    Axis I and II disorders as long-term predictors of mental distress: a six-year prospective follow-up of substance-dependent patients

    <p>Abstract</p> <p>Background</p> <p>A high prevalence of lifetime psychiatric disorders among help-seeking substance abusers has been clearly established. However, the long-term course of psychiatric disorders and mental distress among help-seeking substance abusers is still unclear. The aim of this research was to examine the course of mental distress using a six-year follow-up study of treatment-seeking substance-dependent patients, and to explore whether lifetime Axis I and II disorders measured at admission predict the level of mental distress at follow-up, when age, sex, and substance-use variables measured both at baseline and at follow-up are controlled for. </p> <p>Methods</p> <p>A consecutive sample of substance dependent in- and outpatients (n = 287) from two counties of Norway were assessed at baseline (T1) with the Composite International Diagnostic Interview (Axis I), Millon's Clinical Multiaxial Inventory (Axis II), and the Hopkins Symptom Checklist (HSCL-25 (mental distress)). At follow-up (T2), 48% (137/287 subjects, 29% women) were assessed with the HSCL-25, the Alcohol Use Disorders Identification Test, and the Drug Use Disorders Identification Test. </p> <p>Results</p> <p>The stability of mental distress is a main finding and the level of mental distress remained high after six years, but was significantly lower among abstainers at T2, especially among female abstainers. Both the number of and specific lifetime Axis I disorders (social anxiety disorder, generalized anxiety disorder, and somatization disorder), the number of and specific Axis II disorders (anxious and impulsive personality disorders), and the severity of substance-use disorder at the index admission were all independent predictors of a high level of mental distress at follow-up, even when we controlled for age, sex, and substance use at follow-up.</p> <p>Conclusion</p> <p>These results underscore the importance of diagnosing and treating both substance-use disorder and non-substance-use disorder Axis I and Axis II disorders in the same programme.</p

    Randomized clinical trial examining duration of voucher-based reinforcement therapy for cocaine abstinence.

    BACKGROUND: This is the first study to systematically manipulate duration of voucher-based reinforcement therapy (VBRT) to see if extending the duration increases abstinence during and following VBRT. METHODS: We randomized cocaine-dependent methadone-maintained adults to Standard (12 weeks; n=62) or Extended (36 weeks; n=68) VBRT and provided escalating voucher amounts contingent upon urinalysis verification of cocaine abstinence. Urinalysis was scheduled at least every 2 weeks during the 48-week study and more frequently during VBRT (3/week) and 12 weeks of Aftercare (2/week). RESULTS: Extended VBRT produced longer durations of continuous cocaine abstinence during weeks 1-24 (5.7 vs 2.7 weeks; p=0.003) and proportionally more abstinence during weeks 24-36 (X(2)=4.57, p=.03, OR=2.18) compared to Standard VBRT. Duration of VBRT did not directly predict after-VBRT abstinence; but longer continuous abstinence during VBRT predicted abstinence during Aftercare (p=0.001) and during the last 12 weeks of the study (p \u3c 0.001). Extended VBRT averaged higher monthly voucher costs compared to Standard VBRT (96vs96 vs 43, p \u3c .001); however, the average cost per week of abstinence attained was higher in the Standard group (8.06vs8.06 vs 5.88, p \u3c .001). Participants in the Extended group with voucher costs exceeding $25 monthly averaged 20 weeks of continuous abstinence. CONCLUSIONS: Greater abstinence occurred during Extended VBRT, but providing a longer duration was not by itself sufficient to maintain abstinence after VBRT. However, if abstinence can be captured and sustained during VBRT, then providing longer durations may help increase the continuous abstinence that predicts better long-term outcomes

    Radar Shape Modeling Of (8567) 1996 HW1 Combined With Thermal Observations

    We observed near-Earth asteroid (8567) 1996 HW1 at the Arecibo Observatory on six dates in September 2008, obtaining radar images and spectra. By combining these data with an extensive set of new lightcurves taken during 2008-2009 and with previously published lightcurves from 2005, we were able to reconstruct the object's shape and spin state. 1996 HW1 is an elongated, highly bifurcated object, and appears to be a contact binary. A convex shape model derived from the lightcurves alone produces a pole orientation that is consistent with the pole derived from the radar data. It is instructive to compare these two shape models. We have also obtained near-infrared spectra at the NASA IRTF using SpeX in both prism and LXD modes. The prism spectra show pyroxene and olivine bands. We obtained LXD spectra (2-4 microns) on 19 August, 1 September and 1 October 2008. Thermal modeling using the shape model and derived pole and rotation constrain the regolith properties.
