    Sürnümerer premolar (parapremolar) dişler: olgu sunumu

    ÖZET Sürnümerer dişler normal diş sayısından fazla olan dişler olarak tanımlanmaktadır. Hem süt hem de daimi dentisyonda sürnümerer dişlere rastlanabilir. Etiyolojileri tam olarak bilinmemek-tedir. Genelde gömülüdürler ve radyografilerde tesadüfen görülürler. Tek taraflı, çift taraflı, tek veya çok sayıda bir çenede veya her iki çenede de bulunabilirler. Bu olgu sunumunda alt çenede dört adet sürnümerer premolar dişlere sahip bir vakada tanı ve tedavi yaklaşımları sunulmaktadır. Anahtar kelimeler: Sürnümerer diş, sürnümerer premolar, suplemental, panoramik radyografi, parapremolar   ABSTRACT Supernumerary teeth are described as the teeth formed in excess of the normal dental formula. They have been reported in both the primary and the permanent dentition. Their etiology is still not clearly understood. They usually do not erupt and are seen incidentally on radiographies. Supernumerary teeth may occur singly, multiply, unilaterally or bilaterally and in one or both jaws. In this case report, diagnosis and treatment approaches of a patient with four supernumerary premolar teeth in mandibula are presented.     The Relationship Between Malocclusion Perceptions of Patient and Clinician

    Aim:To evaluate the Aesthetic Component (AC), which is part of Index of Orthodontic Treatment Need (IOTN) according to the patient and the clinician's point of view and to examine whether there is a difference between them in terms of awareness according to gender and pubertal situation.Subjects and Method:The study material of 100 patients (50 males, 50 females) with mean age 13.9 consisted of pre-treatment orthodontic models and hand-wrist films. AC images used in determination of the patient and clinician perceptions of malocclusion.Results:These 100 patients gave the same score with 32 clinicians, percentage of compliance of all patients was 32.0 % and there was no statistically significant differences (p>0.05) between clinician and patient's perceptions of malocclusion. Perceptions of malocclusion was also examined according to genders, there was no statistically significant difference between female and male patients, and clinician (p>0.05). Perceptions of malocclusion was also examined according to puberta, there was only no statistically significant difference between patients in pubertal period and clinician (p>0.05), but there was statistically significant difference (p<0.05) between patients with pre-and postpubertal period, and clinician.Conclusion:Although there was not difference between perceptions of malocclusion of the genders, the patients in pubertal period were more objective, more aware of their malocclusions and dental aesthetics than the pre-and postpubertal patients So that, a greater number of this patients referred for orthodontic treatment

    Effects of Transillumination Method on Bond Strenght of an Orthodontic Adhesive

    The purpose of this study was to determine the effect of transilüminasyon on the bond strength of orthodontic brackets. One hundred human maxillary premolars were used in this study. Brackets with orthodontic composite adhesive were placed on the labial surface of the premolars and light cured from either the labial and/or the lingual (transilüminasyon). The control sample was cured from the labial for a total of 40 seconds of light exposure. Experimental samples were cured from the buccal for 20 seconds, the lingual (transilüminasyon) for 20 or 40 seconds and both buccal and lingual for 40 seconds. The shear-peel bond strengths were tested at 24 hours after light application. The results of this study demonstrated statistically significant difference between 40 seconds of labial curing, the both buccal and lingual curing and the only lingually cured groups. Actual bond strengths were lower for only lingually cured samples but higher for the both buccal and lingual cued sles. The samples tested received 20 seconds of labially cured were nearly the same as the control values. This study demonstrated that transilüminasyon of maxillary premolars is not alone an acceptable method of cunng orthodontic adhesive. However, the bond strengths can be higher when the transilüminasyon is used in addition to the buccal curing particularly if the exposure time is inceased to 40 seconds

    Use of Class Fiber to Reinforce Ortohodontic Plates Against Mechanical Forces

    This study investigated the different effects of quantitative percentages of glass fiber intensity on the transverse strength and maximum deformation of orthodontic acrylic resin. Forty specimens were formed in a specially designed mold to produce identical specimens in accordance with the manufacturer's recommendations. Four different percentages of glass fiber reinforcements in loose random form were studied. Each glass fiber treatment percentage (0%, 1%, 2%, 5%) was related to the total powder/liquid mass. The results indicated that 2% glass fiber treatment enhanced the transverse strength of the tested specimen. Maximum deformation at failure in the tested treatment groups was found to be different. The interaction of glass fiber concentration on fracture strength and deformation was significant. In conclusion the 2% glass fiber concentration was found to yield optimum fracture strength and deformation results. Increased amount of glass fiber incorporation resulted in decreased mechanical properties of the resin

    The Photographic comparison of enamel surfaces roughened by different methods

    Amaç: Bu çalışmanın amacı üç farklı mine pürüzlendirme metodunun mine yüzeyine etkisini incelemek ve mine yüzeyleri üzerine en az zararlı olan pürüzlendirme yöntemini saptamaktır. Yöntem: Yeni çekilmiş 30 adet küçük azı dişi rasgele üç gruba ayrıldı. Grup I (n10) %37’lik ortofosforik asit ile asitlendi. Grup II (n10) %18 HCL ve pomza karışımı ile pürüzlendirildi (mikrabrazyon). Grup III (n10) ise 50 ? çapında alüminyum oksit kullanılarak kumlandı. Dijital fotoğraf makinesi kullanılarak standart uzaklıktan vestibül yüzey fotoğrafları alındı ve bu fotoğraflar skorlandı. Tanımlayıcı istatistikler her bir grup için yapıldı. Fotoğraf görüntülerinin karşılaştırılmasında Wilcoxon Signed Ranks testi kullanıldı. P değeri .05 ten küçük olanlar istatistiksel olarak anlamlı olarak değerlendirildi. Bulgular: Grupların hepsinde mine yüzeyi pürüzlenmesi görüldü. En belirgin pürüzlenmenin Grup I’de, en az pürüzlenmenin ise Grup II’de meydana geldiği izlendi. (P0,03) Sonuçlar: Alternatif bir mine pürüzlendirme yöntemi olan %18 HCL ve pomza karışımı ile mikroabrazyon işlemi orijinal mine yüzeyine en yakın görüntüleri vermiştir. Mikroabrazyondan sonra mine yüzeyinde meydana gelen ince cilalı yüzey ortodontik tedavi süresince dental plağın kontrolünde kolaylık sağlamaktadır.Objective: Aim of this study was to investigate the effect of three different roughening methods on the enamel surface, and also to determine less harmful the roughening method on the enamel surfaces. Materials and Methods: Newly extracted thirty premolar teeth were divided into three groups randomly. Group I (n10) were etched with 37% phosphoric acid. Group II (n10) were roughened with 18% HCL and pumice mixture (microabrasion). Group III (n10) were sandblasted with 50 µ aluminum oxide particles. Standardized surface photographs were taken by using a digital photograph machine, and these photographs are scored. The descriptive statistics were calculated for each group. Wilcoxon Signed Ranks was executed to evaluate differences of scores. P values less than to .05 were considered statistically significant. Results: Enamel roughening was observed in all groups, with the most extensive roughening in Group I and the smallest in Group II. There was appeared that microabrasion method gave clinically the finest enamel surface. (P0.03) Conclusions: Microabrasion with 18% HCL and pumice mixture which alternative enamel roughening method has given very close appearances to original enamel surface. Fine polished enamel surfaces occur after the microabrasion convenience of dental plaque control during orthodontic treatment

    Evaluation of the orthodontic treatment need of middle anatolian region using by icon index

    Amaçlar: Bu çalışmanın amacı, İç Anadolu bölgesinde yaşayan bireylerin, ortodontik tedavi ihtiyaçlarını ICON indeksi kullanarak değerlendirmek ve ortodontik tedavi ihtiyaçlarının cinsiyete ve pubertal büyüme dönemlerine göre değişip değişmediğini incelemektir. Yöntem: Mevcut çalışmada yaşları 9-38 arasında değişen 154 hastanın (87 kadın, 67 erkek) ortodontik modelleri ve panoramik filmleri ICON indeksi kullanılarak incelenmiştir. Bulgular: 154 hastanın 90’ında (% 58,4) ortodontik tedavi ihtiyacının olduğu, 87 bayan hastanın 49’unda (% 56,3) ve 67 erkek hastanın ise 41’inde (% 61,2) ortodontik tedavi ihtiyacının olduğu tespit edilmiştir. Ayrıca, prepubertal dönemde olan 37 hastanın 16’sında (% 43,2), pubertal dönemde olan 95 hastanın 62’sinde (% 65,3) ve postpubertal dönemde olan 22 hastanın ise 12’sinde (% 54,5) ortodontik tedavi ihtiyacının olduğu tespit edilmiştir. Estetik kaygılarından dolayı, bayanların ve pubertal dönem içindeki hastaların ortodontik tedavi için daha fazla başvuruya sahip oldukları görülmüştür. Sonuç: Sonuç olarak, cinsiyetlerin ve pubertal büyüme dönem gruplarının tedavi ihtiyaçları arasında önemli farklılık bulunmamıştır.Aims: The aim of the present study is to evaluate the treatment needs of persons lived in Middle Anatolian area using by ICON index. Additionally, it is aimed to examine orthodontic treatment needs according to gender and pubertal period. Methods: In current study, orthodontic models and panoramic films of 154 patients (87 women, 67 men) with the age range of 9-38 years were investigated using by ICON index. Results: 90 (58,4%) of 154 patients had orthodontic treatment need, 49 patients (56,3%) of 87 female patients and 41 patients (61,2%) of 67 male patients had orthodontic treatment need. In addition, the numbers of patients had orthodontic treatment need were; 16 (43,2%) of 37 patients in prepubertal period, 62 (65,3%) of 95 patients in pubertal period and 12 (54,5%) of 22 patients in postpubertal period. Due to the aesthetic anxieties, female and patients in pubertal period had more appeal for orthodontic treatment than the other groups. Conclusion: As a result, there was no significant difference in the treatment needs between the groups of gender and pubertal period

    Investıgatıon Of The Influence Of Compensatıon Treatments On Soft Tıssues In Class Iı Dıvısıon 1 Mandıbular Retrognathıe Cases

    The aims of this study were, 1. To determine the changes in soft tissues with Holdaway soft tissue analysis of Class II Division 1 mandibular retrognathie patients treated with upper two first premolars extraction, 2. To investigate the sexual differences between pre- and post-treatment values of boy and girl subjects, 3. To compare these values with the Holdaway soft tissue Turkish norms. The study group comprised the lateral cephalo-grams of 14 boys (mean age: 16.07+1.08 years) and 14 girls (mean age: 17.05+1.05 years) totally 28 (mean age: 17.00±1.06 years), skeletal Class II, dental Class II division 1 patients. Ten linear and 2 angular measurements were analyzed according to the definition provided by Holdaway on pre and post treatment radiographs. For statistical evaluation paired samples t-test and student t-test were performed. Soft tissue subnasale to H line, H angle, upper lip sulcus depth, skeletal profile convexity and upper lip strain measurements decreased and nose prominence, upper lip thickness and soft tissue chin thickness measurements increased during treatment period. Statistically significant sex differences were only found in nose prominence and H angle measurements between pre- and post-treatment values. In the comparison of these values with Holdaway soft tissue Turkish norms, soft tissue facial angle, nose prominence, soft tissue subnasale to H line, basic upper lip thickness, upper lip thickness and H angle measurements become closer to the Turkish norms. All changes were desirable for improvement of patients' soft tissue profile with extraction of upper two first premolars. The results of this study indicated that, generalizations concerning the negative effects of extraction compensations treatments on the profile of Class II Division 1 mandibular ret-rognathie patients are not true. It is necessary to investigate this difference in further investigations, using the parameters for soft-tissue analysis, need to be repeated with a larger sample

    Comparison of Intermaxillary Tooth Size Discrepancies Among Different Malocclusion Groups

    The aim of this study was to investigate the Bolton ratio differences between normal occlusion cases and Class I, Class II division 1, Class II division 2 and Class III malocclusion cases and to determine the role of sex differences. The patients were divided into 5 different groups. Class I, Class II division 1, Class II division 2, Class III malocclusions and normal occlusion. Each group had 60 boys and 60 girls. Mesiodistal tooth size measurements of upper and lower teeth were performed on 600 dental casts using a digital caliper. To determine the discrepancy, anterior ratios and total ratios mentioned in Bolton Analysis were calculated. These ratios were compared between different malocclusion and gender groups. The statistical analyses were performed using SPSS for Windows (version 10.0) statistics software. The intergroup comparisons were made using unidirectional variance analysis (ANOVA) and Tukey HSD tests. Lettering method was used in the one by one comparison between different groups. In this study, it was found that there were no differences in tooth dimension ratios between different sex groups in malocclusion cases (Class I, Class II division 1, Class II division 2, Class III malocclusion), but in normal occlusion group, anterior ratio was found to be higher in girls than males. It was observed that there was a significant difference in anterior ratios between Class III and Class II division 1 malocclusions in girls. Statistically significant differences were also observed in total ratios between normal occlusion and Class II division 1 and 2 malocclusion. Since it was a fact that the total and anterior ratio values determined for Turkish population were higher than Bolton's values, it would be better to take these specific values for Turkish people into consideration for treatment planning
