    Um dos maiores desafios para o terceiro setor é, sem dúvida, o desenvolvimento de uma estrutura de gestão adequada a suas peculiaridades. É interessante observar a relativa escassez de produção intelectual e acadêmica a respeito, em contraste com o destaque e importância econômica que o setor vem ganhando ultimamente. Este trabalho apresenta algumas idéias a respeito do modo de ação gerencial apropriado às organizações deste setor, formuladas a partir do desenvolvimento e implementação do projeto Pra não Dizer que não Falei das Flores. O projeto, elaborado por acadêmicos da Universidade Federal do Rio Grande do Sul, foi implementado em uma organização de amparo a menores e idosos de Porto Alegre. Conclui-se que a forma de ação administrativa mais apropriada a estas organizações é a de projeto transformador, de cunho educativo e formativo, e não caritativo. Para que o desenvolvimento organizacional assim obtido seja harmônico, deve atuar nas dimensões política, econômica e cultural destas organizações de forma evolutiva e nunca revolucionária. A busca do desenvolvimento sustentado nas organizações beneficentes é um processo de mudança cultural a longo prazo que deve ser conduzido a partir da capacitação gerencial.One of the greatest challenges for the third sector is the development of an appropriate management structure. It is interesting to observe the relative small amount of intellectual and academic production about it, in contrast to the economic and social projection that this sector has gained lately. This paper is based on the case-project Pra não Dizer que não Falei das Flores and presents some ideas regarding the managerial action adapted to non-profitorganizations. The project was developed by students of the Universidade Federal do Rio Grande do Sul and implemented in an organization that supports poor children and elders in Porto Alegre, Brazil. The paper concludes that projects are the best way to deal with these organizations. These projects must transform the organization's reality and also be educational and formative, not merely an act of charity. The organizational development should be harmonic, developing the political, economical and cultural dimensions of those organizations, making an evolution and never a revolution. The development of sustainable systems for non-profit-organizations is a long-term process of change and it begins by an efficient training of its managers

    ¿Notas explicativas explican? : Análisis de la comunicación de la gestión de riesgos a partir de técnicas de Text Mining

    O estudo objetivou analisar a efetividade da comunicação do gerenciamento de risco nas notas explicativas considerando técnicas de text mining. A partir da análise de 241 trechos de texto de 32 instituições financeiras, utilizou-se a similaridade de cosseno como medida de distância para avaliar a relação da variabilidade dos textos com os índices de risco Basileia e Inad90. Os resultados indicam que as notasanalisadas não estão em conformidade com a OCPC 07, pois seu conteúdo não reflete a flutuação dos índices de risco. A utilidade para o usuário em processos de tomada de decisões pode ser prejudicada, dado o indício de desconexão entre os textos e a política de riscos. Apresentam-se casos de repetição integral dos trechos de texto. Como contribuição, esse artigo avança nos estudos textuais das demonstrações financeiras, indo além de análises léxicas e volumétricas, trazendo o conteúdo semânticoem volume e sua relação com indicadores financeiros.The study aimed to analyze the effectiveness of risk management communication in the explanatory notesconsidering text mining techniques. Based on the analysis of 241 text excerpts from 32 financial institutions, we used the cosine similarity as a distance measurement to assess the relationship between the variability of the texts and the Basel and Inad 90 risk indices. The results indicate that the analyzed notes are not in compliance with OCPC 07, as their content does not reflect the fluctuation of risk indices. The benefit of the notes for the user in decision-making processes may be impaired, given the evidence of disconnection between the texts and the risk policy. Cases of complete repetition of the text excerpts are presented. As a contribution, this article advances in the textual studies of financial statements, going beyond lexical and volumetric analyses, bringing the semantic content in volume and its relationship with finance indicators.El estudio tuvo como objetivo analizar la efectividad de la comunicación de la gestión de riesgos en las notas explicativas considerando técnicas de minería de textos. A partir del análisis de 241 extractos de texto de 32 instituciones financieras, se utilizó la similitud del coseno como medida de distancia para evaluar la relación entre la variabilidad de los textos y los índices de riesgo de Basilea e Inad90. Los resultados indican que las notas analizadas no están en conformidad con OCPC 07, ya que su contenido no refleja la fluctuación de los índices de riesgo. La utilidad para el usuario en los procesos de toma de decisiones puede verse afectada, dada la evidencia de desconexión entre los textos y la política de riesgos.     Relationship of arterial and exhaled CO2 during elevated artificial pneumoperitoneum pressure for introduction of the first trocar.

    The present study evaluated the correlation between arterial CO2 and exhaled CO2 during brief high-pressure pneumoperitoneum. Patients were randomly distributed into two groups: P12 group (n=30) received a maximum intraperitoneal pressure of 12mmHg, and P20 group (n=37) received a maximum intraperitoneal pressure of 20mmHg. Arterial CO2 was evaluated by radial arterial catheter and exhaled CO2 was measured by capnometry at the following time points: before insufflation, once intraperitoneal pressure reached 12mmHg , 5 minutes after intraperitoneal pressure reached 12mmHg for the P12 group or 20mmHg for the P20 group, and 10 minutes after intraperitoneal pressure reached 12mmHg for the P12 group or when intraperitoneal pressure had decreased from 20mmHg to 12mmHg, for the P20 group. During brief durations of very high intraperitoneal pressure (20mmHg), there was a strong correlation between arterial CO2 and exhaled CO2. Capnometry can be effectively used to monitor patients during transient increases in artificial pneumoperitoneum pressure

    Invasive monitoring of the clinical effects of high intra-abdominal pressure for insertion of the first trocar.

    Background: To analyze the effects of transitory, high intra-abdominal pressure on clinical, hemodynamic, blood gas and metabolic parameters.

Methods: Sixty-seven laparoscopic patients were divided into groups P12 (n = 30, maximum intra-abdominal pressure of 12 mmHg) and P20 (n = 37, maximum intra-abdominal pressure of 20 mmHg). Through radial artery cannulation, mean arterial pressure (MAP) was assessed and blood gas analysis – pH, arterial oxygen tension (PaO2), arterial carbon dioxide tension (PaCO2), bicarbonate (HCO3) and base excess (BE) – was performed. These parameters were evaluated in both groups at time point zero, before CO2 insufflation; at time point one (TP1), when intra-abdominal pressure of 12 mmHg was reached in both groups; at time point two (TP2), 5 minutes after reaching intra-abdominal pressure of 12 mmHg in group P12 and of 20 mmHg in group P20; and at time point three (TP3), 10 minutes after reaching intra-abdominal pressure of 12 mmHg in group P12 and 10 minutes after TP1 in group P20, when intra-abdominal pressure decreased from 20 mmHg to 12 mmHg. Values out of the normal range or the occurrence of atypical phenomena suggestive of organic disease indicated clinical changes.

Results: Significant variations in MAP, pH, HCO3 and BE were observed in group P20; these changes, however, were within normal limits. Clinical changes were also within normal limits, and no pathological phenomena were observed.

Conclusions: Brief, intra-abdominal hypertension for the insertion first trocar insertion causes variations in MAP, pH, HCO3 and BE without adverse effects, and it may protect from iatrogenic injury

    Compreendendo que conhecer o passado é importante para planejar o futuro, neste artigo buscamos resgatar a história de sistemas de informação no Brasil, tendo como marco inicial o ano de 1988, quando a área de administração da informação (ADI) surge pela primeira vez de forma independente em um dos principais congressos de administração do Brasil, o EnANPAD. Como marco final, o ano de 2014, que representa a comemoração dos 20 anos da AMCIS (Americas Conference on Information System), evento que é referência na área de ADI, e que incitou um chamado para um olhar sobre a história de ADI através da área Celebrating the 20th Anniversary of AMCIS: Examining IS History. Para analisar estes 27 anos brasileiros frente à comunidade internacional, foram classificados 1.335 artigos de ADI publicados nos eventos da ANPAD nas áreas da AMCIS, trazendo um olhar longitudinal ao período. Como resultados, identificou-se que a área de Gestão de Projetos apresenta a maior tendência de crescimento. Ainda, apresenta-se um panorama de evolução e estado atual das pesquisas em SI no Brasil, bem como principais autores, instituições e temáticas trabalhadas ao longo do tempo.Know the past to plan the future. In this article, we seek to rescue the history of information systems area in Brazil, starting with the year 1988, when the information system area (IS) emerges for the first time independently in one of the main management congresses in Brazil, EnANPAD. As a final milestone, the year 2014, which represents the 20th anniversary celebration of AMCIS, which prompted a call for a look at the history of SI through the track “Celebrating the 20th Anniversary of AMCIS: Examining IS History”. In order to analyze these 27 Brazilian years concerning an international community, we chose to classify the 1,335 articles retrieved in ANPAD meetings on AMCIS tracks, bringing a longitudinal look to the period. As a result, we identified that Project Management presented the highest growth tendency. Furthermore, we present an overview of the evolution and current state of research in IS in Brazil, as well as the main authors, institutions and themes worked over time

    The management of COVID-19 cases through telemedicine in Brazil

    In Dec 2020 Brazil became one of the worldwide epicenters of the COVID-19 pandemic with more than 7.2M reported cases. Brazil has a large territory with unequal distribution of healthcare resources including physicians. Resource limitation has been one of the main factors hampering Brazil’s response to the COVID-19 crisis. Telemedicine has been an effective approach for COVID-19 management as it allows to reduce the risk of cross-contamination and provides support to remote rural locations. Here we present the analyses of teleconsultations from a countrywide telemedicine service (Telessa´udeRS-UFRGS, TRS), that provides physician-to-physician remote support during the COVID-19 pandemic in Brazil. We performed a descriptive analysis of the teleconsultation incoming calls and a text analysis from the call transcripts. Our findings indicate that TRS teleconsultations in Brazil experienced an exponential increment of 802.% during a period of 6 days, after the first death due to COVID-19 was reported. However, the number of teleconsultations cases decreased over time, despite the number of reported COVID-19 cases continuously increasing. The results also showed that physicians in low-income municipalities, based on GDP per capita, are less likely to consult the telemedicine service despite facing higher rates of COVID-19 cases. The text analysis of call transcripts from medical teleconsultations showed that the main concern of physicians were “asymptomatic” patients. We suggest an immediate reinforcement of telehealth services in the regions of lower income as a strategy to support COVID-19 management.Telemedicin

    Helicobacter pylori eradication : influence of interleukin-1beta –31 C/T polymorphism

    Aim: To analyze the influence of the –31 C/T polymorphism of the interleukin-1 gene on Helicobacter pylori eradication therapy success in patients with functional dyspepsia. Methods: Functional dyspepsia was diagnosed according to the Rome III criteria. All patients underwent upper gastrointestinal endoscopy, and gastric biopsies were obtained at screening and 12 months after randomization (last follow-up visit). Urease test and histological examination were performed to define the H. pylori status. Patients received twice-daily amoxicillin, clarithromycin and omeprazole for 10 days. Genotyping of the interleukin-1beta –31 C/T polymorphism (rs1143627)was performed using polymerase chain reaction-restriction fragment length polymorphism. Results: One hundred forty-nine patients received treatment with triple therapy for H. pylori eradication. Only one patient was lost to follow-up, and adherence to study medication was 94.6%. A total of 148 patients (mean age 46.08±12.24 years; 81.8% women) were evaluated for the influence of the interleukin-1beta –31 C/T polymorphism on the outcome of H. pylori eradication therapy. After treatment, bacteria were eradicated in 87% of patients (129/148). Genotype frequencies of the polymorphism were as follows: CC, 38/148 (25.7%); CT, 71/148 (47.9%); and TT, 39/148 (26.4%). Successful eradication rate was 78.9%, 94.4% and 82.1% for the CC, CT and TT genotypes, respectively. The CT genotype was significantly associated with successful H. pylori eradication (p = 0.039). Conclusion: This study suggests that the CT genotype of the interleukin-1beta –31 C/T polymorphism plays a role in the successful eradication of H. pylori among patients with functional dyspepsia
