate. The timing of manoeuvre performing, in particular the final verification test after therapeutic sessions, is important to prevent the immediate reflux of particles into canals
Effects of PARP-1 Deficiency on Th1 and Th2 Cell Differentiation
T cell differentiation to effector Th cells such as Th1 and Th2 requires the integration of multiple synergic and antagonist signals. Poly(ADP-ribosy)lation is a posttranslational modification of proteins catalyzed by Poly(ADP-ribose) polymerases (PARPs). Recently, many reports showed that PARP-1, the prototypical member of the PARP family, plays a role in immune/inflammatory responses. Consistently, its enzymatic inhibition confers protection in several models of immune-mediated diseases, mainly through an inhibitory effect on NF-ÎşB (and NFAT) activation. PARP-1 regulates cell functions in many types of immune cells, including dendritic cells, macrophages, and T and B lymphocytes. Our results show that PARP-1KO cells displayed a reduced ability to differentiate in Th2 cells. Under both nonskewing and Th2-polarizing conditions, naĂŻve CD4 cells from PARP-1KO mice generated a reduced frequency of IL-4+ cells, produced less IL-5, and expressed GATA-3 at lower levels compared with cells from wild type mice. Conversely, PARP-1 deficiency did not substantially affect differentiation to Th1 cells. Indeed, the frequency of IFN-Îł+ cells as well as IFN-Îł production, in nonskewing and Th1-polarizing conditions, was not affected by PARP-1 gene ablation. These findings demonstrate that PARP-1 plays a relevant role in Th2 cell differentiation and it might be a target to be exploited for the modulation of Th2-dependent immune-mediated diseases
Modifying Anthocyanins Biosynthesis in Tomato Hairy Roots:A Test Bed for Plant Resistance to Ionizing Radiation and Antioxidant Properties in Space
Gene expression manipulation of specific metabolic pathways can be used to obtain bioaccumulation of valuable molecules and desired quality traits in plants. A single-gene approach to impact different traits would be greatly desirable in agrospace applications, where several aspects of plant physiology can be affected, influencing growth. In this work, MicroTom hairy root cultures expressing a MYB-like transcription factor that regulates the biosynthesis of anthocyanins in Petunia hybrida (PhAN4), were considered as a testbed for bio-fortified tomato whole plants aimed at agrospace applications. Ectopic expression of PhAN4 promoted biosynthesis of anthocyanins, allowing to profile 5 major derivatives of delphinidin and petunidin together with pelargonidin and malvidin-based anthocyanins, unusual in tomato. Consistent with PhAN4 features, transcriptomic profiling indicated upregulation of genes correlated to anthocyanin biosynthesis. Interestingly, a transcriptome reprogramming oriented to positive regulation of cell response to biotic, abiotic, and redox stimuli was evidenced. PhAN4 hairy root cultures showed the significant capability to counteract reactive oxygen species (ROS) accumulation and protein misfolding upon high-dose gamma irradiation, which is among the most potent pro-oxidant stress that can be encountered in space. These results may have significance in the engineering of whole tomato plants that can benefit space agriculture
Rapporto tecnico sulla valutazione della biomassa ittioplanctonica mediante l'utilizzo del Multi Plankton Sampler (MPS)
Il piano di campionamento della campagna oceanografica BANSIC'14, condotta a bordo della N/O "URANIA" dal 22 Luglio al 9 Agosto 2014, lungo transetti sotto costa e a largo delle coste meridionali della Sicilia, ha avuto l’obiettivo generale dello studio delle relazioni tra le strutture oceanografiche a mesoscala (vortici verticali ed orizzontali, upwelling, etc.) e le strutture spaziali dei fenomeni biologici relativi ai primi anelli della catena trofica (zooplancton, distribuzione e abbondanza di larve di piccoli pelagici e grandi pelagici) (vedi Rapporto finale BANSIC 2014) per la stima dell’abbondanza dello stock riproduttore. E' una campagna di ricerca nell'ambito del WP3 del progetto SSD-Pesca, finanziato dal MIUR su fondi MISE, a supporto della pesca italiana nelle Regioni Obiettivo 1 e del progetto RITMARE (SP2_WP4_AZ2_UO04). Il campionamento ittioplanctonico è inserito anche nel piano di lavoro del progetto regionale MIPAF-FAO “MedSudMed” (“Assessment and Monitoring of the Fishery Resources and the Ecosystems in the Straits of Sicily”).
Il campionamento dell’ittioplancton, durante questa campagna, oltre ai metodi tradizionali quali le reti di tipo Bongo, ha visto l'utilizzo del Multi Plankton Sampler (MPS) MultiNet.
I campionatori ittioplanctonici hanno lo scopo di prelevare porzioni di mesozooplancton da un massimo di 100 m fino alla superficie, in quanto le uova di pesci pelagici possiedono una galleggiabilità tale che nonostante le turbolenze superficiali dell’acqua, un campionamento entro i primi metri restituisce un dato affidabile della distribuzione anche se alcune uova possono trovarsi a maggiore profondità (Ahlstrom, 1959).
Il campionatore MPS consente, a differenza di altri strumenti, di prelevare la frazione di zooplancton d'interesse con diverse modalitĂ di campionamento: orizzontale, verticale e obliquo e, allo stesso tempo, permette di campionare a differenti quote di profonditĂ .
Le informazioni così ottenute sono state utilizzate per valutare la variazione della biomassa ittioplanctonica, l'abbondanza e la composizione delle specie lungo gli strati della colonna d'acqua anche in relazione alle componenti oceanografiche.
Grazie all'utilizzo di questo strumento è possibile validare e verificare alcune informazioni e acquisirne delle nuove sull’ecologia delle specie larvali e sul mesozooplancton, sul modo in cui queste si distribuiscono lungo la colonna d'acqua e sulle interazioni intra ed interspecifiche legate anche a fattori oceanografici. Ciò consente di ottenere maggiori informazioni e contribuire al miglioramento della comprensione della biologia e dell'ecologia delle specie rinvenute e, nel contempo, approfondire e migliorare le conoscenze sugli stadi di sviluppo di uova di specie ittiche che allo stadio embrionale sono ancora poco conosciuti
Engraulis encrasicolus (Linnaeus, 1758): technical report about extraction and recognition of the digestive tract contents in early stages of the life cycle
The study of ichthyio-plankton stages and its relations with the environment and other organisms is therefore crucial for a correct use of fishery resources. In this context, the extraction and the analysis of the content of the digestive tract, is a key method for the identification of the diet in early larval stages, the determination of the resources they rely on and possibly a comparison with the diet of other species. Additionally this approach could be useful in determination on occurrence of species competition. This technique is preceded by the analysis of morphometric data (Blackith & Reyment, 1971; Marcus, 1990), that is the acquisition of quantitative variables measured from the morphology of the object of study. They are linear distances, count, angles and ratios. The subsequent application of multivariate statistical methods, aims to quantify the changes in morphological measures between and within groups, relating them to the type and size of prey and evaluate if some changes appear in food choices along the larvae growth
