    The globalized state: Measuring and monitoring governance

    In the last 20 years, there has been an explosion of 'governance indicators' purporting to measure and track the quality of governance (especially public administration) among states. These indicators are sponsored by international agencies such as the World Bank, NGOs such as Transparency International and Freedom House, and private sector risk assessors. We argue that this web of standards marks a distinctive feature of globalized, if loose, coordination among states and an increase in monitoring and auditing functions. The article reviews the major governance indicators, their characteristics and limitations. We conclude that these indicators are a little noticed, but supremely powerful mechanism of discordant control and discipline on state systems around the world

    Utilization of Lipids, Polymers by Modern Techniques for Innovative Pharmaceutical Formulations and Medical Devices

    With advancing science and technology, new futuristic technologies have been upcoming that are having a greater impact on the pharmaceutical and biomedical fields. Many lipids and polymer of natural and synthetic origin have spread horizons enabling to reach in fabricating delivery systems for unmet medical needs. In this modern biomedicine era, we must outline and scrutiny the roles of these biomaterials and the modern technologies that are shaping up the advancing medicines. The real challenges in modulating, fine-tuning the biomaterials for their bioactive properties are vital for assessing the in-vivo performance of the developed systems. Therefore, the present review discusses the detailed analysis of the current research reported in the technologies of utilizing the lipids, polymers, and their role in delivering the drugs, genes, and designing advanced implantable medical devices. The scrutiny also focuses on key optimization tools, analytical methods, formulation, and biomedical techniques helpful in developing the solutions that can be administered different routes in different therapeutic indications

    A study on the ayurveda intervention (Virechana - therapeutic purgation and Rasayana - rejuvenation) on molecular gene expression profiling in familial breast cancer patients

    Background: The study was conducted to assess the ayurvedic intervention Virechana - therapeutic purgation and Rasayana - rejuvenation on predisposed familial breast cancer expression profiling of BRCA1 and BRCA2 genes. Materials and Methods: The total RNA was extracted from blood of five subjects of familial breast cancer predisposition patients using PAX Gene Blood RNA Kit who had undergone the ayurvedic intervention Virechana - therapeutic purgation and Rasayana - rejuvenation therapy. BRCA1 and BRCA2 gene expression was assessed by reverse transcription polymerase chain reaction and quantitative polymerase chain reaction (qPCR). The qPCR-based BRCA1 gene expression results were analyzed for fold variation based on Ct values using the following formula: 2 (−ΔΔCt), where ΔCt is the Ct (GOI) − avg.(Ct (HKG)), GOI is the gene of interest, and HKG is the housekeeping gene. Results: In one subject (S1), the upregulation (61.82) showed further increase (843.36) by therapeutic purgation and then decreased below the baseline level (11.3) in the rejuvenation phase. In three subjects (S2, S3, and S5), the upregulated genes (1488.87, 15825.9, and 19.16) showed down trend continuously till rejuvenation phase (1.92, 4.17, and 3.97) but not to downregulation. Another subject (S4) showed reversal of genetic expression, i.e., downregulated gene (−1) showed upregulation (237,900.70) continuously throughout the therapy, which is in conformity of the proposed hypothesis, i.e., biopurification (therapeutic purgation) followed by rejuvenation leads to upregulation of gene. Conclusion: With the available limited and diversified data, it may be concluded that therapeutic purgation followed by rejuvenation (Narasimha Rasayana) therapy exerted effect on gene expression, but further study needs to be conducted with more number of samples

    Assessment of radiological changes involving the articular surface of the temporomandibular joint in patients with osteoarthritis and rheumatoid arthritis using computed tomography scan: A prospective clinico-radiological study

    Background and Objective: The purpose of the present study was to assess the radiological changes involving the articular surfaces of temporomandibular joints (TMJs) in patients with osteoarthritis (OA) and rheumatoid arthritis (RA) using computed tomography (CT) scans. Materials and Methods: A total of 20 patients with OA and 20 patients with RA were subjected to a detailed examination, routine radiography, and CT imaging of the TMJs. CT scanning was carried out for the direct axial view and reconstructed to coronal and sagittal planes with contiguous slice thickness of 2 mm using bone window. All the images were evaluated for the presence of osteophytes, flattening of the articular surfaces, sclerosis, and narrowing of the joint space and subjected to statistical analysis. Results: The female to male ratio of the study group for OA and RA was 2:1, respectively. Of all the CT findings, namely, osteophytes, flattening of the articular surfaces, sclerosis, and narrowing of the joint space, statistically significant values (P = 0.056) for osteophytes were obtained. Flattening and narrowing of joint space were seen in both types of arthritides, however, a relatively higher percentage of such patients was seen in RA group. Conclusion: OA and RA of the TMJ are the two most commonly seen conditions which can impair the functional capacity of the entire masticatory system. Their in-depth clinical and radiological evaluation is a must to assess the disease activity. Likewise, CT is a valuable tool in assessing osseous abnormalities and should be used in cases where osseous involvement of the TMJs is suspected
