X-ray variability and spectral scaling: a measure of BLR sizes in AGN
We apply a new method of determination of the size of the broad emission-line
region (BLR) in active galactic nuclei. This method relates the radius of the
broad-line region of AGN to the soft X-ray luminosity and spectral index.
Comparing the BLR distances calculated from our photoionization scaling model
to the BLR distances determined by reverberation mapping shows that the scaling
law agrees with the empirical relation. We investigate a
complimentary method of estimating the BLR distance - based on the Keplerian
broadening of the emission lines and the central mass estimated from X-ray

Reverberation Mapping and the Physics of Active Galactic Nuclei
Reverberation-mapping campaigns have revolutionized our understanding of AGN.
They have allowed the direct determination of the broad-line region size,
enabled mapping of the gas distribution around the central black hole, and are
starting to resolve the continuum source structure. This review describes the
recent and successful campaigns of the International AGN Watch consortium,
outlines the theoretical background of reverberation mapping and the
calculation of transfer functions, and addresses the fundamental difficulties
of such experiments. It shows that such large-scale experiments have resulted
in a ``new BLR'' which is considerably different from the one we knew just ten
years ago. We discuss in some detail the more important new results, including
the luminosity-size-mass relationship for AGN, and suggest ways to proceed in

A dust-parallax distance of 19 megaparsecs to the supermassive black hole in NGC 4151
The active galaxy NGC 4151 has a crucial role as one of only two active
galactic nuclei for which black hole mass measurements based on emission line
reverberation mapping can be calibrated against other dynamical methods.
Unfortunately, effective calibration requires an accurate distance to NGC 4151,
which is currently not available. Recently reported distances range from 4 to
29 megaparsecs (Mpc). Strong peculiar motions make a redshift-based distance
very uncertain, and the geometry of the galaxy and its nucleus prohibit
accurate measurements using other techniques. Here we report a dust-parallax
distance to NGC 4151 of Mpc. The measurement is
based on an adaptation of a geometric method proposed previously using the
emission line regions of active galaxies. Since this region is too small for
current imaging capabilities, we use instead the ratio of the
physical-to-angular sizes of the more extended hot dust emission as determined
from time-delays and infrared interferometry. This new distance leads to an
approximately 1.4-fold increase in the dynamical black hole mass, implying a
corresponding correction to emission line reverberation masses of black holes
if they are calibrated against the two objects with additional dynamical

Healthcare providersâ perceptions of potentially preventable rural hospitalisations: a qualitative study
Potentially preventable hospitalisations (PPHs) are common in rural communities in Australia and around the world. Healthcare providers have a perspective on PPHs that may not be accessible by analysing routine patient data. This study explores the factors that healthcare providers believe cause PPHs and seeks to identify strategies for preventing them. Physicians, nurses, paramedics, and health administrators with experience in managing rural patients with PPHs were recruited from southern Tasmania, Australia. Semi-structured telephone interviews were conducted, and reflexive thematic analysis was used to analyse the data. Participants linked health literacy, limited access to primary care, and perceptions of primary care services with PPH risk. The belief that patients did not have a good understanding of where, when, and how to manage their health was perceived to be linked to patient-specific health literacy challenges. Access to primary healthcare was impacted by appointment availability, transport, and financial constraints. In contrast, it was felt that the prompt, comprehensive, and free healthcare delivered in hospitals appealed to patients and influenced their decision to bypass rural primary healthcare services. Strategies to reduce PPHs in rural Australian communities may include promoting health literacy, optimising the delivery of existing services, and improving social support structures
Estimating black hole masses of blazars
Estimating black hole masses of blazars is still a big challenge. Because of
the contamination of jets, using the previously suggested size -- continuum
luminosity relation can overestimate the broad line region (BLR) size and black
hole mass for radio-loud AGNs, including blazars. We propose a new relation
between the BLR size and emission line luminosity and present
evidences for using it to get more accurate black hole masses of radio-loud
AGNs. For extremely radio-loud AGNs such as blazars with weak/absent emission
lines, we suggest to use the fundamental plane relation of their elliptical
host galaxies to estimate the central velocity dispersions and black hole
masses, if their velocity dispersions are not known but the host galaxies can
be mapped. The black hole masses of some well-known blazars, such as OJ 287, AO
0235+164 and 3C 66B, are obtained using these two methods and the M -
relation. The implications of their black hole masses on other related studies

Occurrence of testicular microlithiasis in androgen insensitive hypogonadal mice
<b>Background</b>: Testicular microliths are calcifications found within the seminiferous tubules. In humans, testicular microlithiasis (TM) has an unknown etiology but may be significantly associated with testicular germ cell tumors. Factors inducing microlith development may also, therefore, act as susceptibility factors for malignant testicular conditions. Studies to identify the mechanisms of microlith development have been hampered by the lack of suitable animal models for TM.<BR/>
<b>Methods</b>: This was an observational study of the testicular phenotype of different mouse models. The mouse models were: cryptorchid mice, mice lacking androgen receptors (ARs) on the Sertoli cells (SCARKO), mice with a ubiquitous loss of androgen ARs (ARKO), hypogonadal (hpg) mice which lack circulating gonadotrophins, and hpg mice crossed with SCARKO (hpg.SCARKO) and ARKO (hpg.ARKO) mice.<BR/>
<b>Results</b>: Microscopic TM was seen in 94% of hpg.ARKO mice (n=16) and the mean number of microliths per testis was 81 +/- 54. Occasional small microliths were seen in 36% (n=11) of hpg testes (mean 2 +/- 0.5 per testis) and 30% (n=10) of hpg.SCARKO testes (mean 8 +/- 6 per testis). No microliths were seen in cryptorchid, ARKO or SCARKO mice. There was no significant effect of FSH or androgen on TM in hpg.ARKO mice.<BR/>
<b>Conclusions</b>: We have identified a mouse model of TM and show that lack of endocrine stimulation is a cause of TM. Importantly, this model will provide a means with which to identify the mechanisms of TM development and the underlying changes in protein and gene expression
A novel targeted/untargeted GC-Orbitrap metabolomics methodology applied to Candida albicans and Staphylococcus aureus biofilms
Introduction:
Combined infections from Candida albicans and Staphylococcus aureus are a leading cause of death in the developed world. Evidence suggests that Candida enhances the virulence of Staphylococcusâhyphae penetrate through tissue barriers, while S. aureus tightly associates with the hyphae to obtain entry to the host organism. Indeed, in a biofilm state, C. albicans enhances the antimicrobial resistance characteristics of S. aureus. The association of these microorganisms is also associated with significantly increased morbidity and mortality. Due to this tight association we hypothesised that metabolic effects were also in evidence.
Objectives:
To explore the interaction, we used a novel GC-Orbitrap-based mass spectrometer, the Q Exactive GC, which combines the high peak capacity and chromatographic resolution of gas chromatography with the sub-ppm mass accuracy of an Orbitrap system. This allows the capability to leverage the widely available electron ionisation libraries for untargeted applications, along with expanding accurate mass libraries and targeted matches based around authentic standards.
Methods:
Optimised C. albicans and S. aureus mono- and co-cultured biofilms were analysed using the new instrument in addition to the fresh and spent bacterial growth media.
Results:
The targeted analysis experiment was based around 36 sugars and sugar phosphates, 22 amino acids and five organic acids. Untargeted analysis resulted in the detection of 465 features from fresh and spent medium and 405 from biofilm samples. Three significantly changing compounds that matched to high scoring library fragment patterns were chosen for validation.
Conclusion:
Evaluation of the results demonstrates that the Q Exactive GC is suitable for metabolomics analysis using a targeted/untargeted methodology. Many of the results were as expected: e.g. rapid consumption of glucose and fructose from the medium regardless of the cell type. Modulation of sugar-phosphate levels also suggest that the pentose phosphate pathway could be enhanced in the cells from co-cultured biofilms. Untargeted metabolomics results suggested significant production of cell-wall biosynthesis components and the consumption of non-proteinaceous amino-acids
Ruptured ovarian cystic teratoma in pregnancy with diffuse peritoneal reaction mimicking advanced ovarian malignancy: a case report
<p>Abstract</p> <p>Introduction</p> <p>This case illustrates the unusual complication of granulomatous peritonitis following rupture of a dermoid cyst in pregnancy resembling disseminated ovarian carcinoma. To the best of the authors' knowledge, this is the first report of this complication during advanced pregnancy in the literature.</p> <p>Case presentation</p> <p>A dermoid cyst ruptured during surgical removal in the second trimester of pregnancy in a 27-year-old primigravida. Postoperatively the patient suffered pulmonary embolism and leakage of sebaceous material through the abdominal wound. She gradually developed significant abdominal distension, gastrointestinal symptoms and lost more than 8 kg of weight in the 12 weeks postoperatively. The baby was delivered at 31 weeks by a technically challenging caesarean section owing to severe dense adhesions obscuring the uterus. Bowel resection was performed for suspected malignant infiltration and adhesion causing obstruction. She had a protracted convalescence with an ileostomy and mucus fistula. Histology confirmed granulation without malignancy. One year following the surgical treatment, she had recovered well and was planning her next pregnancy.</p> <p>Conclusion</p> <p>Although granulomatous peritonitis following rupture of a dermoid cyst is very rare, awareness is the key to diagnosis and appropriate management. Per-operative frozen section may be helpful.</p
