    National audienceLa réfection du complexe aréolo-mamelonnaire est l’étape finale de la reconstruction mammaire. Elle restitue l’identité visuelle du sein. Les options les plus courantes pour refaire une aréole sont la dermopimentation, la greffe de peau totale de la racine interne de la cuisse et la greffe de peau totale prélevée sur l’aréole controlatérale. Les solutions de reconstruction mamelonnaire sont le dédoublement du mamelon controlatéral, le lambeau local, la greffe de peau totale et le tatouage en trompe l’œil. Le dédoublement du mamelon est la technique de choix parce qu’elle offre d’emblée une couleur, une texture et une projection optimales. La dermopigmentation aréolaire est la procédure la plus courante pour sa simplicité et sa reproductibilité

    Oncological safety of nipple-sparing prophylactic mastectomy: A review of the literature on 3716 cases

    INTRODUCTION: The objective of our study was to evaluate the risk of cancer after prophylactic nipple-sparing mastectomy (PNSM). MATERIAL AND METHODS: The PubMed database was consulted using the following key-words: "nipple-sparing mastectomy", "prophylactic", "locoregional recurrence", "oncological risk". Articles published between January 1995 and December 2016 were searched. RESULTS: Out of the 270 articles found, 19 were included. Overall, 15 studies were retrospective, 2 prospective, 2 prospective and retrospective and 3 were multicentric. All told, they involved 3890 patients corresponding to 6786 mastectomies, among which the total number of prophylactic nipple-sparing mastectomies was 3716. Average age of the patients was 44.4years and average follow-up was 38.4months (8-168months); 29.4% of them had a BRCA 1 or 2 mutation; 85 and 15% underwent prosthetic and autologous reconstructions, respectively. Average cancer rates exterior to and within the nipple areolar complex (NAC) were 0.2 and 0.004%, respectively. The overall average rate of histological pre-malignant lesions in the nipple areolar complex was 1.5%. The overall complication rate was 20.5%, and necrosis rates of the nipple areolar complex and the skin were 8.1 and 7.1%, respectively. CONCLUSION: In prophylactic breast surgery, conservation of the nipple areolar complex does not seem to increase the risk of cancer development. However, short follow-up time and the different methodologies applied in the different studies presently preclude generalization of the technique

    Relation parents-enfant porteur de FLP : importance du délai précédant la première réparation chirurgicale

    Une écoute attentive des familles dont l’un des enfants est porteur de Fente(s) Labiale(s) associée(s) ou non à une Fente Palatine (FLP) révèle l’importance du contexte psychologique et les risques d’une structuration dysharmonieuse des relations parents-enfant avec ses conséquences sur le développement de l’enfant. La naissance d’un enfant porteur de FLP est un événement particulièrement stressant et traumatisant pour les parents. Dans le cadre d’un Projet Hospitalier de Recherche Clinique (PHRC), les perceptions psychologiques des parents d’enfants porteurs de FLP dans l’année qui suit la naissance de leur enfant sont évaluées par une étude prospective, multidisciplinaire et multicentrique. Le but est d’analyser l’état de souffrance psychique de l’enfant ainsi que les relations parentsenfant. Les résultats sont comparés entre les groupes de quatre centres utilisant un protocole chirurgical différent en distinguant les sous-groupes de parents ayant appris le diagnostic in utero et de ceux l’ayant découvert à la naissance. La dimension psychique et psychologique due à la malformation et à sa correction seront analysées pour les parents (importance du diagnostic prénatal, développement relationnel avec l’enfant, image de soi, qualité de vie) mais également pour l’enfant (détresse et retrait éventuels). Les résultats de cette étude pourraient permettre d’élaborer de nouvelles façons d’atténuer, dans la mesure du possible, l’impact psychologique de la malformation pour les parents et leur enfant et d’améliorer le traitement de ces enfants sur le long terme

    Relationships between parents and children with labio-palatal clefts: the importance of a delay preceding the first surgical repair

    Any attentive observer of families who have a child afflicted with a cleft lip whether or not associated with a palatal cleft would be certain to be impressed by the importance of the psychological context of their interpersonal exchanges and the risks of a breakdown in the parent-child relationship adversely affecting the child’s development. The birth of a child with a cleft lip inevitably becomes a painfully stressful and traumatic event for its parents. In the framework of the Programme Hospitalier de Recherche Clinique (PHRC) we have evaluated the psychological perceptions of parents of cleft lip and palate patients during the child’s first year of life in a multi-disciplinary and multi-group prospective study. Our goal is to analyze both the child’s psychic suffering and the parent-child relationship. We compared the results obtained in four hospital centers using different surgical protocols, distinguishing between the sub-groups of parents who had learned of the defect from an in utero examination and parents who did not become aware of the cleft until the child was born. We analyze for the parents the psychic and psychological dimensions of the deformity and its correction, touching on the importance of a prenatal diagnosis, the development of a relationship with the child, self-images, and the quality of life as well focusing on the infant’s distress and eventual recovery from it. The results of this study should make it possible for health care workers to construct new ways of attenuating, insofar as possible, the psychological impact of the deformity on parents and their children and to improve long term care for these patients
