Hernie diaphragmatique post-traumatique de lâenfant: Ă propos dâun cas au Centre Hospitalier Universitaire PĂ©diatrique Charles de Gaulle de Ouagadougou
La hernie diaphragmatique post-traumatique est une urgence chirurgicale rare chez lâenfant mais pouvant mettre rapidement en jeu le pronosticvital. Les auteurs rapportent le cas dâun garçon de 04 ans admis aux urgences pour douleur abdominale suite Ă une contusion thoraco-abdominale par accident de la voie publique. Le bilan radiologique initial a consistĂ© en une Ă©chographie abdominale qui a rĂ©vĂ©lĂ© un hĂ©mopĂ©ritoine de petite abondance sans lĂ©sion focale. Douze heures aprĂšs son admission, le patient a prĂ©sentĂ© une dĂ©tresse respiratoire avec tableau clinique depneumothorax gauche qui a nĂ©cessitĂ© une exsufflation en urgence. Le diagnostic de hernie diaphragmatique gauche a Ă©tĂ© posĂ© Ă la radiographiedu thorax rĂ©alisĂ©e aprĂšs la ponction. Lâenfant a bĂ©nĂ©ficiĂ© dâune cure chirurgicale. LâĂ©volution a Ă©tĂ© favorable. La hernie diaphragmatique posttraumatique, bien que rare chez lâenfant, devrait ĂȘtre systĂ©matiquement recherchĂ©e par une radiographie thoracique ou un scanner thoracoabdominal devant tout traumatisme abdominal avec hyper pression. Les perforations iatrogĂšnes de lâĆsophage chezlâenfant. A propos de quatre cas
La perforation de lâĆsophage est une complication rare, grave, et la plus mortelle des perforations du tractus digestif. Nous rapportons 4 cas de perforations iatrogĂšnes de lâĆsophage pris en charge dans notre service. LâĂąge de nos patients Ă©tait respectivement de 2 ans, 3 ans, 4 ans et 5 ans. Lâon comptait 3 patients de sexe masculin et 1 patient de sexe fĂ©minin. Trois patients Ă©taient suivis pour une stĂ©nose caustique de lâĆsophage pour laquelle deux avaient fait lâobjet dâune gastrostomie dâalimentation avec pose dâun fil sans fin. Un patient avait un antĂ©cĂ©dent dâingestion dâune pile bouton enclavĂ©e dans lâĆsophage depuis 5 mois. La perforation Ă©tait survenue chez trois patients au cours dâune dilatation instrumentale de lâĆsophage et chez un patient pendant une oesophagoscopie. Tous les patients ont Ă©tĂ© admis en unitĂ© de rĂ©animation oĂč tous ont bĂ©nĂ©ficiĂ© dâune triple antibiothĂ©rapie, dâun drainage pleural, dâun arrĂȘt alimentaire et dâune nutrition parentĂ©rale pour deux d'entre eux. LâĂ©tat gĂ©nĂ©ral Ă©tait considĂ©rablement altĂ©rĂ© chez deux patients qui nâont pas pu bĂ©nĂ©ficier dâune reprise chirurgicale. Une colo-oesophagoplastie a Ă©tĂ© rĂ©alisĂ©e chez un patient aprĂšs un mois et demi de rĂ©animation. Les suites aprĂšs 18 mois de reculsontsimples. Un patient est en attente de son remplacement Ćsophagien. Les deux autres sont dĂ©cĂ©dĂ©s. Les perforations iatrogĂšnes de lâĆsophage sont graves. La prĂ©cocitĂ© du diagnostic, de la rĂ©animation et de la prise en charge chirurgicale conditionne le pronostic.Mots clĂ©s : perforation iatrogĂšne, Ćsophage, enfant, Ouagadougou.Perforation of the esophagus is a rare, serious complication, and the most dangerous perforation of the digestive tract. We report 4 cases of iatrogenic perforations of the esophagus managed in our department. The age of our patients was 2, 3, 4 and 5 years,respectively. We had 3 male patients and 1 female patient. Three patients were followed for caustic stenosis of the esophagusfor which two patients had benefited from feeding gastrostomy with placement of an endless thread. Only one had a history of ingesting a button cell that had been blocked in the esophagus for 5 months. Perforation occurred in three patients during instrumental dilatation of the esophagus and in one patient during oesophagoscopy. All patients were admitted to an intensive care unit where all received triple antibiotic therapy, pleural drainage, nothing per os, and two had parenteral nutrition. The general condition was significantly altered in two patients who could not benefit from surgical treatment. A colo-oesophagoplasty was performed in a patient after 6 weeks of resuscitation. The outcome after 18 months of follow-up are simple. A patient is awaiting esophageal replacement and the other two have died. Iatrogenic perforations of the esophagus are serious.
