Modifiable Psychological Factors Affecting Functioning in Fibromyalgia.
OBJECTIVE: To examine the role of several interrelated, potentially modifiable psychological factors (i.e., mindfulness and catastrophizing) in influencing patient-reported functioning. METHODS: In this cross-sectional study, 107 patients with fibromyalgia completed self-report assessments of pain severity, functioning and impact of symptoms, mindfulness, and pain catastrophizing. Linear regression and bootstrapping mediation analyses were performed to assess the relationships between these factors. RESULTS: Pain intensity was significantly and positively associated with pain catastrophizing and impact of fibromyalgia on functioning. Linear regression analyses indicated that pain intensity, catastrophizing, and mindfulness affect functioning in fibromyalgia. Follow-up mediation analysis revealed a significant indirect effect of pain catastrophizing on the relationship between pain intensity and fibromyalgia functioning. CONCLUSION: Individuals with fibromyalgia who have higher levels of pain and catastrophizing, and lower levels of mindfulness, are more likely to experience impaired functioning. Our findings suggest that pain catastrophizing appears to be an especially important variable contributing to reduced functioning in women with fibromyalgia. Therefore, catastrophizing-reducing treatments (e.g., cognitive behavioral therapy) are likely to have direct, beneficial impacts on functioning

Diffusion tensor and volumetric magnetic resonance imaging using an MR-compatible hand-induced robotic device suggests training-induced neuroplasticity in patients with chronic stroke
Stroke is the third leading cause of mortality and a frequent cause of long-term adult impairment. Improved strategies to enhance motor function in individuals with chronic disability from stroke are thus required. Post-stroke therapy may improve rehabilitation and reduce long-term disability; however, objective methods for evaluating the specific impact of rehabilitation are rare. Brain imaging studies on patients with chronic stroke have shown evidence for reorganization of areas showing functional plasticity after a stroke. In this study, we hypothesized that brain mapping using a novel magnetic resonance (MR)-compatible hand device in conjunction with state-of-the-art magnetic resonance imaging (MRI) can serve as a novel biomarker for brain plasticity induced by rehabilitative motor training in patients with chronic stroke. This hypothesis is based on the premises that robotic devices, by stimulating brain plasticity, can assist in restoring movement compromised by stroke-induced pathological changes in the brain and that these changes can then be monitored by advanced MRI. We serially examined 15 healthy controls and 4 patients with chronic stroke. We employed a combination of diffusion tensor imaging (DTI) and volumetric MRI using a 3-tesla (3T) MRI system using a 12-channel Siemens Tim coil and a novel MR-compatible hand-induced robotic device. DTI data revealed that the number of fibers and the average tract length significantly increased after 8 weeks of hand training by 110% and 64%, respectively (p<0.001). New corticospinal tract (CST) fibers projecting progressively closer to the motor cortex appeared during training. Volumetric data analysis showed a statistically significant increase in the cortical thickness of the ventral postcentral gyrus areas of patients after training relative to pre-training cortical thickness (p<0.001). We suggest that rehabilitation is possible for a longer period of time after stroke than previously thought, showing that structural plasticity is possible even after 6 months due to retained neuroplasticity. Our study is an example of personalized medicine using advanced neuroimaging methods in conjunction with robotics in the molecular medicine era
Η ενσυνειδητότητα και οι εφαρμογές της στην ευεξία και την εκπαίδευση
The present dissertation aims at examining the literature concerning mindfulness as it relates to educational psychology and overviews three research studies conducted to add empirical research on mindfulness, well-being and learning. The contribution of the Thesis is that different theories and methods of mindfulness application are demonstrated and discussed, offering an important tool for modern educational syllabi. In part A, the present Thesis examines the theoretical background regarding mindfulness as it relates to education and well-being and learning. In part A, The theoretical background is divided into three main chapters: Chapter 1) discusses Well-being, learning and education, Chapter 2) explores mindfulness, definitions, implications and Chapter 3) examines mindfulness as a disposition in educational settings. The main aim of the theoretical background is to critically evaluate the concept and attributes of mindfulness, well-being and promote mindfulness as a dispositional way of thinking and antidote to stress incorporated in educational settings. Contrasting definitions, views and evidence of mindfulness are provided as well as potential links to philosophy, well-being, stress, emotional burdens, cognition, neuroscience and education. Results of the analyses for this study suggest that mindfulness is dispositional (trait-like) and could affect burnout and stress levels in students. Limitations in the current study are discussed. The findings implicate (a) the role and influence of mindfulness as a behavior trait and (b) the effect of previous knowledge on a student’s propensity for mindfulness. The positive implications of mindfulness suggest a need for further research (a) on long-term applied mindfulness program in actual classrooms and (b) on a combined Kabat-Zinn (1989) and Langer (1989) form of mindfulness instruction

στα εκπαιδευτικά ιδρύματα και να επιδείξει ολιστικούς τρόπους προσέγγισης και κατανό της μάθησης. Οι στόχοι της διατριβής είναι να δείξει πως η «ενσυνειδητότητα» μπορεί να ένα ζωτικό εργαλείο για να βελτιωθεί η μάθηση και η ευεξία καθώς και να μειωθεί το στρε εκπαιδευτικά ιδρύματα. Η διατριβή χωρίζεται σε δύο μέρη: το θεωρητικό (μέρος Α) κ πρακτικό/ερευνητικό μέρος (μέρος Β). Στο πρώτο μέρος, παρουσιάζεται το θεωρητικό πλ γύρω από την ευεξία, την εκπαίδευση και το ρόλο της «ενσυνειδητότητας» ως αντίδοτου στρες καθώς και η κριτική της εκτίμηση. Στο δεύτερο μέρος, παρουσιάζονται και συζητο τρεις έρευνες που βασίζονται πάνω στο θεωρητικό πλαίσιο. Ο βασικός σκοπός του θεωρη• μέρους είναι να εκτιμήσει κριτικά την ιδέα και τα χαρακτηριστικά της «ενσυνειδητότητα της ευεξίας και να παρουσιάσει την «ενσυνειδητότητα» ως χαρακτηριστικό και ένα αντ στο άγχος εφαρμοσμένο στα εκπαιδευτικά ιδρύματα. Στο σημείο αυτό παρουσιάζονται τα μοντέλα της ενσυνειδητότητας το διαλογιστικό (Kabat-Zinn) και το κοινωνικογνωστικό (Langer). Αρχικά, παρουσιάζονται πιθανές συνδέσεις της ενσυνειδητότας με τη νευροεπισ και το στρες. Έπειτα συζητείται η σύνδεση της έννοιας της ενσυνειδητότητας από φιλοσοφική σκοπιά και κυρίως τη φαινομενολογία και τη Χουσερλιανή σχολή. Στο τελε κεφάλαιο παρουσιάζονται πιθανές εφαρμογές της ενσυνειδητότητας στην εκπαίδευση καθώ συσχετισή της με την ευεξία και άγχος σε μαθητές και φοιτητές. Τα αποτελέσματα αυτή διατριβής δείχνουν ότι η «ενσυνειδητότητα» ως χαρακτηριστικό μπορεί να επηρεάσε επαγγελματική εξουθένωση και τα επίπεδα του άγχους στους φοιτητές. Τα αποτελέσ δείχνουν ότι α) ο ρόλος και η επίδραση της «ενσυνειδητότητας» ως χαρακτηριστικό συμπεριφοράς και β) η επίδραση της «ενσυνειδητότητας» στη μάθηση. Οι θετικές επιδρ της «ενσυνειδητότητας επιδεικνύουν μια ανάγκη για περαιτέρω έρευνα α) μέσω μακροχρόνιου προγράμματος «ενσυνειδητότητας» στα σχολεία και β) μέσω ενός συνδυα του διαλογιστικού μοντέλου (Kabat-Zinn) και κοινωνικού-γνωστικού μοντέλου ενσυνειδητότητας (Ellen Langer). Η παρούσα διατριβή βασίζεται κυρίως θεωρία της ενσυνειδητότηας της Ellen Langer που επιδεικνύει το πώς η επίγνωση ενσυνειδητότηταας μπορεί να διακόψει τους κύκλους της συνήθειας και να αφαιρέσε διάσταση της νοοτροπίας και να ανοίξει νέες επιλογές και πιθανότητες για δημιουρ επιλογές και θετικής εξέλιξης. Στην παρούσα διατριβή διερευνώ την καταλληλόλητο «ενσυνειδητότητας» ως εκπαιδευτικός σκοπός και παρουσιάζω τη σημασία της δημιουργία «ενσυνειδητότητας» ως χαρακτηριστικό της προσωπικότητας παρά ως μία προσω κατάσταση. Επιπλέον, η διατριβή προσπαθεί να παρουσιάσει την ενσυνειδητότητα ως ένι εργαλείο που επιφέρει ευεξία και μειώνει το άγχος στα εκπαιδευτικά ιδρύματα. Σύμφωνα μ Ellen Langer (1989), για να αναπτύξουμε την «ενσυνειδητότητα» στην εκπαίδευση, θα π να αναγνωρίσουμε τις παρακάτω ποιότητες: να παρατηρούμε τις λεπτομέρειες, την έρευνα πιθανοτήτων και τις πολλαπλές επιλογές καθώς και τη κριτική των τωρινών δεδομένα διατριβή ανακεφαλαιώνει, ερευνώντας πως μπορεί να αναπτυχθεί το χαρακτηριστικό «ενσυνειδητότητας» μέσα σε σχολικές τάξεις. Αυτή η διατριβή περιέχει ένα θεωρητικό πλ συμπεριλαμβανομένου διάφορες έρευνες πάνω στις εφαρμογές της «ενσυνειδητότητας» εκπαίδευση όπου ερευνάται η «ενσυνειδητότητα» ως μία κατάσταση και ως χαρακτηριστικό.
Η διατριβή χωρίζεται σε δύο μέρη: το μέρος Α παρουσιάζει το θεωρητικό υπόβαθρο
«ενσυνειδητότητας» καθώς και ερευνά και συνδέει την ευεξία και το στρες με την φιλοσ της εκπαίδευσης. Πιο αναλυτικά, στο κεφάλαιο 1, παρουσιάζεται η ευεξία ως προϋπόθεσ αποτελεσματική μάθηση. Αυτή η αρχική συζήτηση συνθέτει ένα χρήσιμο υπόβαθρο στηρίζονται περαιτέρω συζητήσεις. Στο κεφάλαιο 2, ο ρόλος του άγχους αναπτύσσετα αναλγητικός παράγοντας στην ευεξία και μάθηση. Η ανάλυση δείχνει ότι το άγχος παίζει καταστρεπτικό παράγοντα στη διαδικασία της μάθησης και στην ευεξία των μαθη Επιπλέον, αυτό το κεφάλαιο δείχνει την αναγκαιότητα των εκπαιδευτών να κατανοήσουν καταστρεπτική επίδραση του άγχους στην υγεία των μαθητών, στην συμπεριφορά και μά καθώς και την σημασία αυτών των πλεονεκτημάτων στην δημιουργία μίας ήρεμης κατάστα Στο κεφάλαιο 3, η θεωρία και τεχνικές της «ενσυνειδητότητας» αναπτύσσονται ως θεραπεία άγχος. Επιπλέον, σε αυτό το μέρος, παρουσιάζονται πολλαπλές προσεγγίσεις και ορισμο ενσυνειδητότητας. Επίσης, παρουσιάζονται και συζητούνται τα δύο βασικά ρεύματα «ενσυνειδητότητας»: το κοινωνικο-γνωστικό και διαλογιστικό. Τέλος, στο κεφάλαι παρουσιάζεται μια συστηματική επισκόπηση σχετικά με την «ενσυνειδητότητα» εκπαιδευτικά ιδρύματα. Επιπλέον, παρουσιάζεται και συζητείται η πτυχή «ενσυνειδητότητας» ως χαρακτηριστικό όπως εφαρμόζεται στα εκπαιδευτικά πλαίσια. Σ μέρος, παρουσιάζονται τρεις έρευνες πάνω στην επίδραση της θεωρίας της «ενσυνειδητότη και εφαρμογή της στην εκπαίδευση. Η πρώτη έρευνα ερευνά την μάθηση και απόδοση σ φοιτητές Ιατρικής μετά από μία παρέμβαση με «ενσυνειδητότητα». Ερευνά κατά πόσο πρόγραμμα πέντε εβδομάδων βελτιώνει την απόδοση στις κλινικές δεξιότητες και συγκεκριστην υγιεινή. Η δεύτερη έρευνα δείχνει κατά πόσο στις τάξεις που διδάσκουν ενσυνεί
καθηγητές, οι μαθητές αποδίδουν καλύτερα. Η τρίτη έρευνα διερευνά δύο διαφορετικά είδ
«ενσυνειδητότητας» στους φοιτητές. Η πρώτη πειραματική ομάδα εξασκήθηκε σε ένα κλ( πρόγραμμα διαλογισμού ενώ η άλλη πειραματιή ομάδα εξασκήθηκε ακριβώς στην ίδια άσ με επιπλέον οδηγίες στην παρατήρηση της διαφορετικότητας (attention to variability αναζήτησης του νέου (novelty seeking). Η «ενσυνειδητότητα» φαίνεται να επιδρά στο ρ της καρδιάς σε αγχωμένους φοιτητές και να μειώνει το άγχος. Αυτή επίδραση στη φυσιολ του σώματατος επιδικνύει την επίδρασης της «ενσυνειδητότητας» στο άγχος και ανη< στους φοιτητές. Πετυχημένοι εκπαιδευτικοί είναι αυτοί που μπορούν να δημιουργήσου ομάδα μάθησης, στην οποία οι μαθητές βελτιώνονται ακαδημαϊκά, συναισθηματικά κοινωνικά και παράλληλα οι εκπαιδευτικοί αναπτύσσονται επαγγελματικά και προσωπικ μάθηση στο σχολείο είναι περίπλοκη κυρίως γιατί οι μαθητές και οι δάσκαλοι έ προσωπικές γνώσεις και ικανότητες που μεταφέρονται σε ένα διαδραστικό περιβάλλον «ενσυνειδητότητα» είναι ένας συνειδητός και σκόπιμος τρόπος, ώστε να επιτευχθεί πνευμο σωματική και ψυχολογική ευεξία στα εκπαιδευτικά πλαίσια.
Σύμφωνα με τις σύγχρονες συνθήκες μάθησης και έκδηλης επιθετικότητας στα σχολε ενσυνειδητότητα μπορεί να είναι η λύση για τη δημιουργία ενός ισσοροποημένου εκπαιδευ πλαισίου γεματό ηρεμία, διαύγεια και συναισθηματική ρύθμιση. Αυτά αποτελούν τα βα συστατικά για μια εποικοδομητική διδασκαλία και μάθηση. Τα αποτελέσματα των ερε αυτής της διατριβής αποτελούν τον ακρογωνιαίο λίθο για σύχρονες παρεμβάσεις που μπο να εφαρμοστούν σε οποιοδήποτε εκπαιδευτικό πλαίσιο χωρίς κόστος και μεγάλες αλλαγές σχολικού ή ακαδημαικού προγράμματος. Yoga and Mindfulness as Therapeutic Interventions for Stroke Rehabilitation: A Systematic Review
Aim. This paper reports a systematic review and critical appraisal of the evidence on the effectiveness of behavioral therapies such as yoga and mindfulness practices for stroke rehabilitation. Background. The experience of stroke can have a negative impact on both psychological and physical health and on quality of life. Yoga and relevant practices are promising therapies that have been used with patients with a variety of conditions. In order to draw conclusions on effectiveness for stroke patients, the evidence requires systematic assessment. Methods. A comprehensive search of major biomedical and complementary medicine databases was conducted. Relevant research was categorized by study type and appraised according to study design. Results. Five randomized controlled clinical trials and four single case studies were found. Additionally, one qualitative research study was identified. Studies reported positive results, including improvements in cognition, mood, and balance and reductions in stress. Modifications to different yoga practices make comparison between studies difficult, and a lack of controlled studies precludes any firm conclusions on efficacy. Conclusion. Yoga and mindfulness could be clinically valuable self-administered intervention options for stroke rehabilitation. Further research is needed to evaluate these specific practices and their suitability in stroke rehabilitation
An exploratory study of Mindfulness Based Stress Reduction for emotional eating
Emotional eating is an important predictor of weight loss and weight regain after weight loss. This two part study\u27s primary aim was to explore changes in emotional eating in a general population of individuals taking the Mindfulness Based Stress Reduction (MBSR) program, with a secondary aim to explore whether changes in mindfulness predicted changes in emotional eating. Self-reported survey data exploring these questions were collected before and after the intervention for two sequential studies (Study 1 and Study 2). While there were no control groups for either study, in both studies emotional eating scores following the MBSR were significantly lower than scores prior to taking the MBSR (p \u3c 0.001; p \u3c 0.001) In Study 2, changes in mindfulness were correlated with changes in emotional eating (r = 0.317, p = 0.004). These results suggest that MBSR may be an effective intervention for emotional eating, and that further research is warranted to examine effects on weight loss and maintenance

Interactive effects of pain catastrophizing and mindfulness on pain intensity in women with fibromyalgia
The objective of this study was to examine the association between facets of trait mindfulness, pain catastrophizing, and pain severity in a sample of patients with fibromyalgia. Patients with fibromyalgia completed validated baseline and diary assessments of clinical pain, mindfulness, and pain catastrophizing. Multilevel modeling analyses indicated that the daily association between catastrophizing and pain intensity was moderated by certain mindfulness facets. Our findings suggest that various aspects of mindfulness may interact differently with pain and catastrophizing, which may have implications for the design and testing of interventions targeting mindfulness and catastrophizing in fibromyalgia patients

Variability in conditioned pain modulation predicts response to NSAID treatment in patients with knee osteoarthritis
Background: Patients with painful knee osteoarthritis (OA) demonstrate hyperalgesia and altered pain-modulatory responses. While some prior work has demonstrated cross-sectional associations between laboratory and clinical pain measures, it is unknown whether individual variability in quantitative sensory testing (QST) responses at baseline can prospectively predict analgesic treatment responses. Method Patients with knee OA (n = 35) were compared on QST responses to a demographically-matched pain-free control group (n = 39), after which patients completed a month-long treatment study of diclofenac sodium topical gel (1 %), applied up to 4 times daily. Results: OA patients demonstrated reduced pain thresholds at multiple anatomic sites, as well as reduced conditioned pain modulation (CPM) and enhanced temporal summation of pain. The most pain-sensitive patients tended to report the most intense and neuropathic OA pain. Following diclofenac treatment, the knee OA cohort showed a roughly 30 % improvement in pain, regardless of the presence or absence of neuropathic symptoms. Baseline CPM scores, an index of endogenous pain-inhibitory capacity, were prospectively associated with treatment-related changes in clinical pain. Specifically, participants with higher CPM at baseline (i.e., better functioning endogenous pain-inhibitory systems) showed more reduction in pain at the end of treatment (p < .05). Conclusions: These results support prior findings of amplified pain sensitivity and reduced pain-inhibition in OA patients. Moreover, the moderate to strong associations between laboratory-based measures of pain sensitivity and indices of clinical pain highlight the clinical relevance of QST in this sample. Finally, the prospective association between CPM and diclofenac response suggests that QST-based phenotyping may have utility in explaining inter-patient variability in long-term analgesic treatment outcomes. The impact of anxiety and catastrophizing on interleukin-6 responses to acute painful stress
Objective: To examine the influence of anxiety and pain-related catastrophizing on the time course of acute interleukin-6 (IL-6) responses to standardized noxious stimulation among patients with chronic pain. Methods: Data were collected from 48 participants in the following demographically matched groups: patients with chronic pain (n=36) and healthy controls (n=12). Participants underwent a series of Quantitative Sensory Testing (QST) procedures assessing responses to mechanical and thermal stimuli during two separate visits, in a randomized order. One visit consisted of standard, moderately painful QST procedures, while the other visit involved nonpainful analogs to these testing procedures. Blood samples were taken at baseline, and then for up to 2 hours after QST in order to study the time course of IL-6 responses. Results: Results of multilevel analyses revealed that IL-6 responses increased across assessment time points in both visits (p<0.001). While patients with chronic pain and healthy controls did not differ in the magnitude of IL-6 responses, psychological factors influenced IL-6 trajectories only in the chronic pain group. Among patients, increases in catastrophizing over the course of the QST session were associated with elevated IL-6 responses only during the painful QST session (p<0.05). When controlling for anxiety, results indicated that the main multilevel model among patients remained significant (p<0.05). Conclusion: Under specific conditions (eg, application of a painful stressor), catastrophizing may be associated with amplified proinflammatory responses in patients with persistent pain. These findings suggest that psychosocial interventions that reduce negative pain-related cognitions may benefit patients’ inflammatory profiles
