    Regional and Seasonal Variability of Rainfall Characteristics in Nepal

    Regional and seasonal variability of rainfall characteristics was investigated in Nepal on the southern slopes of the Himalayas using long-term daily rainfall data. Cluster analysis was applied to define subregions in Nepal based on the seasonal progression of precipitation. The results identified four subregions. The characteristics of the seasonal progression of daily average precipitation indicate the significance of premonsoon rainfall in April and May in the eastern part of Nepal. Comparisons of seasonal changes in rainfall characteristics in each subregion showed that increases in rainfall amount, number of days on which rain falls, and rainfall intensity occur during the summer monsoon season with a peak in July, expect in the western mountainous region. In the western region and northern part of the eastern mid-hill region, rainfall amount and rainfall intensity increase in the winter season. Moreover, a significant change in rainfall intensity occurs in the eastern Terai region between the summer and winter season

    Incidental discovery of circle contact lens by MRI: you can’t scan my poker face, circle contact lens as a potential MRI hazard

    BACKGROUND: Circle contact lenses, also known as color contact lenses and big eye contact lenses, are a type of cosmetic contact lens. It is not generally known that a circle contact lens usually contains iron oxide and other metals, which means their use during magnetic resonance imaging (MRI) is a potential hazard. CASE PRESENTATION: We present a rare case of incidental discovery of circle contact lenses by MRI and MRI images of circle lenses in vitro. CONCLUSIONS: Circle contact lenses usually contain iron oxide, which is a known source of susceptibility artifact on MRI. Not only radiologists and radiographers but also referring physicians should be familiar with the imaging findings and potential risk of scanning circle contact lenses by MRI

    Internet-Based Inquiries From Users With the Intention to Overdose With Over-the-Counter Drugs: Qualitative Analysis of Yahoo! Chiebukuro

OTC drug abuse has serious effects on the mind and body, such as poisoning symptoms, and often requires specialized treatments. In contrast, there is concern about people who potentially abuse OTC drugs whose symptoms are not serious enough to consult medical institutions or drug addiction rehabilitation centers yet are at high risk of becoming drug dependent in the future. [Objective:] Consumer-generated media (CGM), which allows users to disseminate information, is being used by people who abuse (and those who are trying to abuse) OTC drugs to obtain information about OTC drug abuse. This study aims to analyze the content of CGM to explore the questions of people who potentially abuse OTC drugs. [Methods:] The subject of this research was Yahoo! Chiebukuro, the largest question and answer website in Japan. A search was performed using the names of drugs commonly used in OTC drug abuse and the keywords overdose and OD, and the number of questions posted on the content of OTC drug abuse was counted. Furthermore, a thematic analysis was conducted by extracting text data on the most abused antitussive and expectorant drug, BRON. [Results:] The number of questions about the content of overdose medications containing the keyword BRON has increased sharply as compared with other product names. Furthermore, 467 items of question data that met the eligibility criteria were obtained from 528 items of text data on BRON; 26 codes, 6 categories, and 3 themes were generated from the 578 questions contained in these items. Questions were asked about the effects they would gain from abusing OTC drugs and the information they needed to obtain the effects they sought, as well as about the effects of abuse on their bodies. Moreover, there were questions on how to stop abusing and what is needed when seeking help from a health care provider if they become dependent. It has become clear that people who abuse OTC drugs have difficulty in consulting face-to-face with others, and CGM is used as a means to obtain the necessary information anonymously. [Conclusions:] On CGM, people who abused or tried to abuse OTC drugs were asking questions about their abuse expectations and anxieties. In addition, when they became dependent, they sought advice to quit their abuse. CGM was used to exchange information about OTC drug abuse, and many questions on anxieties and hesitations were posted. This study suggests that it is necessary to produce and disseminate information on OTC drug abuse, considering the situation of those who abuse or are willing to abuse OTC drugs. Support from pharmacies and drugstores would also be essential to reduce opportunities for OTC drug abuse

    Resminostat in EGFR-mutated lung cancer

    Drug-tolerant cells are mediators of acquired resistance. BIM-intron2 deletion polymorphism (BIM-del) is one of the mechanisms underlying the resistance to epidermal growth factor tyrosine kinase inhibitor (EGFR-TKI)-mediated apoptosis that induces drug tolerance. Here, we investigated whether resminostat, a histone deacetylase inhibitor, circumvents BIM-del-associated apoptosis resistance. The human EGFR-mutated non-small cell lung cancer (NSCLC) cell line PC-9 and its homozygous BIM-del-positive variant (PC-9 BIMi2-/-), established by editing with zinc finger nuclease, were used. In comparison with PC-9 cells, PC-9 BIMi2-/- cells were less sensitive to apoptosis mediated by EGFR-TKIs such as gefitinib and osimertinib. The combined use of resminostat and an EGFR-TKI preferentially induced the expression of the pro-apoptotic BIM transcript containing exon 4 rather than that containing exon 3, increased the level of pro-apoptotic BIM protein (BIMEL), and stimulated apoptosis in vitro. In a subcutaneous tumor model derived from PC-9 BIMi2-/- cells, gefitinib monotherapy decreased tumor size but retained residual lesions, indicative of the presence of tolerant cells in tumors. The combined use of resminostat and gefitinib increased BIMEL protein level and induced apoptosis, subsequently leading to the remarkable shrinkage of tumor. These findings suggest the potential of resminostat to circumvent tolerance to EGFR-TKIs associated with BIM deletion polymorphism

    Clinical Application of MPRAGE Wave Controlled Aliasing in Parallel Imaging (Wave-CAIPI): A Comparative Study with MPRAGE GRAPPA

    PURPOSE: To compare reliability and elucidate clinical application of magnetization-prepared rapid gradient-echo (MPRAGE) with 9-fold acceleration by using wave-controlled aliasing in parallel imaging (Wave-CAIPI 3 × 3) in comparison to conventional MPRAGE accelerated by using generalized autocalibrating partially parallel acquisition (GRAPPA) 2 × 1. METHODS: A total of 26 healthy volunteers and 33 patients were included in this study. Subjects were scanned with two MPRAGEs, GRAPPA 2 × 1 and Wave-CAIPI 3 × 3 acquired in 5 min 21 s and 1 min 42 s, respectively, on a 3T MR scanner. Healthy volunteers underwent additional two MPRAGEs (CAIPI 3 × 3 and GRAPPA 3 × 3). The image quality of the four MPRAGEs was visually evaluated with a 5-point scale in healthy volunteers, and the SNR of four MPRAGEs was also calculated by measuring the phantom 10 times with each MPRAGE. Based on the results of the visual evaluation, voxel-based morphometry (VBM) analyses, including subfield analysis, were performed only for GRAPPA 2 × 1 and Wave-CAIPI 3 × 3. Correlation of segmentation results between GRAPPA 2 × 1 and Wave-CAIPI 3 × 3 was assessed. RESULTS: In visual evaluations, scores for MPRAGE GRAPPA 2 × 1 (mean rank: 4.00) were significantly better than those for Wave-CAIPI 3 × 3 (mean rank: 3.00), CAIPI 3 × 3 (mean rank: 1.83), and GRAPPA 3 × 3 (mean rank: 1.17), and scores for Wave-CAIPI 3×3 were significantly better than those for CAIPI 3 × 3 and GRAPPA 3 × 3. Image noise was evident at the center for additional MPRAGE CAIPI 3 × 3 and GRAPPA 3 × 3. The correlation of segmentation results between GRAPPA 2 × 1 and Wave-CAIPI 3 × 3 was higher than 0.85 in all VOIs except globus pallidus. Subfield analysis of hippocampus also showed a high correlation between GRAPPA 2 × 1 and Wave-CAIPI 3 × 3. CONCLUSION: MPRAGE Wave-CAIPI 3 × 3 shows relatively better contrast, despite of its short scan time of 1 min 42 s. The volumes derived from automated segmentation of MPRAGE Wave-CAIPI are considered to be reliable measures

    The CgHaa1-regulon mediates response and tolerance to acetic acid stress in the human pathogen Candida glabrata

    To thrive in the acidic vaginal tract C. glabrata has to cope with high concentrations of acetic acid. The mechanisms underlying C. glabrata tolerance to acetic acid at low pH remain largely uncharacterized. In this work it is demonstrated the essential role of the CgHaa1 transcription factor (encoded by ORF CAGL0L09339g) in the response and tolerance of C. glabrata to acetic acid. Transcriptomic analysis showed that CgHaa1 regulates, directly or indirectly, the expression of about 75% of the genes activated under acetic acid stress. CgHaa1-activated targets are involved in multiple physiological functions including membrane transport, metabolism of carbohydrates and amino acids, regulation of the activity of the plasma membrane H+-ATPase and adhesion. Under acetic acid stress CgHaa1 increased the activity and the expression of the CgPma1 proton pump and contributed to increased colonization of vaginal epithelial cells by C. glabrata. CgHAA1, CgTPO3 and CgHSP30, two identified CgHaa1-activated targets, are herein demonstrated to be determinants of C. glabrata tolerance to acetic acid. The protective effect of CgTpo3 and of CgHaa1 was linked to a role of these proteins in reducing the accumulation of acetic acid inside C. glabrata cells. In response to acetic acid stress, marked differences were found in the regulons controlled by CgHaa1 and by its S. cerevisiae ScHaa1 ortholog, demonstrating a clear divergent evolution of the two regulatory networks.     Blood Flow Restriction Increases the Neural Activation of the Knee Extensors During Very Low-Intensity Leg Extension Exercise in Cardiovascular Patients:A Pilot Study

    Blood flow restriction (BFR) has the potential to augment muscle activation, which underlies strengthening and hypertrophic effects of exercise on skeletal muscle. We quantified the effects of BFR on muscle activation in the rectus femoris (RF), the vastus lateralis (VL), and the vastus medialis (VM) in concentric and eccentric contraction phases of low-intensity (10% and 20% of one repetition maximum) leg extension in seven cardiovascular patients who performed leg extension in four conditions: at 10% and 20% intensities with and without BFR. Each condition consisted of three sets of 30 trials with 30 s of rest between sets and 5 min of rest between conditions. Electromyographic activity (EMG) from RF, VL, and VM for 30 repetitions was divided into blocks of 10 trials and averaged for each block in each muscle. At 10% intensity, BFR increased EMG of all muscles across the three blocks in both concentric and eccentric contraction phases. At 20% intensity, EMG activity in response to BFR tended to not to increase further than what it was at 10% intensity. We concluded that very low 10% intensity exercise with BFR may maximize the benefits of BFR on muscle activation and minimize exercise burden on cardiovascular patients
