Association of intestinal helminthic infection and nutritional status of primary school children in Gombe State, Nigeria
Background: Intestinal helminthic infections are among the commonest infections worldwide. It often affects the poorest communities and has similar geographic distribution with malnutrition. Intestinal helminthic infection contributes to undernutrition through subtle reduction in digestion and absorption of food, chronic inflammation and loss of nutrients. The objective of this study is to determine the prevalence of intestinal helminthic infection and its relationship with nutritional status of primary school children in Gombe, Gombe State, Nigeria.
Methodology: This was a cross sectional study of 350 pupils selected through multistage random sampling technique from 24 primary schools in Gombe, Gombe State, Nigeria from July 2018 to January 2019. Demographic information including age, gender, height, and weight were collected from each participant with a designed collection form. The data were analysed using SPSS version 24.0, and presented as frequency distribution and mean ± SD. The Chi-square test (with Odds ratio and 95% confidence interval) was used to test for association between prevalence of helminthiasis and factors such as gender, age group and school type. A p value of less than 0.05 was considered statistically significant at 95% confidence interval.
Results: The prevalence of intestinal helminthic infection was 23.7% (83/350). Eighty (96.4%) of the 83 infected pupils were in public schools while only 3 (3.6%) were in private schools (p<0.001). The prevalence of helminthiasis was significantly higher in underweight pupils (34%, OR=2.113, p=0.0065)) and significantly lower (5.4%, OR=0.1637, p=0.0037) in overweight pupils while the prevalence was not significantly associated with normal weight (p=0.5482) or obesity (p=1.000).
Conclusions: Intestinal helminthic infection is a public health problem in children with adverse significant relationship with nutritional status. Provision of toilet facilities in schools and periodic de-worming of pupils aimed at reducing loss of nutrients from intestinal helminthiasis are recommended
Bio-efficacy of an organophosphorous bait (Snip®) against wild populations of synanthropic flies Musca domestica and Lucilia species
The common housefly, Musca domestica and the green-bottle fly Lucilia species are Diptera belonging to the Suborder Cyclorrapha. The former species is associated with mechanical transmission of certain diarrhoeal diseases such as dysentery, typhoid fever, and cholera, which afflict man. Other important diseases include, anthrax, eye infections and bovine mastitis. It is the manner in which M. domestica exudes a “vomit-drop” to sugar, dried blood, pus, excreta, sputum and other substances that makes it an efficient vector of human diseases. Bacteria may also adhere to the hairy body of the fly and to the hairy puvilli on the feet. Lucilia sp. is mostly associated with diseases of livestock such as sheep and fowls. This species causes myiasis of sheep called “strike”, which results in larval development in the skin. Usually, environmental sanitation involving elimination of fly breeding sites by proper disposal of refuse, manure, compost, human excreta and other waste is the fundamental measure for fly control. However, there are instances where control has to be supplemented with insecticides. Among the most commonly used insecticidal methods of control is the use of baits. These are placed or applied to surfaces where adult flies congregate to feed. This control measure takes advantage of the fly's mechanism of feeding. This paper discusses findings of an experiment designed to test attractiveness and bio-efficacy of an organophosphate-based bait against M. domestica and Lucilia species.

The Perceptions on Male Circumcision as a Preventive Measure Against HIV Infection and Considerations in Scaling up of the Services: A Qualitative Study Among Police Officers in Dar es Salaam, Tanzania.

In recent randomized controlled trials, male circumcision has been proven to complement the available biomedical interventions in decreasing HIV transmission from infected women to uninfected men. Consequently, Tanzania is striving to scale-up safe medical male circumcision to reduce HIV transmission. However, there is a need to investigate the perceptions of male circumcision in Tanzania using specific populations. The purpose of the present study was to assess the perceptions of male circumcision in a cohort of police officers that also served as a source of volunteers for a phase I/II HIV vaccine (HIVIS-03) trial in Dar es Salaam, Tanzania. In-depth interviews with 24 men and 10 women were conducted. Content analysis informed by the socio-ecological model was used to analyze the data. Informants perceived male circumcision as a health-promoting practice that may prevent HIV transmission and other sexually transmitted infections. They reported male circumcision promotes sexual pleasure, confidence and hygiene or sexual cleanliness. They added that it is a religious ritual and a cultural practice that enhances the recognition of manhood in the community. However, informants were concerned about the cost involved in male circumcision and cleanliness of instruments used in medical and traditional male circumcision. They also expressed confusion about the shame of undergoing circumcision at an advanced age and pain that could emanate after circumcision. The participants advocated for health policies that promote medical male circumcision at childhood, specifically along with the vaccination program. The perceived benefit of male circumcision as a preventive strategy to HIV and other sexually transmitted infections is important. However, there is a need to ensure that male circumcision is conducted under hygienic conditions. Integrating male circumcision service in the routine childhood vaccination program may increase its coverage at early childhood. The findings from this investigation provide contextual understanding that may assist in scaling-up male circumcision in Tanzania.\u
Ethnobotanical survey of herbs used in the management of diabetes mellitus in Southern Katanga Area/DR Congo
peer reviewedIntroduction: Diabetes is becoming a public health burden for sub-Saharan countries due to its prevalence which is growing rapidly. Traditional medicine is more and more used to treat diabetes in RD Congo as well as in other African countries. This study was undertaken in order to list plants used in the management of diabetes by traditional healers in four agglomerations of southern area of Katanga in the Democratic Republic of Congo. Methods: Forty-nine traditional healers were randomly met and interviewed about diabetes treatment in traditional medicine. The survey concerned the plant identification, their part used, method of preparation and the route of administration. The inquest concerned also traditional medicine users. Results: Ninety-five plants from 47 families were indicated as antidiabetic. Fabaceae (24.2%), Euphorbiaceae (7.4%), Apocynaceae and Strychnaceae (4.2 each) are the more representative families. This inventory showed that the root is the most used part of the cited plants, the decoction with water as the main preparation method and the oral administration as the principal way to give antidiabetic traditional formulations. Conclusion: In Lubumbashi region, many plant species are used to treat diabetes either through traditional praticians or by anyone from well-known ancestral knowledge
Effect of Improved access to Antiretroviral Therapy on clinical characteristics of patients enrolled in the HIV care and treatment clinic, at Muhimbili National Hospital (MNH), Dar es Salaam, Tanzania

Sub-Saharan Africa has been severely affected by the HIV and AIDS pandemic. Global efforts at improving care and treatment has included scaling up use of antiretroviral therapy (ART). In Tanzania, HIV care and treatment program, including the provision of free ART started in 2004 with a pilot program at Muhimbili National Hospital in Dar es Salaam. This study describes the socio-demographic and clinical features of patients enrolled at the care and treatment clinic at MNH, Dar es Salaam, Tanzania. A cross-sectional study looking at baseline characteristics of patients enrolled at the HIV clinic at MNH between June 2004-Dec 2005 compared to those enrolled between 2006 and September 2008. Of all enrolled patients, 2408 (58.5%) were used for analysis. More females than males were attending the clinic. Their baseline median CD4 cell count was low (136 cells/microl) with 65.7% having below 200 cells/microl. Females had higher CD4 cell counts (150 cells/microl) than males (109 cells/microl) p < 0.001). The most common presenting features were skin rash and/or itching (51.6%); progressive weight loss (32.7%) and fever (23.4). Patients enrolled earlier at the clinic (2004-5) were significantly more symptomatic and had significantly lower CD4 cell count (127 cells/microl) compared to CD4 of 167 cells/microl in those seen later (2006-8) (p < 0.001). Patients enrolled to the MNH HIV clinic were predominantly females, and presented with advanced immune-deficiency. Improved access to HIV care and treatment services seems to be associated with patients' early presentation to the clinics in the course of HIV disease.\u
Preferential Targeting of Conserved Gag Regions after Vaccination with a Heterologous DNA prime - Modified Vaccinia Ankara (MVA) boost HIV-1 vaccine regimen
Prime-boost vaccination strategies against HIV-1 often include multiple variants for a given immunogen for better coverage of the extensive viral diversity. To study the immunologic effects of this approach, we characterized breadth, phenotype, function and specificity of Gag-specific T cells induced by a DNA-prime Modified Vaccinia Ankara (MVA)-boost vaccination strategy, which uses mismatched Gag immunogens in the TamoVac 01 phase IIa trial. Healthy Tanzanian volunteers received three injections of the DNA-SMI vaccine encoding for a subtype B and AB-recombinant Gagp37 and two vaccinations with MVA-CMDR encoding subtype A Gagp55 Gag-specific T-cell responses were studied in 42 vaccinees using fresh peripheral blood mononuclear cells. After the first MVA-CMDR boost, vaccine-induced IFN-γ(+) Gag-specific T cell responses were dominated by CD4(+) T cells (compared to CD8(+) T cells, p<0.001) that co-expressed IL-2 (66.4%) and/or TNFα (63.7%). A median of 3 antigenic regions were targeted with a higher median response magnitude to Gagp24 regions - more conserved between prime and boost - as compared to regions within Gagp15 (not primed) and Gagp17 (less conserved, both p<0.0001). Four regions within Gagp24 were each targeted by 45% to 74% of vaccinees upon restimulation with DNA-SMI-Gag matched peptides. The response rate to individual antigenic regions correlated with the sequence homology between the MVA and DNA Gag encoded immunogens (p=0.04, r(2)=0.47). In summary, after the first MVA-CMDR boost, the sequence-mismatched DNA-prime MVA-boost vaccine strategy induced a Gag-specific T cell response that was dominated by polyfunctional CD4(+) T cells and that targeted multiple antigenic regions within the conserved Gagp24 Protein.IMPORTANCE Genetic diversity is a major challenge for the design of vaccines against variable viruses. While including multiple variants for a given immunogen in prime-boost vaccination strategies is one approach that aims to improve coverage for global virus variants, the immunologic consequences of this strategy have been poorly defined so far. It is unclear whether inclusion of multiple variants in prime-boost vaccination strategies improves recognition of variant viruses by T cells and by which mechanisms this would be achieved; either by improved cross-recogniton of multiple variants for a given antigenic region or rather through preferential targeting of antigenic regions more conserved between prime and boost. Engineering vaccines to induce adaptive immune responses that preferentially target conserved antigenic regions of viral vulnerability might facilitate better immune control after preventive and therapeutic vaccination for HIV and for other variable viruses
