    Tükürük, ağız boşluğunun ve genel sağlığın korunmasında kritik öneme sahiptir. Tükürük bileşiminde çeşitli elektrolitleri, küçük organik maddeleri, proteinleri ve peptitleri içeren, berrak, hafif asidik serömüsinöz bir salgıdır. Ağız dokularını kayganlaştırma yanında, yutma ve konuşma gibi ağız işlevleri tükürük aracılığı ile gerçekleşir, aynı zamanda dişleri ve mukozal yüzeyleri korur. Tükürüğün salgılanması ve özellikleri yaşlanma ile birlikte değişir. Bu durum ağız kuruluğuna neden olabilir ve yaşlıları disfaji, diş çürükleri, periodontal hastalık, tat alma bozukluğu, mukozit, ağrı, kandidiyazis ve diğer oral enfeksiyon hastalıkları gibi rahatsızlıklara yatkın hale getirir. Ağız kuruluğu, diş protezine karşı toleransın zayıflamasına ve yetersiz beslenmeye neden olarak genel sağlığı olumsuz yönde etkiler. Bu durum yaşlıların yaşam kalitesinde azalmaya neden olur. Bu derlemede, yaşlanma süreci ile tükürük özelliklerinde meydana gelen değişiklikler hakkında bilgi sunulması amaçlanmıştır.Anahtar Kelimeler: Tükürük; yaşlanma; ağız kuruluğ

    The effect of conscious sedation on salivary alpha-amylase levels during third molar surgery

    ABSTRACT Aim: The aim of the present study was to investigate whether salivary alpha-amylase levels could be decreased by conscious sedation in the patients undergoing impacted third molar extraction. Material and methods: A total of 18 male patients were recruited. All patients were administered the Modified Dental Anxiety Scale test. Patients were divided into a test group (procedures under sedation) and a control group (procedures under local anesthesia). Systolic blood pressure, diastolic blood pressure, oxygen saturation, and heart rate were monitored at different study time-points. Five samples of saliva were taken from each patient: the first time the patient came to the clinic, the patient sat in the chair for extraction, before local anesthesia, immediately after extraction, at 4 h after extraction. Results: Although no statistically important difference was found for systolic blood pressure (p>0.05) between groups, postoperative diastolic blood pressure level of control group was statistically higher than the test group (p=0.030). Also, a statistically significant decrease was found in the oxygen saturation level in postoperative time compared to preoperative time (p<0.05). Conclusion: Even though conscious sedation may be a solution for dental anxiety and phobia, our results indicated that sedation did not affect acute stress levels during oral surgery.     Saliva profiles in children with congenital heart disease

    Purpose The low salivary pH and buffering capacity are caused by using heart failure medications. For this reason oral health should be supported in cardiac patients, it is necessary that they attend dental clinics for regular follow up. The aim of this study is to evaluate the relationship between the salivary oxidative stress markers and salivary pH, salivary buffering capacity, salivary flow rate and dental caries of children with congenital heart disease (CHD). Material and Methods This cross sectional study was carried out with 42 CHD and 42 healthy children who applied to XXXXXXX. The participants’ gender, age, general health and medications, and caries scores (dfs/DMFS) were written down, then their unstimulated saliva samples were collected. These specimens were evaluated in terms of the salivary secretion rate, salivary buffering capacity, pH, protein levels, superoxide dismutase (SOD), ferric reducing antioxidant power (FRAP), the thiobarbituric acid reactive substances (TBARS), protein carbonyl, protein thiols, total sialic acid. Results Both groups showed caries at similar levels. The salivary pH and buffering capacity were significantly less in the children with CHD than in the controls. The levels of TBARS and protein carbonyl were significantly higher in the children with CHD than in the controls. There was not any significant difference relating to the mean salivary secretion rate, protein levels, SOD, FRAP, protein thiols and total sialic acid. Conclusion The elevated TBARS and protein carbonyl levels in the patients with CHD were observed as an indicator of the free radical damage leading to oxidative stress.Istanbul Universit
