Symptomatic Porphyria

The administration of hexachlorobenzene (HCB) to rats for periods up to 67 days resulted in the development of gross cytoplasmic vacuolation and the formation of cytoplasmic inclusions. Some of the inclusions consisted of slender tubular structures about 200 A in diameter. Although it was suspected that the inclusions represented precipitated porphyrins, proof of this could not be obtained. The prior administration of iron to some of the animals before commencing feeding with HCB resulted in a more extensive development of the lesions in the liver, and this could account for the higher levels of excreted porphyrins in the iron-laden animals.S. Gender shift in realisation of preferred type of gp practice: longitudinal survey over the last 25 years

Abstract

Background

An increasing number of newly trained Dutch GPs prefer to work in a group practice and as a non-principal rather than in a single-handed practice. In view of the greater number of female doctors, changing practice preferences, and discussions on future workforce problems, the question is whether male and female GPs were able to realise their initial preferences in the past and will be able to do so in the future.

Methods

We have conducted longitudinal cohort study of all GPs in the Netherlands seeking a practice between 1980 and 2004. The Netherlands Institute of Health Services Research (NIVEL) in Utrecht collected the data used in this study by means of a postal questionnaire. The overall mean response rate was 94%.

Results

Over the past 20 years, an increasing proportion of GPs, both male and female, were able to achieve their preference for working in a group practice and/or in a non-principal position. Relatively more women than men have settled in group practices, and more men than women in single-handed practices; however, the practice preference of men and women is beginning to converge. Dropout was highest among the GPs without any specific practice preference.

Conclusion

The overwhelming preference of male and female GPs for working in group practices is apparently being met by the number of positions (principal or non-principal) available in group practices. The preference of male and female GPs regarding the type of practice and job conditions is expected to converge further in the near future.</p

BACKGROUND: There are number of studies showing that general practice is one of the most stressful workplace among health care workers. Since Baltic States regained independence in 1990, the reform of the health care system took place in which new role and more responsibilities were allocated to general practitioners' in Lithuania. This study aimed to explore the psychosocial stress level among Lithuanian general practitioner's and examine the relationship between psychosocial stress and work characteristics. METHODS: The cross-sectional study of 300 Lithuanian General practitioners. Psychosocial stress was investigated with a questionnaire based on the Reeder scale. Job demands were investigated with the R. Karasek scale. The analysis included descriptive statistics; interrelationship analysis between characteristics and multivariate logistic regression to estimate odds ratios for each of the independent variables in the model. RESULTS: Response rate 66% (N = 197). Our study highlighted highest prevalence of psychosocial stress among widowed, single and female general practitioners. Lowest prevalence of psychosocial stress was among males and older age general practitioners. Psychosocial stress occurs when job demands are high and job decision latitude is low (χ(2 )= 18,9; p < 0,01). The multivariate analysis shows that high job demands (OR 4,128; CI 2,102–8,104; p < 0,001), patient load more than 18 patients per day (OR 5,863; CI 1,549–22,188; p < 0,01) and young age of GP's (OR 6,874; CI 1,292–36,582; p < 0,05) can be assigned as significant predictors for psychosocial stress. CONCLUSION: One half of respondents suffering from work related psychosocial stress. High psychological workload demands combined with low decision latitude has the greatest impact to stress caseness among GP's. High job demands, high patient load and young age of GP's can be assigned as significant predictors of psychosocial stress among GP's

Ethanol Induces the Production of Antibodies to Acetaldehyde-Modified Epitopes in Rats

Rats treated with ethanol for periods of 3-27 months were found to produce an immune response to acetaldehyde-modified proteins, whereas treatment for a shorter period (3 weeks) did not lead to such a response. The reactivity towards the modified proteins was shown to be independent of the carrier protein as antibodies were reactive with modified human plasma proteins and modified bovine haemoglobin. The data presented here support the hypothesis that acetaldehyde-modified proteins may act as neoantigens in alcoholics and may play a role in the aetiology of alcoholic liver disease
