    Psychosocial factors at work and inflammatory markers: protocol for a systematic review and meta-analysis

    Introduction Chronic inflammation may be a mediator for the development of cardiovascular disease (CVD), metabolic diseases and psychotic and neurodegenerative disorders. Meta-analytic associations between work-related psychosocial factors and inflammatory markers have shown that work-related psychosocial factors could affect the flexibility and balance of the immune system. However, few systematic reviews or meta-analyses have investigated the association between work-related psychosocial factors and inflammatory markers. Based on prospective studies, the present investigation will conduct a comprehensive systematic review and meta-analysis of the association between work-related psychosocial factors and inflammatory markers.Methods and analysis The systematic review and meta-analysis will include published studies identified from electronic databases (PubMed, EMBASE, PsycINFO, PsycARTICLES, Web of Science and Japan Medical Abstracts Society) according to recommendations of the Meta-analysis of Observational Studies in Epidemiology guideline. Inclusion criteria are studies that: examined associations between work-related psychosocial factors and increased inflammatory markers; used longitudinal or prospective cohort designs; were conducted among workers; provided sufficient data for calculating ORs or relative risk with 95% CIs; were published as original articles in English or Japanese; and were published up to the end of 2017. Study selection, data extraction, quality assessment and statistical syntheses will be conducted by 14 investigators. Any inconsistencies or disagreements will be resolved through discussion. The quality of studies will be evaluated using the Risk of Bias Assessment Tool for Non-randomized Studies.Ethics and dissemination The investigation study will be based on published studies, so ethics approval is not required. The results of this study will be submitted for publication in a scientific peer-reviewed journal. The findings may be useful for assessing risk factors for increased inflammatory markers in the workplace and determining future approaches for preventing CVD, metabolic diseases and psychotic and neurodegenerative disorders

    Flow cytometric analysis of skin blister fluid induced by mosquito bites in a patient with chronic active Epstein-Barr virus infection

    金沢大学医薬保健研究域医学系In chronic active Epstein-Barr virus (EBV) infection (CAEBV), ectopic EBV infection has been described in T or natural killer (NK) cells. NK cell-type infection (NK-CAEBV) is characterized by large granular lymphocytosis, high IgE levels and unusual reactions to mosquito bites, including severe local skin reactions, fever and liver dysfunction. However, the mechanisms underlying these reactions remain undetermined. Herein, we describe a patient with NK-CAEBV whose blister fluid after mosquito bites was analyzed. The patient exhibited significant increases in the percentage of CD56+ NK cells in the fluid compared with a simple mosquito allergy, in which the majority of infiltrated cells were CD203c+ cells, indicating basophils and/or mast cells. His fluid also contained CD203c+ cells, and his circulating basophils were activated by mosquito extracts in vitro. These results suggest that CD203c+ cells as well as NK cells may play pathogenic roles in the severe skin reactions to mosquito bites in NK-CAEBV. © 2009 The Japanese Society of Hematology.出版者に照会中.2010年12月より全文公開予定

    本稿は、QUIK(self completed questionnaire for QOL)及び改訂版QUIK-R(self completed questionnaire for QOL revised)の文献的展望である。本論文は、QOLの概念、QOLの測定、QOLと気分障害との関連、さまざまな障害を有する患者のQOL、健常者のQOLと介護者のQOLの検討した内容を含んでいる。われわれが、ここで強調したいことは、QOL-Oriented-Medicineは、身体的-心理的-社会的-実存的な存在である個人のオートノミーと幸福を反映させることのできる医療(Medical Service)なのである。The article is a review of the literature on a self completed questionnaire for QOL (QUIK) and its revised version (QUIK-R) assessing QOL. It includes the contents of evaluating the concept of QOL, the measurement of QOL, correlation between QOL and mood disorders, QOL in patients with various disorders, QOL in healthy subjects and QOL of caregivers.     Association of Job Stressors With Panic Attack and Panic Disorder in a Working Population in Japan:A Cross-Sectional Study

    OBJECTIVE: This study aimed to investigate associations of job stressors with panic attack (PA) and panic disorder (PD) among Japanese workers. METHODS: A cross-sectional online questionnaire survey was conducted of 2060 workers. Job strain, effort/reward imbalance, and workplace social support were measured by the job content questionnaire and effort/reward imbalance questionnaire. These variables were classified into tertiles. PA/PD were measured by self-report based on the mini international neuropsychiatric interview (MINI). Multiple logistic regression was conducted, adjusting for demographic, lifestyle, and health-related covariates. RESULTS: Data from 1965 participants were analyzed. Adjusted odds ratio (OR) of PA/PD was significantly greater for the group with high effort/reward imbalance compared with the group with low effort/reward imbalance (ORs, 2.64 and 2.94, respectively, both P < 0.05). CONCLUSION: This study found effort/reward imbalance was associated with having PA/PD among Japanese workers

    米国Institute for Family-Centered Careによれば, Family-Centered Care(以後FCCとする)は, [尊厳と尊敬][情報の共有][参加][協働]の4つの概念からなるケアの理念である. NICU看護においてFCCは, 大変重要な分野であり, 最近, 大きな関心を集め, さまざまな取り組みがなされてきているものの, 面会に関する規定や子どもの病状や治療に関する説明のあり方, 家族のための設備, 家族同士の交流など施設での格差があることも報告されている. また, FCCの実践には, マンパワーや施設の環境だけでなく, 医療者の考え方や態度も大きく影響するとされる. 私たちには, 子どもの最善を家族と共に考えるパートナーとなり, 子どもと家族が主役になれるようなかかわり方や関係づくり, 環境づくりが求められている. 人材育成の強化や組織文化の刷新も課題である. このワークショップでは, 古くて新しいFCCの考え方に関する説明, 日本の現状に関する研究報告, 臨床での実践とスタッフ教育に関する取り組み, 先駆的なアメリカでの取り組みなどについて情報を提供し, 臨床現場においてFCCを推進し, 根付かせていくための戦略について, 参加者の方々と意見交換を通じて検討していきたいと考えた.I Family-Centered Careの概念とその変遷 首都大学東京健康福祉学部看護学科 浅井 宏美 II Family-Centered Careに関する臨床での実際の取り組み 昭和大学病院総合周産期母子医療センター 井出 由

    The effect of a behavioral activation program on improving mental and physical health complaints associated with radiation stress among mothers in Fukushima:a randomized controlled trial

    Abstract Background Mothers living with small children in Fukushima prefecture may experience radiation anxiety and related symptoms after the Fukushima Dai’ich Nuclear Power Plant Accident. A behavioral activation (BA) program was developed to improve their psychosomatic symptoms. The purpose of this randomized controlled trial was to examine the effectiveness of a BA program for improving psychological distress and physical symptoms among mothers with preschool children in Fukushima-prefecture 3 years after the Fukushima Daiichi Nuclear Power Plant accident. Methods Participants were recruited from mothers living with a preschool child(ren) in Fukushima city and surrounding areas though a newspaper advertisement, posters, and flyers. Participants allocated to the intervention group received a newly developed group-based BA program, which consisted of two 90- min lessons with a 1-week interval. Psychological distress and physical symptoms as a primary outcome, and radiation anxiety and positive well-being (liveliness and life satisfaction) as a secondary outcome, were measured at baseline, 1- and 3-month follow-ups. Results Participants were randomly allocated to either an intervention or a control group (18 and 19, respectively). The BA program showed a marginally significant intervention effect on psychological distress (p = 0.051) and physical symptoms (p = 0.07) at 1-month follow-up, while the effect became smaller at 3-month follow-up. The effect sizes at 1-month were medium to large (-0.72 and -0.56, respectively). There was a significant intervention effect on increasing liveliness at 3-month follow-up (p = 0.02); and there were marginally significant effects on life satisfaction at 1- and 3-month follow-ups (both p = 0.09). Conclusions This BA program may be effective for improving psychological distress, physical symptoms, and well-being, at least for a short duration, among mothers with preschool children after the nuclear power plant accident in Fukushima, while a further large-scale study is needed.
