    Demographic and cognitive profile of individuals seeking a diagnosis of Autism Spectrum Disorder in adulthood

    Little is known about ageing with autism spectrum disorder (ASD). We examined the characteristics of adults referred to a specialist diagnostic centre for assessment of possible ASD, 100 of whom received an ASD diagnosis and 46 did not. Few demographic differences were noted between the groups. Comorbid psychiatric disorders were high in individuals with ASD (58%) and non-ASD (59%). Individuals who received an ASD diagnosis had higher self-rated severity of ASD traits than non-ASD individuals. Within the ASD group, older age was associated with higher ratings of ASD traits and better cognitive performance. One interpretation is that general cognitive ability and the development of coping strategies across the lifespan, do not necessarily reduce ASD traits but may mitigate their effects

    Developmental expression of orphan g protein-coupled receptor 50 in the mouse brain

    [Image: see text] Mental disorders have a complex etiology resulting from interactions between multiple genetic risk factors and stressful life events. Orphan G protein-coupled receptor 50 (GPR50) has been identified as a genetic risk factor for bipolar disorder and major depression in women, and there is additional genetic and functional evidence linking GPR50 to neurite outgrowth, lipid metabolism, and adaptive thermogenesis and torpor. However, in the absence of a ligand, a specific function has not been identified. Adult GPR50 expression has previously been reported in brain regions controlling the HPA axis, but its developmental expression is unknown. In this study, we performed extensive expression analysis of GPR50 and three protein interactors using rt-PCR and immunohistochemistry in the developing and adult mouse brain. Gpr50 is expressed at embryonic day 13 (E13), peaks at E18, and is predominantly expressed by neurons. Additionally we identified novel regions of Gpr50 expression, including brain stem nuclei involved in neurotransmitter signaling: the locus coeruleus, substantia nigra, and raphe nuclei, as well as nuclei involved in metabolic homeostasis. Gpr50 colocalizes with yeast-two-hybrid interactors Nogo-A, Abca2, and Cdh8 in the hypothalamus, amygdala, cortex, and selected brain stem nuclei at E18 and in the adult. With this study, we identify a link between GPR50 and neurotransmitter signaling and strengthen a likely role in stress response and energy homeostasis

    Convalescent plasma in patients admitted to hospital with COVID-19 (RECOVERY): a randomised controlled, open-label, platform trial

    SummaryBackground Azithromycin has been proposed as a treatment for COVID-19 on the basis of its immunomodulatoryactions. We aimed to evaluate the safety and efficacy of azithromycin in patients admitted to hospital with COVID-19.Methods In this randomised, controlled, open-label, adaptive platform trial (Randomised Evaluation of COVID-19Therapy [RECOVERY]), several possible treatments were compared with usual care in patients admitted to hospitalwith COVID-19 in the UK. The trial is underway at 176 hospitals in the UK. Eligible and consenting patients wererandomly allocated to either usual standard of care alone or usual standard of care plus azithromycin 500 mg once perday by mouth or intravenously for 10 days or until discharge (or allocation to one of the other RECOVERY treatmentgroups). Patients were assigned via web-based simple (unstratified) randomisation with allocation concealment andwere twice as likely to be randomly assigned to usual care than to any of the active treatment groups. Participants andlocal study staff were not masked to the allocated treatment, but all others involved in the trial were masked to theoutcome data during the trial. The primary outcome was 28-day all-cause mortality, assessed in the intention-to-treatpopulation. The trial is registered with ISRCTN, 50189673, and ClinicalTrials.gov, NCT04381936.Findings Between April 7 and Nov 27, 2020, of 16 442 patients enrolled in the RECOVERY trial, 9433 (57%) wereeligible and 7763 were included in the assessment of azithromycin. The mean age of these study participants was65·3 years (SD 15·7) and approximately a third were women (2944 [38%] of 7763). 2582 patients were randomlyallocated to receive azithromycin and 5181 patients were randomly allocated to usual care alone. Overall,561 (22%) patients allocated to azithromycin and 1162 (22%) patients allocated to usual care died within 28 days(rate ratio 0·97, 95% CI 0·87–1·07; p=0·50). No significant difference was seen in duration of hospital stay (median10 days [IQR 5 to >28] vs 11 days [5 to >28]) or the proportion of patients discharged from hospital alive within 28 days(rate ratio 1·04, 95% CI 0·98–1·10; p=0·19). Among those not on invasive mechanical ventilation at baseline, nosignificant difference was seen in the proportion meeting the composite endpoint of invasive mechanical ventilationor death (risk ratio 0·95, 95% CI 0·87–1·03; p=0·24).Interpretation In patients admitted to hospital with COVID-19, azithromycin did not improve survival or otherprespecified clinical outcomes.     Friendship in young people with Asperger's syndrome

    The understanding and experiences of twenty young people with Asperger's syndrome (AS) were compared with those of nineteen age-matched neurotypical young people. The participants were aged between eleven and nineteen years of age. They were asked: first, to define "a friend"; second, to say whether or not they thought that friends were important; third, to complete a questionnaire that asked about what they did with their friends; and fourth, to rate the behaviours of two friends in each of six story vignettes. Supplementary information was obtained by means of a brief semi-structured interview with each of the participants and, about the Asperger's group, by means of questionnaires completed by some of their parents and staff It was found that, whilst the definitions of friendship that the AS group gave were similar in content to those of the neurotypical group, some, at least, had a rather theoretical quality. Although most of the AS group thought that friendships were important, significantly fewer of them than of the neurotypical group mentioned the importance of companionship in explaining why. Responses to the friendship questionnaire suggested that there were differences between the two groups in terms of the security of their friendships, that is the extent to which they felt able to rely upon their friends and to transcend any differences arising. There were significant between-group differences in the responses to five of the six friendship vignettes, suggesting that young people with AS have a different perspective on what is and is not appropriate social behaviour. Information from the semi-structured interviews and the parent and staff questionnaires was used to provide illustrations of the between-groups differences that emerged and also to highlight instances where the formal measures perhaps failed to access fully the lived experience of the young people with AS. The difficulties that the AS group faced in forming and maintaining friendships were explored, with a view to considering how their friendships could best be supported. Directions for future research were suggested and possible methodologies were considered

    Zur Messung von Staat-Kirche-Beziehungen. Eine vergleichende Analyse neuerer Indizes

    Im Zuge der erhöhten Aufmerksamkeit, welche dem Faktor Religion in der Politik zuteil wird, ist das spannungsreiche Verhältnis von Staat und Kirche wieder vermehrt in den Blick geraten. Gleichwohl werden bisherige Forschungsbemühungen der quantitativ-vergleichenden Politikwissenschaft oftmals durch einen Mangel an geeigneten Messkonzepten gehemmt. Erst in allerjüngster Zeit sind verschiedene Vorschläge zur Messung von Staat-Kirche-Beziehungen hervorgebracht worden. Ziel des vorliegenden Beitrags ist es, diese neuen Messvorschläge vorzustellen und in einem kritischen Vergleich ihre jeweiligen methodischen Stärken und Schwächen herauszuarbeiten. Genauer gesprochen wird der Versuch unternommen, den von Munck und Verkuilen (Comparative Political Studies 35:5–24, 2002) im Rahmen der Demokratiemessung vorgeschlagenen und von Müller und Pickel (Politische Vierteljahresschrift 48:511–539, 2007) präzisierten Kriterienkatalog zur methodologischen Bewertung von Messkonzepten auf insgesamt fünf verschiedene Indizes zur Messung von Staat-Kirche-Verflechtungen anzuwenden. Konkret wird dabei anhand spezifischer Beurteilungskriterien untersucht, inwieweit die methodischen Herausforderungen der Konzeptualisierung, der Messung sowie der Aggregation bei der Konstruktion der betrachteten Indizes überzeugend bewältigt wurden. Neben der vergleichenden Analyse und Bewertung von neuen Messinstrumenten wird damit gleichzeitig die Brauchbarkeit dieses Evaluationsschemas, sowie seine Übertragbarkeit in alternative Anwendungskontexte erprobt
