Operative BehandlungsmoÌglichkeiten und anatomische Einflussfaktoren bei Patienten mit degenerativer Spinalkanalstenose
In dieser Habilitationsschrift wurden unterschiedliche operative Behandlungsmöglichkeiten und anatomische Einflussfaktoren bei Patient*innen mit degenerativer Spinakanalstenose untersucht. Es konnten die Chancen und Risiken unterschiedlicher ventraler Oprationstechniken an der HalswirbelsĂ€ule dargestellt werden. Hierbei konnte zunĂ€chst ein gutes operatives Resultat bei der ventralen Fusion durch intervertebrale Cages oder durch einen Wirbelkörperersatz bestĂ€tigt werden, es zeigte sich jedoch, dass bei der 2-Höhen-Korporektomie die postoperative Komplikationsrate durch eine sekundĂ€re InstabilitĂ€t sehr hoch war. Diese Komplikation konnte durch eine zusĂ€tzliche dorsale Instrumentierung verhindert werden. Die klinischen Ergebnisse nach einer mikrochirurgischen Dekompression bei Patient*innen mit lumbaler Spinalknalstenose waren wie in vorausgehenden Studien gut. Es konnte zudem gezeigt werden, dass durch eine operative Dekompression bei Patient*innen mit einer durch epidurale Lipomatose entstandenen Spinalkanalstenose ein gutes Langzeitergebnis erzielt werden kann. Wider Erwarten hatte die prĂ€operative Sagittale Balance keinen signifikanten Einfluss auf den klinischen Verlauf von Patient*innen mit lumbaler Spinalknalstenose nach mikrochirurgischem Eingriff. Relevant fĂŒr das postoperative Ergebnis war vielmehr das Sagittale Profil. Insbesondere Patient*innen mit einer ausgeprĂ€gten und kurzen lumbalen Lordose und vermehrter Belastung auf den dorsalen Strukturen der LendenwirbelsĂ€ule durch eine langstreckige thorakolumbale Kyphose litten im postoperativen Verlauf unter starken Lumbalgien. Bei diesen Patient*innen sollte ĂŒber alternative Behandlungsmöglichkeiten nachgedacht werden
In vivo analysis of microglia dynamics in malignant gliomas
Time Lapse in-vivo-Mikroskopie als Nachweis der individuellen Dynamik der
Mikroglia-Tumormilieu-Interaktion â eine neue Rolle des perivaskulĂ€ren Raums
als Schnellstrasse fuÌr aktivierte Mikrogliazellen Mikrogliazellen sind
entscheidend fuÌr das Wachstum und den Progress von malignen Gliomen. Jedoch
ist nur sehr wenig uÌber das intratumorale Verhalten von Mikrogliazellen und
die dynamische Interaktion mit dem Tumor bekannt. Aktuell basiert das
begrenzte Wissen uÌber das Erscheinungsbild der Mikroglia in Gliomen auf
histologischen und in-vitro-Studien. Um das Muster der MikrogliaaktivitÀt und
-motilitÀt sowie die Migration der Zellen zu verstehen, haben wir eine
intravitale Studie in einem orthotopen, murinen Gliommodell mit CX3CR1-eGFPGFP
/wt-MĂ€usen konzipiert. Wir haben die Dynamik der intratumoralen Mikroglia-
Akkumulation und -aktivitÀt und auch die Interaktion der Mikrogliazellen mit
BlutgefĂ€Ăen des Tumors mit intravitaler Epi- und Zwei-Photonen-
Fluoreszenzmikroskopie untersucht. Zudem haben wir die zellulÀren Funktionen
und Gewebsfunktionen einschlieĂlich der Messung der EnzymaktivitĂ€t der NADPH-
Oxidase durch in-vivo-Fluoreszenzlebenszeitmessung im Tumor und in der
Mikroglia dargestellt. Zudem haben wir den Einfluss der antivaskulÀren
Therapie auf das Verhalten der Mikrogliazellen sowie deren Migration und
Proliferation untersucht. Wir konnten drei morphologische PhÀnotypen von
Tumor-assoziierter Mikroglia mit vollkommen unterschiedlichen ZellgröĂen und
Bewegungsmustern identifizieren. Im Gegensatz zu vorrausgegangenen Studien
konnten wir beobachten, dass sich eine Fraktionen der Mikrogliazellen sehr
schnell im Tumorgewebe bewegt und groĂe Strecken binnen weniger Minuten
zuruÌcklegt wohingegen sich die anderen Fraktionen wenig oder gar nicht
bewegen. Wir stellten fest, dass sich die NADPHOxidase-AktivitÀt in den
unterschiedlichen Mikroglia-Subtypen dramatisch unterscheidet und hierdurch
verschieden groĂe Mengen von reaktiven Sauerstoffradikalen produziert werden.
Dies weist auf die vielseitige Funktion der verschiedenen Mikroglia-Subtypen
hin. Wir haben beobachtet, dass die gröĂte MotilitĂ€t der Mikroglia in der
peri-vaskulÀren Nische zu finden ist, was auf die hohe Relevanz der Mikroglia-
TumorgefĂ€Ă-interaktion hindeutet. Entsprechend zeigte sich unter
AntivaskulĂ€rer Therapie eine relevante GefĂ€Ăreduktion im Tumor und konsekutiv
eine Abnahme der Mikrogliazellen im Gliom. Die vorgestellten Daten bestÀtigen
die Relevanz des TumorgefĂ€Ă-Kompartiments auf die Migration und die Biologie
der Mikroglia. Zusammengefasst konnten wir neue Einsichten in das Verhalten
der Mikrogliazellen in vivo hinsichtlich ihrer MotilitÀt und Funktion im
malignen Gliom eröffnen.Time Lapse In Vivo Microscopy Reveals Distinct Dynamics of Microglia-Tumor
Environment Interactions â A New Role for the Tumor Perivascular Space as
Highway for Trafficking Microglia Microglial cells are critical for glioma
growth and progression. However, only little is known about intratumoral
microglial behavior and the dynamic interaction with the tumor. Currently the
scarce understanding of microglial appearance in malignant gliomas merely
originates from histological studies and in vitro investigations. In order to
understand the pattern of microglia activity, motility and migration we
designed an intravital study in an orthotopic murine glioma model using
CX3CR1-eGFPGFP/wt mice. We analysed the dynamics of intratumoral microglia
accumulation and activity, as well as microglia/tumor blood vessel interaction
by epi-illumination and 2-photon laser scanning microscopy. We further
investigated cellular and tissue function, including the enzyme activity of
intratumoral and microglial NADPH oxidase measured by in vivo fluorescence
lifetime imaging. Furthermore we investigated the influence of antivascular
therapy on microglia cell behavior, migration and proliferation within
malignant gliomas. We identified three morphological phenotypes of tumor-
associated microglia cells with entirely different cell size and motility
patterns. In contrast to previous studies we could observe that one fraction
of microglia cells showed a very fast cell movement within the tumor tissue
and covers long distances within a few minutes. We found that NADPH oxidase
activation is highly divergent in these different microglia subtypes leading
to distinct production levels of reactive oxygen species (ROS) indicating
various functions of microglia subtypes. We observed that microglia motility
is highest within the perivascular niche, suggesting relevance of
microglia/tumor blood vessel interactions. In line, a reduction of tumor blood
vessels by antivascular therapy resulted in a decrease of microglia cells in
gliomas. This confirmed the relevance of the tumor vessel compartment on
microglia migration and its biology in brain tumors. In summary, we provide
new insights into in vivo microglial behavior, regarding both morphology and
function, in malignant gliomas
Treatment results for lumbar epidural lipomatosis: Does fat matter?
PurposeThe lumbar epidural lipomatosis (LEL) is a rare disease that can cause sciatic pain syndrome or neurological deficits comparable to symptoms caused by a classical spinal canal stenosis. In severe cases surgical decompression was conducted. However, the outcome after decompressive surgery has only been investigated in small case series. In this study we compared the outcome of LEL patients after microsurgery with the outcome of patients with classical spinal stenosis (CSS).MethodsPatients with LEL (n=38) and patients with CSS (n=51), who received microsurgical decompression, were followed in a prospective observational study for 3years. The clinical results including the Oswestry Disability Index, Numeric Pain Rating Scale (NRS), Roland and Morris Disability Questionnaire, the Short Form-36 Score and the Walking Distance were analysed and compared between both groups.ResultsPatients with LEL improved significantly after microsurgical decompression in a 3-year follow-up concerning back pain, leg pain and pain-associated disability equal to patients with CSS (NRSback_LEL_preop.=6.4; NRSback_CSS_preop.=6.3; NRSback_LEL_3-years=3.2; NRSback_CSS_3-years=3.6; NRSleg_LEL_preop.=6.3; NRSleg_CSS_preop.=6.5; NRSleg_LEL_3-years=2.5; NRSleg_CSS_3-years=2.9; ODILEL_preop.=52.7; ODICSS_preop=51.8; ODILEL_3-years=32.3; ODICSS_3-years=27.6). The microsurgical decompression had a positive effect on the health-related quality of life, and patient satisfaction was high in both groups (LEL group71%, CSS group69%).ConclusionsLEL can influence the quality of life dramatically and cause a high degree of disability. A surgical decompression is a safe and effective procedure with a good clinical outcome comparable to the results in patients with an osteoligamentous spinal stenosis. Therefore, microsurgical decompression can be recommended in patients with LEL if conservative treatment fails. [GRAPHICS]
Radiofrequency denervation for treatment of sacroiliac joint pain-comparison of two different ablation techniques
The radiofrequency treatment (RFD) for sacroiliac joint pain (SIP) is well-established, but there is still scarce evidence on its clinical outcome. The classical monopolar RFD is limited by a high recurrence rate. This might be caused by an incomplete denervation of the dorsal rami. The Simplicity III probe was invented to optimise pain fibre recruitment by its multi-electrode design. However, the clinical superiority of this procedure was never proven. The aim of this study was to illustrate the effectiveness of RFD and to compare both denervation techniques. One hundred twenty-one patients were included, and their clinical course was analysed. Fifty-seven patients received conventional treatment with multiple percutaneous monopolar RFDs (monolesion probe group, MoLG) and 64 patients with the Simplicity III probe (multilesion probe group, MuLG). All patients were followed 1, 3, 6 and 12 s after RFD. Clinical outcome scores were analysed (numeric pain rating scale (NPRS), Roland-Morris Disability Questionnaire, Oswestry Disability Index (ODI), Odom's criteria, Short Form 36 score). The MuLG showed a clearly advanced improvement concerning the clinically relevant pain relief (>= 50%) (1 month/3 months /6 months/12 months = 72%, 55%, 36%, 27% vs. 1 month/3 months/6 months/12 months = 39%, 28%, 16%, 11%) as well as an advanced improvement of pain-associated disability and a higher satisfaction rating compared to the MoLG (NPRSMuLG_(preop) = 8,3; NPRSMuLG_(12months) = 5.8; NPRSMoLG_preop = 7,7; NPRSMoLG_12months = 5.8; ODIMuLG_preop = 52; ODIMuLG_12months = 42; ODIMoLG_preop = 52; ODIMoLG_12months = 47; ODOMSMuLG_good/excellent = 54%; ODOMSMoLG_good/excellent = 28%). RFD of the SIP with the Simplicity III probe is effective and delivers a distinct pain reduction even after 1 year of treatment. This technique shows clear advantages compared to the conventional monolesion technique and is a useful treatment for patients with recurrent SIP
High frequency of low-virulent microorganisms detected by sonication of implanted pulse generators: so what?
Introduction: Deep brain stimulation (DBS) has become a well-established treatment modality for a variety of conditions over the last decades. Multiple surgeries are an essential part in the postoperative course of DBS patients if nonrechargeable implanted pulse generators (IPGs) are applied. So far, the rate of subclinical infections in this field is unknown. In this prospective cohort study, we used sonication to evaluate possible microbial colonization of IPGs from replacement surgery. Methods: All consecutive patients undergoing IPG replacement between May 1, 2019 and November 15, 2020 were evaluated. The removed hardware was investigated using sonication to detect biofilm-associated bacteria. Demographic and clinical data were analyzed. Results: A total of 71 patients with a mean (±SD) of 64.5 ± 15.3 years were evaluated. In 23 of these (i.e., 32.4%) patients, a positive sonication culture was found. In total, 25 microorganisms were detected. The most common isolated microorganisms were Cutibacterium acnes (formerly known as Propionibacterium acnes) (68%) and coagulase-negative Staphylococci (28%). Within the follow-up period (5.2 ± 4.3 months), none of the patients developed a clinical manifest infection. Discussions/Conclusions: Bacterial colonization of IPGs without clinical signs of infection is common but does not lead to manifest infection. Further larger studies are warranted to clarify the impact of low-virulent pathogens in clinically asymptomatic patients
